Abstract:Aortic dissection is a life-threatening emergency disease, which needs immediate and effective clinical interventions. In recent years, owing to the widespread use of multidetector computed tomography, the rate of diagnosis of aortic dissection has significantly improved. As a convenient and noninvasive examination, multidetector computed tomography angiography has been used widely in the preoperative diagnosis of aortic dissection and postoperative follow-up. Improving CT scan protocols and powerful post-processing function make the diagnosis of aortic dissection more accurate and provide adequate evidences for clinical treatments. This review summarized the recent application and research progress of multidetector computed tomography angiography in the diagnosis of aortic dissection and its future perspective.
64-Slice Spiral CT Angiography in the Diagnosis of Aortic Dissection

Abstract:Objective:To investigate the clinical value of multi-slice spiral CT angiography(MSCTA) in the diagnosis and preoperative assessment of aortic dissection(AD).Methods:Fifty-eighty consecutive patients with AD underwent 64-slice spiral CT angiography.The original data were post-processed with MPR,MIP and VR techniques and the results were analyzed.Results:Based on Debakey classification,the MSCTA results of these fifty-eight patients were classified as TypeⅠ(14 patients),TypeⅡ(2 patients) and Type Ⅲ(42 patients).True and false lumen and intimal flap tearing could be clearly revealed in all of these 58 patients.Furthermore,the mean CT value after enhancement in true lumen is higher than that in false lumen in arterial phase,with significant statistical difference(P=0.0000.05).Initial entry sites were well defined in all patients,and one or multiple re-entry sites were shown in 55 patients,but did not showed in 3 patients with Type Ⅲ dissection.In 39 patients with Type Ⅲ dissection,the initial entry site located in descendent part of aortic arch or proximal part of thoracic aorta,the length and the width of proximal neck of aortic aneurysm were ranged from 0.4cm to 10.8cm(mean,3.5cm) and ranged from 2.2cm to 3.6cm(mean,2.8cm) respectively,and the maximum diameter of aortic aneurysm was between 3.2cm and 9.2cm(mean 5.4cm).All of the main branches of aorta involved by dissection and the thrombus within false lumen were accurately showed by MSCTA Conclusion:The advantages of 64-slice spiral CT angiography are fast,non-invasive and highly effective which is one of the reliable imaging methods in the diagnosis and preoperative evaluation of aortic dissection.

Abstract:Objective To investigate the clinical applied value of multi-slice spiral CT(MSCT) in acute aortic syndrome(AAS).Methods Sixty-two patients who complained of acute pain in the chest and back were scanned by 16-slice spiral CT. Results Sixty-two cases included 41 cases of aortic dissection(AD),12 cases of intramural hematoma(IMH) and 9 cases of penetrating atherosclerotic ulcer(PAU). MSCT could depict the imaging features of AAS as follows:the intimal flap,true and false lumens in AD;crescent or circular thickening of aortic wall in IMH; a niche filling with contrast medium and protruding the aortic lumen.Conclusion MSCT is a fast and noninvasive examination technique,it can provide important information for the diagnosis of AAS.

Abstract:Purpose: To evaluate 16 slices spiral CT in the diagnosis of aortic dissection(AD).Materials and Methods:10 cases with AD were performed by 16 slices spiral CT angiography,all data were processed with 2D and 3D reconstructions,the main methods included:MPR,CPR,SSD,MIP and VR.Results:3 Debakey type Ⅰ,3 cases,type Ⅱ,2 cases and type Ⅲ,5 cases of aortic dissection wee found to show specific signs,i.e.intimal flaps and true and flase double aortic lumen.The non-specific signs included thicking and calcification of the aortic wall,irregular aortic dilation and intraluminal thrombus.MPR and CPR could clearly show the extent,size and details of lesions.The space relationship and calcifation of aorta were displayed in SSD,MIP and VR.Conclusion:16 slices spiral CT is an effective and non-invasive method in diagnosis,classification and locationof intimal rupture of AD.

Abstract:Objective To explore the clinical features and etiology of aortic dissection,CT diagnosis about aortic dissection.Methods Fifty-seven aortic dissection patients treated in our hospital were retrospectively analyzed.The clinical and pathological features of aortic dissection were summarized,and the aortic dissection results detected by CT scan and CT angiography imaging scan compared.Results The clinical symptoms of aortic dissection patients were sudden pain(52 cases,91.2%)and hypertension(48 cases,84.2%).In 57 aortic dissection patients,Stanford A type 39 cases,Stanford B 18 cases,Debakey Ⅰ,Ⅱ type 27 cases,Debakey Ⅲ type 30 cases;CT angiography imaging detection was significantly higher than CT scan in detection rate(χ2 =4.940,P <0.05).Conclusion Aortic dissection is a highly lethal disease,hypertension may be the main predisposing factor.And CT scan and CT contrast-enhanced scan have high sensitivity and specificity,and also can provide a fast and accurate diagnosis for aortic dissection.

Abstract:Post-processing techniques of computed tomography imaging are playing a dominant and critical role in evaluation of AD. It is vivid for showing the whole contour of aorta, from different view. Post-processing techniques not only provide information of location, but also provide information of invading range. It is referred as preferred investigative modality for the diagnosis of aortic dissection, and has been widely used in clinic. Recently, it has become the first choice for clinical to diagnose AD, then it could provide important information for clinical selection of AD of surgical methods.

Abstract:Objective:To present the CT findings of aortic intramural hematoma(AIH) and determine the diagnostic value of multislice spiral CT(MSCT) for AIH.Methods:Twenty-one patients who complained of acute pain in the chest and back were scanned by 16-slice spiral CT and were diagnosed as AIH.Results:Five patients were diagnosed as having Stanford type A,the others as type B.The direct features of AIH were crescent or circular thickening of aortic wall without intimal flap as well as true and false lumen.The indirect features included: calcification ingression(5 cases),penetrating ulcer(9 cases),atherosclerosis(13 cases) and leakages(5 cases).The complication features included: pericardial effusion(3 cases),pleural effusion(10 cases),involvement of the large branches(3 cases),aortic dissection(6 cases) and aneurysm(2 cases).Conclusion:MSCT is a fast and noninvasive examination technique.It can provide important information for the diagnosis and treatment of AIH.MSCT can be used as the first means in the diagnosis and following-up of AIH.
The CT Imaging of Atypical Aortic Dissection

Abstract:Objective To discuss the CT imaging of atypical aortic dissection(AAD). Materials and Methods 24 cases of AAD were found by spiral CT scan before and after enhancement,11 cases were treated surgically.Results (1)AAD had no typical aortic dissection CT imaging features:no split intimal flap and double channels; (2)The aortic wall was crescentic or circumferential incrassation (false lumen) to 5～30 mm; (3)On unenhanced CT scan, the density of false lumen washigher or isodensity or lower than that of true lumen respectively in 12 cases, 8 cases and 4 cases; (4)The false lumen had nonenhancment on postcontras CT scan; (5)22 cases(92%)were lack of serious depression in the involved aortic lumen; (6)Some changes were shown in atypical false lumen with follow-up CT scan. There were some common features in AAD and AD as follows: (1)The location of the AAD was consistent with the typical aortic dissection; (2)medial displacement of intimal calcifications were found in 4 cases; (3)63% of cases were accompanied with pleural effusion (4)67% of cases with pericardial effusion in type A dissection; (5)The entry was difficult to indentify, which was found only in 36% cases corresponding with operation. Hyperattenuation in 6 false channels on unenhanced CT scans was shown as fresh thrombus existed between media and intima on operation, and the hypoattenuation in other 5 cases was older thrombus. 6 AAD were absorbed on follow-up scans. Conclusion The typical image features in AAD were no split intimal flap and double channels, and the aortic wall was crescentic or circumferential incrassation. On unenhanced CT scan, the density of false lumen was higher or isodensity or lower than that of true lumen. The false lumen will be not enhanced, conservative treatment is effective.

Abstract:AIM:To investigate the clinical applicable value of multi-slice spiral CT angiography in aortic dissection. METHODS: 16-slice CT angiography was performed on 53 patients with aortic dissection. The volume data of all patients were reconstructed with the 3D techniques including multiplanar reconstruction (MPR), curved planar reformation (CPR), maximum intensity projection (MIP) and volume rendering (VR). RESULTS: Fifty-three cases included 15 cases of type I, 2 cases of typeⅡand 36 cases of type Ⅲ. Among them, 12 cases were aty-pical aortic dissection. Transverse images clearly showed intimal flap, entry tears and origination of branches. 3D reconstruction allowed a better definition of anatomical details. CONCLUSION: Multi-slice spiral CT angiography can fully display the details of aortic dissection and is therefore of important value for the diagnosis and preoperative and postoperative evaluations.
[Relationship Between Multi-Slice Spiral CT Angiography Imaging Features and In-Hospital Death of Patients with Aortic Dissection].

Abstract:Objective: To explore the imaging manifestations of multi-slice spiral CT angiography (CTA) and relationship with in-hospital death in patients with aortic dissection (AD). Methods: The clinical data of 429 patients with AD who underwent CTA in Zhongshan Hospital of Fudan University between January 2009 and January 2016 were retrospectively analyzed. AD patients were divided into 2 groups, including operation group who underwent surgery or interventional therapy (370 cases) and non-operation group who underwent medical conservative treatment(59 cases). The multi-slice spiral CTA imaging features of AD were analyzed, and multivariate logistic regression analysis was used to investigate the relationship between imaging manifestations and in-hospital death in AD patients. Results: There were 12 cases (3.24%) of in-hospital death in operation group, and 28 cases (47.46%) of in-hospital death in non-operation group(P<0.001). AD involved different vascular branches. Multi-slice spiral CTA can clearly show the dissection of true and false lumen, and intimal tear was detected in 363 (84.62%) cases, outer wall calcification was revealed in 63 (14.69%) cases, and thrombus formation was present in 227 (52.91%) cases. The multivariate logistic regression analysis showed that the number of branch vessels involved (OR=1.374, 95%CI 1.081-1.745, P=0.009) and tearing false lumen range(OR=2.059, 95%CI 1.252-3.385, P=0.004) were independent risk factors of in-hospital death in AD patients, and the number of branch vessels involved (OR=1.600, 95%CI 1.062-2.411, P=0.025) was independent risk factor of in-hospital death in the operation group, while the tearing false lumen range (OR=2.315, 95%CI 1.019-5.262, P=0.045) was independent risk factor of in-hospital death of non-operation group. Conclusions: Multi-slice spiral CTA can clearly show the entire AD, true and false lumen, intimal tear, wall calcification and thrombosis of AD patients. The number of branch vessels involved and tearing false lumen range are the independent risk factors of in-hospital death in AD patients.

Abstract:Objective To investigate the clinical value of dual-source CT(DSCT) angiography in the diagnosis and preoperative assessment of aortic dissection.Methods Thirty-five patients with aortic dissection from Jul 2009 to Feb 2011 were retrospectively analyzed.Imaging reconstruction and analysis of all patients were finished at the workstation.The reconstruction techniques included multiplanar reconstruction,maximum intensity projection,and volume rendering technique.The extension,type,and involvement of important branches of aortic dissection were focused.Results According to involvement extension of aortic dissection,there were 13 cases of DeBakey type I,4 cases of DeBakey type II and 18 cases of DeBakey type Ⅲ.Lacerated intimal flap and initial entry site of aortic dissection were observed in all patients,and involvements of main branches of aorta were accurately showed.Conclusions DSCT angiography should be one of important imaging Methods in diagnosis and preoperative assessment of aortic dissection,for its simple operation and accuracy.
Clinical application of pulse-gated non-enhanced rapid magnetic resonance imaging in the definitive diagnosis of aortic dissection

Abstract:Objective: Aortic Dissection (AD) is one of the most fatal acute diseases in cardiovascular diseases, with rapid onset and progression and a high fatality rate. This study aims to investigate the clinical values of non-enhancement peripheral pulse-gating rapid magnetic resonance imaging in deterministic diagnosis of AD. Methods: Aorta magnetic resonance imaging was performed in 21 healthy volunteers at a 1.5t MR scanner sequences including cardiac-gated and peripheral pulse-gated True-FISP and HASTE were carried out separately. Acquisition Time (TA), Signal to Noise Ratio (SNR), Contrast Noise Ratio (CNR), and entirety of vessel wall blood flow artifacts were measured and compared. A total of 56 AD cases were displayed by non-enhancement peripheral pulse-gating fast MR imaging, and the results were compared with pathological findings or CTA of the aorta. The dissection rupture, tear film, true and false lumen, thrombosis, hydropericardium, and the main branches of AD were evaluated respectively. Results: There were no significant differences in SNR, CNR, entirety of the vessel wall, and blood flow artifact between cardiac-gated and peripheral pulse-gated fast MR imaging. Non-enhancement pulse-gated fast scanning takes less TA time. By the pulse-gated non-enhancement fast MR imaging, the dissection rupture, tear film, true and false cavity, thrombosis, hydropericardium, and the main branches of aortic dissection were shown clearly. Multi-planar and multi-angle scans helped to show the extent of entrapment rupture, whereas partial complex tears or bi-directional tears were slightly less well visualized. Conclusion: Non-enhancement peripheral pulse-gated rapid magnetic resonance imaging can be used for deterministic diagnosis of AD.
Diagnosis of Dual-source CT in Acute Aortic Dissection

Abstract:Purpose To explore the value of 128-slice dual source CT (DSCT) three-dimensional post-processing techniques in the diagnosis of acute aortic dissection (AAD). Materials and Methods All image data of 116 patients with AAD who underwent conventional and enhanced DSCT scan by dual-energy scanning technology were retrospectively analyzed, and the multi-planar reconstruction (MPR), volume rendering (VR) and maximum intensity projection (MIP) were conducted in the workstation. Taking digital subtraction angiography (DSA) as the diagnostic gold standard, we analyzed the imaging manifestation of the original and 3D-reconstruction images and evaluated the specificity and the sensitivity of diagnostic accuracy and the image quality. Results The diagnostic accuracy of conventional scan was 37.1%(43/116). The display rates of MPR for initial break, intimal flap and true and false lumen were 93.1%, 100.0%and 100.0%, respectively. The display rates of VR and MIP for the initial break were 33.62%and 6.90%, respectively, which was both lower than that of MPR. The display rate of MIP for the true and false lumen was 23.28%. The overall display capability of MPR was significantly better than that of VR and MIP (P<0.01), and the display capability of VR was better than that of MIP (P<0.01). The sensitivity and specificity of enhanced DSCT in the diagnosis of AAD were both 100.0%. Conclusion DSCT has a fast and reliable diagnostic value on AAD. Conventional CT signs should be highly valued in the evaluation of chest or abdominal pain; and a thin layer of MPR and VR should be chosen in the three-dimensional reconstruction;whilst MIP reconstruction may be unnecessary for AAD.
Study on screening and diagnosis of aortic dissection based on non-enhanced CT and deep learning

Abstract:Objective To develop a machine learning algorithm to detect aortic dissection on non-contrast-enhanced CT and evaluate the diagnostic ability of the algorithm compared with those of radiologists. Methods This study developed a machine learning algorithm using single-center data collected between May 1st, 2022, and April 30th, 2023. Included in the study were 220 patients (110 with AD and 110 without AD). An AD detection algorithm was developed using a 3D full-resolution U-net architecture. We have continuously trained and developed an algorithm based on machine learning to segment the true and false lumens of the aorta and then determine whether there is aortic dissection. The diagnostic capabilities of our algorithm and three radiologists were also compared. Results The developed algorithm achieved an accuracy of 95.8%, a sensitivity of 93.2%, and a specificity of 92.6%. For radiologists, accuracy, sensitivity, and specificity were 88.6%, 90.6%, and 94.2%, respectively. The algorithm's performance was not significantly different from the mean performance of radiologists in terms of accuracy, sensitivity, or specificity. Conclusion The proposed algorithm showed comparable diagnostic performance to radiologists for detecting AD on non-contrast-enhanced CT, which suggests that the proposed algorithm has the potential to reduce misdiagnosis of AD to improve clinical outcomes.Aortic SegmentationAlgorithm development

Advanced Warning of Aortic Dissection on Non-Contrast CT: The Combination of Deep Learning and Morphological Characteristics

Abstract:Background: The identification of aortic dissection (AD) at baseline plays a crucial role in clinical practice. Non-contrast CT scans are widely available, convenient, and easy to perform. However, the detection of AD on non-contrast CT scans by radiologists currently lacks sensitivity and is suboptimal. Methods: A total of 452 patients who underwent aortic CT angiography (CTA) were enrolled retrospectively from two medical centers in China to form the internal cohort (341 patients, 139 patients with AD, 202 patients with non-AD) and the external testing cohort (111 patients, 46 patients with AD, 65 patients with non-AD). The internal cohort was divided into the training cohort ( n = 238), validation cohort ( n = 35), and internal testing cohort ( n = 68). Morphological characteristics were extracted from the aortic segmentation. A deep-integrated model based on the Gaussian Naive Bayes algorithm was built to differentiate AD from non-AD, using the combination of the three-dimensional (3D) deep-learning model score and morphological characteristics. The areas under the receiver operating characteristic curve (AUCs), accuracy, sensitivity, and specificity were used to evaluate the model performance. The proposed model was also compared with the subjective assessment of radiologists. Results: After the combination of all the morphological characteristics, our proposed deep-integrated model significantly outperformed the 3D deep-learning model (AUC: 0.948 vs. 0.803 in the internal testing cohort and 0.969 vs. 0.814 in the external testing cohort, both p < 0.05). The accuracy, sensitivity, and specificity of our model reached 0.897, 0.862, and 0.923 in the internal testing cohort and 0.730, 0.978, and 0.554 in the external testing cohort, respectively. The accuracy for AD detection showed no significant difference between our model and the radiologists ( p > 0.05). Conclusion: The proposed model presented good performance for AD detection on non-contrast CT scans; thus, early diagnosis and prompt treatment would be available.

Clinical Application of CT Virtual Endoscopy in the Diagnosis of Aortic Diseases

Abstract:Objective To evaluate the display ability and the clinical application of CT virtual endoscopy (CTVE) in the diagnosis of aortic disease. Methods In this group, 147 cases with clinically suspected aortic disease (113 males, 34 females) underwent CTVE and 58 cases had DSA for comparison. 37 cases were confirmed by operation. Results (1)In the 147 cases ,139 cases showed successful CTVE images. (2)CTVE images could clearly show the inner wall and inner structures of the vessels, the true and false lumina of aortic dissections, and the lesion of false aneurysm. Conclusion CTVE could provide more information of the vessel by displaying the inner wall and allowing users to map the vessels in a display. It is helpful in correct diagnosis and guiding surgery.
Multi-slice CT Angiography: Its Application in Postoperative Check of Aortic Disease

Abstract:Objective To evaluate multi slice CT angiography in the reexamination of aortic disease after endoluminal stent grafts therapy or arterial bypass.Materials and Methods Multi slice CT angiography was performed in 34 cases after surgical therapy for aortic disease. 3D reconstruction techniques, including volume rendering (VR) SSD and MIP, were adopted, and the images thus obtained were compared with each other. Results Of 34 cases in this study, 23 had abdominal or thoracic aortic aneurysms or dissection and received endoluminal stent implantation, 7 received artificial artery replacement, and 4 received both stent implantation and arterial bridge. Perigraft leakage was occurred in 2 cases, and both displayed on VR images. In one case, the perigraft leakage could be revealed only on SSD and MIP images. VR images were superior to MIP and SSD in revealing the location and shape of the stent or artificial vessels.Conclusion Multi slice helical CT can rapidly scan a large scale in a very short time, which is favorable for examining the aortic diseases. VR is superior to SSD and MIP in its affording more postoperative information.

Abstract:Objective To explore the MRI diagnostic features of aortic dissection (AD).Methods MRI plain scannings, MRI enhancement scannings, MRI film imaging, and MRI angiography were performed in all 32 cases with AD. Results The true and false channels, the intimal flap, the thrombus, the location of the rupture and the aortic coverage were analyzed. According to the classification of DeBakey,6 cases were of typeⅠ;4 were of typeⅡ;22 were of typeⅢ.Conclusion MRI scanning has more important value for diagnosis,classification and clinical treatment of the dissecting aorta than other examinations.
Acute Aortic Dissection Type A with Acute Coronary Involvement: A Novel Classification

Abstract:Background: Acute coronary involvement (ACI) due to acute aortic dissection (AAD) type A is potentially fatal. We examined selected patients with AAD type A, which had evolved over 14 years, and acute coronary involvement. The purpose of this study was to determine the characteristics of patients with ACI due to AAD type A.Methods: Between 1997 and 2011, we recruited 20 patients (14.1%) with ACI (14 men, 6 women; mean age: 51.8 +/- 11.8 years; age range: 35-79 years) from 142 patients who had undergone surgical repair of AAD type A.Results: We propose a novel 4-category classification scheme based on the surgical pathological findings. The right coronary artery was involved in 15 patients, and the left was involved in 5 patients. Fourteen patients had preoperative myocardial ischemia. In the other 6 patients, acute coronary involvement was found intra-operatively. Patients with ACI were significantly younger than those without ACI (51.8 +/- 11.8 vs. 61.0 +/- 11.8; p = 0.001), a lower prevalence of intramural hematoma (5.0% vs. 32.8%; p = 0.011), a higher aortic regurgitation rate (95.0% vs. 53.5%; p = 0.001). Patients presenting with ACI had an in-hospital mortality rate of 20.0% (4/ 20), while those without ACI had an in-hospital mortality rate of 19.7% (24/ 122).Conclusions: Acute coronary involvement due to AAD type A is not always associated with coronary malperfusion. Patients with ACI were much younger, had a higher aortic regurgitation rate, and, less commonly, had intramural hematoma. This new classification scheme would make it more convenient for surgeons to decide on treatment options for this special cohort. (C) 2013 Elsevier Ireland Ltd. All rights reserved.

Abstract:Conventional computed tomography (CT) with intravenous contrast injection effectively demonstrates the features of aneurysms of the thoracic aorta (dilatation, calcification, intraluminal thrombus, and displacement and erosion of adjacent structures). In aortic dissection, CT can establish the diagnosis by demonstrating (a) double channels with an intimal flap or (b) displaced intimal calcifications if one channel is thrombosed. Dynamic CT following a contrast bolus shows the relative rate of filling of the true and false channels and demonstrates the intimal flap with optimum clarity. Reformatted CT images (paraxial and oblique) display the entire thoracic aorta in the plane of the arch, resembling an aortogram. CT is noninvasive and can be easily repeated to assess progress. Although aortography is still required prior to surgery in some cases, CT is a safe screening procedure that may avoid some invasive angiographic procedures.
