Laparoscopy-assisted remnant stomach-preserving dual pathways restruction in treatment for upper gastric cancer

Abstract:Objective To explore the evaluation of laparoscopy-assisted remnant stomachpreserving dual pathways restruction in treatment for upper gastric cancer.Methods 25 consecutive patients of upper gastric cancer underwent laparoscopy-assisted radical proximal gastrectomy and remnant stomach-preserving dual pathways restruction,which included end-to-side esophagus-jejunal anastomosis, side-to-side remnant stomach-jejunal anastomosis and side-to-side jejun-jejunal anastomosis.Results The surgery for the 25 patients were completed laparoscopily.Average operation time is(235±24) min,incised margin is negative,average lymphoid nodes dissected are 22±5,and there are not complications such as anastomotic fistula or stenosis.Barium swallow visualization were all made after operation,displaying most of Barium immediately passed the jejunum and less passed the pathway of remnant stomach-duodenum and no Barium back flows esophagus.Moreover,no backstreaming esophagitis was detected and satifactory quality of life in the near future was obtained.Conclusions Remnant stomach-preserving dual pathways restruction randomly complete the range of dissection and excision.The role of pouch was preserved by remnant stomach,which can better prevent backstreaming esophagitis and vert syndrome,preserve the pathway of duodenum and promote the quality of life.Therefore we think it is comparatively ideal restruction of disgestive tract in treatment for upper gastric cancer.
