Abstract:Background/Aims: To evaluate whether local injection of exosomes derived from human adipose-derived stem cells (hADSCs) facilitates recovery of stress urinary incontinence (SUI) in a rat model. Methods: For the in vitro study, a Cell Counting Kit-8 (CCK-8) array and proteomic analysis were performed. For the in vivo study, female rats were divided into four groups: sham, SUI, adipose-derived stem cell (ADSC), and exosomes (n = 12 each). The SUI model was generated by pudendal nerve transection and vaginal dilation. Vehicle, hADSCs, or exosomes were injected into the peripheral urethra. After 2, 4, and 8 weeks, the rats underwent cystometrography and leak point pressure (LPP) testing, and tissues were harvested for histochemical analyses. Results: The CCK-8 experiment demonstrated that ADSC-derived exosomes could enhance the growth of skeletal muscle and Schwann cell lines in a dose-dependent manner. Proteomic analysis revealed that ADSC-derived exosomes contained various proteins of different signaling pathways. Some of these proteins are associated with the PI3K-Akt, Jak-STAT, and Wnt pathways, which are related to skeletal muscle and nerve regeneration and proliferation. In vivo experiments illustrated that rats of the exosome group had higher bladder capacity and LPP, and had more striated muscle fibers and peripheral nerve fibers in the urethra than rats of the SUI group. Both urethral function and histology of rats in the exosome group were slightly better than those in the ADSC group. Conclusions: Local injection of hADSC-derived exosomes improved functional and histological recovery after SUI.

Recent advances in cell technology have aroused interest in the use of autologous stem cell therapy to restore the ability of urinary control. The present study reviewed several types of stem cells for the treatment of SUI in the experimental and clinical stages.
Clinical Observation on Heat Sensitive Moxibustion Combined with Abdominal Acupuncture Therapy and Kegel Exercise in Treating 28 Females' Stress Urinary Incontinence

Abstract:Objective To examine the therapeutic effect of heat sensitive moxibustion combined with abdominal acupuncture therapy and Kegel exercise in treating female stress urinary incontinence (SUI).Methods Fifty-six female patients with SUI were randomized into treatment group and control group,28 cases in each group.The control group was treated by Kegel exercise and abdominal acupuncture therapy.The treatment group was added heat sensitive moxibustion.Heat sensitive moxibustion was given twice every day (once in the morning,once in the afternoon),45 minutes to 2 hours each time,for 5 days.From the 6th day,the moxibustion was given once every day,for 25 times.Abdominal acupuncture therapy was given once every day,retaining the needle for 25min each time,for 30 days.Kegel exercise was given three times every day,15 to 30minutes each time,for 30 days.Patients' 1-hour pad test leakage volume,leakage times and total scores of International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) in both groups were examined before and after treatment.The clinical effect was evaluated.Results The total effective rate of clinical effect in the treatment group was 96.43％,while 75.00％ in the control group.The treatment group was superior to the control group (P ＜0.05).Compared before treatment,patients' 1-hour pad test leakage volume,leakage times and ICIQ-SF total scores in both groups decreased after treatment (P ＜0.01).Compared with the control group,patients' 1-hour pad test leakage volume,leakage times and ICIQ-SF total scores in the treatment group decreased (P ＜ 0.05).Conclusion Heat sensitive moxibustion combined with abdominal acupuncture therapy and Kegel exercise treating female SUI might improve female leakage symptoms of incontinence and improve their life quality.

Abstract:Objective Using adipose-derived stem cells (ADSCs) to halt stress urinary incontinence (SUI) progression and regenerate urethral sphincter (US) has emerged as a promising method to restore normal voiding function. However, traditional two-dimensional (2D) cell culture systems have limitations in mimicking in vivo conditions losing some important properties. This study aims to examine the potential and mechanism of three-dimensional (3D) cultures of ADSCs in the treatment of SUI in a rat model simulating childbirth injury. Methods ADSCs were used to generate micro-tissues (MTs) with a hanging drop method. A total of 48 postpartum Sprague-Dawley (SD) rats were developed SUI models by 4 h vagina dilation (VD) followed by bilateral ovariectomy (OV). Ten rats underwent sham OV without VD as control group. The SUI rats were divided into three groups and received urethral injection of PBS, ADSCs and MTs. specimens were harvested for histology and tracking CM-Dil-Labelled ADSCs at day 1, 3, 7 (n=3), and 28 (n=7) post-injection. At day 28, the remaining rats in each group were examined for voiding function. Western blot, immunofluorescence (IF) and immunohistochemistry (IHC) staining were performed to examine the histological change and cytokines. Results Compared with ADSCs, MTs expressed increased vascular endothelia growth factor (VEGF), TNFα stimulated gene/protein 6 (TSG-6) in vitro and represented a higher retention rate in vivo. The voiding function and histopathological recovery were significantly better in MTs group than ADSCs group. Conclusions Urethral injection of MTs better restored the voiding function than ADSCs. The up-regulation of some bioactive factors and the improved cellular retention in the urethra may be responsible for the underlying mechanisms.

In this paper, we review recent publications and clinical trials investigating the applications of several stem cell types as potential treatments for SUI and the underlying challenges of such therapy.

Abstract:OBJECTIVE To review the progress and clinical application of cellular therapy for stress urinary incontinence (SUI). METHODS The literature about cellular therapy of SUI was extensively reviewed. RESULTS Because of having no or poor regeneration capacity, the clinical application of chondrocytes and myoblasts were limited. Based on the rapid progress in stem cell biology, an increasing number of animal experiments and clinical trials about cellular therapy of SUI have been reported with encouraging results. All these show that cellular therapy has great potential in clinical application. Stem cells are considered as ideal seeded-cells for treatment of SUI. CONCLUSION Cellular therapy, especially stem cells, provides a novel approach for treatment of SUI, but the mechanism needs further study.
Exosomes secreted by urine-derived stem cells improve stress urinary incontinence by promoting repair of pubococcygeus muscle injury in rats

Abstract:BackgroundPrevious studies revealed that urine-derived stem cells (USCs) could promote myogenesis after the impairment of the sphincter muscles. However, the effects of exosomes secreted by USCs (USCs-Exo) were not elucidated. Exosomes are nanosized membrane vesicles secreted by the cells. They have been proved to be effective in protecting against tissue injury and therapeutic in tissue repair. USCs are ideal sources of exosomes because of the noninvasive obtaining method and self-renewal abilitiy. This study aimed to show the therapeutic effects of USCs-Exo on improving stress urinary incontinence (SUI).MethodsRat SUI models were established in this study using vaginal balloon inflation, and urodynamic and histological examination were carried out after exosome application. The proliferation and differentiation of muscle satellite cells (SCs) were evaluated using EdU, Cell Counting Kit 8, immunofluorescence staining, and Western blot analysis. mRNAs and proteins related to the activation of SCs were detected by reverse transcriptase quantitative polymerase chain reaction (RT-qPCR) and Western blot analysis.ResultsAfter exosome injection, the urodynamic parameters significantly improved and the injured muscle tissue recovered well. The activation, proliferation, and differentiation of SCs were promoted. The phosphorylation of extracellular-regulated protein kinases (ERK) was enhanced. When ERK was inhibited, the promoting effect of USCs-Exo treatment disappeared.ConclusionThe findings of this study elucidated the functional roles of USCs-Exo in satellite cell ERK phosphorylation and identified a novel agent for skeletal muscle regeneration, providing a basis for further exploring a cell-free correction for SUI.

Abstract:Background: Stress urinary incontinence (SUI) is a common disorder with high prevalence in women across their life span, but there are no non-surgical curative options for the condition. Stem cell-based therapy, especially endogenous stem cell therapy may be a potential treatment method for SUI. The aims of this study are to identify, isolate, and assay the function of urethral striated muscle derived stem/progenitor cells (uMDSCs) and to assess uMDSC response to microenergy acoustic pulses (MAP). Methods: Urethral striated muscle was identified utilizing 3D imaging of solvent organs (3DISCO) and immunofluorescence (IF). uMDSCs were isolated and purified from Zucker Lean (ZL) (ZUC-LEAN) (ZUC-Leprfa 186) rats, with magnetic-activated cell sorting (MACS) and pre-plating methods. The stemness and differentiation potential of the uMDSCs were measured by cell proliferation, EdU, flow cytometry, IF, and Western blot. Results: Comparison of the cell proliferation assays between MACS and pre-plating reveals the advantage of MACS over pre-plating. In addition, the study reveals that uMDSCs form myotubes when treated with MAP. Conclusions: The uMDSCs within female rat urethral striated muscle could be a therapeutic target of MAP in managing SUI.
Activation of VEGF and ERK1/2 and Improvement of Urethral Function by Adipose-derived Stem Cells in a Rat Stress Urinary Incontinence Model

Abstract:OBJECTIVE:To investigate the injected autologous adipose-derived stem cells (ADSCs) in improving stress urinary incontinence in a rodent model of parturition-related stress incontinence and the possible mechanism.METHODS:The 40 rats were developed stress urinary incontinence models by postpartum balloon dilation of the vagina for 4 hours followed by bilateral ovariectomy. ADSCs were isolated from the peri-ovarian fat and labeled with thymidine analog 5-ethynyl-2-deoxyuridine (EdU). Twenty stress urinary incontinence rats received peri-urethral injection of phosphate-buffered saline as the negative controls and the other 20 stress urinary incontinence rats received peri-urethral injection of EdU-labeled ADSCc. Twenty control rats underwent sham ovariectomy without balloon dilation and served as positive controls. Four weeks later, voiding function was assessed by cystometry. Urethral histologic examination (Masson trichrome stain, picrosirius red stain, Hart elastin stain, Gordon and Sweet stain, and immunohistochemical stain) and Western blot were performed on urethral tissues.RESULTS:Both leak point pressure and bladder capacity were significantly increased in ADSC-treated rats, compared to the balloon-injured ovariectomized rats. Histologic examination revealed normalized appearance of the fibromuscular structure of the urethra as well as increased peri-urethral blood vessel density in ADSC-treated rats. On Western blot, vascular endothelial growth factor and P-extracellular signal-regulated kinases (ERKs)1/2 protein was expressed at a higher rate in tissues from ADSC-treated rats compared to phosphate-buffered saline-treated rats.CONCLUSION:Peri-urethral injection of ADSCs is associated with more normal urinary function and urethral structure in rats with parturition-related incontinence. The activation of vascular endothelial growth factor and ERK1/2 may be responsible for the paracrine effects from ADSCs.

Abstract:Pelvic floor disorders (PFDs) represent a group of common and frequently-occurring diseases that seriously affect the life quality of women, generally including stress urinary incontinence and pelvic organ prolapse. Surgery has been used as a treatment for PFD, but almost 30% of patients require subsequent surgery due to a high incidence of postoperative complications and high recurrence rates. Therefore, investigations of new therapeutic strategies are urgently needed. Stem cells possess strong multi-differentiation, self-renewal, immunomodulation, and angiogenesis abilities and they are able to differentiate into various cell types of pelvic floor tissues and thus provide a potential therapeutic approach for PFD. Recently, various studies using different autologous stem cells have achieved promising results by improving the pelvic ligament and muscle regeneration and conferring the tissue elasticity and strength to the damaged tissue in PFD, as well as reduced inflammatory reactions, collagen deposition, and foreign body reaction. However, with relatively high rates of complications such as bladder stone formation and wound infections, further studies are necessary to investigate the role of stem cells as maintainers of tissue homeostasis and modulators in early interventions including therapies using new stem cell sources, exosomes, and tissue-engineering combined with stem cell-based implants, among others. This review describes the types of stem cells and the possible interaction mechanisms in PFD treatment, with the hope of providing more promising stem cell treatment strategies for PFD in the future.

Abstract:Objective To review the study on adipose derived stem cells(ADSCs) in the therapy of urological diseases.Methods The recent literature concerning ADSCs in bladder repair,urethral reconstruction,incontinence treatment,and erectile dysfunction treatment was reviewed.Results The application of tissue engineering using ADSCs has made significant achievements in the treatment of urological diseases and in animal studies,and has been initially used in clinical and has achieved a good therapeutic effect.Conclusion Tissue engineering using ADSCs has good prospects in the study on urological diseases,and is expected to widely used in the treatment of urological diseases.

Abstract:Cell-based injectable therapy utilizing stem cells is a promising approach for the treatment of stress urinary incontinence (SUI). Applying a magnetically controlled cell delivery approach has enormous potential to enhance cell retention capability within the specified site. To assess the therapeutic efficacy of cellular magnetic targeting, we applied an external magnetic force to target an adipose-derived stem cell based therapy in a rat model of SUI. The results revealed that magnetic attraction of transplanted cells under the magnetic field was generated by cell uptake of superparamagnetic iron oxide nanoparticles in vitro. More importantly, magnetic targeting improved the retention rate of transplanted cells and facilitated the restoration of sphincter structure and function in a rat SUI model according to the results of histological examination and urodynamic testing. Therefore, magnetically guided targeting strategy might be a potential therapy method for treatment of SUI.
Low Intensity Pulsed Ultrasound Influences the Myogenic Differentiation of Muscle Satellite Cells in a Stress Urinary Incontinence Rat Model.

Abstract:OBJECTIVE To investigate the therapeutic effect of low intensity pulsed ultrasound (LIPUS) in a stress urinary incontinence (SUI) rat model and its influence on myogenic satellite cells. METHODS Fifty Sprague-Dawley rats underwent vaginal distension and bilateral ovariectomy mimicking par- tum injury and menopause to construct SUI models, which were further randomized into 100 mW/cm(2) LIPUS, 200 mW/cm(2) LIPUS, 300 mW/cm(2) LIPUS, and none-treatment control sub-groups with 10 rats per subgroup. Ten rats served as mock operation control. Leak point pressure and bladder capacity were recorded 1 week after LIPUS treatment. Immunofluorescence staining and Western blot were performed to examine histological changes, myodifferentiation, and signaling pathway. RESULTS Here, we found the leak point pressure and bladder capacity were restored in 200 mW/cm(2) LIPUS and 300 mW/cm(2) LIPUS groups, but not in 100 mW/cm(2) LIPUS group. More robust striated muscle regeneration was observed in 200 mW/cm(2) LIPUS group comparing with the SUI none-treatment group. Moreover, we found LIPUS activated the myodifferentiation of muscle satellite cells, which is correlated to p38 phosphorylation level. CONCLUSION LIPUS restored the leak point pressure and bladder capacity, and activated satellite cell myodifferentiation in SUI rat model. (C) 2018 Published by Elsevier Inc.

Abstract:AbstractObjectiveTo explore the myoblast formation around the urethra and increase in urethral resistance of bone marrow mesenchymal stem cells or muscle‐like cells/calcium alginate composite gel injection therapy and effect on LPP in SUI rat model.MethodsIsolation, cultivation, and identification of SD rat bone marrow mesenchymal stem cell were performed. 5‐Azacytidine was introduced to induce muscle‐like cells. SUI was produced in 72 6‐week‐old female Sprague–Dawley rats, which were divided into four groups: stem cell‐gel group, muscle‐like cell‐gel group, Gel group, and mock control group. One, 4, and 8 weeks after injection, the leak point pressure (LPP) was measured. HE staining of Desmin and α‐skeletal muscle actin (α‐SMA) were performed.ResultsAt 4 and 8 weeks after injection in stem cell‐gel group and muscle‐like cell‐gel group, growth of blood vessels gradually increased at gel edge, BMSC, and muscle‐like cells gathered around the new blood vessels observed by fluorescence tracer, muscle‐like cells grew into elongated spindle‐like cells, Desmin, and α‐SMA staining were obviously positive expression. LPP determinations of the mock control group compared with the Gel groups were significantly different.ConclusionsCompound of BMSC, muscle‐like cells, and calcium alginate composite gel has the potential to differentiate into muscle cells in the microenvironment of SUI rat model. It is found by LPP measurement that the correlation between the increase in urethral resistance and the volume effect of calcium alginate gel is high. Neurourol. Abstract:Abstract To establish a strategy for culitivation of human adipose-derivde stem cells(ADSCs) and differentiation of adipose-derived stem cells into smooth muscle cells.Methods ADSCs were isolated from liposuction tissue by collagenase treatment,then were cultured and expanded in vitro.The growth curve and surface antigen were identified.Characterization and multiple-differentiation of adipose-derived stem cells were observed;TGF-β and ascorbic acid(AA) were used to the strategy for differentiation of adipose derived stem cells into smooth muscle cells.The induced cells were subsequently harvested for determination of smooth muscle cells by immunohistochemical examination and RT-PCR.Results The adipogenic,osteogenic and chondrogenic differentiation of human adipose-derived stem cells was identified;The induced cells exhibited swirl morphology like smooth muscle cells,The result of RT-PCR of the induced cells showed expression of a-SMA,calponin and SM-22 genes,and the express of MHC gene was not detected.Conclusion ADSCs can be differentiated into smooth muscle cells by TGF-β and AA induced effects.

Abstract:In the study by Sarveazad et al. adipose tissue-derived stem cells were injected to reinforce anal sphincter repair. The authors came to the conclusion that injection of stem cells during repair surgery for fecal incontinence may cause replacement of fibrous tissue, which may be a key point in treatment of fecal incontinence. The authors emphasized in their "Discussion" section that the ability of stem cells to differentiate into muscle fibers, replacing the fibrous tissue at the site of repair, is their main action, which may not be accurate. We think that healing of repaired anal sphincter begins with granulation tissue formation, which then matures into fibrous tissue that becomes infiltrated by muscle fibers from the approximated cut ends of the sphincter, resulting in regain of sphincter muscle continuity. This is supported by many experimental studies that have evaluated local injection of stem cells during sphincteroplasty in rats and shown that the injected stem cells do not differentiate into muscle fibers but may induce healing by a strong fibrous tissue. Further studies are needed to determine the main mechanism of action of mesenchymal stems cells in augmenting anal sphincter repair.

Abstract:Pelvic organ prolapse (POP) has a high incidence rate among Chinese women. Repeated mechanical stimulation is an important factor causing POP, but the injury mechanism has not yet been elucidated. The purpose of this study is to explore the related mechanisms of pelvic floor supporting tissue damage caused by mechanical force and the application of stem cell therapy. First, we obtained vaginal wall and sacral ligament tissue samples from clinical patients for examination. Pelvic floor support tissues of POP patients displayed high expression of inflammation and immune disorders. Then, we constructed a rat model of childbirth injury. In vivo and in vitro experiments investigated the key mechanism of pelvic floor support tissue injury caused by mechanical force. We discovered that after mechanical force, a large number of reactive oxygen species (ROS) and macrophages rapidly accumulated in pelvic floor tissues. ROS stimulated macrophages to produce NLRP3 inflammatory complex, induced the release of interleukin (IL-1β) and pyroptosis and exacerbated the inflammatory state of damaged tissues, persisting chronic inflammation of fibroblasts in supporting tissues, thus causing the pelvic floor's extracellular matrix (ECM) collagen metabolic disorder. Resultingly impeding the repair process, thereby causing the onset and progression of the disease. Through their paracrine ability, we discovered that adipose mesenchymal stem cells (ADSCs) could inhibit this series of pathological processes and promote tissue repair, asserting a good therapeutic effect. Simultaneously, to overcome the low cell survival rate and poor therapeutic effect of directly injecting cells, we developed a ROS-responsive PVA@COLI hydrogel with ADSCs. The ROS-scavenging properties of the gel could reshape the site of inflammation injury, enhance cell survival, and play a role in subsequent treatment. The findings of this study could serve as a basis for early, targeted intervention therapy for POP and representing a promising approach.

Abstract:OBJECTIVE:To investigate the feasibility and effectiveness of human umbilical cord mesenchymal stem cells (HUCMSC) transplantation in the treatment of female stress urinary incontinence (SUI) in rats.METHODS:The 14 female SD rats of SUI model were established by vaginal balloon dilation after birth and maintain this status for four hours bilateral ovariectomy were performed after two weeks and were routinely reared for two months, then 12 SUI rat model were made. Two months later, transfected with plasmid pEGFP-N1 of HUCMSC were injected into the region surrounding the urinary tract matched with saline injection as control group. To get genitourinary tissue after testing urodynamic indicators, and observe the pathological changes of the bladder, urethra and the surrounding tissue; fluorescent cell of the experimental groups specimens were observed by fluorescence microscope.RESULTS:The leak point pressure(LPP) was (23.8 ± 4.2) mm Hg(1 mm Hg = 0.133 kPa) of the SUI rats. Transplanting mesenchymal stem cells of SUI rats, the positive rate of sneeze test was 1/6 in SUI group and 5/6 in control group, which reached statistical significance (P < 0.05); LPP was (30.6 ± 2.8) mm Hg in SUI group and (21.4 ± 7.0) mm Hg in control group, which reached statistical significance (P < 0.05) .In SUI rate model, connective tissue content were increased in urethra and the surrounding tissue and more fluorescent cell were observed.CONCLUSIONS:A rat model of female SUI was established successfully through postpartum vaginal balloon dilation and bilateral ovariectomy. MSC can be survived and proliferated in the urethral and the surrounding tissue of injured rats, and improve the urodynamic indicators and the positive rate of sneeze test. Morphology shows renovation of the support structures around the urethra.
