Abstract:Objective: To evaluate the therapeutic effect of and adverse reaction to Sodium Ibandronate (SI) in patients with osteodynia resulting from osseous metastasis from malignant tumors. Methods: Sixty-eight patients were randomly assigned to group A that received 4 mg SI i.v., and 73 patients were randomized into group B that received 60 mg PD i.v., using a randomized, double-blind clinical trial. The change in bone pain intensity and any side effects that occurred within three weeks were observed. Results: A total of 141 patients were enrolled in the study, with 136 cases suitable for efficacy appraisal. The effective rate was 72.3% (47/65) in group A and 63.4% (45/71) in group B. There was no significant difference between the 2 groups. The adverse effects were similar in group A and B. Conclusion: The SI injection, with a pronounced effect, can relieve bone pain caused by osseous metastasis of malignant tumor. The curative and adverse effects were similar to those found with pamidronate disodium.

Abstract:Objective:To evaluate the efficiency and safety of ibandronate at the initial loading dose in the treatment of metastatic bone pain(MBP) induced by non-small cell lung cancer(NSCLC).Methods: Thirty cases of NSCLC patients who were admitted to our hospital from May 2006 to July 2007 were retrospectively analyzed.They received loading dose of ibandronate for MBP treatment.Ibandronate was given to the patients intravenously at initial loading dose of 6 mg for more than 15 min on 3 consecutive days.Then ibandronate was administered repeatedly at 6 mg every 3 to 4 months.The response time,pain score,QOL score,daily morphine consumption dose were recorded.The serum markers such as calcium,creatinine,alkaline phosphatase(AKP) were monitored.Results:Twenty nine cases were eligible to evaluate the efficiency and safety.Among them,the MBP of 25 cases(86.2%) were relieved.Their average response time was 2.9±0.8 days.The pain score was significantly decreased after treatment and the life qualities of patients were markedly improved(P0.001).The morphine consumption dose was reduced by about 20 to 40 mg in 4 cases after treatment.The serum calcium level increased in 7 patients before treatment and declined to the normal level after treatment.The adverse reaction was relieved after treatment and no renal toxicity was observed.Conclusion: Administration of ibandronate at initial loading dose had quick on-set and mild side effects.It could significantly relieve metastasis bone pain and improve QOL,reduce the morphine consumption dose and avoid morphine-induced adverse reaction,and effectively decrease serum calcium level.Administration of ibandronate at initial loading dose is a new promising option for treatment of the MBP in NSCLC.

Abstract:Objective To evaluate the efficacy and safety of zoledronic acid in the treatment of pain of skeletal metastases in cancer patients. Methods A randomized double-blind, multicenter, comparative trial was performed in 210 patients with osteolytic metastases and pains. One hundred and five patients were treated with both zoledronic acid (group A), others were treated with pamidronate disodium (group B).Results The efficacy of group A and B in relieves pain of osteolytic were 71.43% and 65.72%, respectively. There was no significant difference between the two groups (P0.05). The performance status of group A is lower than that of group B on the 7th day after administration. There was significant difference between the two groups (P0.05). The adverse reaction of the group A and B were 46.76% and 52.38%, respectively. No significant difference was found between the two groups(P0.05). Conclusion Zoledronic acid was safe and effective in the treatment of pain patients caused by osteolytic metastases.

Abstract:Objective:To observe the effect of sodium ibandronate on bone pain and ameliorating physical motion of cancer patients with bone metastases.Methods:Thirty-five cancer patients with bone metastases were administrated 2 mg AIBEN every 2 to 4 weeks and twenty-five cases received chemotherapy or radiation.Results:The total effective rate of bone pain relief was 85% (30/35),with better results for breast cancer and multiple myeloma.The effect was more obvious when accompanied by chemotherapy or radiation.Seventy-seven 77% of the cases obtained good physical motion and 3 cases demonstrated complete response on screening.Conclusions:Ibandronate can effectively relieve bone pain caused by neoplasm metastasis and improve the physical motion of the patients.

Abstract:The objective of this study was to evaluate the efficacy and safety of zoledronic acid in the treatment of pain of skeletal metastases in lung cancer patients. Fifty patients were randomly assigned to a zoledronic acid group,and pamidronate disodium group. The total rates of efficacy of the zoledronic acid group and pamidronate disodium group in relieves pain of skeletal metastases were 76%(19/25) and 72%(18/25), respectively. There was no significant difference between the two groups, P=0.798. The initial time of effect was 1-11 d (median time, 5 d) in the zoledronic acid group, and 2-13 d (median time, 5 d) in the pamidronate disodium group. There was no significant difference between the two groups,P=0.702. The persistent time of effect was 2-32 days (median time, 24 days) in the zoledronic acid group, and 1-29 days (median time, 23 days) in the pamidronate disodium group. There was no significant difference between the two groups,P=0.509. In conclusion, the zoledronic acid injection, with a pronounced effect, can relieve bone pain caused by metastasis of lung cancer. The curative and adverse effects are similar to those found with pamidronate disodium.

Abstract:Objective To retrospectively evaluate the efficacy and safety of pamidronate disoclium and zoledronic acid in treating non-small-cell lung cancer(NSCLC) patients with bone metastasis.Methods This study included 74 patients who were treated with bisphosphonate between June 2007 and December 2008.Fifty were treated with pamidronate disodium,and 24 with zoledronic acid.Pamidronate disodium was administered intravenously once for 3 hours every 4 weeks at a dose of 90 mg.Zoledronic acid was given intravenously once for 15 minutes every 4 weeks at a dose of 4 mg.Various clinical,pathological factors and treatment methods related to the occurring time of skeletal related events(SRE) and survival rate were analyzed.Kaplan-Meier curve and Log rank were adopted to detect the difference in survival rate between patients treated with different medicine,and we used χ2 test to discover the rate of adverse events of the patients.Results Eighteen-month SRE-free survival and overall survival rate in the pamidronate disodium and zoledronic acid group were 19.3% vs.28.9%(P=0.253),and 33.4% vs.38.2%(P=0.745) respectively.There were 8(8/50) cases of adverse events in the pamidronate disodium group,and 6(6/24) in the zoledronic acid group(χ2=0.200,P=0.655).The SRE-free survival time for seven patients who were treated with zoledronic acid after pamidronate disodium failed was 2 months(95%CI: 0-4.6).Conclusion Compared with zoledronic acid,pamidronate has equal efficacy in delaying SRE and incidence of adverse effects.Administering zoledronic acid after pamidronate failed can also delay the occurring time of SRE.
Comparison of Clinical Effect in Treatment of Bone Tumor Between Zoledronic Acid Needle and Ibandronate Needle.

Abstract:To observe and analyze the clinical effect of zoledronic acid needle and ibandronate needle in treatment of bone tumor. 100 patients who have been treated in our hospital for bone tumor were selected as research objects. They were randomly divided into research group and control group, each containing 50 patients. The research group was applied with ibandronate needle therapy, while the control group was given with zeledronic acid needle therapy. After treatment, the clinical effects of the two groups were observed and analyzed. Through comparing the pain relief rate after treatment between the two groups, it can be known that the pain relief rate of research group was relatively higher, P<0.05; the rate of adverse effect in research group was relatively lower, P<0.05; the quality of life score (QLS) of research group was significantly superior to that of control group, P<0.05. theibandronate needle therapy is a more reliable and superior method in treatment of bone tumor compared with zoledronic acid needle therapy, which should be promoted in clinical treatment.

Abstract:Objective: To evaluate the clinical effect and safety profiles of zoledronic acid in the treatment of bone metastasis pain in cancer patients. Methods: A multicenter, double-blind and double-dim clinical trial on pamidronate was conducted in 237 cancer patients with bone metastasis pain from 12 centers in China. The patients were randomized into 2 groups, i.e., the experiment group and control group. Zoledronic acid (4 mg) was injected for 15 min in the experiment group and pamidronate (90 mg) i.v. within 4 h in the controls. The daily change of bone ache recorded by a visual analog scale (VAS) and the side effects were observed within 4 weeks as well. Results: The total 219 of 239 cases were eligible for final analysis, 112 from experimental group and 107 from control group were involved in the clinical response evluation. For the best clinical response within 14 days, after injection with those two biphosphanates, the complete response rate (CR rate), partial response rate (PR rate ) and clinical benefit rate was 10.08%, 53.78% and 90.75% respectively in the experiment group, compared with those data of 5.08%, 54.24% and 83.90% in the control group. There was no significant difference between the 2 groups. The daily clinical improvements of pain reduction and/or relief in the experiment group during the 5th, 7th till 12th day were absolutely better than those from the controls.There was no significant difference between the 2 groups for the consideration of pain improvement with 28 days . The medium maintain time (MMT) of complete response within 14 days were 5.51 and 3.01 days respectively between the 2 groups (P=0.0302), with the contrast of the MMT of effective responses (CR+PR) were 9.02 and 8.02 days (P>0.05). The incidence rate of the main side effects, like fever, hypocalcaemia, and fatigue was 23.94% and 27.98% respectively in the experiment group and the controls. No severe side effect was found in both groups. Conclusion: Zoledronic acid can effectively relieve bone metastasis pain. The response rate and side effect were similar to those of pamidronate.

Abstract:OBJECTIVETo evaluate the efficacy and safety of zoledronic acid in the treatment of bone pain in patients with bone metastasis from solid tumor or multiple myeloma.METHODSA randomized, double-blind, double-simulated and multi-center phase III clinical trail with pamidronate as control was conducted. Patients with moderate to severe bone pain (VAS > 50 mm) induced by solid tumor or multiple myeloma were randomized to receive intravenous zoledronic acid 4 mg or pamidronate 90 mg. Then the change of VAS and urinary NTX/Cr and CTX/Cr were observed in two groups.RESULTSFrom July 2005 to September 2006, 228 patients with bone pain induced by bone metastasis from 15 cancer centers were randomize into two groups: 116 patients in zoledronic acid group and 112 patients in pamidronate group. The VAS value was decreased gradually after treatment in these two groups. Significant improvement in bone pain after treatment were observed both in zoledronic acid group and the control group when compared with baseline VAS on D8 (-11.77% vs. -10.87%), D15 (-24.60% vs. -21.06%) and D28 (-32.37% vs. -31.26%) (P< or =0.0001), but no significant difference existed between two groups (P =0.6587). Compared with baseline, urine NTX/Cr and CTX/Cr level were decreased rapidly after treatment in both groups, the nadir was on D8, the median decreased on D28, which was -36.9% vs. -32.1% for NTX/Cr (P = 0.7922) and -63.2% vs. -47.9% for CTX/Cr (P =0.834). The frequently observed adverse events were pyrexia (19.0% vs. 31.3%), vomiting (6.0% vs. 8.9%), nausea (4.3% vs. 4.5%), fatigue (3.4% vs. 2.7%) and constipation (2.6% vs. 1.8%) in the two groups. Compared with baseline, the serum creatinine level was not significantly increased throughout the study.CONCLUSIONIntravenous injection of 4 mg zoledronic acid can significantly reduce bone pain and bone resorption marker in urine in the Chinese patients with bone metastasis from solid tumor or multiple myeloma, which is tolerable and also comparable to pamidronate in the efficacy and safety.
Therapeutic Effect of Ibandronate Combined with Chemotherapy on Bone Metastases of Advanced Breast Cancer

Abstract:Objective To evaluate therapeutic effect and adverse effect of ibandronate combined with chemotherapy in breast cancer patients with bone metastases.Methods 65 patients with advanced breast cancer and bone metastases were randomly assigned to received ibandronate combined with chemotherapy of Epirubicin and Vinorelbine(group A),or receive ibandronate alone(group B).Results Overall response rate of pain relief was 87.5%(28/32) and 81.3%(26/32) in group A and B,showing no significant difference(P0.05)between the two groups.There were more adverse effects in group A than in group B.Most of them were medulla regression,gastrointestinal reaction.Conclusion Ibandronate have a satisfactory pain relief effect in patients with advanced breast cancer and bone metastases.The effect can't be enhanced when ibandronate combined with chemotherapy is used.

Abstract:BACKGROUND:Alendronate has a marked enhancing effect on bone mineral density. Ibandronate is the newest (the third generation) bisphosphonates. Evidence-based medicine data on the therapeutic effects ofboth twodrugs remain lacking because of different administration routes and time. <br> OBJECTIVE:To compare the therapeutic effects of ibandronate and alendronate on postmenopausal osteoporosis. <br> METHODS:A computer-based online search was conducted in CNKI, PubMed, CECDB, BMA, VIP, CQVIP, CBM, EMbase, ASP, The Cochrane Library, and EMCC databases from August 1999 toAugust 2015 to screen the relevant randomized controled trials. Meta-analysis was performed using Stata/SE version 12.0 software by extracting data from the relevant articles. <br> RESULTS AND CONCLUSION:A total of 10 randomized controlled trials involving 725 patients (ibandronate and alendronate for 379 and 346 patients, respectively) were included. The meta-analysis results showed that bone mineral density was significantly increased (P=0.174, I2=29.4%,WMD=0.03, 95%CI (0.01-0.04))at 6 months after treatment and incidence rate of fracture was significantly decreased(P=5.810,I2=0%,OR=0.32, 95%CI:(0.10-1.00))during 1-year treatment with ibandronate compared with alendronate treatment. However, no significant differences were found between two drug treatmentsin lumbar spine bone mineral density at 1 year after treatment and the incidence rate of fracture during 6-month treatment. Six-month treatment ofibandronate can improve bone mineral density and reduce the incidence rate of fracture of patients withpostmenopausal osteoporosis.

Abstract:Objective:To evaluate the pharmacoeconomics of zoledronate in treatment of metastatic bone pain of malignancy,compared with ibandronate.Methods:Total of 45 patients with metastatic bone pain of malignancy were randomly divided into two groups to receive 4 mg zoledronate or 4 mg ibandronate.Total cost,cost composition,pain relief response rate and side effects in the first treatment cycle were assessed.A cost-benefit analysis was conducted.Results: The total cost of zoledronate group (n=23) and ibandronate group (n=22) was 2409.22 yuan and 3903.64 yuan respectively (P<0.05).The pain relief rate was 78.3% and 72.3% respectively (P>0.05).The cost / effect of zoledronate group was lower than that of ibandronate group (31.75 vs 53.99).There were no significant differences in incidence of adverse reactions (P>0.05).Conclusion:Zoledronate is a safe,efficient and economic alternative in the treatment of metastatic bone pain of malignancy.

Abstract:19127 Background: Bone metastasis is a common complication in non-small cell lung cancer(NSCLC) and in which pain is the commonest presentation, besides conventional radiation and pain killers, bisphosphonates especially the third generation are indicated in MBP. We did this study to evaluate the efficacy and safety of initial loading dose Ibandronate in the treatment of NSCLC metastatic bone pain. Methods: 48 cases of NSCLC patients with MBP were errolled into this study from May 2006 to Oct 2007, received loading dose Ibandronate, initially 6 mg administered intravenously on 3 consecutive days for about 15 minutes, then 6 mg maintained once a time per 3–4 weeks. Pain scores and QOL scores were accessed on the first 7 days and after 3–4 weeks by numeric rating scale and LCSS respectively, daily morphine consumption doses were recorded, also the serum markers such as calcium, creatinine, alkaline phosphatase and urine protein were monitored. Results: 47 cases were eligible to evaluate the efficacy and safety, including 29 males and 18 females with the midian ages of 55 years old. 40 cases (85.1%)were effective and the average initial response time was 3.0±0.7 days; There were a significant decrease both in pain scores(5.5±1.8 VS 2.9±2.7 P<0.001)and QOL scores (11.2±3.8 VS 5.9±4.7 P<0.001) after receiving the loading dose therapy. 8 patients reduced morphine consumption amount for about 20–60mg and the serum calcium of 11 hypercalcemia patients decreased to normal range after the treatment. Side effects were mild and acceptable, including 10 cases of mild fever who recovered without any additional treatment and 2 cases of muscular spasms who recovered after calcium supplementation, no renal toxicity and osteonecrosis of the jaw happened. Conclusions: Loading dose, consecutive three day administration of ibandronate took effects quickly with mild side effects, it could relive metastasis bone pain and improve QOL significantly, reduced the morphine consumption and will be a new promising option to treat the MBP of NSCLC. No significant financial relationships to disclose.

Abstract:Objective To evaluate efficacy and adverse reactions of ibandronate combined with chemotherapy in treatment of patients with bone metastases of advanced lung cancer.MethodsSixty-seven patients with bone metastases of advanced lung cancer were assigned to two groups(group A and group B). Group A was treated with ibandronate combined with chemotherapy and group B was treated with ibandronate alone.Results Overall objective response rate of bone pain relief was 79.4%(27/34) and 72.7%(24/33) in the 2 treatment groups respectively,with no statistical difference (P0.05).ConclusionIbandronate combined with chemotherapy can treat patients with bone metastases of advanced lung cancer.The treatment is effective and safe.

Abstract:Objective To evaluate the efficiency and safety of loading dose of Ibandronate in the treatment of metastatic bone pain(MBP) induced by malignant tumors.Methods Eighteen patients with malignant tumor who were to be in our hospital and treated with Ibandrnate were analyzed retrospectively.Ibandronate was administered intravenously 4 mg per day at loading dose in more than 30 min for 4 consecutive days.Onset time,pain score,KPS score and daily morphine consumption were recorded.The serum markers such as calcium,creatinine were monitored.Results Eighteen cases were eligible to evaluate the efficiency and safety.Among them,the pain of 13 cases(72.2%) was relieved.The average onset time was(3.7±0.8)days.The pain score was significantly decreased after treatment and the life quality of patients was markedly improved(P0.05).The morphine consumption was reduced by about 20 mg to 60 mg in 4 cases after treatment.The adverse reaction was relieved and no renal toxicity was observed.Conclusion Administration of loading dose of Ibandronate had quick onset time and mild side effects.It could significantly relieve metastasis bone pain and improve KPS,reduce the morphine consumption dose, avoid morphine-induced side effect,and effectively decrease serum calcium level.

Abstract:Objective:To compare the clinical effects of 89Sr combined Zoledronic acid as a palliative therapy method for cancer patients with bone metastases.Methods: Total of 64 patients with bone metastases were randomly divided into two groups,treated with 89Sr(pure group) and 89Sr combined Zoledronic acid(experimental group).Results: In the pure group and experimental group effective rates were 81.2% and 90.6%.Bone relief rates were 75% and 87.5%.No obvious side-effect was found.Conclusion: The total effect rate of two groups.The effect of the experiment group is obvious,and could be used repeatedly.It's safe for patients and could improve life quality of patients.

Patients were randomized to receive daily oral minodronate 1 mg with supplements of 500 mg calcium and 200 U vitamin D3 (n=130) or placebo (n=132) with daily supplements of 500 mg calcium and 200 U vitamin D3, for 48 weeks. The primary endpoint was the average bone mineral density (BMD) change in the lumbar vertebrae 48 weeks post-treatment. Secondary outcome measures was the incidence of vertebral fractures. Safety assessments included the rate of adverse events. Results: At the end of 48 weeks treatment, the average BMD change rate from baseline were: full analysis set results: (3.52±4.82)% in the minodronate group and (2.00±5.74)% in the placebo group; per-protocol set results: (3.99±5.05)% in the minodronate group and (2.07±6.20)% in the placebo group; the differences were all significant (all P<0.05). Vertebral fracture occured in 3 patients (2.3%, 3/132) in the placebo group, and 1 case (0.8%, 1/130) in the minodronate group (P>0.05). The incidence of adverse events was 71.5% (93/130) in the minodronate group and 78.0% (103/132) in the placebo group (P>0.05). Conclusion: Minodronate is effective and safe in the treatment of postmenopausal osteoporosis without severe side effects.
Systematic Literature Review and Network Meta-Analysis Comparing Bone-Targeted Agents for the Prevention of Skeletal-Related Events in Cancer Patients with Bone Metastasis.

Abstract:BACKGROUND Complications from skeletal-related events (SREs) constitute a challenge in the care of cancer patients with bone metastasis (BM). OBJECTIVES This study evaluated the comparative effectiveness of pamidronate, ibandronate, zoledronate, and denosumab in reducing the morbidity of SREs in cancer patients with BM. METHODS Medline (1948 to January 2014), Embase (1980 to January 2014), the Cochrane Library (2014 issue 1), and Web of Science with Conference Proceedings (1970 to January 2014) were searched. Only randomized controlled trials assessing denosumab, bisphosphonates, or placebo in cancer patients with BM were included. The primary outcomes were SREs and SREs by type. The network meta-analysis (NMA) was performed with a random-effects Bayesian model. RESULTS The NMA included 14 trials with 10,192 patients. Denosumab was superior to placebo in reducing the risk of SREs (odds ratio [OR]: 0.49; 95% confidence interval [CI]: 0.31-0.75), followed by zoledronate (OR: 0.57; 95% CI: 0.41-0.77) and pamidronate (OR: 0.55; 95% CI: 0.41-0.72). Ibandronate compared with placebo could not reduce the risk of SREs. Denosumab was superior to placebo in reducing the risk of pathologic fractures (OR: 0.50; 95% CI: 0.32-0.79), followed by zoledronate (OR: 0.61; 95% CI: 0.43-0.86). Denosumab was superior to placebo in reducing the risk of radiation (OR: 0.51; 95% CI: 0.35-0.75), followed by pamidronate (OR: 0.67; 95% CI: 0.52-0.86) and zoledronate (OR: 0.70; 95% CI: 0.52-0.96). CONCLUSION This NMA showed that denosumab, zoledronate, and pamidronate were generally effective in preventing SREs in cancer patients with BM. Denosumab and zoledronate were also associated with reductions in the risk of pathologic fractures and radiation compared with placebo. Denosumab was shown to be the most effective of the bone-targeted agents.

Abstract:Objective To evaluate the effect of ibandronate and alendronate on bone mass in postmenopausal osteoporosis.Methods Sixty-four postmenopausal women aged 47 to 80 years old were randomly divided into two groups.Patients in Group I were administered oral ibandronate 150 mg per month,and patients in Group II were administered oral alendronate 70 mg per week.All patients received calcium 500 mg and vitamin D 200 IU daily for one year.Bone mineral density in the lumbar vertebra and the hip were examined before and after the treatment.Results BMDs in the lumbar spine of patients in Group I were increased by 6.25% and 9.64%,respectively,after a half-year and one-year treatment(P0.05),and increased in patients in Group II by 6.82% and 11.4%,respectively(P0.05).In both group,BMDs of the hip also increased.However,the most obvious BMD increased occurred in the lumbar spine.Conclusions Treatment with both bisphosphonates improved BMD in postmenopausal osteoporosis.Their clinical efficacies were similar.However,the clinical tolerance of ibandronate was better.

Abstract:Objective: To evaluate and compare the safety and efficiency of the combined therapy of 89Sr plus zoledronic acid and those of 89Sr-chloride alone, in patients with painful bone metastases. Methods: A total of 87 patients with osseous metastasis were ran-domly divided into treatment groups of 89Sr-chloride alone (group A, 53 patients) and 89Sr plus zoledronic acid (group B, 34 patients). A total of 17 patients in group B received zoledronic acid 2-14 days after 89Sr therapy, and 13 other patients in the group received 89Sr 4-7 days after zoledronic-acid therapy. Pain response and KPS score were evaluated after the different treatments. Results: No obvious bone marrow suppression and liver damage were found in all cases. All patients who received both 89Sr-chloride and 89Sr plus zoledronic acid showed reduced bone pain and total discomfort, as well as improved KPS score, but the response was more pronounced in group B (P=0.047; P=0.036). No statistical differences in pain score and KPS scores were observed between the groups treated with zoledronic acid first and 89Sr therapy first (P=1.000; P=0.667). Comparison of bone pain relief and changes in the KPS score of different primary tumors after treatment with 89Sr-chloride or 89Sr plus zoledronic acid showed no statistical significance. Conclusion: Compared with 89Sr-chloride, treatment with 89Sr plus zoledronic acid was more effective in patients with painful bone metastases. The safety of these two treatments are similar.
