Vertical transmission of HCMV infection and HCMV gB genotypes in Qinba mountain areas

Abstract:AIM: To assess the distribution of human cytomegalovirus (HCMV) glycoprotein B(gB) genotypes in the maternal and infant HCMV infected population in Qinba mountain areas of Shaanxi,and to explore the possible relationship between genotypes and clinical outcome of intrauterine HCMV infection.METHODS: HCMV gB was detected by a nested PCR in the(sera) of 214 pairs of mother and infant,and gB genotypes were(analyzed) by restriction fragment length polymorphism(RFLP) (analysis).RESULTS: ①The HCMV-DNA positive rate in sera was(6.1%) in pregnant women,and 2.3% in the neonates.In the 13 pregnant women with positive HCMV-DNA,4 were confirmed infected with HCMV in their umbilical cord blood.The rate of transplacental transmission of HCMV was 30.8%.②All of the 13 cases were grouped into the gB genotype 2,including 3 sympto-(matic) infected infants and 2 asymptomatic infants. ③The incidences of premature delivery,fetal growth restriction,low birth weight (<2500 g) and pathological jaundice were much higher in the HCMV infected pregnant women than those in the non-infected(P<0.05).No significant difference was found in the incidence of neonatal asphyxia between the two.CONCLUSION: HCMV gB genotype 2 is a major genetype in maternal and infant infections in Qinba mountain areas of Shaanxi,and it can cause(mother-)to-infant transmission.
