Abstract:Objective To evaluate the efficacy and safety of standardized flexible ureteroscopic holmium laser lithotripsy in the treatment of intrarenal and proximal ureteral stones. Methods From October 2012 to October 2014, 140 patients underwent standardized flexible ureteroscopic holmium laser lithotripsy in our hospital. All the patients received KUB and IVP or CTU preoperatively.Thirty-eight patients with intrarenal stones underwent double-J stent placement for 10-14 days before operation.An optical-fiber or digital-fiber flexible ureteroscopy with 200 μm holmium laser fiber and standardized flexible ureteroscopic technique were used for treatment.A semirigid ureteroscopy was performed followed by guidewire and sheath placement and then flexible ureteroscopic lithotripsy.The burdens of proximal ureteral stones were fragmented and pushed backward into the renal pelvic during semirigid ureteroscopic inspection, then the intrarenal stones and residual fragments retrieved were managed by flexible ureteroscopy.A double-J stent was placed postoperatively.Traditional Chinese medicine was given for 1-2 months after operation.Stone clearance was estimated by KUB within 2 months of follow-up. Results The operation time was 36 -243 min (mean, 114.7 min), and the estimated blood loss was 2-100 ml (mean, 11.4 ml).Except for 4 cases of medullary sponge kidney cases, the stone clearance rates were 91.7% (77/84) for renal stones and 94.8% (129/136) for overall.For stones with cumulative size more than 2 cm, the average operation time was 128.5 min (range, 63-213 min) and the stone clearance rate was 71.4%(10/14).The success rates of flexible ureteroscopy intervention in patients with or without preoperative double-J stent placement were 100% (38/38) and 82.0%(41/50) respectively.The hospital stay after operation was 1-5 d (mean, 3.6 d).There were no major complications identified. Serious postoperative urinary tract infection occurred in 8 patients.The sepsis rate was 5.7%(8/140), including 9.1%(8/88) for renal stones and 28.6% (4/14) for stones with cumulative size more than 2 cm. Conclusions In selected patients, proximal ureteral and intrarenal stones can be treated safely and efficiently with standardized flexible ureteroscopic homium laser lithotripsy. However, the efficiency and safety are lowered in patients with stone cumulative size more than 2 cm.
