Clinical Value of Posterior Cruciate Ligament Index in the Diagnosis of Anterior Cruciate Ligament Injuries

Abstract:Objective To measure the value of posterior cruciate ligament index (PCL index), to assess the relationship of PCL index and stability of joint, and to evaluate the value of it in the diagnosis of anterior cruciate ligament (ACL) injuries. Methods The shortest distance between the femoral and tibial attachment of PCL (X) and the distance from that line to the tip of the arc marked by the PCL (Y) on the sagittal plane images of MRI were measured. The quotient of these two parameters (X/Y) defined the PCL index. The distance of tibial anterior displacement was obtained with both knee flexed at 30°and 90°with pull strength at 15, 20 and 30 pounds and the maximum respectively with KT2000, the difference of displacement of two knee was counted. The relationship of PCL index and displacement of KT2000 was assessed and the clinical diagnostic value of PCL index was evaluated. Results The mean PCL index in whom arthroscopy ruled out ACL injury was 5.0953± 0.6029, in whom arthroscopy showed ACL partial rupture was 3.5867± 0.5276, and in whom with total ACL rupture was 3.4217± 0.5532. Negative correlation was between the PCL index and the displacement of KT2000 at maximum pull strength. Conclusion PCL index could be regarded as the clinical diagnostic index of ACL injuries.
