Abstract:Insomnia, classified under the category of “Bu Mian” in traditional Chinese medicine (TCM), is a chronic condition characterized by the inability to obtain normal sleep. It is closely related to liver function. TCM treatment for insomnia follows the holistic principle of “integrating man with nature” and applies syndrome differentiation to implement individualized treatments. The safety of herbal medicine, combined with the absence of drug dependence, makes it highly favored by patients compared to Western medications. Professor Li Fenping believes that insomnia results from dysfunction in the liver’s regulation of Qi and blood, which prevents the Hun (ethereal soul) from returning, thus disrupting sleep. Based on clinical practice, she has identified four major liver-related patterns in insomnia: liver Qi stagnation with unsettled Hun, deficiency of heart and liver blood (or Yin) leading to poor nourishment of the Shen (spirit), liver fire excess disturbing the Hun, and blood stasis obstructing the meridians and unsettling the Shen. These distinct approaches to liver-focused treatment of insomnia have expanded the clinical applications of TCM in treating the disorder, with remarkable therapeutic outcomes. This paper presents Professor Li Fenping’s clinical experience in treating insomnia from the perspective of liver pathology.

Abstract:Prof.Xu Feng has unique understanding in the treatment of insomnia and has accumula ted rich clinical experiences.Prof.Xu believes that insomnia is divided into two categories,the deficiency and the excess.The deficiency pattern may be transformed into the excess one,or the mixture of both happens.The treatment of insomnia should focus on regulating and consolidating the spleen and the stomach functions.The four diagnostic methods and differentiation should be considered accordingly in the cases of blood stagnation,heat accumulation and damp involved.The modified herbal formulas are used to removing stasis,clearing heat and reducing damp so as to eliminate the pathogens and strengthen the spleen and stomach.As a result,the qi and blood circulation is regulated and the function of spleen and stomach in transformation and transportation is benefited.Additionally,the advice on the proper diet and regulate food intake should be given to cultivate the function of spleen for strengthening the functions of zangfu organs,supplementing qi and blood and harmonizing yin and yang.Consequently,insomnia is prevented.

Abstract:As a common clinical disease, insomnia was usually treated with anti-psychotic or sedative-hypnotic drugs in Western medicine, which showed a fast efficacy, as well as easy drug tolerance and side effects. In traditional Chinese medicine, the basic and key pathogenesis of insomnia are believed to be “the restlessness of the mind”, as well as “the absence of the mind”; and the treatment mainly focus on “regulating mental activities and smoothing the liver”. By combining the theoretical basis and characteristics of Fang’s scalp acupuncture, this paper aims to explain the advantages of Fang’s scalp acupuncture in the treatment of insomnia, and provide new ideas and methods for its clinical treatment.

Abstract:Abstract It has been well known that insomnia and sleep deprivation can lead to many metabolic diseases such as type 2 diabete mellitus (T2DM). Chinese medicine (CM) has been used to improve insomnia and showed many good therapeutic effects. Some Coptidis Rhizoma-contained traditional formulae have been found to exert obvious effects on insomnia, such as Jiaotai Pill (交泰丸), Huanglian Ejiao Decoction (黄连阿胶汤), Huanglian Wendan Decoction (黄连温胆汤), Niuhuang Qingxin Pill (牛黄清心丸), and Zhusha Anshen Pill (朱砂安神丸). Coptidis Rhizoma, a traditional herbal medicine which can purge Xin (Heart) fire, is the most important component of these formulas. Studies have found that these formulae could improve T2DM symptoms. We hypothesized that treating insomnia theoretically may help to prevent diabetes and organized many experimental studies in this review and concluded that Coptidis Rhizoma-contained traditional formulae for insomnia might be a potential to prevent diabetes.

Abstract:With high incidence,insomnia plays a serious impact on people's quality of life and health.The traditional Chinese medicine(TCM) has fairly good effect and less side effects for the treatment of insomnia.This article was going to summarize the action mechanism of TCM treatment to insomnia from the perspectives of adjusting neurotransmitters,influencing factors related to sleep and improving the central nervous structure.TCM has the functions in treating insomnia mainly through three aspects of mechanism:one is to adjust inhibitory neurotransmitters such as 5-HT,GABA,PG,and excitability neurotransmitters such as Glu,DA,NA;Second is to influence cell factors like IL-1,IL-6,TNF-α and CCK-8,OrexinA and other factors related to sleep;Third is to improve central nervous ultrastructure of nuclear such as Brain cortex,Dorsal Raphe Nucleus,etc.Mechanism research of TCM treatment to insomnia has achieved significant progress,however,it still needs further study to know how TCM could take the advantages of multi-level and multi-target and coordinate the interaction of so many sleep factors in treating insomnia.

Abstract:Insomnia has become an urgent clinical problem in modern society. Current research has found that the gut flora-gut-brain axis plays an important role in regulating insomnia. The state-target theory is a product of the combination of traditional Chinese medicine and modern molecular biology technology. This paper clarifies the correlation between the state-target theory, the intestinal flora-gut-brain axis, and liver-depression-spleen-deficiency insomnia. The use of traditional Chinese medicine to regulate the structure and abundance of intestinal flora was also explored, aiming to integrate traditional Chinese medicine with Western medicine for the prevention and treatment of liver-depression-spleen-deficiency insomnia.
[Comparison of Therapeutic Effects Between Thermosensitive Moxibustion and Medication in the Treatment of Insomnia of Liver- Qi Stagnation Pattern]

Abstract:Objective: To compare the therapeutic effect of thermosensitive moxibustion (TSM) and medication in the treatment of insomnia patients with the syndrome of stagnation of liver-qi. Methods: A total of 60 insomnia patients with the syndrome of liver-qi stagnation were randomly divided into TSM group and medication group (n ＝ 30 in each). Mild moxibustion was applied to bilateral Taichong (LR 3) and Qiuxu (GB 40) alternatively till the patient experienced an intense warmth (diathermic, heat transmission, etc.) feeling in the local region. The treatment was conducted once daily for 15 d. Patients of the medication group were ordered to take Estazolam tablets (1 mg/d) 1 h before sleeping, once daily for successive 15 days. The sleep quality (sleeping quality, falling asleep time, sleep duration, sleep efficiency, sleep disorders, hyponotic and day time dysfunction, 0－21 points) was evaluated by using Pittsburgh Sleep Quality Index (PSQI). The therapeutic effect was assessed in accordance with the "Guide Principles for Clinical Trials on New Drugs of Chinese Materia Medica (1993)" and international united sleep efficiency value. Results: Following the treatment, the PSQI of both groups were significantly decreased relevant to their own pre-treatment (P<0.05), but without significant difference between the two groups (P>0.05). Of the two 30 cases in the medication and moxibustion groups, 12 (40.00%) and 20 (66.67%) had marked improvement, 7 (23.33%) and 6 (20.00%) were effective, and 11(36.67%) and 4 (13.33%) were invalid, with the effective rates being 63.33 % (19/30) and 86.67% (26/30), respectively. The effective rate of the TSM group was significantly higher than that of the medicine group (P <0.05). Conclusion: Thermosensitive moxibustion applied to the source points of the Liver and Gallbladder Meridians can improve the quality of sleep in insomnia patients with syndrome of liver-qi stagnation.

Abstract:To explore the mechanism of insomnia caused by "stomach disorder could lead to excess of yang-qiao meridian" and clinical application of treating insomnia with acupoints in qiao meridian as the main points. From meridian theory, intersection between stomach meridian of Foot-Yangming and yang-qiao meridian is through Chengqi (ST 1). Qiao meridian for sleep is mainly because it is connected with eyes through the Bladder Meridian of Foot-Taiyang. For Stomach Meridian of Foot-Yangming is intersected with the Bladder Meridian of Foot-Taiyang in Jingming (BL 1), and intersected with yin and yang qiao meridian beside the mouth and under the eye, once functional disorder of the stomach, it can affect qi movements of the whole body and give rise to various pathological changes that cause insomnia. Meanwhile examples are given to explain the clinical application of treating subborn insomnia with corresponding acupoint of stomach and yang-qiao meridian.

Abstract:Insomnia is one of the sleep disorders that seriously affect the quality of life of people all over the world. Several studies have shown that acupuncture and moxibustion have the advantages of significant curative effect and high total effective rate in the treatment of insomnia. Traditional Chinese medicine believes that insomnia is in the heart, closely related to liver, spleen and kidney, and its pathogenesis is Yang flourishing Yin failing, Yin and Yang disconnecting. Western medicine believes that insomnia is closely related to changes in neurotransmitter content, endocrine dysfunction, immune inflammatory response, and activation of antioxidant defense system. Based on the results of clinical trials and animal experiments at home and abroad in the past 10 years, this study discusses the latest research status of acupuncture treatment mechanism of insomnia and summarizes, providing help and reference for the mechanism research of acupuncture treatment of insomnia, and providing new ideas for clinical acupuncture treatment of insomnia.

Abstract:Insomnia belongs to the category of "sleepless" in Chinese medicine, the main clinical manifestations are difficult to sleep, or sleep and easy to wake up, or can not sleep after waking up, and sometimes wake up when sleeping, severe sleepless all night. Later generations of doctors will spleen and stomach disharmony, phlegm and damp block, diet stagnation, causing sleepless nights are attributed to this. As early as in Suwen, it was mentioned that "stomach disharmony leads to sleeping restlessness". With the in-depth study of brain and intestinal axis and intestinal flora, the relationship between spleen and stomach and insomnia has been further understood, and it is concluded that one of the important pathogenesis of insomnia is spleen and stomach disharmony, which can cause deficiency of Qi and blood biochemistry, disharmony of Ying and wei, and loss of balance of Yin and Yang. Traditional Chinese herbal formulas modulate gut microbiome and improve insomnia in patients with distinct syndrome types: insights from an interventional clinical study

Abstract:Background: Traditional Chinese medicine (TCM) comprising herbal formulas has been used for millennia to treat various diseases, such as insomnia, based on distinct syndrome types. Although TCM has been proposed to be effective in insomnia through gut microbiota modulation in animal models, human studies remain limited. Therefore, this study employs machine learning and integrative network techniques to elucidate the role of the gut microbiome in the efficacies of two TCM formulas - center-supplementing and qi-boosting decoction (CSQBD) and spleen-tonifying and yin heat-clearing decoction (STYHCD) - in treating insomnia patients diagnosed with spleen qi deficiency and spleen qi deficiency with stomach heat. Methods: Sixty-three insomnia patients with these two specific TCM syndromes were enrolled and treated with CSQBD or STYHCD for 4 weeks. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) and Insomnia Severity Index (ISI) every 2 weeks. In addition, variations in gut microbiota were evaluated through 16S rRNA gene sequencing. Stress and inflammatory markers were measured pre- and post-treatment. Results: At baseline, patients exhibiting only spleen qi deficiency showed slightly lesser severe insomnia, lower IFN-α levels, and higher cortisol levels than those with spleen qi deficiency with stomach heat. Both TCM syndromes displayed distinct gut microbiome profiles despite baseline adjustment of PSQI, ISI, and IFN-α scores. The nested stratified 10-fold cross-validated random forest classifier showed that patients with spleen qi deficiency had a higher abundance of Bifidobacterium longum than those with spleen qi deficiency with stomach heat, negatively associated with plasma IFN-α concentration. Both CSQBD and STYHCD treatments significantly improved sleep quality within 2 weeks, which lasted throughout the study. Moreover, the gut microbiome and inflammatory markers were significantly altered post-treatment. The longitudinal integrative network analysis revealed interconnections between sleep quality, gut microbes, such as Phascolarctobacterium and Ruminococcaceae, and inflammatory markers. Conclusion: This study reveals distinct microbiome profiles associated with different TCM syndrome types and underscores the link between the gut microbiome and efficacies of Chinese herbal formulas in improving insomnia. These findings deepen our understanding of the gut-brain axis in relation to insomnia and pave the way for precision treatment approaches leveraging TCM herbal remedies.
Clinical observation on acupuncture treatment of intractable insomnia

Abstract:Objective: To observe the effects of acupuncture for treatment of intractable insomnia. Methods: Totally 90 patients were randomly divided into a treatment group (50 cases), treated by the body acupuncture plus auricular-plaster therapy, and a control group (40 cases), the Baihui (GV 20)-through-Sishencong (EX-HN1). puncture, auricular-plaster therapy and moxibustion on the back-shu points were not used in the control group. The clinical effects were observed. Results: The total effective rate was 98.0% in the treatment group and 77.5% in the control group. The therapeutic effect in the treatment group was much better than that in the control group (P < 0.01). Conclusion: The body acupuncture plus auricular-plaster therapy may show better effect for intractable insomnia.

Abstract:Insomnia is a very common disease worldwide. It seriously affects the quality of human life and even endangers health. Traditional Chinese medicine (TCM) has unique advantages in the intervention and treatment of insomnia. However, its underlying mechanism has yet to be elucidated. This study was performed to explore the potential biomarkers and mechanisms of insomnia, and treatment TCM and classical prescriptions. The gene microarray data of insomnia is downloaded and preprocessed. Differentially expressed genes (DEGs) and GO and KEGG enrichment analyses were performed. The protein-protein interaction network (PPI) was constructed. Small molecule drugs for curing insomnia were identified using cMap and CTD databases. We searched the TCM corresponding to small molecule drugs and the classic prescriptions corresponding to TCM by the TCMSP database. We constructed a network of “ingredient-TCM-classic prescriptions”. The molecular docking was performed to validate the screening results. We obtained a total of 124 DEGs, including 78 up-regulated genes, 46 down-regulated genes, 10 Hub genes and 3 key modules. A total of 125 significant GO entries and 15 significant KEGG were enriched (P < 0.05). The main biological processes involve neuronal apoptosis, autophagy, cell growth and apoptosis, etc. These signaling pathways may be involved in molecular regulatory mechanisms of insomnia, such as autophagy regulation, Alzheimer's disease, pathways to neurodegenerative diseases and neurotrophic factor signaling pathways. We identified 10 traditional Chinese medicines and 2 classical prescriptions of potential value. In addition, the molecular docking results indicated that small molecule ligands were nicely bound to the Hub gene, and the binding affinity ranged from −7.6 to −9.7 kcal/mol. This study provides a foundation for the clinical treatment of insomnia, explains the molecular mechanisms, and efficiently develops TCM and classical prescriptions.

Abstract:Insomnia is a common disease and its impact on human health cannot be ignored. At present, there are 3 main clinical treatments for insomnia, including traditional Chinese medicine treatment, sedative hypnotic drug therapy, and cognitive behavioral therapy. Traditional Chinese medicine (TCM) treatment for insomnia is widely used due to its advantages of low side effects, good efficacy, and no drug dependence. This paper summarizes the pathogenesis of insomnia in the theories of traditional Chinese and Western medicine. Modern medical research generally believes that sleep-wake disorder is the main pathological mechanism of insomnia, involving many factors such as monoamine neurotransmitter disorder, cytokine imbalance and intestinal flora imbalance. TCM mainly divides the pathogenesis of insomnia into 9 kinds of syndrome types: Liver depression transforming into fire, hyperactivity of fire due to yin deficiency, phlegm-heat attacking internally, disharmony between heart and kidney, deficiency of both heart and spleen, qi deficiency of both heart and gallbaldder, stomach qi disharmony, exuberance of heart fire, and internal blockade of static blood. According to these 9 kinds of pathogenesis of insomnia, the corresponding classical prescriptions such as Longdanxiegan decoction, Suanzaoren decoction, Huanglian-Wendan decoction, Jiaotai pill and Guipi decoction were analyzed and summarized. There is evidence that traditional Chinese medicine could treat insomnia mainly by increasing the level of 5-hydroxytryptamine, reducing the levels of dopamine, noradrenaline, tumor necrosis factor α, and interlukin-6, decreasing the ratio of glutamic acid to γ-aminobutyric acid, and inhibiting the hypothalamic-pituitary-adrenal axis.

[Potential targets of Euodiae Fructus in treatment of insomnia based on network pharmacology]

Abstract:The acupoint application of Euodiae Fructus at Yongquan(KI1) can significantly improve the sleep quality of patients with insomnia with berberine as the main effective component for the efficacy. Nineteen active compounds and 203 drug targets were screened out from Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP). After comparison with GeneCards and Online Mendelian Inheritance in Man(OMIM), 24 common genes of diseases and drugs were obtained. STRING 11.0 was used to construct a protein-protein interaction(PPI) network of the overlapping genes, and Matthews correlation coefficient(MCC) was employed to screen the core genes, which were then subjected to enrichment analysis with gene ontology(GO) and Kyoto encyclopedia of genes and genomes(KEGG). The results revealed that the main compounds of Euodiae Fructus, such as berberine and rutaecarpine, participated in the biological processes(such as neurotransmitter receptor activity) by regulating C-reactive protein(CRP), estrogen receptor 1(ESR1), 5-hydroxytryptamine(5-HT) receptor, and interleukin-6(IL-6) to exert sedative, anxiolytic, and antidepressant effects. Sixty 4-week-old SPF mice were randomly divided into a control group, a model group, a positive drug(diazepam tablets) group, and low-, medium-, and high-dose berberine groups. Medication with corresponding drugs was performed for one week. The results demonstrated that berberine was potent in reducing the activities and standing times of mice, down-regulating the levels of CRP and IL-6 mRNA in the hypothalamus, and up-regulating the expression of 5-HT(P<0.01); however, no significant effect on ESR1 was observed. The network of Euodiae Fructus in treating insomnia was constructed by network pharmacology and verified by tests. The findings indicated that the therapeutic efficacy of Euodiae Fructus in treating insomnia was achieved by participating in multiple biological processes, such as neurotransmitter receptor activity, which provided a scientific basis for its clinical application.
[Clinical efficacy of shallow-needle therapy combined with estazolam on insomnia of liver stagnation transforming into fire pattern: a randomized controlled trial]

Abstract:Objective: To observe the clinical efficacy of shallow-needle therapy combined with estazolam on insomnia differentiated as liver stagnation transforming into fire and its effect on adrenocorticotropic hormone (ACTH) and cortisol (CORT), so as to explore the mechanism of this combined treatment. Methods: A total of 119 patients with insomnia of liver stagnation transforming into fire pattern were randomly divided into shallow-needle therapy group (n=40), medication group (n=39), and shallow-needle therapy combined with medication group (combined therapy group,n=40). In the shallow-needle therapy group, the patients were treated with finger pressure and operation with shallow stimulating at Zhenjing (Dong's extra point, sedative point) and Taichong (LR3). In the medication group, the patients were administered with estazolam (1 mg) orally. In the combined therapy group, both shallow-needle therapy and medication were administered. The treatment was given once daily in each group, 10 days as one session of treatment and 2 sessions were required. Before and after the treatment, Pittsburgh sleep quality index scale (PSQI) and Self-rating anxiety scale (SAS) were used to assess sleep and anxiety status. ELISA was used to detect the contents of ACTH and CORT in plasma. The clinical efficacy was evaluated in each group. Results: In within-group comparison, PSQI scores, SAS scores and the concentrations of ACTH and CORT in plasma were all decreased (P<0.05) after treatment for the patients of three groups. After treatment, the total PSQI score, the score for sleep latency, sleep duration and daytime dysfunction, as well as SAS score in the combined therapy group were all lower than those of the shallow-needle therapy group (P<0.05); the total PSQI score, the score for sleep duration and sleep efficiency, as well as SAS score were lower when compared with the medication group (P<0.05). The total effective rates were 87.50% (35/40), 82.05% (32/39) and 95.00% (38/40) in the shallow-needle therapy group, the medication group and the combined therapy group, respectively. The total effective rate in the combined therapy group was higher than those of the shallow-needle therapy group and the medication group separately (P<0.05). Conclusion: Shallow-needle therapy combined with estazolam is effective on insomnia of liver stagnation transforming into fire pattern, and its underlying effect mechanism is related to the reduction of plasma ACTH and CORT levels.

Abstract:Objective: To explore and analyze the clinical efficacy of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia. Method: 80 patients with primary insomnia were selected as the study sample, with 20 cases randomly selected as the control group and the rest as the observation group. The control group was treated with ordinary western medicine, while the observation group patients were treated with traditional Chinese medicine combined with acupuncture and moxibustion based on this syndrome differentiation. The comparison items were treatment effect, sleep quality, mental health, quality of life, specific sleep indicators, cerebral blood flow and neurotransmitters. Result: The treatment effectiveness showed that the observation group had a higher treatment effectiveness rate than the control group (P<0.05); The results of sleep quality showed that after treatment, the scores of the observation group were lower than those of the control group in seven dimensions: sleep quality, sleep time, sleep onset time, sleep efficiency, sleep disorders, hypnotic drugs, and daytime dysfunction score (P<0.05); The quality of life results showed that after treatment, the observation group had higher scores than the control group in eight dimensions: physiological function, physiological function, physical pain, health status, energy, social function, emotional function, and mental health (P<0.05); The results of slow wave sleep time and fast eye movement sleep time showed that after treatment, the HAMA and HAMD scores of the physiological mechanism in the observation group were lower than those in the control group (P<0.05); The results of cerebral blood flow and neurotransmitter indicators show that after treatment, glutamate γ- the peak systolic blood flow velocity of aminobutyric acid, vertebral artery, and basilar artery in the observation group was higher than that in the control group (P<0.05). Conclusion: The clinical treatment of Chinese medicine combined with acupuncture and moxibustion in the treatment of insomnia patients can significantly improve the clinical treatment effect, and improve the cerebral hemorrhage and neurotransmitter status of patients, promote the improvement of their physical and mental status, can more significantly improve the quality of sleep, reduce sleep difficulties, reduce the dosage of sleeping pills, which has important clinical application value.
