Efficacy analysis of selective peripheral neurotomy for spasmodic torticollis

Abstract:Objective To study the effect of CT and electromyographic(EMG) examination on surgical outcome of selective peripheral neurotomy(SPN) for spasmodic torticollis. Methods Clinical data of 26 patients with spasmodic torticollis were analyzed retrospectively. Toronto Western Spasmodic Torticollis Rating Scale(TWSTRS) was used in all the patients preoperatively, while CT scan was performed in 15 and EMG examination in 19. SPN was performed according to the results of preoperative examination and clinical features. Surgical outcomes were evaluated by TWSTRS scores acquired from postoperative telephone follow-up. Results Effective treatment was found in 21 patients and poor efficacy in 5 according to the TWSTRS scores. There was a statistical significance in difference of TWSTRS scores before and after the surgery(P < 0.01). No death or severe complication occurred in the study. There was a significant difference in surgical outcome between the groups with and without CT scan preoperatively(P = 0.02),and a similar result was also seen between the groups with and without CT scan and EMG examination preoperatively(P = 0.047).However, there was no significant difference between the groups with and without EMG examination or injection of creotoxin(P =0.08, P = 0.33). Conclusion SPN is effective for spasmodic torticollis, and the individualized surgical plan guided by cervical thin-section CT before the surgery can improve the surgical outcome.
