Abstract:Objective To investigate the possible factors associated with delay time in the treatment for patients with acute myocardial infarction ( AMI).Methods One hundred and eighty-four patients with AMI admitted to hospitals were divided into two groups,based on their length of prehospital delay,one group delayed for less than six hours (≤6 h) and another group delayed for greater than six hours (6 h).Of these patients,174 cases who underwent reperfusion therapy were re-divided into two groups,one with ≤90 min inhospital delay and another with90 min delay.The effects of a few variables on the prehospital or inhospital delay time,such as age,sex,hypertension,hyperlipidemia,diabetes,cigarette smoking,family history of coronary artery disease,stroke,chest pain,onset at night,history of angina,heart function rating,duration from admission to establishment of diagnosis,or length of time calling physicians to reach the catheterization room of the hospital,etc.,were analyzed.Results Age of the patients with6 h of prehospital delay was elder than that with ≤6 h delay,as well as more female patients and more patients with diabetes,stroke,history of angina in the group with6 h delay than in the group with ≤6 h delay (all P0.05).More patients with90 min delay had atypical angina than those with ≤90 min delay did.Fewer patients were diagnosed within 20 min after arrival to emergency room,as well as the physicians delayed longer to arrive catheterization room in the90 min group than in ≤90 min group.Multivariate logistic regression analysis showed that age,diabetes,atypical angina,onset at night all were independently associated with prehospital delay (all P0.05),and atypical angina,the time interval between admission and diagnosis,the time to arrive catheterization room for physicians all were independently associated with delay in the inhospital treatment (all P0.05).Conclusions Factors,such as age,diabetes,atypical angina,onset at night,all were associated independently with the time of prehospital delay,and atypical angina,length of time from the arrival in emergency room to diagnosis establishment and length of time of physicians arriving catheterization room all related independently to inhospital delay for therapy.
