Revaluation of Serum Fibrotic Markers for Predicting Hepatopathological Grading and Staging

Abstract:Objective To build mathematical models for diagnosing liver inflammation and fibrosis degrees in chronic hepatitis,and to revaluate the value of serum fibrotic markers in clinical practice.Methods 81 patients with chronic hepatitis were enrolled into present study.The specimens of liver biopsy were diagnosed for pathological grading(G) and staging(S).Serum fibrotic markers including hyaluronic acid,procollagen type Ⅲ,laminin,type Ⅳcollagen,prolidase and cholylglycine were determined by radioimmunoassay.Taking pathological grading or staging as the grouping variables,and serum fibrotic markers as the independents,the Fisher's linear discriminant functions were built by Bayes's stepwise discriminant analysis.Results According to the discriminant models for predicting pathological grading,the correctly classified rates of the original grouped cases were 62.50% for G1,21.05% for G2 and G3 for 62.96%,respectively;and the correctly classified total rate of the original grouped cases was 43.21%;the susceptibility,specificity,positive and negative predictive value of the discriminant models for predicting G3 were 62.96%,72.22%,53.13% and 79.59%,respectively.According to the discriminant models for predicting pathological staging,the correctly classified rates of the original grouped cases were 80.95% for S1,12.00% for S2,33.33% for S3 and 50.00% for S4,respectively;and the correctly classified total rate of the original grouped cases was 40.74%;the susceptibility,specificity,positive and negative predictive value of the discriminant models for predicting S4 were 50%,83.56%,25% and 93.85%,respectively.Conclusions The serum fibrotic markers have restricted value for predicting liver inflammation and fibrosis degrees in chronic hepatitis;and the main value of the discriminant models for diagnosing the severe liver inflammation(≥G3) and cirrhosis(S4) were negating cirrhosis than affirming cirrhosis.
