Abstract:Objective To quantitatively evaluate the effects of canceling drug price addition policy on the change of using high-priced systemic anti-bacterial drugs in 5 general tertiary public hospitals in Beijing City.Methods Difference-in-difference (DID) model was established to analyze the change of using high-priced systemic anti-bacterial drugs in 5 general tertiary public hospitals in Beijing City by using the unit of defined daily dose as an index.Results After the cancellation of drug price addition,the structure index of high-priced drug use did significantly decrease in one sample hospital,and not significantly in the other one sample hospital,but significantly decreased in the other three sample hospitals.Conclusion There are different effects of canceling drug price addition policy on the use of high-priced anti-infective drugs among the 5 general tertiary public hospitals in Beijing City.Further researches are needed to observe the effects of canceling drug price addition policy.

Abstract:Public hospitals attempt to separate medical and pharmaceutical management,to cancel drug addition, registration fees and medical fees,and to receive medical services fees in Beijing.Hospital outpatient site survey found the hospital changed the traditional management and business model from a focus on revenue-generating to effective cost control,from more drugs to the rational use of drugs.The number of outpatients increased significantly.The majority of patients supported and satisfied with the medical service fee policy.Some hospital administrators believe that the current medical service fees and canceled drug addition did not really cut off the interests between doctors and medicines.Long-term impact on hospitals and health care workers needs further study.
Analysis and Suggestion on the Compensation Mechanism of Public Hospitals after Abolishing Drug Price Addition

Abstract:As an important part of public hospital reform, the compensation mechanism after abolishing drug price addition policy is a problem that the related stakeholders need to face and think. In this paper, new healthcare reform policy documents and pilot compensation mechanism were analyzed. The author proposed that the government should fulfill its major responsibility, improve price formation and adjustment mechanism of medical service, deepen medical insurance payment reform , establish hierarchical diagnosis system, separate administrative functions and standardize drug production and circulation. Meanwhile, public hospitals should innovate management process, strengthen internal management and explore potential cost-saving. In this way, each stakeholder would work together to promote public hospital reform.

Abstract:Objective To probe into reform outcomes of county-level public hospitals in the province,for the purpose of improving reform measures and ensuring validity and sustainability of the reform.Methods 38 general hospitals with ongoing reform since December 2012 were rounded up in the research.From these hospitals the performance indicators of January-June,2012 and January-June,2013 were collected and analyzed for comparison before and after the reform.Results The reform found a significant rise of medical services,an increase in the revenue of both outpatients and inpatients,and a proportional drop of drug sales(from 54.41％ to 49.30％); an increase of outpatient costs per visit (0.75％average annual increase); a drop of hospitalization fees per person(1.41％ average annual decrease); an increase of medical workers' income (6.04％ increase); and a budget surplus for the hospitals.Conclusion The comprehensive reform has reduced medicine cost burden for patients,yet its role in regulating physicians' prescription behavior and alleviating the general out-of-pocket burden of patients in general remains to be confirmed.

Abstract:Objectives:This article is to analyze the change of economic status of hospitals and health expenditure after executing the policy controlling drug mark-up rate,and to put forward the corresponding reimbursement on implementing zero profit drug supply policy as well. Data and methods:Quantitative analyze from annual financial reports from 2003 to 2008 of local hospitals in Shanghai (including the hospitals affiliated of Fudan University). Results:The drug mark-up rate had decreased from 33% to 17% after the policy for controlling drug mark-up rate had been implemented; Medicine revenues had still been the main revenue of hospitals; there had been taken a loss in 37% of hospitals since medicine revenues were reduced; health expenditure had not been saved with the decrease of drug mark-up rate; the total compensation would be 3 billion per year if implementing zero profit drug supply policy; different source of compensation would cause different results of health expenditure. Recommendations:Integrated compensation program should be applied after implementing zero profit drug supply policy,including government input improved,health insurance shared,pharmacy service fee set up and medical service price increased,in order that the long mechanism of controlling health expenditure from hospitals will be established which can not only maintain economic status of hospitals but also lighten the economic burden from patients.

Abstract:Objectives:To analyze the economics status and government input of public hospitals at different levels from the year 2002 to 2007. Data and Methods: Make full use of the data of Shanghai total expenditure on health (STEH),especially on pulic hospitals,and give a descriptive analysis. Results: The revenue of hospitals accounts for around 78% of STEH,mainly in secondary and tertiary hospitals. The surplus turns into deficit in 2007 in public hospitals for the first time. Drug retailing remains the major source of public hospitals revenue. The government input accounted for about 1/5,1/3 and 1/2 personnel expenditure of the tertiary,secondary and primary hospitals respectively. And the drug mark-up rate is restricted to 17.8%,21.0% and 23.5% in 2007 in tertiary,secondary and primary hospitals respectively. Recommendations: A clear increase of government health input is needed so as to adjust the policy of financial subsidies. Meanwhile the medical care price system should be modulated. Cost accounting on medical care and health information system should be strengthened. In addition,hospitals of diverse investment should be introduced to promote public hospitals reform.
Effect of separating drug sales from medical services on hospital revenue and medical services in the county public hospitals of Guangxi Province

Abstract:Objective:To evaluate the effect of separating drug sales from medical services on hospital revenue and medical services in the county-level public hospitals of Guangxi. Methods:The controlled before and after study design was employed. 2009 to 2012 was the pre-intervention period;2013 was the intervention period. Pilot people’s hospitals were included in the intervention group;non-pilot people’s hospitals were included in the control group. Da-ta came from hospitals and new rural cooperative medical statistics from 2009 to 2013 and the Guangxi Statistical Yearbook from 2010 to 2014. The analysis method of difference-in-differences based regression was employed. Re-sults:Separating drug sales from medical services included cancelling medicine markups, increasing price of inspec-tion and nursing services, reducing price of large equipment inspection services and increasing financial assistance. In terms of hospital revenue, compared with non-pilot hospitals, for pilot hospitals, the reform reduced medicine rev-enues by 3. 326 million yuan and increased medical revenue by 10. 75 million yuan. There was no significant change in financial assistance. In terms of medical expenses, compared with non-pilot hospitals, the reform reduced per-visit outpatient drug expenses in pilot hospitals by 3. 51 yuan, increased per-visit outpatient inspection fees by 2. 23 yuan, reduced per-visit inpatient drug expenses by 133. 5 yuan, increased per-visit inpatient inspection fees by 62. 01 yuan, and increased per-visit inpatient nursing fees by 69. 72 yuan. There were no significant change in outpatient and inpa-tient visits, length of stay, outpatient expenses per-visit and inpatient expenses per-visit. Conclusion:County hospi-tals can offset losses due to cancelling medicine markups by medical service pricing adjustment in inpatient departments;in outpatient departments, they can offset losses due to cancelling medicine markups by both medical service pricing ad-justments and medical service utilization adjustments beyond policy adjustments. The reform did not reduce the operating revenue of pilot hospitals or the medical expenses per visit. The reform had little effect on hospital and doctor incentives.

Abstract:Objective: Analysis on the composition of high hospitalization expenses and explores the influencing factors so as to put forward some specific measures and suggestions.Methods: Selected 811 inpatient cases with expensive costs whose total hospitalization expense of indi-vidual patients is over RMB100, 000 at a 3A hospital of Shanghai in 2014.Data is analyzed with SPSS 19.0.Results:Drugs and medical material costs are the highest in high hospitalization expenses.The influencing factors are including gender, medical payment, and classification of disea-ses, department, hospital days and curative effect.Conclusion: By lowering drugs and material costs, strengthen health education of focus groups, monitoring priority diseases and departments, reducing the average hospitalization days, improving medical standards, perfecting clinical pathways, exploring DRGs, the hospital can effectively control the high inpatients.

Impact of China's Public Hospital Reform on Healthcare Expenditures and Utilization: A Case Study in ZJ Province

Abstract:Background: High drug costs due to supplier-induced demand (SID) obstruct healthcare accessibility in China. Drug prescriptions can generate markup-related profits, and the low prices of other medical services can lead to labor-force underestimations; therefore, physicians are keen to prescribe drugs rather than services. Thus, in China, a public hospital reform has been instituted to cancel markups and increase service prices. Methods: A retrospective pre/post-reform study was conducted in ZJ province to assess the impact of the reform on healthcare expenditures and utilization, ultimately to inform policy development and decision-making. The main indicators are healthcare expenditures and utilization. Results: Post-reform, drug expenditures per visit decreased by 8.2% and 15.36% in outpatient and inpatient care, respectively; service expenditures per visit increased by 23.03% and 27.69% in outpatient and inpatient care, respectively. Drug utilization per visit increased by 5.58% in outpatient care and underwent no significant change in inpatient care. Both were lower than the theoretical drug-utilization level, which may move along the demand curve because of patient-initiated demand (PID); this indicates that SID-promoted drug utilization may decrease. Finally, service utilization per visit increased by 6% in outpatient care and by 13.10% in inpatient care; both were higher than the theoretical level moving along the demand curve, and this indicates that SID-promoted service utilization may increase. Conclusion: The reform reduces drug-prescription profits by eliminating drug markups; additionally, it compensates for service costs by increasing service prices. Post-reform, the SID of drug prescriptions decreased, which may reduce drug-resource waste. The SID of services increased, with potentially positive and negative effects: accessibility to services may be promoted when physicians provide more services, but the risk of resource waste may also increase. This warrants further research. It is recommended that comprehensive measures that control SID and promote physician enthusiasm be carried out concurrently.

Abstract:OBJECTIVE:To investigate the satisfaction degree of outpatients on policy of separation of clinical pharmacy from 4 third grade hospitals in Shanghai,and provide reference for promoting medical reform policy. METHODS:Parts of outpatients in the 4 new suburb third grade hospitals were investigated by the questionnaire and interview in Shanghai city,and the data was sta-tistically analyzed by Excel software and SPSS 19.0 software. RESULTS:Totally 400 questionnaires were sent out and 368 were valid with effective recovery of 94.3%. The awareness rate of medical separation policy was 81.3% in the outpatients;55.5% of outpatients thought that medicine cost savings effect was not obvious;the overall satisfaction of patients was 91.6%,and the high-est of satisfaction was the service attitude and the lowest was medical expenses. CONCLUSIONS:Policy of separation of clinical pharmacy has reduced the medical expenses,guided the triage treatment of patients partly and improved the satisfaction of patients. It is suggested to shorten the waiting time by optimizing examination process,strengthen price regulation in drug,standardize pro-duction and distribution process,improve use rate of essential medicines and promote clinical rational drug use for further improve-ment of satisfaction.
Different Impacts of the Zero-Markup Drug Policy on County General and Traditional Chinese Medicine Hospitals: Evidence from Shandong Province, China

Abstract:Background: As a key part of the new round of health reform, the zero-markup drug policy (ZMDP) removed the profit margins of drug sales at public health care facilities, and had some effects to the operation of these institutions. This study aims to assess whether the ZMDP has different impacts between county general and traditional Chinese medicine (TCM) hospitals. Methods: We obtained longitudinal data from all county general and TCM hospitals of Shandong province in 2007-2017. We used difference-in-difference (DID) method to identify the overall and dynamic effects of the ZMDP. Results: On average, after the implementation of the ZMDP, the share of revenue from medicine sales reduced by 16.47% and 10.42%, the revenue from medicine sales reduced by 24.04% and 11.58%, in county general and TCM hospitals, respectively. The gross revenue reduced by 5.07% in county general hospitals. The number of annual outpatient visits reduced by 11.22% in county TCM hospitals. Government subsidies increased by 199.22% and 89.3% in county general and TCM hospitals, respectively. The ZMDP reform was not significantly associated with the revenue and expenditure surplus, the number of annual outpatient visits and the number of annual inpatient visits in county general hospitals, the gross revenue, the revenue and expenditure surplus and the number of annual inpatient visits in county TCM hospitals. In terms of dynamic effects, the share of revenue from medicine sales, revenue from medicine sales, and gross revenue decreased by 20.20%, 32.58% and 6.08% respectively, and up to 28.53%, 63.89% and 17.94% after adoption, while government subsidies increased by around 170% to 200% in county general hospitals. The number of annual outpatient visits decreased by 9.70% and up to 18.84% in county TCM hospitals. Conclusion: The ZMDP achieved its some initial goals of removing the profits from western medicines in county hospitals’ revenue without disrupting the normal operation, and had different impacts between county general and TCM hospitals. Meanwhile, some unintended consequences were also recognized through the analysis, such as the decline of the utilization of the TCM.

Abstract:China's public hospital price change reform was progressively piloted at urban-level tertiary hospitals in 2015, aiming to adjust the healthcare expenditure structure, reorient public hospitals towards social objectives and control inflated healthcare expenditure. This study investigates the impacts of price changes on inpatient expenditure, service quantity, quality and efficiency and whether the impacts varied in different specialities, treatments and hospitals. A difference-in-differences analysis was conducted using data from 25 million patients from 124 nationally representative tertiary hospitals between 2013 and 2018. The study analyses changes in total expenditure, drug, medical services and diagnostic test/medical consumables expenditure per admission, the use of antibiotics, performed surgery, readmission within 30 days and length of stay (LOS). These factors are examined before and after adjusting the price changes for demographic, socioeconomic and clinical covariates. The price changes decreased drug expenditure per admission (-13.5%, P < 0.001, USD 96.6) and increased medical services expenditure per admission (30.9%, P < 0.001, USD 153.3). They also reduced the LOS (-1.2%, P = 0.019, 0.1 days) while not significantly affecting total expenditure per admission, diagnostic test/medical consumables per admission and the use of antibiotics, surgery performed and readmission rates. In heterogeneity analysis, price changes reform increased efficiency in major diagnostic categories (MDCs) with high drug share and improved quality in nonoperating room surgical groups and hospitals with high drug share; however, it increased total expenditure in MDCs with low drug share or surgical groups. China's public hospital price change reform generally adjusted the inpatient expenditure structure and reduced the LOS in tertiary public hospitals. Sufficient compensation from medical services and government subsidies that minimize the income effects may be the key to the success of price change reform.

Abstract:Abstract Background China has initiated a medical pricing reform to combat the overuse of drugs and relieve the financial burden of patients. This paper aims to analyze the effect of medical pricing reform on revenue structure and healthcare expenditure of county public hospitals in Guangdong province. Methods Based on the monthly data from January 2013 to August 2019, we use interrupted time series design to evaluate the effects of medical pricing reform on healthcare expenditure in both outpatients and inpatients. A counterfactual is also established to examine the net effect of the policy. Results The proportion of drug revenue decreased from 35 % to 2015 to 29.7 % in 2019, and the revenue from medical services and inspection increased 3.2 and 3 % respectively. Meanwhile, the increasing trend of total expenditure and its main components is slowed down, especially the drug expense and medical consumable expense for inpatients after the Zero Mark-up Drug policy (coefficient = -18.76, p < 0.01; coefficient = -13.41, p < 0.01, respectively). However, the growth of inspection expense for outpatients continues to increase, while the healthcare expenditure for inpatients experiences an instant increase after the Zero Mark-up Medical Consumables policy. In terms of the net effect, most of healthcare expenditure in both outpatient and inpatient experienced a negative net growth from 2015 to 2019. Conclusions The medical pricing reform is a valuable attempt in controlling the unreasonable increase of medical expenses. In the meantime, the unexpected increase in inspection expenditure and insufficient compensation from medical service adjustment should draw the attention of the policymakers.

Abstract:Irrational use of drugs remains a major challenge especially in developing countries, which contributed to a heavy pharmaceutical expenditure burden. Price regulation has been taken to curb the growth of pharmaceutical expenditures in many countries. This study aimed to investigate the impact of different mark-up drug policies on drug-related expenditures in tertiary public hospitals in Shanghai, China. Data were drawn from the audited financial statement in 24 tertiary public hospitals in Shanghai from January 2015 to December 2018. Drug-related revenue data and per capita cost data pre- and post-intervention were included. Interrupted time series design was applied to assess the actual effects of Fixed Percent Mark-up Drug (FPM) policy and Zero Mark-up Drug (ZMD) policy respectively. Results showed that ZMD policy achieved better intervention effects on declining drug-related expenditures than FPM policy. Apart from a declining trend in drug proportion (coefficient = -0.0017, p = 0.031), no other significant changes were found during FPM implementation. However, ZMD policy was associated with a level decline in per capita outpatient drug cost (coefficient = -12.21, p = 0.025) and a trend decline in per capita inpatient drug cost (coefficient = -25.12, p < 0.001), as well as a level decrease (coefficient = -0.0256, p = 0.001) and a downward tendency (coefficient = -0.0018, p < 0.001) in drug proportion. ZMD policy was effective in regulating drug-related expenditures, while FPM policy was difficult to achieve expected results due to the existence of profit space. Further regulation should be strengthened in the future, especially on drug revenue and per capita drug cost.
