Immediate post-urokinase percutaneous coronary intervention within 3 hours after myocardial infarction with ST-segment elevated:influence on the prognosis

Abstract:Objective To evaluate the safety and effectiveness of immediate post-fibrinolysis percutaneous coronary intervention (PCI) for patients with ST-segment elevation myocardial infarction (STEMI) within 3 h of onset, and the results were compared with those of directly PCI strategy.Methods One hundred and thirty-five consecutive STEMI patients (immediate post-urokinase PCI in 67 patients, primary PCI in 68 patients) were included in the present study and followed up for 90 days. The pre-operation stenosis degree of infarct-related artery (IRA) and thrombolysis in myocardial infarction trial (TIMI) flow, bleeding and the end-point incidences during the 90-day-follow-up, including death, reinfarction, revascularization, stroke,or acute heart failure, were all recorded. Results There were similar bleeding incidence (7.5% vs. 7.4%, P>0.05) between the two groups. Immediate post-urokinase PCI resulted in higher frequency (50.7% vs. 19.1%, P<0.01) of pre-interventional epicardial reperfusion (TIMI 2 or 3 epicardial flow) and seemed to have a lower 90-day composite incidence (6.0% vs. 13.2%, P=0.06). Conclusion Immediate post-urokinase PCI is safe and superior to primary PCI in improving the prognosis of STEMI within 3 hours of onset.
