Current progress on etiology and clinical treatment of bullous keratopathy

Abstract:Bullous keratopathy(BK) is the late(phase condition of endothelial decompensation.The corneal edema developed,associated with bubbles in the epithelium.It is a common corneal disease which has severe clinical symptoms such as irritation of the ocular surface,photophobia,ophthalmalgia,bad visual power,even blinding complication.It is increasingly observed after some intraocular surgeries,ocular trauma and endophthalmitis,but has had no excellent therapies up to now.Today,penetrating keratoplasty (PKP) is the main,safe and effective procedure for treatment of BK and plays an important role in rescuing eyeballs and improving ocular acuity.However,PKP is restrained by the deficiency of corneal resources and the postoperative complications such as high or irregular astigmatism and graft rejection in itself.During the last decade,a novel approach has been developed.It is deep lamellar endothelial keratoplasty (DLEK) which offers considerable theoretical advantages over PKP in treatment of endothelial dysfunction and is being practiced and evaluated clinically.The study of culturing the corneal endothelium has been realized successfully.DELK may replace the conventional PKP for patients with endothelial dysfunction.Available literature related to BK,the etiology and clinical treatments are reviewed in this article.
