Abstract:ObjectiveThe diagnoses of patients with duodenal protruding lesions are difficult when using conventional examinations such as computed tomography (CT) and conventional endoscope etc. Thus, we investigated the clinical value of endoscopic ultrasonography (EUS) with miniature ultrasonic probes on the diagnosis and treatment of duodenal protruding lesions.MethodsPatients with duodenal protruding lesions who were indicated for EUS were examined by EUS with 12∼15 MHz miniature ultrasonic probes and double-cavity electronic endoscope. According to diagnosis of EUS, those patients were indicated for biopsy and treatment received biopsy, endoscopic resection or surgical excision. The postoperative histological results were compared with the preoperative diagnosis of EUS. Those patients without endoscopic resection or surgical excision were periodically followed up with EUS.ResultsA total of 169 patients with duodenal protruding lesions were examined by EUS, of which 40 were diagnosed with cysts, 36 with inflammatory protruding or polyp, 25 with Brunner’s gland adenoma, 19 with ectopic pancreas, 17 with gastrointestinal stromal tumor, 12 with extrinsic compression, 12 with minor papilla, 6 with lipoma, 1 with adenocarcinoma and 1 with lymphoma. After EUS examinations, 75 patients received biopsy, endoscopic resection or surgical excision respectively. The postoperative histological results of 70 patients were completely consistent with the preoperative diagnosis of EUS, with 93.33% diagnostic accuracy. The results of follow-up with EUS indicated that duodenal cyst, Brunner’s gland adenoma, ectopic pancreas, gastrointestinal stromal tumor and lipoma remained unchanged within 1∼3 years. No related complications occurred among all patients that received EUS examinations.ConclusionEUS is an effective and reliable diagnostic method for duodenal protruding lesions.

Abstract:Objective To investigate the diagnostic value of endoscopic ultrasonography (EUS) for duodenal cysts.Methods Clinical manifestations, results of conventional gastroscopy and EUS, as well as follow-up, of patients with duodenal cysts were retrospectively analyzed.Results A total of 105 patients with duodenal cysts presented with no lesion-related symptoms, but displayed characteristic imaging changes under EUS, which could reveal the origin, size and nature of duodenal cysts, and differentiate it from other submucosal lesions.A maximum follow-up of 5 years showed no changes in duodenal cysts.Conclusion Duodenal cyst is a benign disease, and EUS plays a very important role in diagnosis of the disease.
Diagnostic Superiority of Endoscopic Ultrasonography in 38 Cases of Primary Gastric Lymphoma and Its Value in Follow-Up

Abstract:Objective To evaluate the diagnostic superiority of endoscopic ultrasonography (EUS) in primary gastric lymphoma (PGL) and its value in follow-up.Methods From January 2012 to June 2015,38 patients with suspected PGL under regular gastroendoscopy,biopsy and surgery operation received EUS combined with targeted deep biopsy.T test and chi-square test were performed for statistical analysis.Results Among 50 patients suspected for PGL under regular gastroendoscopy,38 patients were confirmed pathologically.The sensitivity of EUS examination with targeted deep biopsy in PGL was 86.8％ (33/38),the specificity was 83.3％ (10/12) and accuracy was 86.0％ (43/50).The accuracy of EUS examination with deep biopsy was higher than that of regular gastroscopy,and the difference was statistically significant (86.0％ (43/50) vs 57.9％ (22/38),x2 =19.4,P＜0.05).The main endosonographic characteristics of PGL were partial or diffuse thickening of stomach wall presented with,even or uneven hypoechoic lesions and extramural enlarged lymph nodes.Compared with pathological stage after surgery,the accuracy of T and N stage of EUS was 8/8 and 7/8.The times to achieve complete remission of Helicobacter pylori (H.pylori) eradication group and H.pylori eradication group were (3.3±0.8) months and (4.6± 0.9) months,respectively,and the difference between the two groups was statistically significant (t =4.3,P＜0.05).Conclusions The detection rate of EUS combined with targeted deep biopsy is high,which could clearly indicate the depth and extent of lesion invasion and guide clinical stage and selection of therapy.EUS could effectively evaluate efficacy of PGL treatment.

Abstract:【Objective】 To To investigate the characteristic manifestation of endoscopic ultrasonography(EUS) in primary gastric lymphoma(PGL),and improve the diagnostic levels of clinicians.【Methods】 The manifestations of EUS of patients with PGL identified by histology were analyzed,and the changes in the process of treatment and follow-up of these patients were observed,and the features of EUS were concluded in PGL.【Results】 All the 24 patients who received the EUS examination were diagnosed as PGL by histology.The tumor appeared as ulcerative type in 14 cases,nodular type in 5,infiltrative type in 3,erosive type in 2.The lesions were located at gastric antrum in 5 cases,antrum and corpus in 7,corpus in 8,fundus in 4.The lesions invaded into submucosa in12 cases,muscular layer in 7,serous membrane in 5,gastric contiguous lymph nodes in 5.15 cases of all were followed up for EUS after treatment.【Conclusions】 By mastering the features of EUS images of PGL,we can improve the levels of diagnosis and differential diagnosis of this disease.EUS play a crucial role in the affect assessment and recurrence follow-up of patients with primary gastric lymphoma.

Abstract:Objective: To examine the values of endoscopic ultrasonography (EUS) on diagnosis and treatment of esophageal hamartoma. Methods: We compared and analyzed various kinds of imaging examinations such as barium esophagram, contrast-enhanced computed tomography (CT) and conventional gastroscopy in retrospectively reviewing the clinical data of an esophageal hamartoma patient seen in our clinic in the First Affiliated Hospital, School of Medicine, Zhejiang University, Hangzhou, China. Having received various imaging examinations, this patient was finally diagnosed with esophageal hamartoma and underwent gastroscopic resection of hamartoma with the diagnostic information obtained from EUS. The patient had been regularly followed up for 13 months after treatment. Results: Barium esophagram, CT and conventional gastroscopy detected the lesion, but were unable to distinguish it from common esophagopolypus and other submucosal lesions, and unable to determine etiopathogenisis. EUS detected the hamartoma and identified its internal structure, echo, exact size, depth of invasion, origin and the relationship between adjacent tissues and organs, differentiating the lesion from other submucosal tumors and clearly defining the diagnosis. EUS-guided fine needle aspiration (FNA) also helped to identify the etiological diagnosis. Conclusion: EUS was superior to other imaging means in diagnosis and treatment of hamartoma.
The Value of Endoscopic Ultrasonography for Management of Esophageal Leiomyoma

Abstract:Objective To investigate clinical value of endoscopic ultrasonography (EUS) on diagnosis and treatment of esophageal leiomyoma.Methods The clinical feature of patients diagnosed as esophageal leiomyoma by EUS was analyzed.All patients underwent conventional gastroscopy and EUS examination as well as blood test.The EUS findings were compared with histopatholoical diagnosis for endoscopically or surgically resected specimens.All patients were followed-up with EUS for 14.3 months (range 2-36 months) for leiomyoma recurrence.Results A total of 191 patients were diagnosed with esophageal leiomyomas by EUS.Clinical manifestation,serum examination,conventional gastroscopy and spiral CT were not helpful for diagnosing esophageal leiomyoma,One hundred and sixteen patients with esophageal leiomyomas were treated by either endoscopic resection (111 cases) or surgical excision (5 cases).of which,101 resected specimens were pathologically confirmed as leiomyomas.The diagnostic accuracy of EUS was 87%.The follow-up study showed that there was no change of leiomyomas in 75 untreated patients and no recurrence was found in 116 surgically treated patients.Conclusion Esophageal leiomyoma is a benign tumor,EUS plays a very important role in diagnosis and treatment of this disease.
Diagnosis and Treatment of Duodenal Gastrointestinal Stromal Tumors

Abstract:OBJECTIVES: The diagnostic value of different noninvasive diagnostic modalities and the endoscopic ultrasoundguided fine-needle aspiration (EUS-FNA) reliability of duodenal gastrointestinal stromal tumors (GISTs) are ambiguous in the present studies. METHODS: Patients with a histopathological diagnosis of the primary duodenal GISTs between the years 2008 and 2018 were analyzed. Data on the treatment and clinicopathological features were recorded. Furthermore, the computed tomography (CT)/magnetic resonance imaging (MRI), EUS, and EUS-FNA results were collected and compared. RESULTS: A total of 142 patients were enrolled into the study. In all patients, the most common symptom was gastrointestinal bleeding (44.4%), followed by abdominal pain and bloating (27.5%). Duodenal GISTs were mostly located in the second duodenal portion (52.1%), followed by the first portion (19.0%). EUS had significantly higher sensitivity and positive predictive values than CT or MRI (P = 0.047 and P = 0.005, respectively). The EUS-FNA sensitivity of duodenal GISTs was also significantly higher than the conventional endoscopic biopsy (73.3% vs 33.3%, P = 0.006). A total of 131 patients underwent surgery, including limited resection or pancreaticoduodenectomy. The tumor size and postoperative complication rates were higher in patients who underwent pancreaticoduodenectomy (P = 0.001 and P < 0.001, respectively). DISCUSSION: The diagnostic value of EUS is significantly higher than that of CT and MRI for duodenal GISTs. The EUS-FNA can provide a histological diagnosis of duodenal GISTs in most cases.

Abstract:Objective To observe the value of endoscopic ultrasonography(EUS) in diagnosis and treatment of gastrointestinal lipoma.Methods A total of 135 patients with gastrointestinal lipoma diagnosed by EUS were retrospectively analyzed,and the endosonographic features were observed.EUS results of 40 patients were compared with pathological results after endoscopic treatment or surgery resection.Results The lipomas presented as submucosal(only a small proportion as subserous) hyperechoic masses with homogenous echo and distinct border and some with ultrasonic attenuation.Among 40 patients receiving treatment,25 underwent endoscopic resection,including 9 by endoloop,5 by snare high-frequency electroresection and 11 by endoscopic submucosal dissection(ESD),the other 15 cases were treated by operation.Thirty eight were diagnosed pathologically as gastrointestinal lipoma,and the diagnostic accuracy of EUS was 95.00%(38/40).Pathologic study showed that 34 tumors originated from submucosa with 1 involved muscular and 1 involved serosa,2 originated from intramural,and 2 from subserous.The accuracy of EUS for the origin of lipoma was 94.74%(36/38).Conclusion EUS can provide definite information of gastrointestinal lipoma about its originating layer,size,form,internal echo and border,which can guide the treatment options.

Abstract:BACKGROUND AND AIM:To investigate the clinical value of endoscopic ultrasonography (EUS) with miniature ultrasonic probes (MUP) for the diagnosis and treatment of esophageal leiomyoma.METHODS:A total of 229 patients with esophageal leiomyoma, diagnosed using EUS, with 12-MHz MUP and a double-cavity electronic endoscope, were enrolled. The clinical characteristics of the patients were analyzed, and those who had therapeutic indications received endoscopic resection or surgical excision. Postoperative histological diagnostic results were compared with the preoperative diagnosis of EUS. All patients, including those with or without endoscopic resection or surgical excision were periodically followed up with EUS.RESULTS:Of the 229 patients, 118 received endoscopic resection, and seven received surgical excision. Postoperative histology showed that 110 patients were completely consistent with the preoperative diagnosis of EUS, and the diagnostic accuracy of EUS was 88.6%. No treatment-related complications occurred among the patients who received endoscopic resection or surgical excision, and no recurrence was observed during the follow-up examinations.CONCLUSIONS:Esophageal leiomyoma is a benign tumor of the esophagus. EUS is a useful technique for the diagnosis of esophageal leiomyoma and for making treatment-related decisions.
The application significance of endoscopic ultrasonography and endoscopic submucosal dissection in upper digestive tract neuroendocrine tumor

Abstract:Objective: To evaluate the feasibility and efficacy of endoscopic ultrasonography (EUS) and endoscopic submucosal dissection (ESD) in treatment of upper digestive tract neuroendocrine tumor (NEN).Methods: A retrospective review of 26 cases of upper digestive tract NEN undergoing EUS and ESD between August 2010 and January 2014 was conducted. All patients were followed-up using endoscopy. The clinical features and outcomes including EUS features, pathological grade, complete resection rate, complications, recurrence and metastasis were evaluated.Results: EUS results showed that 26 patients had 31 NENs (18 originated from muscularis mucosa and 13 from submucosa; 2 located in esophagus, 24 in stomach and 5 in duodenal bulb). All 26 lesions were removed by ESD en bloc; the average maximum diameter of the lesions was 1.0 (0.3-3.1) cm; the complete resection rate was 93.5% (29/31). Two patients with positive basal surgical margin included one patient with pathologic grade 2 NEN in stomach (who then underwent an additional ESD) and one patient with pathologic grade 1 NEN in duodenal bulb (who was treated with argon plasma coagulation after rejecting surgery and ESD). Histological examination determined that 27 NEN lesions were pathologic grade 1 and 4 were pathologic grade 2. No patients underwent delayed bleeding or perforation after ESD. The follow-up rate was 93.5% (29/31). During a mean follow-up period of 21 months (range: 10-41 months), no lymph node metastasis or distant metastasis was observed.Conclusion: EUS is able to distinguish the origin of upper digestive tract NEN and give aid to determing the range and depth of ESD. EUS for Choosing Best Endoscopic Treatment of Mesenchymal Tumors of Upper Gastrointestinal Tract

Abstract:AIM: To evaluate the value of endoscopic ultrasonography (EUS) in the choice of endoscopic therapy strategies for mesenchymal tumors of the upper gastrointestinal tract.METHODS: From July 2004 to September 2010, 1050 patients with upper gastrointestinal mesenchymal tumors (GIMTs) were diagnosed using EUS. Among them, 201 patients underwent different endoscopic therapies based on the deriving layers, growth patterns and lesion sizes.RESULTS: Using EUS, we found 543 leiomyomas and 507 stromal tumors. One hundred and thirty-three leiomyomas and 24 stromal tumors were treated by snare electrosection, 6 leiomyomas and 20 stromal tumors were treated by endoloop, 10 stromal tumors were treated by endoscopic mucosal resection and 8 stromal tumors were treated by endoscopic submucosal dissection. Complete resection of the lesion was achieved in all cases. Of the mesenchymal tumors, 90.38% diagnosed by EUS were also identified by pathohistology. All wounds were closed up nicely and no recurrence was found in the follow-up after 2 mo.CONCLUSION: EUS is an effective means of diagnosis for upper GIMTs and is an important tool in choosing the endoscopic therapy for GIMTs, by which the lesions can be treated safely and effectively. Abstract:Endoscopic ultrasound (EUS) is one of the most important modalities for the diagnosis of digestive tract diseases. EUS has been evolving ever since it was introduced. New techniques such as elastography and contrast enhancement have emerged, increasing the accuracy, sensitivity and specificity of EUS for the diagnosis of digestive tract diseases including pancreatic masses and lymphadenopathy. EUS-elastography evaluates tissue elasticity and therefore, can be used to differentiate various lesions. Contrast-enhanced EUS can distinguish benign from malignant pancreatic lesions and lymphadenopathy using the intravenous injection of contrast agents. This review discusses the principles and types of these new techniques, as well as their clinical applications and limitations.
Endoscopic ultrasonography in diagnosis of duodenal accessory papilla

Abstract:Objective:To explore the diagnostic value of endoscopic ultrasonography (EUS) for duodenal accessory papilla.Methods:Data of 122 cases of duodenal accessory papilla diagnosed by EUS at the endoscopy center of the First Affiliated Hospital of Zhejiang University School of Medicine from February 28, 2006 to February 28, 2018 were analyzed and summarized.Results:Of the 122 duodenal accessory papilla cases, the age was 52.1±12.9, with more males than females. The most common site of duodenal accessory papillae was the descending part above the papilla (88/122, 72.13%), followed by the junction of duodenal bulb and descending part (29/122, 23.77%), and a small proportion of lesions located in the duodenal bulb (5/122, 4.10%). Duodenal accessory papillae were all solitary, whose diameter mostly ranged 0.5-1.0 cm (88/122, 72.13%), a smaller proportion of diameter larger than 1.0 cm (23/122, 18.85%), and only a few with diameter less than 0.5 cm (11/122, 9.02%). Most duodenal accessory papillae were hypoechoic (71/122, 58.20%) or moderate to low echogenic (35/122, 28.68%), and the echoes were mostly homogeneous. The mucosa layer was smooth, with a sphincteroid structure in the submucosa and below. The boundary of the duodenal accessory papillae was mostly clear (121/122, 99.18%) and characteristic lacunar cavity structures were often seen in the center (83/122, 68.03%). The surrounding intestinal wall was normal and no associated enlarged lymph nodes were found around the intestine.Conclusion:EUS can clearly show the structure of duodenal accessory papilla and adjacent organs, and is of high value for the diagnosis of duodenal accessory papilla.
Diagnostic Value of Endoscopic Ultrasonography for Gastrointestinal Leiomyoma.

Abstract:AIM:To investigate the clinical pathologic features of gastrointestinal leiomyoma and the diagnostic value of endoscopic ultrasonography (EUS) on gastrointestinal leiomyoma.METHODS:A total of 106 patients with gastrointestinal leiomyoma diagnosed with EUS were studied. The location, size and layer origin of gastric and esophageal leiomyomas were analyzed and compared. The histological diagnosis of the resected specimens by endoscopy or surgery in some patients was compared with their results of EUS.RESULTS:The majority of esophageal leiomyomas were located in the middle and lower part of the esophagus and their size was smaller than 1.0 cm, and 62.1 % of esophageal leiomyomas originated from the muscularis mucosae. Most of the gastric leiomyomas were located in the body and fundus of the stomach with a size of 1-2 cm. Almost all gastric leiomyomas (94.2 %) originated from the muscularis propria. The postoperative histological results of 54 patients treated by endoscopic resection or surgical excision were completely consistent with the preoperative diagnosis of EUS, and the diagnostic specificity of EUS to gastrointestinal leiomyoma was 94.7 %.CONCLUSION:The size and layer origin of esophageal leiomyomas are different from that of gastric leiomyomas. Being safe and accurate, EUS is the best method not only for gastrointestinal leiomyoma diagnosis but also for the follow-up of patients.
Usefulness of endoscopic ultrasound for acquiring the pathological diagnosis of gastrointestinal lymphoma.

Abstract:BACKGROUND AND STUDY AIMS:This study aims to assess the value of endoscopic ultrasound (EUS) for acquiring a pathological diagnosis of gastrointestinal lymphoma (GIL). PATIENTS AND METHODS:We retrospectively reviewed all GIL patients who underwent EUS from November 2011 to July 2020 at Fudan University Shanghai Cancer Center. All patients with pathologically confirmed GIL were included. The characteristics of the lesions were recorded, and the efficacy for acquiring pathologic diagnosis between white light endoscopy (WLE) and EUS was analyzed. RESULTS:In total, 404 patients with GIL who underwent EUS examination were included in this study. GIL was confirmed in 143 cases by after EUS judgment biopsy (AEJ biopsy), 11 cases by endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA), 293 cases by WLE biopsy, and 10 cases by surgical pathology for repeated negative pathologic results from EUS and WLE. Among all cases, 78.71% (318/404) were T1-T2, whereas 32.18% (130/404) were determined to have multiple lesions in the digestive tract wall. The positive rates of the WLE biopsy and AEJ biopsy of the involved gastric wall were 77.93% (293/376) and 89.38% (143/160), respectively. Twelve cases showed diffuse thickening of the gastric wall, and the total positive rate of EUS was 91.67% but 0% for WLE with this type of GIL. The total positive rate and positive rate during the first examination of EUS were all significantly higher than those of WLE. Moreover, 19.68% of the patients showed negative results during their WLE examination and then received a positive pathologic diagnosis upon EUS examination, but none had the opposite process. CONCLUSIONS:EUS was found to be a better tool for acquiring a pathological diagnosis of GIL than conventional WLE, especially for GIL similar to linitis plastica.
Endoscopic ultrasonography in gastric lymphomas: appraisal on reliability in long-term follow-up

Abstract:The reliability of endoscopic ultrasonography (EUS) in follow-up management of gastric lymphomas has not been clearly validated. We conducted a retrospective analysis on 23 patients, 12 affected by mucosa-associated lymphoid tissue (MALT) lymphoma, eight by diffuse large B-cell lymphoma, and three by high-grade lymphoma with low-grade component, all treated with a stomach-conservative approach. One hundred and twenty matched evaluations with both EUS and endoscopy with biopsy (E-Bx) were performed, according to validated guidelines and clinical judgment. At a median follow-up of 87 months ranged between 9.5 and 166 months, the analysis of progression-free survival and disease-free survival showed a strict relationship between the persistence of EUS abnormalities and the clinical outcome in patients with MALT lymphoma (p = 0.0079; p = 0.02) but not in patients with high-grade lymphoma. In conclusion, EUS evaluation does not seem reliable in follow-up management of high-grade lymphomas, although it could have a great clinical impact in the management of MALT lymphoma.

Abstract:OBJECTIVES:Currently, positron emission tomography/computed tomography (PET/CT)-based Ann Arbor and Lugano staging systems are widely applied for gastric lymphoma, which majorly focused on the extent of systemic invasion, but there is insufficient assessment in extent of gastric wall invasion. Thus, we aimed to assess the value of linear-array EUS in therapeutic response and prognosis prediction, and provide an endoscopic ultrasound (EUS)-based staging system for gastric lymphoma. METHODS:Between January 2012 and October 2019, 191 patients with histologically confirmed gastric lymphoma were included. We analyzed patients' clinicopathologic data, white-light appearance, EUS staging, tumor extension, and therapeutic regime to identify the predictors for complete remission (CR) and progression-free survival (PFS). RESULTS:With a median follow-up period of 547 days, 136 patients achieved CR. Multivariate regression analysis identified EUS T stage three CR predictors, including T stage (T2-T4 vs. T1, OR = 0.271, 95% confidence interval (CI) = 0.108-0.683, P = 0.0085) was the independent predictor. The Ki67 index, lesion surface, EUS stage, tumor extension, and regime predicted PFS, but tumor extension was the only independent predictor (local vs. system, OR = 6.005, 95%CI = 2.365-15.236, P < 0.001). The concordance index (C-index) was higher for CR rate in the EUS-based staging [0.673; 95% CI = 0.628-0.718] than Lugano staging [0.663; 95% CI 0.616-0.710], and for PFS in the EUS-based staging [0.694; 95% CI 0.586-0.802] than Lugano staging [0.684; 95% CI 0.564-0.804]. CONCLUSIONS:An EUS-based staging system is suitable for CR and PFS prediction in gastric lymphoma, with highly informative assessment in extent of gastric wall invasion.

Abstract:OBJECTIVE:To investigate the clinical value of miniature ultrasonic probes (MUP) in the diagnosis and treatment of digestive tract diseases.METHODS:537 patients who were indicated for endoscopic ultrasorography (EUS) underwent EUS with 7.5 - 20 MHz MUP and double-cavity electronic endoscope. According to the diagnoses of MUP, those patients who presented the indication of treatment were treated by endoscopic resection or surgical excision respectively, the postoperative histological results were compared with the preoperative diagnoses of MUP. A follow-up with MUP was periodically made for a few patients without endoscopic resection or surgical excision.RESULTS:Among the 537 patients the diagnosis of gastrointestinal submucosal lesions was made in the 256 cases, polyp in 56 cases, inflammatory prominence in 37 cases, extrinsic compression 53 cases, digestive tract cancer 50 cases, peptic ulcer 17 cases, cholecystolithiasis 11 cases, chronic pancreatitis in 8 cases, and achalasia 2 cases and 47 cases were diagnosed as normal, After examination of MUP, 220 patients were treated by endoscopic resection or surgical excision respectively, the postoperative histological results of 211 patients were completely consistent with the preoperative diagnosis of MUP, and diagnostic accuracy of MUP was 95.9%. The result of follow-up with MUP indicated that gastrointestinal leiomyoma, lipoma, phlebangioma and cyst were unchanged within 1 - 2 years. The patients who were treated by endoscopic resection or centesis showed no complication.CONCLUSION:EUS with MUP is a valuable method in diagnosis of gastrointestinal submucosal lesions, staging of digestive tract cancer and diagnosis of biliary-pancreatic diseases. It plays a very important role for making scientific, effective, safe and economic therapeutic plan.

Abstract:To explore the value of endoscopic ultrasonography (EUS) for diagnostic and therapeutic decision making in ampullary lesions. All patients who underwent EUS primarily for the evaluation of a suspected ampullary lesion (a previous biopsy suggesting adenoma/adenocarcinoma) between Jan 2005 and December 2016 were included. Also, the patients received endoscopic therapy or surgery. Using final endoscopic or surgical pathology as the reference standard, the sensitivity, specificity, and accuracy of EUS for intraductal and local extension were evaluated. and outcomes of the subgroup referred for endoscopic papillectomy evaluation of intraductal extension. Multiple logistic regression analysis was used to explore EUS characteristics of ampullary lesions that predict the need for endoscopic ampullectomy. 1) We identified 102 patients (61 male; median age, 60 years) who underwent EUS for an ampullary lesion. Among them, 79 (77.4%) patients with ampullary tumors underwent endoscopic papillectomy, 19 (18.7%) received surgery, and the remaining 4 (3.9%) cases underwent additional surgery because of incomplete endoscopic resection. Median tumor size was 2.5 cm. Based on histopathology, 8 were nonadenomatous. The remaining (n=94) included adenoma (n=28), advanced adenoma (n=48), and adenocarcinoma (n =18). 2) When using pathology as the reference, the sensitivity, specificity, and accuracy of EUS for local staging was 80%, 93%, and 81%. 3) Compared with ERCP and pathology, the sensitivity, specificity, and accuracy of EUS for any intraductal extension was 56%, 97%, and 74.1%. 4) Complete endoscopic resection rate was 73.8%. Multivariate analysis indicated that lesion size <2.5 cm, lesions limiting to the ampulla of Vater or sphincter of Oddi, and pathology type (adenoma) were independent factors predicting complete endoscopic resection. EUS may be feasible in evaluating tumors of the ampulla of Vater preoperatively. Future prospective studies should evaluate whether EUS can identify characteristics of ampullary lesions that appropriately direct patients to endoscopic or surgical resection.
