Stent angioplasty for very serious stenosis of extracranial internal carotid artery(report of 28 cases)

Abstract:Objective To enhance the safety of carotid artery stenting and its effect on very serious stenosis of extracranial internal carotid artery.Methods The clinical data of 28 patients with very serious stenosis of the extracranial internal carotid artery treated by carotid artery stenting from October,2002 to October,2009 were analyzed retrospectively,including the clinical characteristics,clinical manifestation during the perioperation period,complications related to treatment and follow-up after the stenting.Results Stent angioplasty was successfully performed in all the patients.The decrease in degree of the stenosis of the internal carotid arteries from 90% ~99% before the treatment to 0% ~20% after the treatment was made in the patients,in whom no ischemic event related to stenting occurred,and 3(10.7%) patients died of cerebral hemorrhage due to the over-perfusion within 6 hours after the stenting.The following up from 1 to 17 months showed that no ischemic symptom happened in all the survival patients.Conclusions The ischemic symptom can effectively be improved by the stent angiography in the patients with very serious stenosis of the extracranial internal carotid artery,in whom the rate of cerebral over-reperfusion and hemorrhage after the stenting is relatively higher and the better method to prevent cerebral over-perfusion and hemorrhage after the stenting should be further studied.
