Clinical Analysis of 41 Cases of Pulmonary Cryptococcosis

Abstract:Objective To increase the cognition of pulmonary cryptococcosis from analyzing its clinical characteristic.Methods Forty-one patients diagnosed as pulmonary cryptococcosis in our hospital(2003.1～2006.10)were included and their clinical characteristics,imaging findings,immunological test,pathologies,antifungal therapies and follow-up results were studied.Results ①The patients consisted of 28 males and 13 females.Eighteen cases had basis diseases,and 22 cases had clinical symptoms.Imaging manifestation included mass and nodule change(n=27),pneumonia-like change(n=8),mix change(n=6).②In the 18 cases that had basis diseases:15 cases had clinical symptoms;Imaging findings:pneumonia-like change(n=8),mix change(n=4),mass and nodule change(n=6);2 cases had the complication of cryptococcal meningitis.Majority of the 23 cases without basis diseases had no or slight clinical symptoms and manifested mass and nodule change(n=21).The 23 cases had no complication and their prognosis was good.③36 cases had been treated by anti-fungi drugs(effective rate100%),cure(n=34),mend(n=2).Most of those cases were treated with fluconazole and the course of treatments were in the range from two weeks to two years.Conclusion The immunocompetent patients with pulmonary cryptococcosis had the localized pathology,and their imaging manifestation most were mass and nodule,and their prognosis was good.The immunocompromised patients had the exentsive pathology,and their imaging manifestation most were pneumonia-like change or mix change,and had the relatively highly rate suffering the dissemination of Cryptococcosis.The follow-up results were good by treating with fluconazole.
