Ultrasonography-Guided Multiple Tracts Pertutaneous Nephrolithotomy for Renal Staghorn Calculi

Abstract:Objective:To evaluate the efficacy and safety of ultrasonography-guided multiple tracts pertutaneous nephrolithotomy(PCNL) for the renal staghorn calculi.Methods:We retrospectively analysed the clinical data of 163 patients(130 males and 33 females,163 renal units) who underwent multitract PCNL under ultrasound guidance for staghorn calculi from 2004 to 2008.Of the 163 renal units,105 had a complete staghorn and 58 had a partial staghorn.We divided the patients into two groups,including 131 cases(groupⅠ) treated by multitract PCNL in two sessions and the next 32(groupⅡ) treated by multitract PCNL in a single session.Results:There was no major complication intraoperatively or postoperatively.From groupⅠand groupⅡ,respectively,the stone clearance rate without auxiliary procedures was 81.7%and 87.5%respectively,median operative time was 146 minutes(range,130 to 240 minutes) and 115 minutes(range,90 to 160 minutes),median length of hospital stay was 17 days(range,14 to 23 days) and 10 days(range,7 to 15 days),and blood transfusion rate was 6.1%(8 cases) and 9.4%(3 cases).The patients in groupⅡwere found to have reduced operative times,and length of hospital stay.However,there was no significant difference in the blood transfusion rate and stone clearance rate between two groups,respectively.Conclusions:Ultrasonography-guided multiple tracts pertutaneous nephrolithotomy is a safe,effective,and minimally invasive approach for the management of staghorn calculi.With increasing experience,multitract PCNL in a single session can be used more often without increasing the risk of significant complications.
