Abstract:BACKGROUND:The number of global hematopoietic stem cell transplantation (HSCT) survivors is increasing rapidly. Survivors encounter many challenges, but studies regarding survivorship experiences in China are scarce.OBJECTIVES:This study aimed to explore the survivorship experiences of Chinese patients with hematological cancers after HSCT and to describe the impact of HSCT on the survivors' lives.METHODS:Descriptive qualitative research was used. Purposive sampling was used to recruit HSCT survivors who were treated in a teaching hospital in Zhejiang Province from June 2021 to June 2022. Audio-recorded semistructured interviews were conducted, transcribed verbatim, and analyzed via conventional content analysis.RESULTS:Fifteen HSCT survivors aged 18 to 59 years participated in this study. Four themes and 11 subthemes emerged: (1) transplant being harder than you thought (body function impaired, forced to modify diet, disturbed by survivorship uncertainty), (2) difficulty blending into circles (limited activity space, suffering from discrimination), (3) adjusting value judgment (health being a top priority, contributing to family as much as possible, feeling worthless), and (4) still being the lucky one (recovered better than others, genuine relationships acquired, self-improvement achieved).CONCLUSION:This study offers insight into subjective survivorship experiences of patients with hematological cancers post-HSCT within a Chinese sociocultural context and presents changed perceptions of HSCT, life alterations, adjusted value judgments, and positive self-evaluation since treatment.IMPLICATIONS FOR PRACTICE:Nurses can provide person-centered survivorship care based on the understanding of survivorship experiences of Chinese HSCT survivors. Intervention programs and informational materials should be developed to address difficulties encountered by Chinese HSCT survivors.

Abstract:Allogeneic hematopoietic stem cell transplantation is an effective method for the treatment of hematological malignancies or non-malignant blood diseases. Hematopoietic stem cell transplantation provides patients with a chance of cure. However, patients need to receive large doses of radiotherapy during transplantation, which makes them less immune to disease after transplantation and even have almost zero white blood cells in the bone marrow vacuity stage. Therefore, local and systemic infections are easy to occur, resulting in increased psychological pressure of patients. Narrative nursing means that clinical nursing staff with narrative ability can absorb, explain and respond to patients' stories and difficulties, and provide them with medical care full of respect, vitality and empathy. Nurses can fully understand and perceive patients' pain experience and disease experience, make appropriate responses, and discover nursing points, so as to implement clinical nursing intervention for patients. Abstract:Objective To explore the psychological experience of patients underwent hematopoietic stem cell transplantation during isolation.Methods The qualitative phenomenological research method was adopted in this study.Self-structured in-depth interviews were conducted with 11 hematopoietic stem cell transplant patients.Data were analyzed by content analysis.Results Four themes were extracted including loneliness,fear,sense of guilt and sense of being abandoned.Conclusion Nurses should pay attention to the psychological experience of hematopoietic stem cells transplant patients during isolation and help them to cope with existing psychological problems,then to promote their rehabilitation and mental health.

Abstract:Hematopoietic stem cell transplantation (HSCT) is a therapeutic option for various potentially life-threatening malignant and non-malignant diseases in children, such as malignancies, immunodeficiency syndromes, severe aplastic anemia, and inherited metabolic disorders. During transplantation, many factors can affect the nutritional status of the children, including radiotherapy, chemotherapy, gastrointestinal disorders, graft-versus-host disease, and medications. Malnutrition has been associated with decreased overall survival and increased complications in children undergoing HSCT, making nutritional support a crucial component of their management. However, currently, there is a lack of guidelines or consensus on nutritional support for children undergoing HSCT in China. Therefore, this review summarizes the progress in nutritional support for children undergoing HSCT, aiming to provide clinical guidance.
Psychological states and needs among post-allogeneic hematopoietic stem cell transplantation survivors

Abstract:Objectives: To analyze psychological states and needs of patients after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Methods: Questionnaires were sent to 101 allo-HSCT survivors and 96 questionnaires were returned. The questionnaire covered several categories: (1) demographics and general information, (2) physical conditions, (3) psychologic status and sleep quality, (4) survivor's comments on transplantation, (5) demands and needs, (6) preferred forms and channels of information. Results: Depression and poor sleep quality were major concerns troubling allo-HSCT survivors. A notable discrepancy exists between clinically diagnosed depression (4.2%) and self- reported depression based on BDI-13 (55.2%). Young adults (18-49 years old), chronic graft-versus-host disease, the ECOG performance score of 2-4, surviving within 5 years since HSCT, no or low dose of ATG used and being single were significantly associated with self-reported depression. Based on the PSQI score, 75% of survivors experienced varying degrees of sleep quality impairment. Young adults, chronic GVHD and the ECOG score of 2-4 were significantly associated with worse sleep quality. The majority of patients reported unmet needs on physical and psychosocial aspects. Nutrition information was the most important topic, followed by disease treatments and fatigue. Differences in informational needs were found in the survivors according to age, time since HSCT and gender. WeChat public account and WeChat applet, mobile interactive platform and one-to-one conversation were the favorite channel for information. Conclusions: Clinicians should establish more appropriate survivorship care plans focusing on survivors' psychologic states, demands and needs.

Abstract:Objective:To systematically evaluate the qualitative researches on the real psychological experience and coping of patients with hematopoietic stem cell transplantation during their stay in the laminar flow room, and provide reference for nursing staff to provide supportive care.Methods:The Cochrane Library, PubMed, Web of Science, Scopus, CNKI, China Biology Medicine disc, Wanfang and other Chinese and English databases were searched for qualitative research on the psychological experience of patients undergoing hematopoietic stem cell transplantation during their stay in the laminar flow room. The quality of the included studies was evaluated using the Australian JBI Evidence-based Health Care Center Qualitative Research Quality Evaluation Standard (2016) , and the research results were integrated using the pooled integration method.Results:A total of 15 literatures were included, and 99 results were extracted. They were summarized into 9 new categories and summarized into 3 integrated results, which were the imbalance of the external environment after admission of patients with hematopoietic stem cell transplantation to the laminar flow room, the real experience of hematopoietic stem cell transplant patients after being admitted to the laminar flow room, and the coping strategies for psychological changes after hematopoietic stem cell transplantation patients are admitted to the laminar flow room.Conclusions:The imbalance of the external environment is the main reason for the negative psychological experience of hematopoietic stem cell transplant patients after they are admitted to the laminar flow room. Nursing staff need to accurately identify complex emotions of the patients and provide professional guidance and support to help patients reduce negative emotions.
Mothers' responses during the child's stem cell transplantation: pilot study

Abstract:Purpose: Examine the relationships between the mothers' anxiety and depressive symptomatology and resources (problem-solving orientation and style, coping, and social support) during their child's stem cell transplantation (SCT). Method: A prospective correlation design was used. A convenience sample of 23 mothers completed State-Anxiety Inventory, Center of Epidemiologic Studies Depression Scale, Social Problem-Solving Inventory-Revised, and Stress Support Scale at the child's admission and 10 days after the stem cells infusion. Findings: Significant relationships were found between mothers' negative problem-solving orientation and emotional responses, coping and depressive symptomatology, and between social support and emotional responses but in an inverse direction. Conclusion: Health care professionals need to increase their understanding of mothers' stress responses during the SCT. These responses may affect the mother/child interactions and the mother's ability to provide childcare. A psychosocial intervention that includes these resources may assist the mothers during their child's SCT.

Abstract:This study aimed to summarize the experience of precise nursing in 6 patients who experienced failed allogeneic hematopoietic stem cell transplantations (allo-HSCTs) that underwent second allo-HSCT salvage treatment. The key points of nursing care included strictly implementing infection prevention and control measures to prevent secondary infections, precise symptom management to improve the graft survival rate of patients, formulating reasonable nutrition programs to meet their requirements, and paying attention to the psychological care of patients to enhance their self-confidence in overcoming diseases. The patients developed different degrees of complications in the process of transplantation. During the transplantation, 2 patients had oral mucositis, 2 had hemorrhagic cystitis, 3 had a perianal infection, and one had lower gastrointestinal bleeding. After careful treatment and nursing, the neutrophils transplanted in the 6 patients were alive at a median of 16.5 (13-20) days after the second allo-HSCT, and the patients were successfully transferred out of the laminar flow chamber.
The experience of caregivers of hematological cancer patients undergoing a hematopoietic stem cell transplant: a comprehensive literature review

Abstract:Objective: Hematopoietic stem cell transplant (HSCT) is a demanding procedure with associated physical and psychological sequelae that affects patients and their families. Caregivers to HSCT patients not only have to cope with the life-threatening nature of the disease and treatment, but they also have care-giving responsibilities. This study reviews the literature on the psychosocial impact of being a caregiver to a HSCT patient. Methods: A critical review of the literature published before June 2010 was conducted. Databases searched included CINAHL, Medline, PsycInfo, and Academic Search Complete, as well as a comprehensive reference review. Studies that pertained to caregivers of children (under the age of 18) undergoing a HSCT or caregivers to patients with non-hematological malignancies were excluded. Sixteen quantitative research articles and three qualitative research articles were reviewed and analyzed. Results: Caregiver distress is highest pre-transplant and decreases over time, and caregivers display distress levels comparable to or higher than patients' reported distress levels. Predictors of caregiver distress include female gender, elevated subjective burden, and higher patient symptom distress. Caregivers reported uncertainty, fear of the future, juggling patients' needs with their own, and difficulties adapting to role changes; however, they also reported positive aspects to care giving, such as personal growth and developing a more positive relationship with the patient. Conclusions: There are many limitations to the current research and future directions should include both members of the dyad to evaluate the reciprocal relation between caregiver and patient variables, as well as theory-driven research and research with direct clinical applications.

Abstract:Objective To summarize the perioperative mental problems in patients with cervical esophageal cancer and to provide the effective nursing intervention.Methods Postoperative emotional reactions and mental problems of 17 patients with cervical esophageal cancer in department of thoracic surgery were collected and analyzed.Common problems and reasons were summarized,and targeted nursing intervention methods and measures were proposed.Results Patients and their family members were able to frankly face the problems such as postoperative aphasia,dysphagia and change of respiratory tract,and they were fulfilled with confidence against diseases.Conclusion Effective nursing intervention can enhance the confidence and courage of the patients and enable them to return to society successfully.

Abstract:In order to explore the clinical effect of psychological nursing intervention on postoperative chemotherapy for rectal cancer, 120 cases of rectal cancer patients were selected as the research subjects. The control group received conventional nursing treatment after operation, and the research group received comprehensive psychological nursing intervention on this basis. The self-rating anxiety scale (SAS) scores, self-rating depression scale (SDS) scores, hope level scores, nursing satisfaction, mental state changes, treatment compliance, and immune function of two groups were analyzed and compared. There was no significant difference between the two groups of patients in the preoperative SAS, SDS, and hope level scale scores. After the intervention, postoperative SAS and SDS scores and CD8+ value of the research group were significantly lower than those of the control group. In contrast, the postoperative hope level score, treatment compliance, and postoperative CD4+/CD8+ of the research group were significantly higher, and the nursing satisfaction was better than that of the control group. The application of psychological nursing intervention in postoperative chemotherapy for patients with rectal cancer can effectively relieve anxiety and depression of patients, promote patients to establish a healthy and coordinated mental state, improve treatment compliance, improve immune function, and promote disease recovery.

Abstract:Background: We aimed to evaluate the effect of a psychological nursing intervention on inflammatory factors, negative emotions and quality of life in patients with breast cancer. Methods: Patients at the Cancer Hospital of China Medical University from January 2017 to January 2020 were stratified into two groups based on the way of nursing. Patients in the control group received routine care, and the experimental group received appropriate psychological care interventions in addition to routine care. We compared the inflammatory factors (interleukin-6 (IL-6) and C reactive protein (CRP)), negative emotions (self-rating depression scale (SDS) and self-rating anxiety scale (SAS)) and quality of Life (short form-36 survey (SF-36)) between two groups. Results: A total of 226 patients with breast cancer were enrolled in this study. Before the intervention, the CRP, IL-6, SAS and SDS between two groups were not significantly different (t=0.205, 0.971, 0.590, 1.579; P=0.838, 0.332, 0.556, 0.116). After the intervention, the CRP, IL-6, SAS and SDS between two groups were decreased and significantly different (t=-17.648, -18.079,-18.267,-18.613; P<0.05) Besides, the social function , physiological function, mental health, physical pain, physical limitation, vitality, emotional functions and overall health were improved and significantly different between two groups after intervention (P<0.05). Conclusion: A psychological nursing Intervention has the beneficial effect on the inflammatory factors, negative emotions and quality of life in patients with breast cancer.

Abstract:Objective: Multiple psychosocial factors appear to affect cancer progression in various populations; however, research investigating the relationship between psychosocial factors and outcomes following hematopoietic stem cell transplantation (HCT) is scarce. Subject to adverse immunological and psychological conditions, HCT patients may be especially vulnerable to psychosomatic health sequelae; therefore, we studied whether optimism and anxiety influence the pertinent clinical outcome of days to neutrophil engraftment (DTE). Method: 54 adults undergoing either autologous or allogeneic HCT completed self-report questionnaires measuring optimism and anxiety. We assessed the association between these psychosocial variables and DTE. Results: Greater optimism and less anxiety were associated with the favorable outcome of fewer DTE in autologous HCT recipients, though this relationship was no longer significant when reducing the sample size to only subjects who filled out their baseline survey by the time of engraftment. Conclusion: Our findings are suggestive that optimism and anxiety may be associated with time to neutrophil recovery in autologous, but not allogeneic, adult HCT recipients. Further investigation in larger, more homogeneous subjects with consistent baseline sampling is warranted.

Abstract:Objective: We explored whether a patient's psychosocial background before allogeneic hematopoietic stem cell transplantation (allo-HSCT) could predict the occurrence of psychiatric symptoms during treatment and after hospital discharge. Method: Logistic regression analysis was performed using INTERMED, a scale that comprehensively evaluates psychological factors such as psychiatric history, current mental status, and coping skills, and social factors such as social participation status, relationships with others, and living environment, which were used as independent variables. The Center for Epidemiologic Studies Depression Scale was used to measure depression, while the Profile of Mood States was used to measure anxiety and other symptoms. Both measures were used as dependent variables and were administered upon clean room admission, during clean room stay, at clean room discharge, and at 3, 6, and 12 months after hospital discharge. Results: Participants included 70 patients (45 males and 25 females, mean age 53.3 ± 12.3 years). Thirty-eight patients participated in the program for the entire period, up to 12 months after hospital discharge. The total score on the Japanese version of the INTERMED and psychological factor scores assessed at baseline were significant predictors of depressed mood on discharge; however, there were no significant predictors of scores on the Profile of Mood States. Conclusions: A comprehensive pretransplant evaluation of psychosocial background can help predict the appearance of psychiatric symptoms after allo-HSCT. In patients who are expected to develop psychiatric symptoms after allo-HSCT, it is important to consider early intervention by a specialist and close monitoring by a medical team.

Abstract:Breast cancer (Breast Cancer, BC) is one of the most common malignant tumors in women in the world. It is the first malignant tumor that causes the death of women in developing countries. It seriously threatens the lives and health of women and causes damage to the family, economy and society. Through psychological nursing intervention, it has a positive effect on the perioperative mood and NK cell activity of breast cancer patients. The purpose of this article is to explore the effects of psychological intervention on the perioperative mood and NK cell activity of breast cancer patients. This article is based on the concept and theory of psychological nursing intervention for female breast cancer patients, and establishes personalized intervention measures in conjunction with clinical practice to intervene the emotions of breast cancer patients during the perioperative period. This article analyzes the nature, intensity, and causes of negative psychology of the subject through in - depth understanding of the subject's psychological emotions, coping ability, personality characteristics, past emotional experience, and social support, and discovers the different emotional characteristics of the patient, and formulates the personality psychological intervention measures to stimulate positive and optimistic attitudes and ease the degree of negative psychology. This article analyzes the psychological characteristics of several breast cancer patients. Take corresponding psychological intervention measures. Cope with a series of bad psychology caused by image damage caused by total mastectomy. The results and data in this article show that the incidence of anxiety and depression in the control group is higher than that of the psychological intervention group at 7 days after the operation, with P values of 0.0059 and 0. 0215. Psychological intervention reduces the incidence of negative emotions and reduces the negativeness of patients. The intensity of emotion has played a good clinical effect.

Abstract:Sexual health is important for the quality of life of patients who have received haematopoietic stem-cell transplantation (HSCT). Sexual dysfunction and couple dissatisfaction can seriously affect a patient's recovery and treatment process. However, this aspect of post-transplantation recovery is still usually neglected in clinical practice. In this Series paper, we aim to elucidate the emotional and psychosocial factors affecting the sexual function in these patients, with a special focus on the partner's role and the psychological consequences of some adverse effects of HSCT. Moreover, we provide an overview of the management approaches and assessment tools of psychological issues associated with sexual dysfunction reported in the literature. These tools can help clinicians in this field to plan essential lifestyle and clinical interventions to help their patients. In conclusion, screening for psychological issues is indispensable when approaching sexual dysfunction in patients with HSCT. Health-care teams in transplantation units should be trained to discuss this aspect of recovery and provide the required treatment and follow-up plan.

Methods: Questionnaires were completed by 260 parents (146 mothers and 114 fathers) 11-198 months after HSCT: the Hospital Anxiety and Depression Scale, the Shirom-Melamed Burnout Questionnaire, the post-traumatic stress disorders checklist, civilian version, and the PTG inventory. Additional variables were also investigated: perceived support, time elapsed since HSCT, job stress, partner-relationship satisfaction, trauma appraisal, and the child's health problems. A hierarchical cluster analysis and a k-means cluster analysis were used to identify patterns of psychological responses. Results: Four clusters of parents with different psychological responses were identified. One cluster (n = 40) significantly differed from the other groups and reported levels of depression, anxiety, burnout symptoms, and post-traumatic stress symptoms above the cut-off. In contrast, another cluster (n = 66) reported higher levels of PTG than the other groups did. Conclusions: This study shows a subgroup of parents maintaining high levels of several aspects of distress years after HSCT. Differences between clusters might be explained by differences in perceived support, the child's health problems, job stress, and partner-relationship satisfaction.
