Abstract:The aim of this study was to evaluate whether emodin can overcome the chemoresistance of the gemcitabine-resistant cancer cell line (Bxpc-3/Gem) in vitro. The cell line Bxpc-3/Gem was derived from the human pancreatic cancer cell line Bxpc-3. We found that Bxpc-3/Gem cells were characterized by a series of morphological changes with a resistance index of 43.51 comparing with the parental cell line. Emodin reduced Bxpc-3/Gem cell proliferation in a dose-dependent manner. Emodin and gemcitabine combination treatments resulted in decreased cell proliferation and increased apoptosis in Bxpc-3/Gem cells. In addition, combination treatments resulted in downregulation of gene and protein expression of MDR-1 (P-gp), NF-κB, XIAP, survivin, as well as inhibition of NF-κB activity and P-gp function. These observations suggest that emodin may sensitize the pancreatic cancer gemcitabine-resistant cell line Bxpc-3/Gem to gemcitabine therapy via inhibition of survival signaling.
Anti-tumor Activity of Emodin on Human Pancreatic Cancer Cell Line BXPC-3 in Vivo

Abstract:Objective: To investigate the effect and mechanism of emodin (EMO) on human pancreatic cancer cell line BxPC-3 in vivo. Methods: After the pancreatic cancer model in nude mice was established, the mice were divided into four groups: control group (NS 0.2mL/d by i.p. injection), EMO group (EMO 40mg?kg-1?d-1 by i.p. injection), and Gemcitabine (GEM) group (GEM 80mg/kg, twice/week by i.p. injection) with 8 mice each group. After 2 weeks of administration, the mice were sacrificed, detected the body-weight change of nude nice before and after the experiment, and recorded the growth inhibition rate of tumor (TGI). Immunohistochemical (IHC) staining for ki-67 and terminal deoxynucleotidyl transferase-mediated nicked labeling assay (TUNEL) were undertaken to detect the cell proliferation and cell apoptosis in tumor tissue in xenograft nude mice. Results: The inter-group comparisons in body-weight of nude mice showed no significant difference in comparing group NS(27.0±1.64)g with group EMO(25.1±1.58)g and GEM(25.6± 1.47)g.The EMO group was 38.46%, the GEM group was 44.23%. The inter-group comparisons in immunohistochemical analysis of ki-67 showed significant difference in comparing group NS IOD(219.5±17.98) with group EMO IOD(146.6± 11.57)and GEM IOD(139.5±12.55), (P0.01), and the expression of ki-67 in EMO and GEM groups were lower than that in NS group, P0.05; while the expression of ki-67 between EMO and GEM groups showed no significant difference. The inter-group comparisons in TUNEL analysis showed significant difference in comparing group NS IOD(23.6±6.02) with group EMO IOD(65.7±4.97)and GEM IOD(67.4±5.59), (P0.01), and the TUNEL-positive cells in EMO and GEM groups were higher than that in NS group; while the TUNEL-positive cells between EMO and GEM groups showed no significant difference. Conclusion: Emodin can inhibit the growth of pancreatic cancer cells BXPC-3 in nude mice; and this affection was achieved by suppressing cell proliferation and inducing cell apoptosis.

Abstract:Gallbladder carcinoma is known to be an aggressive malignancy and non-sensitive to routine chemotherapy; its prognosis is quite poor. In this study, we show that emodin (1,3,8-trihydroxy-6-methylanthraquinone), an active component from Chinese medicinal herbs, can enhance apoptosis of gallbladder cancer cells induced by cisplatin (CDDP) in a reactive oxygen species (ROS)-dependent manner. The expression of survivin, which is involved in the inhibition of apoptosis, was measured after drug treatment and it was found that this could be suppressed by CDDP. Co-treatment with emodin additively inhibited survivin expression in a ROS-dependent manner. Further experiments proved that emodin potentiated the antitumor effects of CDDP in vivo by downregulating the expression of survivin without causing detectable toxic effects on normal tissues.
Emodin Reverses Gemcitabine Resistance of Pancreatic Cancer Cell Lines Through Inhibition of IKKβ/NF-κB Signaling Pathway

Abstract:BACKGROUND: Pancreatic cancer is one of the most malignant tumors, and gemcitabine has been considered as the standard treatment and been widely utilized as a first-line drug for advanced pancreatic cancer, but gemcitabine-resistance always occurs after a short period of treatment.METHODS: Two pancreatic cancer cell lines Panc-1 and MIA-PaCa-2 were used as the study subject and their gemcitabine-resistant cells were established. Both drug-resistant cells were divided into four groups: blank, emodin, gemcitabine, and emodin+gemcitabine. Cell viability was detected by MTT assay. Flow cytometry was performed to detect cell apoptosis rate and P-gp function. Quantitative real-time polymerase chain reaction and Western blotting were used to detect Survivin, XIAP, Caspase-9/3, NF-κB p65, IKKβ and IκB-α mRNA/protein expressions, respectively. Electrophoretic mobility shift assay (EMSA) was performed to detect NF-κB binding activity. Rhodamine 123 efflux assay was used to detect P-gp function.RESULTS: Emodin could inhibit cell activity in all cell lines. Both emodin and gemcitabine can significantly increase the apoptosis rate, and the combination of the two drugs can further significantly increase the apoptosis rate in normal pancreatic cancer cell lines. In both drug-resistant pancreatic cancer cell lines, it can be observed that although gemcitabine can increase the apoptosis rate, the effect of promoting apoptosis is significantly lower than that of emodin; the drug combination can still significantly increase the apoptosis rate on the basis of emodin alone. Emodin can significantly reduce the mRNA and protein expression levels of Survivin, XIAP, NF-κB, and IKKβ, and significantly increase the mRNA and protein expression levels of Caspase-3/9 and IκB-α. Emodin significantly reduced NF-κB activity and emodin significantly promoted P-gp fluorescence intensity from Rhodamine 123 efflux assay.CONCLUSION: Emodin inhibits the expression of IKKβ, thereby inhibiting the expression and activity of downstream NF-κB, and inhibits P-gp function at the same time, ultimately achieving the purpose of reversing the drug-resistance of pancreatic cancer cell lines.

Abstract:Pancreatic ductal adenocarcinoma (PDAC) is a highly aggressive malignancy due to its broad resistance to chemotherapy. Gemcitabine is used as a standard chemotherapeutic drug for PDAC treatment, either alone or in combination with other chemotherapeutics. However, in patients with advanced disease, survival is rarely improved. This study aimed to investigate the therapeutic efficacy of N1-guanyl-1, 7-diaminoheptane (GC7) combined with gemcitabine in PDAC therapy. We measured eukaryotic translation initiation factor 5A2 (eIF5A2) expression and gemcitabine sensitivity in different PDAC cell lines (Panc-1, BxPC-3, and T3-M4). The synergistic cytotoxic effects of gemcitabine combined with GC7 were measured using Cell Counting Kit-8 assays. Western blots were performed to measure eIF5A2 and multi-drug resistance 1 (MDR1) protein expression in PDAC cells. The present findings demonstrated that combined treatment with GC7 and gemcitabine significantly inhibited PDAC cell line viability (P<0.05). EdU incorporation assays also indicated that GC7 co-treatment remarkably enhanced gemcitabine sensitivity in PDAC cells. Furthermore, downregulation of eIF5A2 diminished the regulatory role of GC7 in gemcitabine cytotoxicity. Western blotting data indicated that GC7 downregulated the expression of MDR1 while gemcitabine induced MDR1 upregulation. These findings showed that GC7 combination therapy may enhance the therapeutic efficacy of gemcitabine in PDAC by downregulating MDR1 expression.

Abstract:Gemcitabine (GEM) is a commonly used treatment for advanced pancreatic cancer. However, chemoresistance and toxic side effect limits its clinical success. In an earlier study, our laboratory found that the curcumin analogue, (3E,5E)-3,5-Bis(pyridin-3-methylene)-tetrahydrothiopyran-4-one (FN2) had strong inhibitory effect on human pancreatic cancer cells. In the present study, we investigated the effects of FN2 in combination with GEM on growth inhibition and apoptosis in human pancreatic cancer Panc-1 cells. The results showed that the combination of FN2 and GEM synergistically inhibited the growth of Panc-1 cells. Panc-1 cells survived the GEM treatment became partially resistant to the drug. Treatment with FN2 in combination with GEM strongly inhibited the growth and stimulated apoptosis in the GEM resistant Panc-1 cells. Mechanistic studies showed that inhibition of cell growth and induction of apoptosis in the GEM resistant Panc-1 cells were associated with decreases in activation of NF-κB and Akt. FN2 in combination with GEM also decreased the level of Bcl-2 and increased the level of Bax. Results of the present study indicate that GEM in combination with FN2 may represent an effective strategy for improving the efficacy of GEM and decreasing the resistance of pancreatic cancer to GEM chemotherapy.

Bisdemethoxycurcumin Potentiates Gemcitabine to Induce Mitochondrial Dysfunctions and Apoptosis in Human Pancreatic Adenocarcinoma Cells

Abstract:Pancreatic cancer is a highly aggressive malignancy, gemcitabine is indicated as a standard single agent for the treatment of pancreatic cancer, however it only extends survival by a mere 3–6 weeks. In this present study we investigate whether bisdemethoxycurcumin (BDMC), a derivative of curcumin, potentiates gemcitabine in human pancreatic cancer cells. The result suggested that BDMC produced a synergistic effects with gemcitabine by inducing mitochondrial dysfunctions and apoptosis in PANC‐1 and MiaPaCa‐2 pancreatic cancer cells. By using 2D‐MS, we found 13 essential proteins were significantly changed in response to gemcitabine alone or combined with BDMC. Pharmacological network analysis revealed that glucose‐regulated protein 78 (GRP78) as the hub activated by BDMC. We then demonstrated that BDMC upregulates GRP78 and facilitated apoptosis through eIF2α/CHOP pathway. In addition, DJ‐1 and prohibitin, two identified markers of chemoresistance, were increased by gemcitabine in PANC‐1 cells. This could be reversed by BDMC, suggesting that BDMC partially offsets the chemoresistance induced by gemcitabine. Coix Seed Emulsion Synergistically Enhances the Antitumor Activity of Gemcitabine in Pancreatic Cancer Through Abrogation of NF-κB Signaling.

Abstract:Clinical outcomes in patients with pancreatic cancer (PC) continue to be dismal, in part due to de novo and acquired chemoresistance. In the present study, we provide preclinical evidence that pre-treatment with coix seed emulsion, an injectable agent extracted from coix seeds, synergistically sensitized PC cell lines (BxPC-3, PANC-1 and AsPC-1) to gemcitabine, both in vitro and in vivo. Such pretreatment led to significant induction of pro-apoptosis proteins, including caspase-3, cleaved-PARP and Bax (P<0.05), after lower doses of gemcitabine compared to monotherapy. We also showed that coix seed emulsion suppressed the constitutive and gemcitabine-induced activation of nuclear factor-κB (NF-κB), as shown with the use of electrophoretic mobility shift, reporter and immunoblotting analyses. Coix seed emulsion pretreatment also downregulated the NF-κB-dependent anti‑apoptotic molecules Bcl-2, survivin and cyclooxygenase-2. In vivo, coix seed emulsion combined with gemcitabine had a much greater antitumor effect than the effect of either agent alone, consistent with the downregulation of the proliferation index, and the results of immunostaining for Ki-67, or for the NF-κB subunit p65. Overall, our data demonstrated that coix seed emulsion abrogated gemcitabine-induced activation of NF-κB, and synergistically sensitized PC cells to gemcitabine therapy.
Emodin-Enhanced Arsenic-Induced Apoptosis on Esophageal Tumor Cells

Abstract:Objective To determine if natural emodin can enhance arsenic-induced apoptosis via increasing reactive oxygen species(ROS) on cultured esophageal tumor cells in vitro and transplanted neoplasms with esophageal tumor cells in vivo,and to demonstrate the effect of emodin on pro-apoptosis signaling as well as anti-apoptosis signaling.Methods In vitro study,after the esophageal cells were exposed to arsenic combined with emodin,the apoptotic rate,release of cytochrome C from mitochondrion,activity of survival-related transcription factor NF-κB and expression of targeted protein survivin were determined.In vivo study,the animal models bearing esophageal tumor cells were administrated with emodin and arsenic intraperitoneally.After chemotherapy,the tumors were weighted and(apoptosis) of tumor cells,translocation of NF-κB to nuclei and expression of survivin in neoplasms were assayed.(Results)Emodin enhanced apoptosis of esophageal tumor cells induced by arsenic via augmenting release of cytochrome C from mitochondrion and inhibiting NF-κB activity in ROS dependent manner.Meanwhile,cotreatment of emodin and arsenic enhanced apoptosis of neoplasms,inhibited translocation of NF-κB and down-regulated survivin in neoplams. Conclusion Emodin enhanced apoptosis of esophageal tumor cells in vitro and in vivo.The mechanisms of emodin involved enhancement of pro-apoptotic signaling pathway as well as inhibition of anti-apoptotic signaling pathway.There exists the practical feasibility of emodin in chemotherapy.

Abstract:Glutathione conjugation and transportation of glutathione conjugates of anticancer drugs out of cells are important for detoxification of many anticancer drugs. Inhibition of this detoxification system has recently been proposed as a strategy to treat drug-resistant solid tumors. Gallbladder carcinoma is resistant to many anticancer drugs, therefore, it is needed to develop a novel strategy for cancer therapy. In the present study, we tested the effect of emodin (1,3,8-trihydroxy-6-methylanthraquinone), a reactive oxygen species (ROS) generator reported by our group previously, in combination with cisplatin (CDDP), carboplatin (CBP) or oxaliplatin in treating the gallbladder carcinoma cell line SGC996. Our results showed that co-treatment with emodin could remarkably enhance chemosensitivity of SGC996 cells in comparison with cisplatin, carboplatin or oxaliplatin treatment alone. We found that the mechanisms may be attributed to reduction of glutathione level, and downregulation of multidrug resistance-related protein 1 (MRP1) expression in SGC996 cells. The experiments on tumor-bearing mice showed that emodin/cisplatin co-treatment inhibited the tumor growth in vivo via increasing tumor cell apoptosis and downregulating MRP1 expression.
Β2-Adrenoceptor Blocker Synergizes with Gemcitabine to Inhibit the Proliferation of Pancreatic Cancer Cells Via Apoptosis Induction.

Abstract:The stimulation of β2-adrenoceptor, which is a major mediator for chronic stress-induced cancers, has been implicated in the progression in the number of cancer cells, including pancreatic cancer, which remains one of the most aggressive and lethal diseases worldwide. Whether β-adrenoceptor antagonists potentiate gemcitabine, a standard first-line treatment for advanced pancreatic cancer that offers only modest benefit due to acquired chemoresistance, has not been elucidated. Thus, we studied the antiproliferative and apoptotic effects and the underlying mechanisms of gemcitabine combined with the β2-adrenoceptor blocker ICI 118551 (1-[2,3-(dihydro-7-methyl-1H-iden-4-yl)oxy]-3-[(1-methylethyl)amino]-2-butanol), in human pancreatic cancer BxPC-3 and MIA PaCa-2 cells. Results show that ICI 118551 significantly synergized the antiproliferative and pro-apoptotic effects induced by gemcitabine in both BxPC-3 and MIA PaCa-2 cells (P<0.05 combination vs. control or gemcitabine alone). When cells were treated with the combination of gemcitabine and ICI 118551, NF-κB activation was blocked; the expression of Bax protein was substantially increased; and Bcl-2 protein was downregulated. Taken together, the data suggest that ICI 118551 potentiates the antiproliferative effects of gemcitabine by inducing apoptosis in pancreatic cancer cells. Our study implies that this combination may be an effective therapeutic strategy for pancreatic cancer.
Study of the survivin expression in pancreatic carcinoma cell line PANC-1 induced by gemcitabine

Abstract:Objective To study the relationship between chemoresistance and the expression of survivin,induced by gemcitabine,in pancreatic carcinoma cell line PANC-1.Methods The pancreatic carcinoma cell line PANC-1 was cultured by RPMI 1640 and gemcitabine with the concentration of 1 μg /ml and 10 μg /mL respectively,and survivin expression was examined by RT-PCR and Western blot.The relationship of the survivin level of the cell and its chemoresistance to gemcitabine was analyzed.Results Tweenty-four and 48 hours after culture with gemcitabine concentration of 1 μg /ml and 10 μg /mL respectively,survivinmRNA level was increased(1.34±0.12),(2.40±0.17),and(3.33±0.20),(4.41±0.18) folds,and the protein expression was enhanced(1.20±0.07),(1.48±0.19),and(2.90±0.04),(4.50±0.20) folds respectively.There was a significant difference between the two groups(P<0.05).Conclusions The intensity of pancreatic carcinoma cell lines chemoresistance could be enhanced by the action of gemcitabine on the up-regulated expression of survivin.

Abstract:Objective:To explore the expression of apoptotic inhibitory protein survivin in pancreatic cancer cell line PaTu8988,and to study its role in the drug resistance of PaTu8988 cells to gemcitabine(GEM).Methods: The inhibitory effect of GEM(0.01,0.1,1.0,2.5,5.0,and 10.0 μg/ml) on PaTu8988 cells was detected by MTT assay;apoptosis rate of PaTu8988 cells treated with GEM was determined by flow cytometry;and the survivin mRNA expression in PaTu8988 cells was examined by RT-PCR.Results: High dosage of GEM(≥1.0 μg/ml) greatly inhibited growth and promoted apoptosis of PaTu8988 cells,while low dosage of GEM(0.01,0.1 μg/ml) showed no effects.Low dose of GEM time-dependently increased expression of survinin mRNA in PaTu8988 cells;high dosage of GEM gradually inhibited survivin mRNA expression within the first 48 h,and then survivin mRNA expression gradually increased as time went by.Conclusion: Survivin mRNA is highly expressed in pancreatic cancer cell line PaTu8988,which may be one of the reasons for drug resistance to GEM.
Gemcitabine Enhances OSI-027 Cytotoxicity by Upregulation of Mir-663A in Pancreatic Ductal Adenocarcinoma Cells

Abstract:Pancreatic ductal adenocarcinoma (PDAC) is well-known to be the most deadly malignancy with the worst survival rate of all cancers. Gemcitabine-based chemotherapy is the most common treatment option for pancreatic ductal adenocarcinoma. However, it offers little therapeutic value in many cases due to the rapid development of chemoresistance. MicroRNAs (miRNAs) have been found to play pivotal roles in the chemotherapeutic resistance of PDAC. In the present study, we examined the molecular basis for the effective combination of OSI-027 and gemcitabine (GEM). Firstly, we identified a specific miRNA expression profile in PDAC cells after treatment with either of these drugs. We found that miR-663a was significantly upregulated after treatment with GEM and downregulated after OSI-027 treatment. With combination of the two drugs, miR-663a level was lower than the GEM group, but higher than the OSI-027 group. Real-time quantitative PCR confirmed these observations. To further establish the role of miR-663a in OSI-027 and GEM resistance in pancreatic cancer, we transfected PDAC cells with miR-663a mimic or miR-663a inhibitor. Cell viability and proliferation assays showed that miR-663a mimic enhanced drug sensitivity, while inhibitor promoted drug resistance. Moreover, we found that the combined effect of OSI-027 and GEM disappeared after inhibiting miR-663a. Our study clearly demonstrates that GEM upregulates miR-663a, thereby promoting the sensitivity of pancreatic cancer cells to OSI-027. Our study suggests that miR-663a expression may be a useful indicator of the potential for chemoresistance and provides a potential new therapeutic target to avert chemoresistance in PDAC.
Enhancing Apoptosis and Overcoming Resistance of Gemcitabine in Pancreatic Cancer with Bortezomib: A Role of Death-Associated Protein Kinase-Related Apoptosis-Inducing Protein Kinase 1

Abstract:Aims and background. To investigate the role of the apoptosis gene, DAP (death-associated protein) kinase-related apoptosis-inducing protein kinase 1 (DRAK1), which is involved in enhancing cell sensitivity and overcoming cell resistance to gemcitabine in pancreatic cancer cells by the proteasome inhibitor bortezomib.Methods. Cultured human pancreatic cancer gemcitabine-sensitive cell lines (bxpc3) and gemcitabine-resistant (panc-1) cell lines were divided into four groups: control, treatment with bortezomib, treatment with gemcitabine, and the two-drug combination. Expression of DRAK1 genes in each group was detected by using reverse transcription-polymerase chain reaction and western blot. Apoptosis in the pancreatic cancer cell lines was measured by flow cytometry.Results. We found that the effects of growth inhibition and apoptosis of gemcitabine on both pancreatic cancer cell lines were enhanced by bortezomib. Treatment of panc-1 and bxpc-3 cells with bortezomib (100 nM) and gemcitabine (50 mu g/ml and 0.05 mu g/ml,respectively) induced an increase in the levels of DRAK1 mRNA compared with the control and single-agent treatment. Furthermore, immunblotting analysis in panc-1 but not bxpc-3 cells showed similar changes in the expression of DRAK1 protein produced by combination therapy.Conclusions. Our results demonstrated that bortezomib enhanced cell sensitivity and overcame cell resistance to gemcitabine in pancreatic cancer cells, which may be attributed to DRAK1 induced by bortezomib and the combination with gemcitabine.

Abstract:Background/objectives: There are increasing evidences suggesting that chemotherapeutic agents can enhance the cytotoxic T lymphocytes (CTLs) antitumor effect, but the precise mechanism is not fully explained. This study aims to investigate whether gemcitabine (GEM) can sensitize pancreatic cancer cells to the CTLs antitumor response, and explore the potential mechanism.Methods: Cell counting kit-8 assays (CCK-8) were performed to determine the tumor cell proliferation. Flow cytometric analysis was conducted to analyze maturation of DCs and the expression of Fas. An Annexin V FITC Apoptosis Detection Kit was performed to detect tumor cell apoptosis. CytoTox 96 Nonradioactive Cytotoxicity assays were used to determine T cell-mediated tumor cell lysis.Results: First, it was demonstrated that Bacillus Calmette Guerin (BCG) could be used to induce effective CTLs antitumor response. Then, GEM inhibited the growth of SW1990 cells, induced apoptosis and upregulated the Fas expression even at a low concentration. When antagonistic anti-Fas mAb ZB4 was preincubated with GEM-treated SW1990 cells, the lysis induced by CTLs was reduced. Moreover, agonistic anti-Fas mAb CH11 induced more apoptosis of GEM-treated SW1990 cells.Conclusion: Our results show that GEM sensitizes pancreatic cancer cells to the CTLs antitumor response, and the sensitization is associated with upregulation of Fas on pancreatic cancer cells. EGFR overexpression may be detected in up to 90% of pancreatic tumors. However, drug resistance of EGFR inhibitors targeting treatment severely limits its clinical application. Methods: In this study, Western blotting was used to detect the expression of p-Stat3, EGFR, Bcl-2, cleaved-caspase3 and Bax. Cell apoptosis was evaluated via flow cytometry. The colon assay and MTT assay were applied for detecting the cell proliferation in vitro. The xenograft mouse model was used to examine the cell proliferation in vivo. Results: Emodin remarkably enhanced the anti-cancer effect of EGFR inhibitor on pancreatic cancer cells. In addition, emodin promoted afatinib-induced apoptosis by inhibiting the Stat3 signaling pathway. Meanwhile, siRNAs against Stat3 significantly increased the apoptosis of pancreatic cancer cells. EGFR inhibitor promoted phosphorylation of Stat3 in pancreatic cancer cells. Interestingly, emodin combined with EGFR inhibitor inhibited the proliferation of pancreatic cancer cells in vitro. The tumor xenograft mice model was further confirmed that emodin possessed a synergy anticancer effect with afatinib on pancreatic cancer cells by regulating the Stat3 expression. Conclusion: These results indicate that the combination of emodin with EGFR inhibitor is an effective therapeutic strategy to sensitize human pancreatic cancer. Abstract:Gemcitabine is a first-line chemotherapeutic agent used in the treatment of pancreatic cancer; however resistance of the disease to the drug often develops over time. Agents that can either enhance the effects of gemcitabine, or help to overcome the chemoresistance to the drug are needed for the successful treatment of pancreatic cancer. Oridonin is one such agent which is safe and multi-targeted and has previously been shown to induce apoptosis in other tumor cells, through mitochondrial signaling pathways. The aims of the present study were to evaluate whether oridonin may enhance the effects of gemcitabine on pancreatic cancer in vitro and to investigate the possible mechanisms of this enhancement. In vitro studies have previously shown that oridonin can inhibit the proliferation of the PANC-1 pancreatic cancer cell line, and potentiate gemcitabine-induced apoptosis, which was shown to be associated with cell cycle arrest in the GI phase. Western. blot and quantitative polymerase chain reaction analyses demonstrated that the expression levels of the anti-apoptotic gene Bcl-2 and the Bcl-2/Bax ratio in the oridonin and the oridonin plus gemcitabine groups were significantly down-regulated as compared with the gemcitabine treatment and control groups. The expression levels of pro-apoptotic genes Bax, cytochrome c (cyt c), and caspase-3 and -9 in the oridonin and the combination groups were significantly upregulated as compared with the other two groups. The results suggested that oridonin improved the anti-tumor effects of gemcitabine through the enhancement of gemcitabine-induced apoptosis. This mechanism may be through the downregulation of Bcl-2 expression and the upregulation of Bax expression, resulting in the reduction of the Bcl-2/Bax ratio. These effects may promote the release of cyt c from the mitochondria into the cytoplasm thus triggering the mitochondrial apoptosis signaling pathway. Furthermore, caspase-3 and -9 were shown to be activated as a result of the induction of apoptosis.

Abstract:Background: Pancreatic cancer remains one of the most lethal malignancies, and has limited effective treatment. Gemcitabine (GEM), a chemotherapeutic agent, is commonly used for clinical treatment of pancreatic cancer, but it has characteristics of low drug delivery efficiency and significant side effects. The study tested the hypothesis that human bone marrow mesenchymal stem cell (MSC)-derived exosomes loaded with GEM (Exo-GEM) would have a higher cytotoxicity against human pancreatic cancer cells by enhancing their apoptosis. Aim: To investigate the cytotoxicity of MSC-derived Exo-GEM against pancreatic cancer cells in vitro. Methods: Exosomes were isolated from MSCs and characterized by transmission electron microscopy and nanoparticle tracking analysis. Exo-GEM through electroporation, sonication, or incubation, and the loading efficiency was evaluated. The cytotoxicity of Exo-GEM or GEM alone against human pancreatic cancer Panc-1 and MiaPaca-2 cells was assessed by MTT and flow cytometry assays. Results: The isolated exosomes had an average size of 76.7 nm. The encapsulation efficacy and loading efficiency of GEM by electroporation and sonication were similar and significantly better than incubation. The cytotoxicity of Exo-GEM against pancreatic cancer cells was stronger than free GEM and treatment with 0.02 μM Exo-GEM significantly reduced the viability of both Panc-1 and MiaPaca-2 cells. Moreover, Exo-GEM enhanced the frequency of GEM-induced apoptosis in both cell lines. Conclusion: Human bone marrow MSC-derived Exo-GEM have a potent cytotoxicity against human pancreatic cancer cells by enhancing their apoptosis, offering a promising drug delivery system for improving therapeutic outcomes.
