Abstract:Objective To observe the effects of a low dose of ketamine on related symptoms of acute stress disorder in patients undergoing orthopedic surgery and the positive rate of post-traumatic stress disorder checklist-civilian version(PCL-C).Methods Sixty-six fracture patients diagnosed as acute stress disorder were divided randomly and equally into ketamine group(group K)and saline group(group C).The patients in group K or group C received intravenous infusion of ketamine(0.5 mg/kg with the additional volume of saline to total 50ml)or equal volume saline during 40minutes via an infusion pump after intravertebral anesthesia, respectively;the related symptoms of acute stress disorder in all the patients were evaluated by acute stress disorder scale(ASDS),self-rating anxiety scale(SAS)and self-rating depression scale(SDS)on the day before operation, the 1st,3rd, and 7th day after operation. In addition, after 3months,all patients were also evaluated by PCL-C via telephone follow-up.Results The scores of ASDS had no significant difference between groups K and C on the 1st day after operation. The scores of ASDS in group K were lower than those in group C on the 3rd and 7th day after operation(P<0.05).The scores of SAS and SDS in group K significantly decreased compared with group C on the 1st,3rd and 7th day after operation(P<0.01).The positive rate of PCL-C in group K was lower than group C,but not significant. Conclusion The related symptoms of acute stress disorder in the fracture patients with acute stress disorder was obviously ameliorated by the intravenous infusion of a low dose ketamine.
