Clinical Analysis of 373 Patients with Non-ST Elevation Acute Coronary Syndrome in a Teaching Hospital

Abstract:Aim To analysis of patients with non-ST elevation acute coronary syndrome in 2008,and scale the gap between real world pratice and evidence-based guidelines.Methods Clinical data of the dianosis and management of 373 patients with non-ST elevation acute coronary syndrome from January to December in 2006,and scale the gap between real world pratice and evidence-based guidelines.Results Elevated cardiac markers,chest X-ray,echocardiography,dynamic electrocardiogram,exercise electrocardiograph (ECG),coronary angiography were used more extensively,but the usage of cardiac stress imaging and electron-beam computed tomography was low.Significant increases in the use of coronary angiography,aspirin,using 300 mg first dosage of aspirin,β-blocker,angiotensin converting enzyme inhibitor (ACEI),thienopyridine,statins and invasive strategies such as percutaneous coronary intervention (PCI) (51.1%),early invasive intervention occurred.The utilization rate of morphine sulfate,nitrates,unfractionated heparin were low.Conclusions There are great progress in the dianosis and management of patients with non-ST elevation acute coronary syndrome.Still,further improvements are needed for optimal implementation of these guidelines such as the use of β-blocker,adding dosage of β-blocker,platelet glycoprotein Ⅱb/Ⅲa inhibitor,low-molecular-weight heparin.Prompt and effective measures should be taken to enhance the education of evidence-based guidelines.
Disease awareness, self-efficacy status and influencing factors of hospitalized coronary heart disease patients

Abstract:Objective To investigate coronary heart disease patients hospitalized for disease awareness and self-efficacy status and influencing factors. Methods From February 2012 to December 2012, 230 patients with coronary artery dis-ease as an object of study in Hangzhou Normal University Affiliated Hospital were collected general information and disease-related information, the use of coronary heart disease knowledge and cognitive scale and patients with coronary heart disease awareness and self-efficacy survey by self-efficacy scale. Results The group of patients with coronary heart disease knowledge and cognitive scale score points was (12.31 ±2.22), score index was (0.61± 0.02), low cognitive level. Of different ages, education levels, economic levels, and on the job and retirement, urban and rural, with or without a family history of patients score differences were statistically significant( P < 0.05), the multi-factor analysis showed that level of education, occupational status, living, the economic level and family history of coronary heart dis-ease risk awareness of the disease were statistically significant( P < 0.05). The group of patients with coronary heart disease self-efficacy score was (84.19 ±6.54), score index was (0.70±0.03), self-efficacy was not high. Different ages,education level, economic level and high salt and high-salt diet, adhere to exercise, urban and rural, multi-factor analysis showed that patients with or without a family history of score differences were statistically significant( P < 0.05), age,level of education, exercise, place of residence and economic level of self-efficacy in patients with coronary heart dis-ease were statistically significant( P < 0.05). Conclusion The level of knowledge of the disease and self-efficacy of coronary heart disease hospitalized patients is low. Developed nursing management measures for the influencing factors will be beneficial to patients with coronary heart disease awareness and self-efficacy.

Abstract:Background and Aims: Current cardiovascular disease (CVD) prevention is based on diagnosis and treatment of specific disease. Little is known for high risk people with CVD at the community level. In rural China, health records of all residents were established after the recent health reforms. This study aims to describe the characters of the rural population with high CVD risk regarding their clinical indicators, disease patterns, drug treatment and adherence.Methods and Results: 17042 (87%) of all the 19500 rural residents in the two townships had valid health records in 2009. We employed a validated tool, the Asian Equation, to screen 8182 (48%) resident health records of those aged between 40-75 years in 2010. Those who were identified with a CVD risk of 20% or higher were selected for a face-to-face questionnaire survey regarding their diagnosed disease and drug treatment. 453 individuals were identified as high risk of CVD, with an average age of 53 years, 62% males, 50% smoking rate and average systolic blood pressure of 161 mmHg. 386 (85%) participated in the survey, while 294 (76%) were diagnosed with and 88 (23%) were suspects of CVD, hypertension, diabetes or hyperlipidaemia. 75 (19%) took drug regularly and 125 (32%) either stopped treatment or missed drugs. The most often used drugs were calcium channel blockers (20%). Only 2% used aspirins and 0.8% used statins. The median costs of drugs were 17 RMB (USD2.66) per month.Conclusion: The majority of the high risk population in our setting of rural China had already been diagnosed with a CVD related disease, but very few took any drugs, and less still took highly effective drugs to prevent CVD. A holistic strategy focused on population with high risk CVD and based on the current China public health reform is suggested in the context of primary care.

Abstract:Background Coronary heart disease (CHD) is the most common type of heart disease and cause of heart attacks. This study investigated the epidemiological characteristics of CHD and its risk factors in Jiaozhou, Shandong province, to ultimately find a way of reducing the prevalence of cardiovascular disease, and to provide a theoretical basis for establishing a cardiovascular disease management path under the regional medical collaborative mechanism. Methods A questionnaire survey was performed including 1 952 people aged 35 years or older who were questioned by means of stratified, cluster, proportional sampling to investigate the prevalence of CHD and its risk factors. The data were inputted into SPSS11.0 statistical software for processing and analysis. We advised the local medical institutions to establish health files for the residents with CHD and risk factors. They were followed up regularly. Their risk factors and life-style were monitored, and advice was given as to proper medications. Green channels were established, and the patients were transmitted in a timely manner to superior hospitals for better treatment if the necessary treatments were not available in the local hospitals. The control of risk factors was observed after the follow-up for half a year. Results In Jiaozhou, the rates of coronary artery disease, hypertension, diabetes, hyperlipidemia and overweight were 8.15%, 28.54%, 11.43%, 35.46%, and 18.70% respectively. The rates of hypertension, diabetes, hyperlipidemia and overweight were higher than the data published in “The report of Chinese cardiovascular disease 2012”; which are 24%, 9.7%, 18.6%, and 9.7%, respectively. The control of risk factors improved significantly after the guidance of the residents lifestyle and medication for six months. Conclusions The high prevalence of coronary artery disease in Jiaozhou is closely related to age, gender, diet structure, family history of cardiovascular disease, hypertension, diabetes, hyperlipidemia, overweight, and unhealthy lifestyle. Under the regional medical coordination mechanism, the collaborative management of cardiovascular disease can provide new management concepts for the areas short of medical resources, so as to reduce the prevalence of cardiovascular disease.

Abstract:Coronary atherosclerosis is a potentially chronic circulatory condition that endangers human health. The biological cause underpinning cardiovascular disease is coronary atherosclerosis, and acute cardiovascular events can develop due to thrombosis, platelet aggregation, and unstable atherosclerotic plaque rupture. Coronary atherosclerosis is progressive, and three specific changes appear, with fat spots and stripes, atherosclerosis and thin-walled fiber atherosclerosis, and then complex changes in arteries. The progression and severity of cardiovascular disease are correlated with various levels of calcium accumulation in the coronary artery. The therapy and diagnosis of coronary atherosclerosis benefit from the initial assessment of the size and degree of calcification. This article will discuss the new progress in the early diagnosis of coronary atherosclerosis in terms of three aspects: imaging, gene and protein markers, and trace elements. This study intends to present the latest methods for diagnosing patients with early atherosclerosis through a literature review.

Abstract:In this article, we introduce briefly several strategies for preventing atherosclerotic cardiovascular disease and promoting healthcare for non-communicable diseases (NCDs). These novel strategies include four core elements of health – sleep, emotion, exercise, and diet – and consist of SEED intervention (SEEDi) and E(e)SEEDi due to supplementation of the environment as a core element, and Hu's healthy lifestyles intervention (HHLi) which originates from E(e)SEED-BasED healthy lifestyles. They are suitable for the early evaluation of risk factors, and play a key role in the prevention and management of human NCDs when combined with the RT-ABCDEF strategy and the Grade 210 prevention, which include obesity-OSA-hypertension syndrome and C-type hypertension, especially in halting cardiovascular, diabetes and cancer (CDC) strips we first discovered. After successful clinical practice, we may expect our novel strategies for controlling these chronic diseases according to the conception of mass prevention and treatment.

Abstract:Cardiovascular diseases are a major cause of death worldwide, and coronary heart disease (CHD) is a prevalent cardiovascular condition and a significant health burden for the population. In this disease, insufficient blood flow to the heart due to plaque accumulation in the coronary arteries causes chest pain, heart attack, and even death. So, it is vital to identify risk factors, prevention, appropriate treatment, and rehabilitation. Nurses play an indispensable role in managing and caring for patients with CHD. Indeed, they possess a deep understanding of the disease and its complexities, enabling them to provide comprehensive care to patients. Nurses monitor vital signs, administer medications, and perform diagnostic tests, ensuring patients receive timely and appropriate interventions. They also educate patients and their families about CHD, emphasizing lifestyle modifications, medication adherence, and self-care practices. Moreover, nurses offer emotional support, guiding patients through the physical and psychological challenges associated with CHD. Their expertise, compassion, and dedication significantly improve patient outcomes and overall quality of life. Nurses are responsible for assessing, diagnosing, and counseling patients on how to manage their disease, making them the front line of defense in preventing and addressing this serious condition. In the current study, we reviewed the literature to consider the best practices and emerging trends in nursing interventions and care strategies for patients with CHD.

Abstract:In spite of major advancements in the management and prevention strategies of atherosclerosis, the prevalence of coronary artery disease has risen worldwide, and might start at an early age. Cardiovascular diseases among young adults are characterized as a heterogeneous group of disorders due to congenital or acquired causes. Multiple risk factors, such as dyslipidemia, premature coronary heart disease, diabetes mellitus and cigarette smoking potentiate the patient’s risk for early coronary heart disease. Early assessment of patients with coronary artery disease can be performed using various non-invasive imaging methods, which facilitate early selection of effective and preventative therapies for coronary artery disease management; however, it is associated with several challenges due to limitations in risk calculators, and limited sensitivity of various screening methods. In this manuscript, we will discuss about pathophysiology, risk factors, invasive and non- invasive imaging methods, and various management strategies for the early prevention of coronary heart disease in young adults and, importantly, the future directions and preventing disease and sudden cardiac death.

Abstract:Coronary heart disease (CHD) is now the leading cause of death and disability globally. Despite recent declines in age-adjusted death rates from CHD, the number of CHD deaths have been increasing due to a combination of growth in population numbers and their longevity. In addition, manifestation and outcome of CHD varies substantially between and within countries. Unlike many other common medical conditions that disable and kill and remain unpreventable, CHD is to a large extent preventable. There are strong, unconfounded relationships between several risk factors and CHD mortality and non-fatal myocardial infarction. The most important risk factors for CHD are smoking, high blood pressure, dyslipidaemia, diabetes, physical inactivity, unhealthy diet, and obesity. Controlling these risk factors, even in middle-aged individuals, through lifestyle changes, medical treatment, or public health interventions, may reduce CHD incidence by almost one-half.

Abstract:In recent years, benefited from the progress of clinical studies of early diagnosis and early revascularization in acute myocardial infarction (AMI), the guidelines for Europe and the United States and the Chinese academic continue to be updated to guide our clinical practice. However, AMI still remains one of the major causes of death in the worldwide. Over the past 10 years, the incidence of AMI increased rapidly in China, and the mortality kept at a high level. There is still plenty of room to improve in the early diagnosis of cardiac troponin, revascularization strategy optimization of non-infarct-related vascular, optimized new antiplatelet therapy, development of regional synergies network and chest pain center. There is still a long way from the standardized treatment of AMI. It is very important to complete early diagnosis and optimum treatment of AMI.

Abstract:Abstract Risk assessment in coronary artery disease plays an essential role in the early identification of high-risk patients. However, conventional invasive imaging procedures all require long intraprocedural times and high costs. The rapid development of coronary computed tomographic angiography (CCTA) and related image processing technology has facilitated the formulation of noninvasive approaches to perform comprehensive evaluations. Evidence has shown that CCTA has outstanding performance in identifying the degree of stenosis, plaque features, and functional reserve. Moreover, advancements in radiomics and machine learning allow more comprehensive interpretations of CCTA images. This paper reviews conventional as well as novel diagnostic and risk assessment tools based on CCTA.

Abstract:Coronary artery disease (CAD) is one of the most common causes of mortality. Besides mortality, it leads to a significant rate of disability, loss of workforce, and financial burden. [ 1 , 2 ] The cornerstone of preventing CAD is to change the lifestyle and environmental factors that lead to the disease (primary prevention) and to identify high-risk individuals (secondary prevention) and take specific measures for these individuals, as well as to protect patients diagnosed early through optimal treatment from relevant complications (tertiary prevention). [ 3 , 4 ] Preventive healthcare is crucial in family medicine, encompassing 5 types of prevention: primordial prevention, primary prevention, secondary prevention, tertiary prevention, and quaternary prevention. [ 5 ] Primordial prevention involves measures and actions to decrease or eliminate risk factors for diseases within the community. Primary prevention includes activities targeting behavioral changes, reducing exposure to disease-causing factors, managing risky situations, and decreasing disease incidence. Secondary prevention focuses on early disease recognition at the asymptomatic stage and controlling disease progression. Tertiary prevention aims to reduce complications arising from the disease through available treatments and rehabilitation. Additionally, it involves safeguarding patients from overmedicalization and ensuring they receive scientifically and ethically acceptable treatment procedures. Quaternary prevention is also emphasized in the preventive healthcare framework, emphasizing the principle of "First, do no harm." [ 5 ]

Abstract:Cardiovascular disease (CVD) remains the leading cause of morbidity and mortality worldwide and challenges the capacity of health care systems globally. Atherosclerosis is the underlying pathophysiological entity in two-thirds of patients with CVD. When considering that atherosclerosis develops over decades, there is potentially great opportunity for prevention of associated events such as myocardial infarction and stroke. Subclinical atherosclerosis has been identified in its early stages in young individuals; however, there is no consensus on how to prevent progression to symptomatic disease. Given the growing burden of CVD, a paradigm shift is required—moving from late management of atherosclerotic CVD to earlier detection during the subclinical phase with the goal of potential cure or prevention of events. Studies must focus on how precision medicine using imaging and circulating biomarkers may identify atherosclerosis earlier and determine whether such a paradigm shift would lead to overall cost savings for global health.
