Abstract:OBJECTIVE:To summarize and analyze the epidemic situation of human rabies from 1984 to 2002 in China, and to explore the possible factors causing the increase of cases so as to provide evidence for preventive and control measures.METHOD:National and some provincial data on the prevalence of rabies during 1984 to 2002 were collected and analyzed.RESULTS:From 1984 to 1989, the annual reported cases were between 4 000 and 6 000 but decreased after 1990. In 1996, the reported cases decreased to the lowest level from 3 520 in 1990 to 159. However, number of reported cases has been continuously increasing since 1998 which reached 1 122 in 2002, a 7.06 times increase as compared to the number in 1996. The epidemic areas were mainly located in the southeast and southwest parts of the country, such as Sichuan, Hunan, Guangxi, Guangdong, Anhui, Fujian, etc. Furthermore, there was no significant seasonal distribution as it showed before.CONCLUSION:Such facts as the increasing numbers of dogs, low inoculation rate to dogs, poor control on the quality of rabies vaccine, mistreatment to the wounds, and lacking good cooperation between different official departments regarding rabies control might serve as important factors responsible for the recurrence of rabies. Therefore, it is necessary to focus on the above mentioned points and to take comprehensive preventive measures to bring down the prevalence of rabies in China.

Abstract:Objective To analyze epidemiological features and trend of rabies in China,and correspondingly discuss prevention and control strategies.Methods Based on the data from Chinese Infectious Disease Surveillance Reporting System and sentinel surveillance system in 6 provinces in 2013,we made a retrospective analysis using descriptive epidemiological methods.Results A total of 1172 cases were reported by 28 provinces which decreased by 17.75％ compared with those of 2012.Rabies mainly occurred in southern China,and the top five provinces of reported cases were Guangxi,Guangdong,Hunan,Guizhou,and Henan,which account for 46.84％ of total rabies cases.The rabies occurred commonly in summer and fall,and the cases were mainly composed of farmers,students and scattered children.The ratio of male to female was 2.27∶ 1.There were two peaks of case numbers in 0-14 age group and 40-64 age group.The 319 epidemiological questionnaires that were collected showed that 93.83％ of exposure was caused by dogs and 5.48％ by cats.The median of latent period was 67 days.64.36％ of cases with exposure of category Ⅲ,and 11.86％ of cases received vaccine after exposure.Only 1.36％ of cases received full immunization.5.91％ of cases with exposure of category Ⅲ received passive biological immunity.Conclusions Rabies cases continued to decrease in 2013,the cases of south still more than north,the reported provinces did not decrease,indicating a trend of increase in northern areas.Cases occurred commonly in rural areas,and were mainly farmers,students and scattered children.All of the proportions of voluntary and medical treatment,vaccination,passive biological immunity were low.It should continue to strengthen the national rabies surveillance.Attach great importance to the propaganda and education of rabies,focusing on rural areas,strengthen the propaganda education of treatment after rabies exposure.
Analysis on the epidemiological characteristics of rabies in Zhejiang province from 2008 to 2010

Abstract:Objective To summarize the epidemiological characteristics of human rabies according to the surveillance data on rabies in Zhejiang province from 2008 to 2010, and explore the existing problems and the countermeasures. Methods The surveillance data on human rabies from the clinics and the epidemiological characteristics of individual cases in Zhejiang from 2008 to 2010 were collected for descriptive analysis. Results It was found that the incidence of rabies decreased from 2008 to 2010 year by year. The epidemic was spread to the central and western regions of Zhejiang, with the incidence rising in Quzhou and Jinhua. The peak incidence of rabies was seen in summer and autumn, with a second peak in early winter in some areas. Farmer, migrant workers and students were frequently affected. The sex ratio of the cases in the 3 years was 3.2∶1, 2.1∶1 and 2.1∶1, respectively. The number was highest in the age group of 55-70 years, accounting for 39.7%. Dogs were the principal reservoir of the 94 cases from 2008 to 2010. The incubation period was 85 d, with the period for the majority of the cases (35.11%) being between 2 months and half a year. The vast majority of cases after exposure did not receive any treatment (62.77%). Some of the cases after exposure were managed only with simple disinfection or flushing (25.53%). There were only 8.5% of the cases who received vaccination after exposure, of which only 1 case (1.06%) got full immunization with rabies vaccine. 67.02% of all the cases were classified as grade Ⅲ exposure and only one of them (1.06%) was administrated rabies immunoglobulin (RIG) or anti-serum. Conclusion It needs to change the target population of health education about rabies and the focus of control. Training of primary care staff should be strengthened in terms of knowledge related to rabies control, especially the interventions of wound management and infiltration of passive immunity preparations.

Abstract:Objective To summarize and analyze the epidemic situation of human rabies from 1997 to 2007 in Nantong City,and analyze the existing problems in the present prevention process,and state the preventive strategies of the Rabies.Methods National and some provincial data on the prevalence of rabies during 1997 to 2007 were collected and analyzed.Results During 1997 to 2007,the morbidity number were 161,the incidence was between 0.09/100thousand and 0.32/100thousand,reaching the highest in2002,and began to decline slowly from 2003.The cases are mainly distributed in Rugao,Rudong,Tongzhou,and Haian.Seasonal morbidity is not obvious,the most from June to October.The patients are mainly the farmers,next are the students.The morbidity of the over 60-year-old group are the highest,about 30.43%.Conclusion The effective measures to prevent Rabies are the cooperation,public education,the popularization of prevention knowledge,the realization of integrated preventive measures between departments.We should carry out the monitoring of the animal Rabies,especially the carried virus by the persons with non-symptoms,also separate the Rabies from the patients and the dogs.
Analysis of Epidemiological Features of Human Rabies in China, 2012

Abstract:OBJECTIVE:To analyze epidemiological characteristics and trends of rabies and explore control and prevention measures based on the rabies surveillance data of 2012 in China.METHODS:Data of 2012 from China's infectious disease surveillance reporting and management system and sentinel surveillance systems in 6 provinces were used, for a retrospective analysis in descriptive epidemiological methods.RESULTS:1 425 cases were reported in 731 counties of 27 provinces in 2012 and 1 361 deaths were reported due to rabies, with the rabies incidence rate and mortality rate of 0.11/100 000 and 0.10/100 000 respectively, decreasing by 26.0% and 27.9% respectively from 2011. Rabies epidemic was mainly found in southern regions, followed by middle and eastern regions in China. 49.6% of total rabies cases were found in Guangxi, Guangdong, Hunan, Guizhou, and Henan province, which were the top five provinces. The rabies cases were mainly peasants, students and scattered children, accounting for 70.9%, 8.3% and 5.8% of total cases respectively. The male-female ratio in rabies cases was 2.6 : 1. In 2012, 294 epidemiological questionnaires were collected, revealing that 92.1% of the exposure was caused by dogs and 6.8% by cats. The median of latent period was 70 days. 62.4% of the cases were exposed in upper limb, and only 6.9% of such cases were vaccinated after exposure while the proportion of passive immunity biological vaccination was 2.9% for cases with exposure of category III. Surveillance data from PEP (post-exposure prophylaxis) clinics showed that 81.7% of the visitors were hurt by dogs and the exposure categories I, II and III accounted for 7.0%, 50.5% and 42.5% respectively. The proportion of of the exposure categories varied by PEP surveillance clinics.CONCLUSION:Despite continuing decrease of rabies cases in China in 2012, the number of counties (districts) affected fall relatively slow, with a tendency of rabies spreading to the western and northern regions in China. There were more rabies cases in rural areas than urban ones, and children and elderly people constitute the high-risk population. Animal reservoir of rabies was still dogs as before. Poor PEP and insufficient use of vaccination and passive immunity preparations were main causes for onset of human rabies.

Abstract:Rabies is a zoonotic, fatal and progressive neurological infection caused by rabies virus of the genus Lyssavirus and family Rhabdoviridae. It affects all warm-blooded animals and the disease is prevalent throughout the world and endemic in many countries except in Islands like Australia and Antarctica. Over 60,000 peoples die every year due to rabies, while approximately 15 million people receive rabies post-exposure prophylaxis (PEP) annually. Bite of rabid animals and saliva of infected host are mainly responsible for transmission and wildlife like raccoons, skunks, bats and foxes are main reservoirs for rabies. The incubation period is highly variable from 2 weeks to 6 years (avg. 2-3 months). Though severe neurologic signs and fatal outcome, neuropathological lesions are relatively mild. Rabies virus exploits various mechanisms to evade the host immune responses. Being a major zoonosis, precise and rapid diagnosis is important for early treatment and effective prevention and control measures. Traditional rapid Seller's staining and histopathological methods are still in use for diagnosis of rabies. Direct immunofluoroscent test (dFAT) is gold standard test and most commonly recommended for diagnosis of rabies in fresh brain tissues of dogs by both OIE and WHO. Mouse inoculation test (MIT) and polymerase chain reaction (PCR) are superior and used for routine diagnosis. Vaccination with live attenuated or inactivated viruses, DNA and recombinant vaccines can be done in endemic areas. This review describes in detail about epidemiology, transmission, pathogenesis, advances in diagnosis, vaccination and therapeutic approaches along with appropriate prevention and control strategies.

Consideration on the Prevention and Control of Rabies in China

Abstract:In recent years,the human rabies cases have increased rapidly from 159 cases in 1996 to more than 3000 cases in 2006.Several measures have considered for prevention and control of the disease as follows:(1)In order to carry out the mass dog vaccination in China,a kind of safe,potent and cheap inactivated dog vaccines derived from cell cultures should be developed and produced in a large quantity instead of the live-vaccine currently used in China,which are not recommended by WHO for use for parenteral immunization in dogs.(2)Pre-exposure prophylaxis should be expanded for people living in the high-risk areas,especially for the children under 15 years old who are the most frequently exposed age group in canine rabies-infected areas.(3)The rate of persons receiving a full post-exposure vaccination should be increased.In order to attain this purpose,cost for the post-exposure vaccination should be reduced,the most effective measure is to use the WHO recommended multiple intradermal vaccination regimens.The use of this route leads to considerable saving of the total amount of vaccine needed,thereby reducing the cost of post-exposure vaccination.(4)The vaccine failure cases after the post-exposure vaccination should be investigated,especially to study and compare on the genetic and antigenic characteristics among the rabies vaccine seed virus strains and the wild rabies virus strains currently prevalent in animals.

Abstract:Rabies is a vaccine-preventable fatal disease in man and most mammals. Although rabies is recorded in 150 territories and is responsible for at least 60,000 human deaths every year worldwide, it is a neglected tropical problem. Most of the rabies free countries are considered to be fragile free as the disease may re-emerge easily through wild mammals. For the performance of effective rabies eradication programs, a complex set of strategies and activities is required. At the time, a joint project of WHO–OIE–FAO which was announced in 2015, plans to control animal–human–ecosystems rabies interface. For effective rabies control, prophylactic policies must be applied. These include various educational outreaches for farmers and people living in endemic areas, enforced legislation for responsible dog ownership, control programs for the free-ranging stray dog and cat populations, field large-scale vaccination campaigns, and the development of new vaccine delivery strategies for both humans and animals. The present work presents the advances in the development of new safe, effective and economic vaccines for domestic dogs, and oral vaccines for the control of the disease in wild animals. It presents also some therapeutic protocols used for the treatment of patients.

Abstract:Introduction: Rabies is a serious, neglected tropical disease. Zoonotic agents are RNA viruses (Genus Lyssavirus, Family Rhabdoviridae), global in distribution. As an acute, progressive, incurable encephalitis, rabies has the highest case fatality of any infectious disease. Warm-blooded vertebrates are susceptible hosts. Major mammalian reservoirs include mesocarnivores and bats. Given wildlife perpetuation, rabies is not eradicable, but is preventable and controllable, especially under newly available international guidelines.Areas covered: Literature review over the past 5 years reveals development of sensitive, specific diagnostic tests and safe and highly effective human and veterinary vaccines. Yet, tens of thousands of human fatalities occur annually, usually in Africa and Asia, primarily after canine exposure. Human and domestic animal vaccination, before or after exposure, is the single greatest preventative strategy following a rabid animal bite.Expert opinion: Significant progress occurred during the twenty-first century regarding vaccine development, doses, and schedules. Remaining barriers to widespread rabies vaccination include an inter-related set of economic, cultural, social, educational, ecological and technological factors. A basic understanding of local and regional root causes of cases historically allows for broader accessibility to vaccination in a trans-disciplinary fashion to meet the global elimination of human rabies caused via dogs (GEHRD) by 2030.

Abstract:[Objective]To investigate and analyze the epidemiological characteristics of rabies and the risk factors in Henan and provide measures for the prevention and control of rabies in the future.[Methods]The data of rabies from 1951 to 2005 in Henan were collected and analyzed by statistical software.The dog brains collected in Henan were tested and the lyssa virus was isolated from the samples.[Results]3 peaks of incidence occurred in the past 55 years.The incidence increased rapidly in recent years.3 273 cases were reported from 1979 to 2005,of which 97.8% occurred in rural areas.The 5 districts in the south and east of Henan province were the main incidence areas.There was no statistical difference in the incidence of human rabies among seasons but more cases occurred in summer and autumn.Male cases were more than female cases.There were more cases in the age-group below 15 and between 40 and 60.Canine(dog especially)was the main infection resource.Site of bite、treatment after exposure and age were the risk factors.Of the 100 dog brain samples,there were 6 IFAT positive,5 ELISA positive,and 10 PCR positive.4 strains of lyssa virus were isolated.[Conclusion]The management and immunity of Canine(dog especially)are as important as health education of rabies in rural areas,which can strengthen the consciousness of selfguard and increase the rates of correct treatment and inoculation after exposure.Multi-measure is essential for rabies control.

Abstract:In China, rabies is a significant public health concern where dogs remain the main reservoir of disease transmission to humans; rabies-related mortality ranks second in the world. We compiled all published articles and official documents on rabies in mainland China to examine challenges and needs to eliminate rabies in the country. The Chinese authorities have identified rabies as a priority, recognized rabies control in dogs as key to control rabies in humans and required intersectoral collaborations. Efforts have been made to respond effectively to the latest re-emergence of rabies, which peaked in 2007 with >3,300 cases. Despite these outcomes and the increasing volume of publications and regulations in the recent years, our review points to some major information gaps to improve rabies control activities and envisage elimination program. An emphasis on laboratory or pathogen-associated and basic epidemiology research in the literature has contrasted with the absence of information to monitor various systems in humans and animals (e.g. quality of surveillance, response and post-exposure prophylaxis). Information is also lacking to appropriately inform policymakers (e.g. economic disease burden, impact of policies) and assist program managers (e.g. comprehensive and strategic guidance for cost-effective prevention and control activities, public education and dog population management). In conclusion, strategic planning is needed to provide a sense of direction, demonstrate feasibility of elimination in China, and develop a research agenda, addressing country’s operational needs and constraints. The planning should be a multisectoral effort.

Abstract:Background Rabies in China remains a public health problem. In 2014, nearly one thousand rabies-related deaths were reported while rabies geographic distribution has expanded for the recent years. This report used surveillance data to describe the epidemiological characteristics of human rabies in China including determining high-risk areas and seasonality to support national rabies prevention and control activities. Methods We analyzed the incidence and distribution of human rabies cases in mainland China using notifiable surveillance data from 1960-2014, which includes a detailed analysis of the recent years from 2004 to 2014. Results From 1960 to 2014, 120,913 human rabies cases were reported in mainland China. The highest number was recorded in 1981(0.7/100,000; 7037 cases), and in 2007(0.3/100,000; 3300 cases). A clear seasonal pattern has been observed with a peak in August (11.0% of total cases), Human rabies cases were reported in all provinces with a yearly average of 2198 from 1960 to 2014 in China, while the east and south regions were more seriously affected compared with other regions. From2004 to 2014, although the number of cases decreased by 65.2% since 2004 from 2651 to 924 cases, reported areas has paradoxically expanded from 162 prefectures to 200 prefectures and from southern to the central and northern provinces of China. Farmers accounted most of the cases (65.0%); 50-59 age group accounted for the highest proportion (20.5%), and cases are predominantly males with a male-to-female ratio of 2.4: 1 on average. Conclusions Despite the overall steady decline of cases since the peak in 2007, the occurrence of cases in new areas and the spread trend were obvious in China in recent years. Further investigations and efforts are warranted in the areas have high rabies incidence to control rabies by interrupting transmission from dogs to humans and in the dog population. Furthermore, elimination of rabies should be eventually the ultimate goal for China.
Analysis of Rabies Cases in Sichuan Province,2007-2008

Abstract:Objective To analyze the characteristics of rabies cases in Sichuan and take appropriate control measures to reduce the incidence of rabies.Methods The survey forms of 480 rabies cases in 2007 and 2008 were collected and analyzed by using Excel and Epidata software statistically.Results The male to female ratio of the cases was 1.78∶1;the cases among farmers accounted for 72.5%;Among 480 cases,97.3% were caused by dog bite,2.7% were caused by cat bite.Stray animal bite caused 49.4% of the cases and dog attack caused 66.6% of the cases by dog bite.Fifty four cases had the history of wound washing/disinfection and vaccination,The cases without wound treatment accounted for 47.9%;with self treatment accounted for 32.7%;with treatment in medical service accounted for 19.4%,and the cases receiving wound treatment in medical service after head/face exposure accounted for 48.1%;receiving vaccination accounted for 16.25%.The vaccination rates after arm exposure and head/face exposure were 66.7% and 50.0% respectively,and 16.3% of cases received vaccination and immune globulin injection.Conclusion Most rabies cases in Sichuan were male farmers.Many cases didn't receive standard wound treatment,and some cases had been vaccinated.It is necessary to strengthen the health education,increase the rate of standard wound treatment,increase vaccination and immune globulin injection rate and strengthen the management of stray dogs in the prevention and control of rabies.Meanwhile,the analysis on vaccination failure should be conducted to explore the reason of rabies onset after vaccination.

Abstract:Rabies is a deadly neurotropic viral infection but is preventable through vaccination. Its impact on human and animal health is often devastating. Despite advances in research and control of this disease, along with an effective vaccine, rabies continues to be a lingering threat in many parts of the world. Its high incidence reinforces the constant need to address it with scientific and public health measures. In this review article, we will explore the most important characteristics of the disease, its distribution, symptoms, prevention methods, and the global efforts that have been made to eradicate this deadly disease. The most recent research and advancements in the field of rabies will also be discussed in an effort to understand and combat this deadly disease more effectively.

Abstract:Rabies is still a public health threat in China. Evaluating the exposure history, clinical characteristics, and post-exposure prophylaxis (PEP) of the cases could help in identifying approaches to reducing the number of these preventable deaths. We analysed data collected from 10,971 case-investigations conducted in China from 2006 to 2012. Most cases (n = 7,947; 92.0%) were caused by animal bites; 5,800 (55.8%) and 2,974 (28.6%) exposures were from domestic and free-roaming dogs, respectively. Only 278 (4.8%) of these domestic dogs had previously received rabies vaccination. Among all cases, 5,927 (59.7%) cases had category III wounds, 1,187 (11.7%) cases initiated the rabies PEP vaccination and 234 (3.9%) cases with category III wounds received rabies immunoglobulin. In our adjusted logistic regression model, male cases (adjusted odds ratio [aOR] = 1.25, 95% confidence interval [CI]: 1.09-1.44) and farmers (aOR = 1.39, 95% CI: 1.10-1.77) and person older than 55 years (aOR = 1.48, 95% CI: 1.01-2.17) were less likely than females and persons in other occupations or younger than 15 years to initiate PEP vaccination. The median incubation period was 66 days (interquartile range (IQR): 33-167 days). To reduce the number of human deaths due to rabies, rabies prevention campaigns targeting males and farmers and older people should be conducted. Increasing routine rabies vaccination among domestic dogs will be essential in the long term.

Abstract:In many third world countries, where rabies is endemic in dog populations, humans continue to be at risk of contracting the disease. Vaccination is the most effective form of prophylaxis for people, yet it often fails to adequately protect dogs. The most major implications are the costs of post-exposure prophylaxis (PEP) after an exposure occurs and the loss of human life and productivity due to early mortality from rabies (about 60,000 deaths annually). The largest rabies death tolls can be found in the world's poorest regions, where rabies vaccinations for domestic dogs are uncommon and PEP is scarce. Mass vaccination of dogs, neutering programs, patient PEP, strengthening laboratory and human resources, education and awareness, and animal and human rabies surveillance are all common methods used to prevent, control, and ultimately eradicate dog-mediated human rabies. Current rabies control initiatives, however, pay little attention to the role that ecological and socioeconomic variables play in the disease's occurrence and spread. To help better inform rabies control strategies, we address in this work the ways in which ecological and socioeconomic factors affect the occurrence and spread of rabies.
Survey on Rabies Virus Carried by Domestic Dogs in Henan and Shaanxi Province, China

Abstract:Background: Rabies is one of the ancient zoonotic diseases of the central nervous system. In the absence of protection measures, onset of disease with this neurotropic virus almost invariably causes death. Despite of the long history and the remarkable progress in the knowledge for prevention and control, rabies has maintained its global distribution in animals as well as humans still, particularly, in many developing countries. There have been three major rabies epidemics in China from the 1950's. Since 1998, the epidemic of human rabies, once effectively controlled, has consecutively taken a bad turn. Therefore, to look for reasons of current epidemic of human rabies in China, we launch a survey on rabies virus carried by domestic dogs in Henan and Shaanxi province, two definitely typical epidemic areas with high and low incidence of human rabies respectively. Methods & Materials: 121 and 645 domestic canine brain samples were collected from Henan and Shaanxi province. IFA assay was carried out for detecting pathogen of rabies, and mouse intracranial inoculation test was applied to establish rabies virus isolates. By using reverse transcription polymerase chain reaction, entire N-coding sequence of viral genome was amplified. Further analysis was launched through MEGA 5.1 and Lasergene 7 software. Results: 9 positive canine brains were picked out from 121 Henan province samples and the rabies virus isolates named FD Ls09, FD Ls23, FD Ls48, FD Ls53, FD Ls57, FD Ls89, FD Ls101, FD Ls103 were then obtained. No positive sample was detected in 645 Shaanxi province canine brains. The rate of rabies virus carried by domestic dogs in Henan was 7.4% and 0 in Shaanxi. Molecular and phylogenetic analysis on viral N gene demonstrated that 9 isolates were classical RVs, and 99.0%∼99.8% nucleotide homology and 98.4%∼99.8% deduced amino acid homology existed among these viruses. 9 isolates shared higher identity with CTN than with other vaccine strains. Conclusion: High rate of rabies virus carried by domestic dogs probably was the explanation for current rabies epidemic in China. The management and immunity of dogs are of significance for further preventing and controlling rabies.

We review the incidence of rabies in China based on data from 1950 and 2004, obtained mainly from epidemiologic bulletins published by the Chinese Ministry of Health.

Abstract:OBJECTIVE:To better understand the epidemiology of rabies during the past ten years in Yancheng city, Jiangsu province.METHODS:Data was collected and analyzed on rabies cases in Yancheng. Density and vaccination rate on Canine, Rate of injured people bit by dogs, and the information of post-exposure prophylaxis were studied. Rabies virus in the dog brains, collected around the epidemic areas of Yancheng, were detected and analyzed.RESULTS:A total of 135 human rabies cases occurred from 1999 through 2008, and formed the second epidemic peak since 1958. Of these victims, 84% (114) were farmers. In general, the rate of people having dogs were 3% - 6% per 100 people, and the injured person-times of 100 dogs were 6.37 per year. Notably, the vaccination rate of dogs was only 20%. Of those people injured by dogs and other animals, 77% had received post-exposure treatment, and only 5% - 10% had been administered anti-rabies serum. Rabies virus antigen was found in 4 (3.6%) of 111 brain specimens among dogs collected from epidemic areas. Genetic analysis of N and G genes, which were amplified from brain specimens, indicated that these viruses belong to genotype I rabies and expressing a close relationship with the Chinese vaccine strain CTN.CONCLUSION:The large number of dogs with low vaccination rate among them, together with the incorrect and low post-exposure treatment in rural areas seemed to be responsible for the outbreak of rabies in Yancheng city.
