Clinical features of idiopathic pulmonary fibrosis in the elderly

Abstract:Objective To provide theoretical clues for clinical diagnosis, presentation and prognosis by retrospective analysis 47 cases of idiopathic pulmonary fibrosis in the elderly. Methods The diagnostic data, patient histories, physical signs, lung functions, chest high resolution CT features, arterial blood gases and other data in 47 elderly patients with idiopathic pulmonary fibrosis were analyzed. Results Surgical lung biopsy data were available in 8 cases; 35 of the 47 patients were male; the mean diagnostic age was 69.23±6.76 years; the mean diagnostic course was 36.74±19.37 months(median diagnostic course 14 months); 29 cases were maldiagnosed or misdiagnosed; and there were 30 case-time complications. The main clinical features included dry cough, progressive dyspnea and velcro rales on auscultation. Pulmonary function test showed ineffective ventilation and diffusive dysfunction. HRCT findings showed peripheral, basal predominant reticular abnormality, with honeycombing and traction bronchiectasis and bronchiolectasis. Arterial blood gases showed hyoxemia. Pulmonary hypertension was found in some patients. Conclusions Idiopathic pulmonary fibrosis is more in elderly male patients. Many patients were maldiagnosed or misdiagnosed. The clinical features of the condition in the elderly are uniquely characteristic. Diagnosis mainly depends on non surgical lung biopsy data in most cases.
