Abstract:The prevention and cure work of Leprosy was an important page in new China's history and the current domestic academic writings are mostly related to the biological/medical research paths.This paper adopts humanities/social sciences research paths,regards disease and people's opinions on disease as a historical phenomenon and disease prevention and control treatment work as a widely-influenced social movement,focuses on intrinsic associations among technological factors and economic factors and social consciousness and policy.This article believes that despite there were significant differences at guiding principles,management system and specific approach about China's prevention and treatment work of Leprosy around reform and opening up,improving China' s public hygiene situation and eliminating disease to people's health were the original intention of all levels of the party and the government organs.The smooth realization to the goal of"basic elimination"fully reflected the party's leadership and the superiority of the socialist system.

Abstract:Leprosy is associated with the occurrence of various skin lesions such as macules, papules, plaques, nodules, and even diffused infiltration, depending on the patient's immune response. Its clinical presentation is often different from the usual pattern, leading to confusion in diagnosis. We present a case of leprosy with ANA positive that was mistaken for connective tissue disease. In this case, we want to tell that doctors should not only depend on laboratory index for diagnosis, the misdiagnosis may lead to delaying illness and aggravating illness.

Abstract:The incidence of leprosy has decreased significantly in Hong Kong since the 1970s,1 and medical students today will have little exposure to this once-dreaded disease. Leprosy has been described in ancient Chinese medical texts for centuries, but it was during the Ming and Qing dynasties that it became a highly feared and stigmatised condition due to the deformities and purported contagiousness.2 Although the pathogen Mycobacterium leprae was discovered in 1873, it took many years to educate both doctors and the public about the true nature of the disease. Dr James Cantlie, one of the founders of the Hong Kong College of Medicine, was the first to study patients with leprosy who presented to the Alice Memorial Hospital.3 In 19th-century Hong Kong, those with leprosy, whether residents or immigrants, were not welcome and would be repatriated to mainland China by persuasion or force. In the Lepers Ordinance of 1910, it was decreed that all lepers in the Colony, once identified, would either be deported or segregated to Kennedy Town Infectious Disease Hospital by the police. Compulsory segregation was lifted by later legislation (the Lepers Ordinance of 1935) and treatment by private practitioners or outpatient clinics was allowed if deemed medically suitable.4 Chinese patients would be sent to a leper asylum operated by Catholic missionaries in Sheklong (石龍), which received financial support from the government.2

Abstract:Leprosy, also known as Hansen's disease, is a chronic infectious disease caused by Mycobacterium leprae , or less commonly by Mycobacterium lepromatosis [ 1 ] both of which cause similar pathological changes. M leprae belongs to the family Mycobacteriaceae and genus Mycobacterium . [ 2 ] It preferentially invades peripheral nerve dermal histocytes and Schwann cells. [ 2 , 3 ] M leprae is slow growing (approximately 12–14 days) and can survive outside the human body for 46 days. [ 4 , 5 ] The incubation period for leprosy is long and broad, with 2 to 20 years estimates. [ 5 ] M leprae prefers to grow at lower temperatures (27–33 °C); thus, it predominantly affects areas in the human body at lower temperatures. [ 4 , 5 ]

Abstract:This study quantitatively investigated word frequencies in parallel Chinese texts at three different periods, namely, Classical Chinese (1855), Beijing Mandarin (1908) and Modern Mandarin (2010), attempting to shed some light on the evolution of the Chinese language over the last 150 years. With the evolution of Chinese: both the types and tokens increased diachronically, and sharp changes can be found in the high-frequency function words, vocabulary richness, and the distribution of word classes. The syntactic structure has also become increasingly complex. In addition, although the length of words (measured in character) has increased diachronically, disyllabic words have not always been preponderant in Chinese after the opium war of 1840–1842. The results show that the complication is a crucial trend in the evolution process of Chinese.

Abstract:Zhejiang Wukang sanatorium evolved from Hangchow C. M.S. Hospital, which was founded in 1887. It is the leprosy prevention and treatment center of Zhejiang Province after the founding of the People's Republic of China. Most of the leprosy patients (convalescents)in sanatoriums entered the hospital successively in 1960s. Due to the limitation of dapsone monotherapy and the poor living conditions, the convalescents need to receive long-term isolation treatment and engage in collective production. After the implementation of Reform and Opening-up policy, the leprosy patients' health care has been greatly improved. The leprosy prevention and treatment course has been gradually reduced; the widespread knowledge of leprosy gradually freed leprosy patients from the dilemma of being stigmatized. In the post-leprosy era, Wukang sanatorium changed from the original focus on disease treatment to strengthening the psychological construction of patients. Its function changed from isolation and prevention to rest and pension, and the status of leprosy patients changed from "patients" to "convalescents" . These changes reflect the humanistic care and guidance of destigmatization in the process of leprosy isolation and epidemic prevention from prevention to treatment to the aged.

Abstract:Memory Studies, Volume 17, Issue 1, Page 86-102, February 2024. A recent revisit of the history of leprosy in East and Southeast Asia led to the establishment of museums of Hansen's disease. Given that the history of leprosy has been a touchy subject due to its social stigma and complicated colonial past, these museums become spaces for the curious to comprehend memories of the forgotten past. In this article, we investigate contradictory purposes of reappraising the heritage value of the history of leprosy for present needs. Museum exhibitions in three colonial leprosaria in Malaysia, South Korea, and Taiwan are examined, specifically focusing on three areas of interpretation: the medical past in a post-colonial present, the difficult life of former patients, and the unsettled present. In contrast with the traditional museum as a place for the repository of glorious national identity, museums of Hansen's disease offer diverse pathways to new museum culture created by activists, museum visitors, and officials.

Abstract:Abstract Objective: To describe the epidemiological characteristics of leprosy in China in 2018 and provide the essential evidences for leprosy control. Methods: We collected data of leprosy cases nationwide from Leprosy Management Information System in China (LEPMIS) except Hong Kong, Macao, and Taiwan. And then we performed a descriptive and comparative analysis. Results: A total of 521 new leprosy cases were detected nationwide in 2018, with a case detection rate of 0.037/100,000. The proportion of children under 15 years of age, multi-bacillary (MB) cases, and cases with WHO grade 2 disability (G2D) were 1.3%, 89.4%, and 19.0%, respectively. There were 2,479 registered cases of leprosy in China by the end of 2018, accounting for a prevalence rate of 0.178/100,000, with 43 replase cases. Southwest China separately accounted for 312/521 (59.9%) newly detected cases and 1180/2479 (47.6%) registered cases of leprosy in China. Conclusion: China is a low endemic country for leprosy and the detection rate was much lower than the global average. Southwest China is still the key regions for leprosy control, including Yunnan, Guizhou, and Sichuan. In addition, leprosy in low endemic provinces also needs to be focused.

Abstract:The history of historical idea is undoubtedly a part of the history of Chinese historiography. The difficulty lies in how to distinguish it from the intellectual history or the history of Chinese philosophy. As indicated here, traditional Chinese history often took advantage of authoritative Chinese medical theories, such as the concept of “second rate doctors cure disease of human body while first rate doctors heal disease of society”, which prevailed in late Spring and Autumn Period and had been frequently cited until late Qing Dynasty. Sima Qian cited this concept to criticize the Emperor Hanwu and his officials abusing their power to enact new economic policies to deprive the people. And the Yantie Conference later in the Eastern Han embodied the influence of Sima Qian's criticism. Through a textual study of Sima Qian's legend of Bian Que in the light of Canggong medical treatment, the author attempts to get a point by exploring the research method of the history of historical idea.
[On the etiological concept in the literature from unearthed documents of bamboo slips and silk scrolls]

Abstract:There is a substantial number of medical literature in the unearthed bamboo slips and silk scroll literature, the vast majority of which came into being earlier than medical books handed down from the ancient time, and are the documents of the origin of Chinese medicine dated back to the earliest time known thus far. In these documents, the contents of not a few of them deals with the etiology of disease which, by textual criticism and analysis, can be classified into seven different aspects, namely, six climatic pathogenic factors, emotional factors, injury caused by falling, traumatic damage; frostbite and burns, insect or animal bites, drug poisoning, evil spirit haunting, and constitutional factors, reflecting the contemporary etiological concept truthfully.

Abstract:Our knowledge of how society viewed leprosy and treated its victims in the past is still scarce, especially in geographical regions and archaeological periods from where no written sources are available. To fill in some research gaps, we provide the comparative analysis of five previously described, probable cases with leprosy from the Avar-period Trans-Tisza region (Hungary). The five skeletons were subject to a detailed macromorphological (re-)evaluation. Where possible, the biological and social consequences of having leprosy were reconstructed based on the observed bony changes and mortuary treatment, respectively. The retrospective, macromorphology-based diagnosis of leprosy could be established in three cases only. Based on the detected skeletal lesions, all of them suffered from near-lepromatous or lepromatous leprosy. The disease resulted in aesthetic repercussions and functional implications, which would have been disadvantageous for these individuals, and limited or changed their possibilities to participate in social situations. They could have even required heavy time investment from their respective communities. The analysis of the mortuary treatment of the confirmed leprosy cases revealed no evidence of a social stigma. These findings indicate that the afflicted have not been systematically expulsed or segregated, at least in death, in the Early Middle Ages of the Carpathian Basin.

Abstract:Leprosy is a chronic infectious disease that mainly affects the skin and peripheral nerves, it can also invade deeper tissues and organs, including mucous membranes, lymph nodes, testes, eyes, and internal organs. Severe cases can result in deformities and disabilities. We encountered the case of a 39-year-old male with unexplained fever, headache and rash. The patient's lesions were taken for histopathological examination and slit skin smear analysis. Further, the patient was detected of Mycobacterium leprae (M.leprae) nucleic acid sequences in the cerebrospinal fluid (CSF) and plasma, and M.leprae gene targets in the skin lesion tissue and blood. The patient was eventually diagnosed with multibacillary leprosy and type II leprosy reaction. These results suggest the possibility of bacteremia in patients with leprosy to some extent, and observation implies the potential invasion of CSF by M.leprae or its genetic material.

Abstract:Leprosy is a neglected infectious disease caused by acid-fast bacillus Mycobacterium leprae. It primarily affects the skin and then progresses to a secondary stage, causing peripheral neuropathy with potential long-term disability along with stigma. Leprosy patients account for a significant proportion of the global disease burden. Previous efforts to improve diagnostic and therapeutic techniques have focused on leprosy in adults, whereas childhood leprosy has been relatively neglected. This review aims to update the diagnostic and therapeutic recommendations for adult and childhood leprosy. This review summarizes the clinical, bacteriological, and immunological approaches used in the diagnosis of leprosy. As strategies for the diagnosis and management of leprosy continue to develop better and more advanced knowledge, control and prevention of leprosy are crucial.

Abstract:Leprosy remains endemic in some regions and is a global health concern. However, the possible causes and risk factors of the disease remain unclear. Data in Wenshan, China were collected from the Wenshan Institute of Dermatology (1986–2015); data in Nepal were obtained from the Leprosy Control Division, Department of Health Services, Nepal (2011 to 2015); and data from Indonesia, India, and Brazil were collected from WHO records. We assessed the epidemiological trends of leprosy in Wenshan and compared the features of possible causes and risk factors with those of other countries. We then performed a descriptive and statistical analysis to make our study more purposeful and definitive. A total of 3,376 cases were detected in Wenshan from 1986 to 2015. The overall prevalence rate (PR) of leprosy presented a decreasing trend with a peak (4.9/10,000 population) in 1986. The detection of new leprosy cases was higher in males than in females. Visible deformity increased every year since 2005 with a disability of 34.8% in 2015 among new cases. In Nepal, 2,461 leprosy patients received multi-drug therapy (MDT) in 2015 which corresponded to the PR of 0.89/10,000 population. Geographic latitude and socio-economic situations appeared to be the main causes of leprosy, and the healthcare condition was an important factor associated with leprosy incidence. The introduction of MDT effectively reduced leprosy prevalence worldwide. Wenshan (China), Nepal, and other countries share similarities in various aspects with respect to socio-cultural features, geographical distribution, environmental factors, and economic situation, which may contribute to leprosy being endemic in these areas.

Abstract:We examined six skeletons from mediaeval contexts from two sites in England for the presence of Mycobacterium leprae DNA, each of the skeletons displaying osteological indicators of leprosy. Polymerase chain reactions directed at the species-specific RLEP multicopy sequence produced positive results with three skeletons, these being among those with the clearest osteological signs of leprosy. Following in-solution hybridization capture, sufficient sequence reads were obtained to cover >70% of the M. leprae genomes from these three skeletons, with a mean read depth of 4–10×. Two skeletons from a mediaeval hospital in Chichester, UK, dating to the 14th–17th centuries AD, contained M. leprae strains of subtype 3I, which has previously been reported in mediaeval England. The third skeleton, from a churchyard cemetery at Raunds Furnells, UK, dating to the 10th to mid-12th centuries AD, carried subtype 3K, which has been recorded at 7th–13th century AD sites in Turkey, Hungary and Denmark, but not previously in Britain. We suggest that travellers to the Holy Land might have been responsible for the transmission of subtype 3K from southeast Europe to Britain.

Abstract:More than 100 counties, mainly in southwest China, report incidence rates of leprosy >1/100,000. The current study analysed the epidemiology of leprosy in southwest China to improve our understanding of the transmission pattern and improve control programs. 207 counties were selected in southwest China. Leprosy patients and their household contacts were recruited. The data from the medical interview and the serological antileprosy antibody of the leprosy patients were analysed. A total of 2,353 new cases of leprosy were interviewed. The distribution of leprosy patients was partly associated with local natural and economic conditions, especially several pocket areas. A total of 53 from 6643 household contacts developed leprosy, and the incidence rate of leprosy in the household contacts was 364/100,000 person-years. We found that NDO-BSA attained higher positive rates than MMP-II and LID-1 regardless of clinical types, disability and infection time in leprosy patients. By means of combination of antigens, 88.4% patients of multibacillary leprosy were detected, in contrast to 59.9% in paucibacillary leprosy. Household contacts should be given close attention for the early diagnosis, disruption of disease transmission and precise control. Applications of serology for multi-antigens were recommended for effective coverage and monitoring in leprosy control.

Abstract:Schistosomiasis is a modern disease name, but it has been widespread in ancient China and exists in a specific form in traditional Chinese medicine (TCM) . The purpose of the paper is to make clear the existing form of schistosomiasis in TCM and infer the prevalence of schistosomiasis in ancient China. The paper focuses on the period when great progress was made on schistosomiasis by TCM, and sums up the understanding of TCM toward schistosomiasis in this period. By studying and analyzing the literature of schistosomiasis in this period, the paper tries to find out the representative Chinese medicine symptom description and TCM "other name" of schistosomiasis, so as to accurately judge whether the relevant description in ancient TCM books and historical documents can provide scientific basis for schistosomiasis. It is important to understand the prevalence and cognition of schistosomiasis in ancient China.

Abstract:Background: Leprosy is a chronic infectious skin and neurological disease, and genetic background is considered to be one of the major factors of risk. The major histocompatibility complex (MHC) region not only affects susceptibility to leprosy but also its development and outcome. Given the complex traits of the MHC region, variants and the potential mechanism by which HLA influences leprosy development need to be further explored. Methods: We extracted previous genome-wide association study data from the Northern Han Chinese population to perform HLA fine-mapping. Using the 1,000 Genome Project Phase 3 dataset as the reference panel, single-nucleotide polymorphisms (SNP), insertion and deletion (INDEL) and copy number variant (CNV) imputation were carried out. HLA classical alleles and amino acids in the MHC region were imputed using the HAN-MHC database. Further stepwise regression analysis was conducted to analyze independent signals of variants related to leprosy. Results: We identified four independent variants: esv3608598, rs7754498, rs3130781 and rs144388449. Among them, esv3608598 is a CNV and the first HLA CNV associated with leprosy risk. SNP annotation using RegulomeDB, HaploReg, and rVarBase showed that three SNPs are likely to affect the pathogenesis of leprosy. Conclusion: In summary, this is the first study to assess the association between HLA CNV and leprosy susceptibility in a Northern Han Chinese population. By fine mapping of the MHC region in this population, our findings provide evidence for the contribution of HLA to leprosy susceptibility.

Abstract:Since the leprosy cases have fallen dramatically, the incidence of leprosy has remained stable over the past years, indicating that multidrug therapy seems unable to eradicate leprosy. More seriously, the emergence of rifampicin-resistant strains also affects the effectiveness of treatment. Immunoprophylaxis was mainly carried out through vaccination with the BCG but also included vaccines such as LepVax and MiP. Meanwhile, it is well known that the infection and pathogenesis largely depend on the host's genetic background and immunity, with the onset of the disease being genetically regulated. The immune process heavily influences the clinical course of the disease. However, the impact of immune processes and genetic regulation of leprosy on pathogenesis and immunological levels is largely unknown. Therefore, we summarize the latest research progress in leprosy treatment, prevention, immunity and gene function. The comprehensive research in these areas will help elucidate the pathogenesis of leprosy and provide a basis for developing leprosy elimination strategies.
