Abstract:Background: Immunosuppressive therapy after life-saving kidney transplantation increases the risk of infection, cardiovascular diseases, metabolic diseases, and cancer. To date, four centers (three in the USA and one in South Korea) have reported clinical tolerance trials in kidney transplantation. We performed the first Chinese clinical trial in which kidney transplantation was combined with donor hematopoietic stem cell (DHSC) infusion to induce tolerance. This study summarizes the 10-year follow-up results. Methods: From 2009 to 2017, 11 donor/recipient pairs underwent living-related kidney transplantation combined with DHSC infusion. Two of the pairs were human leukocyte antigen (HLA)-matched, and nine were HLA-mismatched. DHSCs were mobilized using granulocyte colony-stimulating factor (G-CSF) and harvested 1 day before transplantation. The recipients received consecutive total lymphoid irradiation (TLI) for 3 days before kidney transplantation. The induction drug was anti-thymocyte globulin (ATG). DHSCs were infused on days 2, 4, and 6 post surgery. All patients were followed-up until Dec 2019. Routine laboratory examinations, chimerism, biopsies, and mixed lymphocyte reactions were performed. Results: One HLA-matched recipient had 30-50% chimerism, while the other patients had less than 1% chimerism. Recipients had donor-specific hyporesponsiveness (DSH) while sustaining normal reactivity to non-donors in mixed lymphocyte reactions. All recipients were followed up for 717-3,918 days. One recipient lost allograft function, and 10 recipients had stable renal function. None of the 11 recipients developed myelosuppression or graft-versus-host disease (GVHD) post transplantation. Our protocol did not increase the risk of infection. Allograft biopsy confirmed that one patient had mild rejection with Banff grade IA, while the other ten recipients did not develop rejection. Five patients were able to reduce the dose of their immunosuppressive therapy. Conclusions: Our immune tolerance induction protocol, which used DHSC infusion and TLI, achieved low dose immunosuppression with long-term stable kidney allograft survival in Chinese patients.

Abstract:Background: Inducing donor-specific tolerance in renal transplant patients could potentially prevent allograft rejection and calcineurin inhibitor nephrotoxicity. Combined kidney and hematopoietic stem cell transplant from an HLA-matched donor is an exploratory and promising therapy to induce immune tolerance. Investigation of molecular mechanisms involved in the disease is needed to understand the potential process of cell therapy and develop strategies to prevent this immunologic rejection. Methods: We enrolled nine patients in a clinical study in which cryopreserved donor hematopoietic stem cells were infused on days 2, 4, and 6 after kidney transplantation. One month post-transplant, 4 plasma samples were collected from combined transplants (C + Tx), and 8 plasma samples from patients with kidney transplantation alone (Tx). High abundance proteins in plasma were depleted and the two-dimensional liquid chromatography-tandem mass spectrometry coupled with iTRAQ labeling was utilized to identify the protein profiling between the two groups. Clusters of up- and down-regulated protein profiles were submitted to MetaCore for the construction of transcriptional factors and regulation networks. Results and discussion: Among the 179 identified proteins, 65 proteins were found in C + Tx with at least a 2-fold change as compared with Tx. A subset of proteins related to the complement and coagulation cascade, including complement C3a,complement C5a, precrusors to fibrinogen alpha and beta chains,was significantly downregulated in C + Tx. Meanwhile, Apolipoprotein-A1(ApoA1), ApoC1, ApoA2, ApoE, and ApoB were significantly lower in Tx compared to C + Tx. Gene ontology analysis showed that the dominant processes of differentially expressed proteins were associated with the inflammatory response and positive regulation of plasma lipoprotein particle remodeling. Conclusions: Thus, our study provides new insight into the molecular events in the hematopoietic stem cell-induced immunologic tolerance.

Abstract:Objective To explore the efficacy of renal transplantation plus hematopoietic stem cell transplantation on inducing immune tolerance and summarize its long-term follow-up outcomes . Methods From 2009 to 2018 ,a total of 11 cases of living related donor kidney transplantation plus hematopoietic stem cell transplantation were performed .Two of them were HLA-matched and the remainder were mismatched for one HLA haplotype . The donor hematopoietic stem cells were mobilized using granulocyte colony-stimulating factor at 5 days pre-transplantation and collected at 1 day pre-operation .The recipients received total lymphoid irradiation for 3 days pre-transplantation and received anti-thymocyte globulin induction during transplantation .The donor hematopoietic stem cells were infused at 2 ,4 and 6 postoperative day .Postoperative regulatory T cells ,chimerism ,B cell activating factor and mixed lymphocyte culture and other parameters were detected and long-term follow-up outcomes tracked .Results The immune tolerance-inducible recipients had a significant increase in activated Treg .One HLA-matched recipient achieved 30％-50％ of chimerism and lost after 6 months .However ,other recipients did not achieve mixed chimerism .The BAFF of recipient spiked sharply after transplantation .Mixed lymphocyte culture indicated that a donor-specific low response was induced .The recipients were followed up for 717 to 3612 days .The first recipient lost renal function and another ten recipients had stable renal function . None of the recipients had myelosuppression or graft-versus-host disease .Allograft biopsy confirmed only one case of mild acute rejection . The dose of immunosuppressive agents was lowered in 5 patients .Conclusions Hematopoietic stem cell transplantation for inducing tolerance is safe during renal transplantation . And chimerism is essential for inducing immune tolerance .

Abstract:The pursuit of transplantation tolerance is the holygrail in clinical organ transplantation. It has been established that regulatory T cells (Tregs) can confer donor-specific tolerance in mouse models of transplantation. However, this is crucially dependent on the strain combination, the organ transplanted and most importantly, the ratio of Tregs to alloreactive effector T cells. The ex vivo expansion of Tregs is one solution to increase the number of alloantigen specific cells capable of suppressing the alloresponse. Indeed, ex vivo expanded, alloantigen specific murine Tregs are shown to preferentially migrate to, and proliferate in, the graft and draining lymph node. In human transplantation it has been proposed that depletion of the majority of direct pathway alloreactive T cells will be required to tip the balance in favour of regulation. Ex vivo expansion of alloantigen specific, indirect pathway human Tregs, which can cross regulate the residual direct pathway has been established. Rapid expansion of these cells is possible, whilst they retain antigen specificity, suppressive properties and favourable homing markers. Furthermore, considerable progress has been made to define which immunosuppressive drugs favour the expansion and function of Tregs. Currently a series of clinical trials of adoptive Treg therapy in combination with depletion of alloreactive T cells and short term immunosuppression are underway for human transplantation with the aim of minimizing immunosuppressive drugs and completely withdrawal.

Abstract:. The transplantation of donor hematopoietic tissue prior to organ xenografting has the potential to induce lasting T cell tolerance and, possibly, tolerance of natural antibody-producing B cells. However, the development of specific and nontoxic methods of overcoming the immunologic and physiologic barriers to xenogeneic marrow engraftment is a major challenge that must be met before this goal can be achieved. A greater understanding of the species specificity of molecular interactions important for hematopoiesis and cell homing is a first step toward transcending these physiologic barriers. Perhaps most promising is the potential associated with the use of nonprimate xenogeneic donors genetically engineered to make donor tissues more readily capable of surviving and, in the case of hematopoietic cells, competing with host tissues for survival in a human environment.
Prevention of kidney allograft rejection in rat by immune tolerance i nduction

Abstract:Objective　To investigate a clinically persp ective way of inducing transplantation tolerance.　Methods　Kidney tansplantation model was established using Lewis (RT1l) rats as recipients and DA (RT1a) rats as donor. Before transplantation the recipient rats were condi tioned with anti-lymphothyte serum (ALS) injected for three times, followed by 108 donor bone marrow cells transfusion (BMT) and injection of 100 mg/kg cyclo pho sphamide (CP), survival time of kidney allografts, mixed lymphocyte reaction (MLR ) and delayed-type hypersensitivity (DTH) of recipient to donor spleen cells we re observed and studied.　Results　5 of the 7 kidney allo grafts survived 73 to 90 days without sign of rejection. The donor specific MLR and DTH were suppressed.　Conclusions　Induction of trans plantation tolerance is a prospective way for the prevention of allograft reject ion.

Abstract:With the development of organ transplantation for nearly half a century,currently the hot issues in clinical renal transplantation have already shifted to the application of low toxicity immunosuppressive drugs and their projects,as well as the research of technique which induces low or no immunity response to the specific donor.At present the main effective strategies of inducing immune tolerance are donor specific blood transfusion and donor specific bone marrow transplantation.This article summarizes the background,mechanism and clinical application of these two strategies.

Abstract:Introduction: Regulatory T cells (Treg) have been shown to prevent allograft rejection and plays an important role in promote donorspecific antigenic tolerance in solid organ transplantation. In this study, we analyzed the ratio of CD4+CD25+Foxp3+/Th17 T cell in the peripheral blood of kidney recipients with living-related donor in different immunosuppressive therapy, and analyzed the relationship between the ratio of CD4+CD25+Foxp3+/Th17 T cell and patients who have long-term survival. Methods: The study was approved by the local ethical committee. Twelve renal transplant recipients with living-related donor received combined donor-specific blood transfusion and kidney transplantation more than six years were enrolled, and ten healthy volunteers as healthy control (HC). Posttransplant patients were divided into two groups depending on their immunosuppressive therapy: calcineurin inhibitors (CNI) and rapamycin (Rapa). Analyzed the expression of CD4+CD25+Foxp3+T cells and Th17 T cells in the peripheral blood under the stimulate condition (phorbol myfismte acetate and ionomycin) and no-stimulate condition for all objects by flow cytometry, and the mixed lymphocyte reaction (MLR) in recipient with their donors and independent third party by methyl thiazolyl tetrazolium (MTT). Results: The proportion of CD4+CD25+Foxp3+ T cells in the peripheral blood under the stimulate condition and no-stimulate condition in group of Rapa were 2.24% and 2.14%, respectively, significant higher than group of CNI (P< 0.05, Table 1), and, there was no significant difference for the proportion of Th17 T cells between the group of Rapa and CNI. The ratio of CD4+CD25+Foxp3+/Th17 T cells in group of Rapa under the stimulate condition and no-stimulate condition were 3.42 and 8.92, respectively, significant higher than group of CNI (P< 0.05, Table 1). In all groups, the result of MLR with independent third party higher than their donors, and the MLR with independent third party in group of Rapa was higher than group of CNI.[Table 1 The expression of Treg and Th17 T cells]Conclusion: This study showed in living-related donor kidney recipients who received combined donor-specific blood transfusion and kidney transplantation obtain better long-term graft and patient survival. Increased numbers of CD4+CD25+Foxp3+ T cells and the ratio of CD4+CD25+Foxp3+/Th17 T cells and higher MLR with independent third party in group of Rapa compared with group of CNI. Therefore, monitoring of Treg in the peripheral blood as a biomarker of tolerance may detect unresponsiveness to the graft.

Abstract:During the last 29 years the dramatic changes that have taken place in our transplant programs and in solid organ transplantation permit us to face tomorrow with courage and confidence. In the future we will be deeply concerned that the long-term benefit of more effective immunosuppression or novel strategies might be unfavorably impacted by the tendency to use organs whose intrinsic lifespan may be limited. We are deeply concerned by the inexorably increasing discrepancy between supply and demand for deceased kidneys, especially for the added morbidity that this inevitably entails for those patients with ESRD who do not have live donors. Our program continues to seek better ways to improve survival and the quality of the life of our patient population.

Abstract:Objective　 To compare the effects of different donor-specific antigens, through different pathways, on the inducement of immunological tolerance. Methods　 After the establishment of a stable rat cervical heterotopic heart transplantation model, donor specific transfusion (DST), donor specific spolenocyte (DSSL) and donor specific bone marrow (DSBM) were given to recipients 3-14 days prior to the transplantation through peripheral vein, portal vein and intrathymic injection respectiviely. Low dose CsA 5mg/(kg*d) was given from the 3rd-14th day pre-transplantation to the 28th day post-transplantation. The survival time of cardiac grafts was monitored, meanwhile, the donor specific mixed lymphocyte reaction (MLR) and the subsets of T lymphocytes CD8+ were determined. Results　 DSBM and the administrative pathway of intrathymic injection achieved the best effects on the inducement of immunological tolerance and the improvement of graft survival. Conclusion　 The donor specific antigens can induce the donor-specific immunological tolerance. It may be a practicable approach for the clinical experiments in the future.

Abstract:Purpose To study immune tolerance induced by immature dendritic cells transfusion and bone marrow transplantation in rat renal transplantation model,and to explore a possible strategy for renal allograft protection in clinic. Methods Immature dendritic cells (imDC) and fresh bone marrow cells (BMC) from DA rat were prepared for use before transplantation. Renal allograft from DA rat was transplanted to Lewis rat. Forty recipient rats were randomly divided into 5 groups: (1) negative group; (2) imDC group; (3) BMT group; (4) imDC+ BMT group; (5) cyclophosphamide (CP) group. Anti-donor cytotoxic T lymphocyte activity was assayed by 51Cr release test. Cytokine in the supernatant of splenic cells culture were measured by ELISA,including IL-4,IFN-γ and TGF-β. Results The median survival time (MST) was (7.12±1.25) days in negative group,(24.36±3.20) days in imDC group, (7.87±2.10) days in BMT group,(18.13±2.16) days in Cp group. When combined imDC transfusion and BMT,the MST was prolonged to (80.75±16.88) days, which had significant difference compared with negative group, imDC group,BMT group and CP group (P0.01).CTL from tolerant rats had decreased cytotoxicity to cells from donor rats (4.3±1.0)% vs (39.6±4.4)%,P0.01),but showed normal cytotoxicity to cell from the third party rat (9.8±3.2) %.IFN-γ in the supernatant of tolerant rat splenic cells culture was (532.78±34.54) ng/L,which was lower than normal and rejection rat (P0.01). IL-4 and TGF-β in the supernatant of tolerant rat splenic cells culture was (185.44±32.17) ng/L and (716.82±197.62) ng/L, which was higher than normal or rejection rat (P0. 01). Conclusions Combined used of imDC transfusion and bone marrow transplantation is an effective way to induce immune tolerance in rat renal transplantation model. Its possible mechanisms may due to immune deviation of cytokine net and rat donor-specific CTL anergy.

Abstract:To investigate and evaluate a clinically perspective way of inducing transplantation tolerance.Multi-cord blood transfusion was performed before kidney transplantation in 20 cases. Donor-specific mixed lymphocyte reaction(MLR) and allograft rejection were observed and studied.The donor-specific MLR was suppressed in every recipient. No allograft rejection took place in the cord blood transfusion group three months after kidney transplantation.Multi-cord blood transfusion is a practical way of inducing transplantation tolerance.

Abstract:While significant progress has been made in the 54 years since the world's first heart transplant in 1967, the longevity of heart transplants is limited by rejection and the innumerable complications of chronic immunosuppression. Abstract:Objective Abstract Objective To investigate the effect and mechanisms of transplantation tolerance induced by donor natural killer T cells (NKT) infused intravenously.Methods Skin of C57BL/6 mice was transplanted to BALB/c mice.Control group:normal saline was given;NKT group:BALB/c mice were given an intravenous infusion of (5×106) donor NKT before transplantation and Sodium Chloride were give after transplantation;CsA group:Ciclosporin(CsA)(4mg/kg) were given after transplantation;NKT+CsA group:NKT were given before transplantation and CsA were given after transplantation.Survival condition of grafted skin and general condition of mice were observed daily;14 days after induction of tolerance the MLR and DTH of tolerance BALB/c mice were examined to confirm the tolerance status.Adoptive transfer assay and cytokine mRNA expression in tolerant mice were performed to further explore tolerance mechanisms.Results Long-lasting skin allograft tolerance was achieved and the tolerance could be adoptively transferred.The levels of IL-10 and IL-4 mRNA in tolerant mice were much higher than those in the control mice.Conclusion Skin allograft tolerance of BALB/c mice could be induced by the infusion of donor NKT.Suppressor cells and deviation of Th1/Th2 cytokines were involved in the process of tolerance.
Study on the Induction of Transplantation Tolerance in Mice by Continuous Infusion of Donor Spleen Cells and Its Mechanisms

Abstract:Objective To study the mechanisms of tolerance induced by continuous infusion of donor derived cells to recipient mice. Methods BALB/c (H-2d) mice received an injection of allogeneic spleen cells via the tail vein from C57BL/6 (H-2b, B6) mice, followed by an intraperitoneal injection of cyclophosphamide (Cy) 2 days later. One day after the treatment of Cy, the mice were injected i.v. with B6 spleen cells for 5 days (one time/day, 107/time). Transplantation tolerance status of the recipient mice was tested by skin grafting and MLR. Adoptive transfer assay and analysis of cytokine mRNA expression in both tolerant and rejected mice were performed to further explore tolerance mechanisms. Results Skin graft tolerance of BALB/c mice to B6 mice was achieved and the levels of chimerism in thymus were related to the state of tolerance; The levels of IL-2 and IFN-γ mRNA expression in tolerant BALB/c mice were much lower than those in the control mice, the levels of IL-10 mRNA in tolerant mice were much higher. Conclusions Treatment of mice with donor spleen cells plus Cy and continuous injection of donor spleen cells can induce a specific tolerance in BALB/c mice to B6 skin graft. Multiple mechanisms, including chimerism, suppressor cells, and deviation of TH1/TH2 cytokines were probably involved in the process.

Abstract:O334 Purpose: Tolerogenic dendritic cells (Tol-DC) play an important role in transplant tolerance through initiating a self-maintaining regulatory feedback loop with CD4+ CD25+ T regulatory cells (Treg) (J Immunol 2003, 170:1304) based on experiments in vitro. This study was designed to evaluate the in vivo immunomodulatory ability of Tol-DC using a murine allogeneic heart transplant model. Methods: Dendritic cells were cultured from donor strain (C57/BL6, H-2b) bone marrow progenitors. Tol-DC were generated by culture in medium conditioned with LF15-0195, a tolerance-inducing NF-κB inhibitor. Intravenous administration of Tol-DC to recipients (Balb/c, H-2d) was performed 7 days prior to MHC-fully mismatched heterotopic heart transplantation. The abilities of Tol-DC to induce immune modulation were assessed in vitro and in vivo. Results: In vitro-generated Tol-DC possessed tolerogenic properties as measured by decreased expression of MHC II and co-stimulatory molecules, as well as, impaired allo-stimulatory capacities. Administration of in vitro-generated Tol-DC (5x106 cells) prior to transplantation resulted in significant prolongation of allograft survival (MST 48 ± 14 days), as compared to the survival of recipients treated with the same number of control DC (MST 13 ± 1 day). In addition, CD4+CD25+ T cells were significantly increased in the Tol-DC treated recipients but not in control DC treated recipients. CD4+CD25+ T cells isolated from Tol-DC treated recipients had regulatory ability in an in vitro inhibitory MLR assay. Furthermore, the cytokine profile of T cells in the Tol-DC treated recipients revealed a Th2 differentiation. Conclusions: In vitro-generated Tol-DC initiate a regulatory feedback loop in vivo through promotion of Treg generation and Th2 differentiation, which subsequently prevents allograft rejection.
