Abstract:We read with interest the study by Lin et al1Lin E.C. et al.Clin Gastroenterol Hepatol. 2016; 14: 203-208Abstract Full Text Full Text PDF PubMed Scopus (58) Google Scholar that reported a high rate of false-positive diagnoses of small bowel bacterial overgrowth (SBBO) by glucose hydrogen breath testing (HBT) with nearly 50% of diagnoses being false-positives caused by rapid transit and early colonic fermentation of unabsorbed substrate. This effect was most marked in patients with previous upper gastrointestinal surgery (65% vs 13% in patients without prior surgery). Lactulose is a poorly absorbed, fermentable disaccharide that has also been used to detect SBBO by HBT. The lactulose HBT is said to be more sensitive than glucose HBT or bacterial culture of jejunal aspirates (the “reference standard”) because it can detect cases with distal SBBO and pathologic bacterial strains that do not culture well.2Schiller L.R. If this is not available routinely for reasons of cost, then patients with abnormal results from glucose or lactulose HBT should be considered for a confirmatory test with concurrent scintigraphy to distinguish SBBO from rapid transit accompanied by early colonic fermentation. The use of serial tests (ie, HBT followed by scintigraphy) may be less ideal because of relatively high day-to-day variation in orocecal transit time. Looking ahead, the results of the combined lactulose HBT with concurrent scintigraphy investigation may help to identify the causes of IBS symptoms and to direct antibiotic therapy to patients that are likely to benefit. We agree, but want to emphasize the importance of the scintigraphy protocol used to avoid a false diagnosis of bacterial overgrowth. In the studies cited by Deng et al, the goal was to distinguish, from among groups of patients with putative irritable bowel syndrome or lactase deficiency, patients whose symptoms might result from bacterial overgrowth.
