Clinical risk factors for acute lung injury in 26 patients after liver transplantation

Abstract:To discuss the relative risk factors of acute lung injury(ALI) developing in the early stage after liver transplantation,the clinical data of 26 liver transplant recipients in Department of Hepatobiliary Surgery,First Affiliated Hospital of Chongqing Medical University collected from August 2007 to August 2008 were retrospectively analyzed.Among the 26 cases,22 cases were male and 4 cases were female,and 4 cases received living donor liver transplantation,and the other cases had orthotopic liver transplantation.Eighteen patients developed ALI after operations,and 1 died of multiple organ system failure on the 7th day postoperatively.Among the 18 patients who developed ALI,2 were diagnosed as hepatic carcinoma,13 as decompensated hepatic cirrhosis,and 3 as chronic serious hepatitis.Liver function of 9 patients was Child B,the others was C.Eight patients had coagulation disorder preoperatively;15 had blood loss or transfusion volume over 6 000 mL during operation;6 had prolonged nonliver or hypotension over 103 minutes;alanine aminotransferase level in 13 patients was above 10 μkat/L after 30 minutes of recovery of blood supply of the allograft.The results show that decompensated hepatic cirrhosis,chronic serious hepatitis,coagulation disorder preoperatively,blood loss or transfusion volume over 6 000 mL,and high alanine aminotransferase during operation are important risk factors for ALI after liver transplantation.Transplant recipients who have one or multiple factors of these need more attention.
