Epithelioid Angiomyolipoma of Kidney: a Report of Three Cases and Review of Literature

Abstract:Purpose To investigate the clinicopathological features,diagnosis and differential diagnosis of renal epithelioid angiomyolipoma(EAML).Methods Three cases of EAML were analyzed by gross examination,light microscopy and immunohistochemistry.The literature on this tumor was reviewed to discuss the histologic features of EAML and its clinical behavior.Results Three patients were 28,40 and 32 years old,respectively.All cases had flank pain with no evidence of tuberous sclerosis,one of them presented with gross hematuria.The location of each tumor was in the kidney.Grossly,the largest diameters of tumors were 6 cm,8 cm and 3 cm,respectively.Tumors presented clear border,and case 2 showed cystic component.Tumors lacked capsule with hemorrhage and necrosis in two cases,and showed fragility or tenacity.Microscopically,the neoplastic cells were large or huge,exhibited polygonal epithelioid appearance with abundant eosinophilic or clear cytoplasm.Some cells contained nucleoli and hyaline pseudoinclusions.Scattered multinucleated cells were also evident.The neoplastic cells were diffuse or nests in arrangement.The necrotic areas in the neoplasm were more evident in two cases showing invasive margin.All cases did not present evidence of AML.Immunohistochemically,the neoplastic cells were positive for HMB-45,focally positive for SMA and vimentin,but negative for CK,EMA and CD10.Conclusions EAML is a monotypic differentiated neoplasm containing large epithelioid cells with more specific HMB-45 expression.The differential diagnosis is conducted from other kidney tumors with auxiliary immunohistochemistry.
