Transscleral fixation of IOL for correction of aphakic eyes in children after vitrectomy

Abstract:Objective To evaluate the efficacy of transscleral fixation of a folda-ble intraocular lens (IOL) for correction of aphakic eyes in children with vitrectomy.Methods This retrospective study comprised 16 aphakic eyes of 16 children who had underwent vitrectomy due to endophthalmitis(8 cases) and retinal detachment(8 cases).All patients had transscleral fixation of four haptics foldable IOL.Anterior chamber maintainer was used during the whole operation.The mean preoperative age was (7.62±2.02)years(ranged from 5 to 11 years).The average preoperative best corrcted visual acuity(BCVA) was 0.25±0.24,the average intraocular pressure(IOP) was (13.14±0.33)mmHg(1 kPa=7.5 mmHg) and 10 eyes were with corneal scar.The mean follow-up time was (12.56±5.73)months (ranged from 6 months to 22 months).Results In cases with endophthalmitis,early postoperative complication was low IOP in 5 eyes,the average IOP was (6.02±1.34)mmHg and recovered to normal level at postoperative 7 days.At the last follow-up time,there was no case with suture exposure,IOL malpositioning or retinal detachment,and the average BCVA was 0.29±0.21.Two eyes had BCVA with less than 0.05,5 eyes were 0.05 to 0.3,and 9 eyes were 0.3 or better.Conclusions Transscleral fixation of foldable IOL is a good option to correct aphakic eyes in children after vitrectomy.However,how to avoid and handle low IOP happening shortly after surgery is still a problem and long-term follow-up is important to assess the postoperative efficacy and complications.
