Intensively gargling with calcium formyltetrahydrofolate to relieve oral mucositis caused by hematopoietic stem cell transplantation

Abstract:Objective To observe the impact of gargling with calcium formyltetrahydrofolate(CF) on the morbidity of oral mucositis in patients undergoing hematopoietic stem cell transplantation. Methods Twenty-eight patients who underwent hematopoietic stem cell transplantation received pretreatment of gargling with CF. They were divided into intensive gargling group(from June 2010 to September 2011, n=15)and non-intensive gargling group(after September 2011, n=13) according to the frequence of application of CF. Antibacterial drugs were routinely given by oral from pretreatment to hematopoietic reconstitution after transplantation. During the treatment of methotrexate to prevent graft-versus-host (GVHD), CF(100 mg) and normal saline(500 ml)were given to gargle as well as common gargle liquids. In non-intensive gargling group, patients gargled with CF liquid optionally. In intensive gargling group, patients gargled with intensive CF liquid for over 3 min every 2 h. The incidence, severity and duration of oral mucositis were compared between two groups. Results The incidence of oral mucositis in intensive gargling group and non-intensive gargling group was respectively 69% and 73%. There was no significant difference between two groups(P>0.05). But 62% patients in intensive gargling group suffered mild oral mucositis(Ⅰ~Ⅱ stage), while 60% of patients in non-intensive gargling group suffered severe oral mucositis(Ⅲ~Ⅳ stage). The duration of oral mucositis in the intensive gargling group was (5.5±2.5)d, while that in the non-intensive gargling group was(18.9±7.8)d. There was significant difference between two groups(P<0.05). No significant adverse reaction was observed in two groups. Conclusions Intensively gargling with CF liquid everyday during treatment can significantly reduce the incidence of oral mucositis and alleviate their suffering in patients undergoing hematopoietic stem cell transplantation.
