Abstract:The pathogenesis of peptic ulcer is complicated.Since the discovery of Helicobacter pylori,the pathogenesis and treatment strategy of peptic ulcer have undergone fundamental changes.For the better understanding of the latest development,this paper will discuss it from the following four aspects.(1)The Recovery from the ulcer disease is closely related to the quality of ulcer healing.(2)The discovery of H.Pylori was a revolution for the etiology and treatment of peptic ulcer disease.The eradication of H.pylori can reduce or prevent the recurrence of peptic ulcer disease.(3)To protect the mucosal barrier of stomach and duodenum is the most basic means of treatment of peptic ulcer.(4)The new strategy of treatment of peptic ulcer should be as follows:protection of gastric mucosa,inhibiting of gastric acid and eradication of H.pylori.

Abstract:Objective:To investigate the therapeutic effect of Chinese medicine"QinLianHeJi"(QLHJ)on chronic gastric ulcer healing in rats,mainly focus on the studies on its treament mechanisms.Methods:A rat chronic gastric ulcer model was made by infecting Hp(NCTC11637 strain)after injecting acetic acid into the subserosa of the stomach.In the experiments,the techniques of immunohistochemical stain and in situ hybridization etc.were used to observe the expression of bFGFmRNA and PCNA in th ulcer.Results:Comparing each treating group with Hp ading acetic acid control group,the expression level of bFGFmRNA was increased,and they all have a remarkable effect on increasing PCNA.Conclusion:QLHJ could accelerate the ulcer healing up,and its main mechanism involve the factors by promoting the expression of bFGFmRNA and increasing the cell proliferation of gastric mucosa.

Abstract:Peptic ulcer disease (PUD) is an upper gastrointestinal (GI) disorder with complex etiology, which is a growing major concern worldwide. The pathophysiology of PUD is relatively well understood, hence there are several specific treatment options for this disorder aiming at various pharmacological targets located in the GI tract. In this chapter we provide an overview on pharmacological targets as well as drugs that are currently available for PUD therapy with regard to the etiology of the disease. We discuss their mechanisms of action, evidences for their effectiveness emerging from clinical trials as well as virtues and drawbacks of the most commonly prescribed medications. Furthermore, we highlight the practical aspects of the use of drugs in PUD, such as dosing regimens, resistance issues, possible adverse events, and contraindications.

Abstract: Peptic ulcer refers to the inflammatory response and necrotic lesions of the mucosa under the action of various pathogenic factors, which goes deeply into the mucosal muscle layer and often occurs to the gastrointestinal mucosa related to gastric acid secretion, among which the stomach and duodenum are the most common. The clinical manifestations include slow onset, prolonged course and weekly upper abdominal pain. Nitric oxide (NO) is an intracellular and intercellular signaling molecule that plays an important role in many physiological and pathological processes. Studies have found that a small amount of NO produced in vivo plays a role in many physiological homeostasis, such as regulating blood pressure, platelet aggregation, nitrogenization of hemoglobin, and regulating proliferation and differentiation of stem cells. However, under the action of some cytokines and oxidative stress, intracellular NO synthase will catalyze the synthesis of large amounts of NO and participate in the inflammatory response, causing beneficial or harmful effect on the body. Numerous basic studies have focused on the relationship between NO and peptic ulcer. The purpose of this review is to summarize the role of NO in peptic ulcer and its possible mechanism.

Abstract:Peptic ulcer disease (PUD) refers to acid-induced injury of the digestive tract, occurring mainly in the stomach (gastric ulcer (GU)) or duodenum (duodenal ulcer (DU)). In the present study, we conducted a large-scale, cross-ancestry meta-analysis of PUD combining genome-wide association studies with Japanese and European studies (52,032 cases and 905,344 controls), and discovered 25 new loci highly concordant across ancestries. An examination of GU and DU genetic architecture demonstrated that GUs shared the same risk loci as DUs, although with smaller genetic effect sizes and higher polygenicity than DUs, indicating higher heterogeneity of GUs. Helicobacter pylori (HP)-stratified analysis found an HP-related host genetic locus. Integrative analyses using bulk and single-cell transcriptome profiles highlighted the genetic factors of PUD being enriched in the highly expressed genes in stomach tissues, especially in somatostatin-producing D cells. Our results provide genetic evidence that gastrointestinal cell differentiations and hormone regulations are critical in PUD etiology.

Abstract:Peptic ulcers, recognized for their erosive impact on the gastrointestinal mucosa, present a considerable challenge in gastroenterology. Epidemiological insights underscore the global prevalence of peptic ulcers, affecting 5–10+% of individuals, with a yearly incidence of 0.3 to 1.9 cases per thousand. Recent decades have witnessed a decline in complications, attributed to improved diagnostics and therapeutic advancements. The review deepens into H. pylori -associated and NSAID-induced ulcers, emphasizing their distinct prevalence in developing and industrialized nations, respectively. Despite advancements, managing peptic ulcers remains challenging, notably in H. pylori -infected individuals facing recurrence and the rise of antibiotic resistance. The pathophysiology unravels the delicate balance between protective and destructive factors, including the intricate molecular mechanisms involving inflammatory mediators such as TNF-α, ILs, and prostaglandins. Genetic and ethnic factors, rare contributors, and recent molecular insights further enhance our understanding of peptic ulcer development. Diagnostic approaches are pivotal, with upper gastrointestinal endoscopy standing as the gold standard. Current treatment strategies focus on H. pylori eradication, NSAID discontinuation, and proton pump inhibitors. Surgical options become imperative for refractory cases, emphasizing a comprehensive approach. Advances include tailored H. pylori regimens, the emergence of vonoprazan, and ongoing vaccine development. Challenges persist, primarily in antibiotic resistance, side effects of acid suppressants, and translating natural compounds into standardized therapies. Promising avenues include the potential H. pylori vaccine and the exploration of natural compounds, with monoterpenes showing therapeutic promise. This review serves as a compass, guiding healthcare professionals, researchers, and policymakers through the intricate landscape of peptic ulcer management. Current and Future Drugs for Acid-Peptic Disease: A Plethora of Opinions on Possible Mechanisms of Action

Abstract:Sixty-seven invited participants involved in the development, evaluation, and use of therapies for acid-peptic disorders participated in a meeting to discuss the scientific basis for healing actions of ulcer drugs and the prospects for future developments (“Realities of Mucosal Protection in the Upper Gastrointestinal Tract”, Lausanne, Switzerland, November 8–10, 1987). Eighty-one key statements were prepared and subsequently analyzed on the basis of a voting system. Of the 45 statements that dealt with existing therapies, only 3 statements showed positive consensus (agreement of two-thirds or more of voters) about mechanisms of ulcer healing. Participants agreed that both (1) hydrogen/potassium adenosine triphosphatase inhibitors and (2) histamine H 2 antagonists healed ulcers solely by acid inhibition, and (3) that sucralfate works by topical action. Substantial uncertainty about the mechanisms by which bismuth compounds and antacids heal ulcers was noted as well as their wide range of effects. The mechanism of ulcer healing by prostaglandins and the clinical relevance of antiulcer effects of drugs demonstrated in acute studies with animals were also controversial. There was greater agreement among participants about unexplored drug effects that might produce ulcer healing. Of the 36 such mechanisms surveyed, the most support went to therapies aimed at enhancement of mucosal blood flow, epithelial restitution, and mucosal alkaline secretion or inhibition of luminal pepsin activity. The diversity of opinions among participants suggests a high level of empiricism in the development of ulcer healing drugs apart from those that inhibit acid secretion. This empiricism probably arises from inadequate understanding of processes of mucosal injury and repair.

Abstract:Peptic ulcer bleeding is one of the most common medical emergencies leading to substantial mortality and morbidity. The last two decades has witnessed some important advances in the management of this condition, and some of these are results from clinical trials conducted in the Asia Pacific region. The optimal use of combined endoscopic hemostasis and the use of pharmacologic control of acid secretion in the stomach have significantly improved clinical outcome. The role of surgery has been redefined. Treatment of Helicobacter pylori infection and prophylactic treatment in non-steroidal anti-inflammatory drug and anti-platelet users have made progress in preventing recurrence of peptic ulcer and bleeding. Instead of merely focusing on safety in the gastrointestinal tract, striking a balance between risk and benefit of continuing anti-platelet agents should be a top agenda.

Abstract:Proton pump inhibitors (PPIs) are some of the most commonly prescribed drugs worldwide, but there are increasing concerns about digestive complications linked to PPIs. Next-generation sequencing studies have suggested that PPIs can significantly affect the composition of the gut microbiota, which in turn may substantially contribute to the development of these complications. Recently, emerging evidence has suggested that the translocation of oral microbes into the gut may be the primary mechanism underlying the alterations in the gut microbiota induced by PPIs in the presence of gastric acid suppression and impaired oral–gut barrier function. Moreover, the significance of oral–gut microbial translocation in health and disease conditions has gained increasing recognition. Consequently, it is imperative to enhance our understanding of the functions of the oral–gut microbiota axis in digestive disorders associated with PPI therapies. This review aims to summarize current research findings and further elucidate the contribution of the oral–gut microbiota to the pathogenesis of PPI-related digestive diseases. We aim to provide a theoretical foundation for future therapeutic and preventive strategies targeting PPI-related digestive complications through modulation of the oral–gut microbiota.

Abstract:Ulcerative colitis (UC) is a chronic inflammatory bowel disorder of the large intestine. Previous studies have indicated that the gut microbiota plays an important role in the triggers, development, and treatment response of UC. Natural active molecules and their nanoformulations show huge potential for treating UC. The nanoparticles can regulate the gut microbiota and metabolites, whereas gut microbiota-mediated effects on nanomedicines can also bring additional therapeutic benefits. Therefore, this review aims to integrate current research on natural active molecule-based nanomedicines for UC therapy and their interaction with the gut microbiota. Here, this discussion focuses on the effects and functions of gut microbiota and metabolites in UC. The use of active molecules and the nanoformulation from natural compounds for UC therapy have been provided. The interactions between the gut microbiota and nanomedicines are derived from natural products and elucidate the possible biological mechanisms involved. Finally, the challenges and future directions for enhancing the therapeutic efficacy of nanomedicine in treating UC are proposed.

Abstract:Helicobacter pylori (H. pylori) treatment requires the development of more effective therapies, mainly owing to the challenges posed by the bacterial resistance to antibiotics. In China, critically high infection and antibiotic resistance rates have limited the application of classic H. pylori eradication therapies. Consequently, researchers are attempting to find new solutions by drawing from traditional medicine. This article reviews basic scientific and clinical progress in the use of integrated Chinese and Western medicine (IM) to treat H. pylori; describes the conflicting results between in vivo and in vitro studies in this regard; discusses the observed clinical effects of IM, with emphasis on traditional patent medicines; and proposes a role for IM in both the diagnosis and treatment of H. pylori, including the use of tongue manifestation as an early diagnostic method and capitalizing on IM's direct and indirect methods for enhancing antibiotic effect.

Abstract:A metal‐organic framework based Mucoadhesive Nanodrugs for the treatment of H. pylori. This nanodrugs integrates the gastric mucosal targeting, multi‐effectively antibacterial capability and inflammatory regulation, which also avoids simultaneously the imbalance of gastrointestinal flora homeostasis. The prevalence of drug‐resistant bacteria presents a significant challenge to the antibiotic treatment of Helicobacter pylori (H. pylori), while traditional antimicrobial agents often suffer from shortcomings such as poor gastric retention, inadequate alleviation of inflammation, and significant adverse effects on the gut microbiota. Here, a selenized chitosan (CS‐Se) modified bismuth‐based metal‐organic framework (Bi—MOF@CS‐Se) nanodrug is reported that can target mucin through the charge interaction of the outer CS–Se layer to achieve mucosal adhesion and gastric retention. Additionally, the Bi—MOF@CS‐Se can respond to gastric acid and pepsin degradation, and the exposed Bi—MOF exhibits excellent antibacterial properties against standard H. pylori as well as clinical antibiotic‐resistant strains. Remarkably, the Bi—MOF@CS‐Se effectively alleviates inflammation and excessive oxidative stress by regulating the expression of inflammatory factors and the production of reactive oxygen species (ROS), thereby exerting therapeutic effects against H. pylori infection. Importantly, this Bi—MOF@CS‐Se nanodrug does not affect the homeostasis of gut microbiota, providing a promising strategy for efficient and safe treatment of H. pylori infection.

Neuroendocrine mechanism of gastric acid secretion: Historical perspectives and recent developments in physiology and pharmacology

Abstract:The physiology of gastric acid secretion is one of the earliest subjects in medical literature and has been continuously studied since 1833. Starting with the notion that neural stimulation alone drives acid secretion, progress in understanding the physiology and pathophysiology of this process has led to the development of therapeutic strategies for patients with acid-related diseases. For instance, understanding the physiology of parietal cells led to the developments of histamine 2 receptor blockers, proton pump inhibitors (PPIs), and recently, potassium-competitive acid blockers. Furthermore, understanding the physiology and pathophysiology of gastrin has led to the development of gastrin/CCK2 receptor (CCK2 R) antagonists. The need for refinement of existing drugs in patients have led to second and third generation drugs with better efficacy at blocking acid secretion. Further understanding of the mechanism of acid secretion by gene targeting in mice has enabled us to dissect the unique role for each regulator to leverage and justify the development of new targeted therapeutics for acid-related disorders. Further research on the mechanism of stimulation of gastric acid secretion and the physiological significances of gastric acidity in gut microbiome is needed in the future.

Abstract:Abstract Ulcerative colitis (UC) faces some barriers in oral therapy, such as how to safely deliver drugs to the colon and accumulate in the colon lesions. Hence, we report an advanced yeast particles system loaded with supramolecular nanoparticles with ROS scavenger (curcumin) to treat UC by reducing oxidative stress state and inflammatory response and accelerating the reprogramming of macrophages. In this study, the dual-sensitive materials are bonded on β-cyclodextrin (β-CD), the D-mannose (Man) is modified to adamantane (ADA), and then loaded with curcumin (CUR), to form a functional supramolecular nano-delivery system (Man-CUR NPs) through the host-guest interaction. To improve gastrointestinal stability and colonic accumulation of Man-CUR NPs, yeast cell wall microparticles (YPs) encapsulated Man-CUR NPs to form Man-CUR NYPs via electrostatic adsorption and vacuum extrusion technologies. As expected, the YPs showed the strong stability in complex gastrointestinal environment. In addition, the Man modified supramolecular nanoparticles demonstrated excellent targeting ability to macrophages in the in vitro cellular uptake study and the pH/ROS sensitive effect of Man-CUR NPs was confirmed by the pH/ROS-dual stimulation evaluation. They also enhanced lipopolysaccharide (LPS)-induced inflammatory model in macrophages through downregulation of pro-inflammatory factors, upregulation of anti-inflammatory factors, M2 macrophage polarization, and scavenging the excess ROS. Notably, in DSS-induced mice colitis model, Man-CUR NYPs can reduce the inflammatory responses by modulating TLR4/NF-κB signaling pathways, alleviate oxidative stress by Nrf2/HO-1 signaling pathway, promote macrophages reprogramming and improve the favorable recovery of the damaged colonic tissue. Taken together, this study not only provides strategy for “supramolecular curcumin nanoparticles with pH/ROS sensitive and multistage therapeutic effects” in “advanced yeast particles”, but also provided strong theoretical support multi-effect therapy for UC.

Abstract:Gastric ulcer is a common gastrointestinal disorder worldwide. Although its pathogenesis is unclear, the overproduction of reactive oxygen species (ROS), which results in an oxidative imbalance, has been reported as a central driving mechanism. Within the scope of this investigation, we developed two different self-assembling redox nanoparticles (RNPs) with ROS-scavenging features for the oral treatment of gastric ulcers. One of them, referred to as RNP N , disintegrates in response to acidic pH, whereas the other, denoted as RNP O , remains intact regardless of pH variations. Both types of RNPs showed different free radical scavenging activities in vitro . Protonation of the amino linkages in the side chains of RNP N caused the micelle structure to collapse and the nitroxide radicals encapsulated in the core were exposed to the outside, resulting in a significant increase in antioxidant capacity as the pH decreases. In contrast, RNP O maintained its spherical structure and consistent antioxidant reactivity irrespective of pH changes. The in vivo gastric retention of orally administered RNP N was significantly improved compared to that of RNP O which might be explained by the increased exposure of cationic protonating segments in RNP N on the negatively charged gastric mucosal surface. Owing to its improved gastric retention and enhanced ROS scavenging capacity under acidic pH conditions, RNP N exhibited superior protective effects against oxidative stress induced by aspirin in a gastric ulcer mouse model compared to RNP O . In addition, neither RNP N nor RNP O resulted in severe lethal effects or significant changes in the morphology of zebrafish embryos, indicating their biosafety. Our results suggest that the oral administration of RNPs has a high therapeutic potential for gastric ulcer treatment.

Abstract:Gastric cancer (GC) is one of the inflammation-associated cancers. Helicobacter pylori is now thought to be responsible for more than 95% of all GCs, and its development is associated with at least four mechanisms that lead to genetic instability of the gastric mucosa. The risk of developing GC can be predicted by assessing the extent and severity of corpus atrophy and the degree of risk can be estimated by using non-invasive methods such as the pepsinogen test, or endoscopic or histological cancer risk scoring systems such as the operative link for gastritis assessment. The eradication of H.pylori will stop the progression of gastritis, prevent atrophy and thus decrease the risk of cancer. H.pylori eradication should follow the dictum use what works best locally. There are several new developments in the diagnosis and treatment of H.pylori infection including serological antibody, fluorescent in situ hybridization and antibiotic resistance tests. It is still necessary to develop a preventive or therapeutic vaccine to prevent GC.
