Clinical Study of Sildenafil for the Treatment of Pulmonary Hypertension

Abstract:Objective:To evaluate the efficacy and safety of oral administration of Sildenafil on pulmo-nary hypertension.Methods:From January 2007 to December 2008,twenty-three patients with pulmonary hy-pertension were enrolled in this study.These patients were orally administered with sildenafil for 12 weeks.The patients were followed-up by telephone.The main end point was the improvement in 6 minutes walk test after 12 weeks’administration.The second end point was improvement of the Brog dyspnea scores,WHO classifica-tion of pulmonary hypertension,systolic pulmonary arterial pressure(SPAP) evaluated by Doppler echocardio-graphy,arterial partial pressure of oxygen(PaO2) in artery blood gas analysis,the blood pressure,heart rate and laboratory test of biochemical indicators.Results: 1.The main end points and second end points were sig-nificantly improved.After the sidenafil therapy,the 6 minutes walk test was improved form(296.78 ± 118.77)m to(400.57 ± 96.58)m(P 0.01),and the Brog dyspnea scores and WHO classification of pulmonary hy-pertension were also obviously improved(P 0.01);systolic pulmonary arterial pressure evaluated by Dopplerechocardiography decreased from(101.68 ± 20.89)mmHg(1mmHg = 0.133kPa) in the baseline to(82.74 ±20.04)mmHg(P 0.01),and the PaO2 in arterial blood gas analysis increased from(62.68 ± 7.92)mmHg to(71.48 ±6.30)mmHg(P 0.01).2.Side effects: During the follow-up,no patient presented lower bloodpressure,Only 1 patient presented headache.The ALT was slightly above the normal range in 2 patients.However,compared with the baseline,the heart rate,blood pressure,liver function and kidney function had nosignificantly difference after treatment(P 0.05).Conclusion: 1.Oral administration of Sildenafil can obvi-ously improve the exercise tolerance,clinical symptoms,WHO classification of pulmonary hypertension and theparameters of haemodynamics of pulmonary hypertension,and hasnt clinical deterioration,which indicate theefficacy of this therapy.2.No report of blood pressure drop and other severe side effects after oral administra-tion of Sildenafil,which confirme its safety in the treatment of pulmonary hypertension.
