Abstract:Fetal heart rate monitoring is very important for improving the quality of perinatal fetus. At present, the common fetal heart rate monitoring is always based on the fetal heart sound, which is difficult for long time fetal heart monitoring, and fetal ECG contains more information about fetal health. Therefore, it is of great value for research on fetal ECG monitoring. Although there is a long time in the research on the fetal ECG monitoring. But, it has not yet reached the goal of mature application. In this article, the current research situation of fetal ECG in domestic and overseas was summarized, and the difficulties of the fetal heart monitoring were analyzed at the same time. Then several methods of fetal heart monitoring and their development were introduced briefly. Next, the advantages and disadvantages of various kinds of algorithms were analyzed.

Abstract:The system is composed of a workstation, fetal monitors and family fetal monitors. The workstation is located at a central hospital. Pregnant women are free of cables, based on cordless transducer technologies, so that they are free to roam around the hospital. Fetal monitors in community clinics are connected to the workstation by Internet, so that the community clinics can be technically supported by the central hospital. Pregnant women can be cared by family monitors which can be connected by PSTN or Internet.

Abstract:Fetal electrocardiography (ECG) monitoring during pregnancy can provide crucial information for assessing the fetus's health status and making timely decisions. This paper proposes a portable ECG monitoring system to record the abdominal ECG (AECG) of the pregnant woman, comprising both maternal ECG (MECG) and fetal ECG (FECG), which could be applied to fetal heart rate (FHR) monitoring at the home setting. The ECG monitoring system is based on data acquisition circuits, data transmission module, and signal analysis platform, which consists of low input-referred noise, high input impedance, and high resolution. The combination of the adaptive dual threshold (ADT) and the independent component analysis (ICA) algorithm is employed to extract the FECG from the AECG signals. To validate the performance of the proposed system, AECG is recorded and analyzed of pregnant women in three different postures (supine, seated, and standing). The result shows that the proposed system can record the AECG in different postures with good signal quality and high accuracy in fetal ECG and heart rate information. Sensitivity (Se), positive predictive accuracy (PPV), accuracy (ACC), and their harmonic mean (F1) are utilized as the metrics to evaluate the performance of the fetal QRS (fQRS) complexes extraction. The average Se, PPV, ACC, and F1 score are 99.62%, 97.90%, 97.40%, and 98.66% for the fQRS complexes extraction,, respectively. This paper shows the proposed system has a promising application in fetal health monitoring.

Abstract:Objective:There are some defects in current electronic fetal monitoring (EFM), such as strong subjectivity in the diagnosis, inconvenience in detecting and lack of detection time. Methods: The fetal/Maternal heart rate monitor based on ECG was selected as data acquisition equipment, and other hardware elements include smart handheld devices, servers and central monitoring station. Python was the programming language, MySql was the database, bluetooth was the connection between the monitor and the handheld devices, HTTP/TCP was the remote data transmission mode, and MVC (Model View Controller) framework was used to organize code, in order to achieve function implementation and test completion.Results: Here a kind of remote fetal monitoring software system was proposed, including pregnant monitoring client, server software and central monitoring station software. Using the fetal/maternal heart rate recorder which is based on ECG, the system could achieve more than 20 hours of remote real-time monitoring, heart rate alarm detection and physiological parameter analysis.Conclusion: The system could be applied in ward-nurse station or home-doctor scenarios, providing more objective basis for clinical diagnosis.

Abstract:Extracting clean fetal electrocardiogram (ECG) signals is very important in fetal monitoring. In this paper, we proposed a new method for fetal ECG extraction based on wavelet analysis, the least mean square (LMS) adaptive filtering algorithm, and the spatially selective noise filtration (SSNF) algorithm. First, abdominal signals and thoracic signals were processed by stationary wavelet transform (SWT), and the wavelet coefficients at each scale were obtained. For each scale, the detail coefficients were processed by the LMS algorithm. The coefficient of the abdominal signal was taken as the original input of the LMS adaptive filtering system, and the coefficient of the thoracic signal as the reference input. Then, correlations of the processed wavelet coefficients were computed. The threshold was set and noise components were removed with the SSNF algorithm. Finally, the processed wavelet coefficients were reconstructed by inverse SWT to obtain fetal ECG. Twenty cases of simulated data and 12 cases of clinical data were used. Experimental results showed that the proposed method outperforms the LMS algorithm: (1) it shows improvement in case of superposition R-peaks of fetal ECG and maternal ECG; (2) noise disturbance is eliminated by incorporating the SSNF algorithm and the extracted waveform is more stable; and (3) the performance is proven quantitatively by SNR calculation. The results indicated that the proposed algorithm can be used for extracting fetal ECG from abdominal signals.

Abstract:In this paper, a fetal electrocardiogram (ECG) monitoring system based on the Android smartphone was proposed. We designed a portable low-power fetal ECG collector, which collected maternal abdominal ECG signals in real time. The ECG data were sent to a smartphone client via Bluetooth. Smartphone app software was developed based on the Android system. The app integrated the fast fixed-point algorithm for independent component analysis (FastICA) and the sample entropy algorithm, for the sake of real-time extraction of fetal ECG signals from the maternal abdominal ECG signals. The fetal heart rate was computed using the extracted fetal ECG signals. Experimental results showed that the FastICA algorithm can extract a clear fetal ECG, and the sample entropy can correctly determine the channel where the fetal ECG is located. The proposed fetal ECG monitoring system may be feasible for non-invasive, real-time monitoring of fetal ECGs.

Abstract:The fetal heart rate is vital for monitoring fetal well-being. Fetal heart rate monitoring based on acoustic techniques is passive and noninvasive. In this work, a fetal heart rate monitoring system based on phonocardiographic method is proposed. A portable low-power stethoscope is customized which meets the need for sensitivity in the monitoring. A noise cancellation method and adaptive matching method are applied to extract the fetal heart rate effectively. Clinical trials are carried out on pregnant women, and the comparison of fetal heart rates given by the proposed system with those given by the Doppler monitor is given to show the accuracy.

Abstract:A central fetus monitoring system is introduced. This system is composed of a central workstation, mother/fetus monitoring inside hospital and family used fetus monitoring. Only one work station is needed for the whole system, including the sector inside the hospital and the external sector for family use and other small hospitals. Pregnant women in obstetrics hospitals and homes can be monitored in real time and all the time at the central workstation. The sector inside the hospital is based on RS 485 bus, while the external sector for family use is based on public telephone network. Unique techniques were adopted in the fetus monitoring. Part of the key circuits was built in the fetal heart sensor for noise reduction and reliability, and unique algorithm for fetal heart rate detection was designed. Besides, the monitors can operate independently or communicate with the central workstation through RS 485 bus. A remote or family used monitoring system was designed. The family used fetus monitor is portable and stable, which can detect doppler sound of the fetus, display the FHR values with its LCD and mark the fetal movements. It can be connected to the central workstation in the hospital throuth telephone line if needed. Thus, the information acquired at family side can be monitored in real time at workstation located in the hospital as if they were in the hosptal. Pregnant women and doctors at hospital can talk with each other while monitoring is going on. Built in chip technology is used and it is an effective solution for remote monitoring of fetus. Computer assistant diagnosis software was designed. Its functions include automatic evaluation of fetal monitoring curves, chart for delivery process, etc.

Abstract:Objective:To establish the fetal heart rate monitoring system based on the internet of things so that pregnant women could perform fetal heart rate monitoring anywhere and anytime.Methods:The research used the embedded microcontroller system to process fetal heart rate and uterine pressure signal of perception layer.The server management software was developed by Eclipse demonstrated the monitoring data and gave feedbacks of the diagnosis.The research also adopted a two-way data transmission and reception with bluetooth between smart phones.The research tested the accuracy of the portable fetal heart monitor among full-term pregnant women as the clinical trials subjects.Results: The results showed that fetal heart rate accuracy was more than 98%, the fetal heart rate detection error was less than ±2 bpm, non-linearity error of pressure survey was between -9.1%and+7.7%.Two measurements had no significant differences ( P>0.05 ) .Conclusion: It could be concluded that the operation stability and detection accuracy of fetal heart monitoring system based on the internet of things had met the design requirements.The fetal heart rate monitor can accurately reflect the fetal situation by displaying correct fetal heart rates and uterine pressure.Real-time transmission of information allows experts to deal with any emergency situations in time.

Abstract:In this paper,the design and implementation of a fetal electrocardiogram(ECG) extraction system based on surface electrode placing on maternal abdominal region is introduced.The system consists of an acquisition circuit and ECG analysis software.The hardware module includes eight channels ECG amplifiers(extensible),analog filter and A/D circuit.The software module contains multi-channel data acquisition and access control,digital filter,Independent Component Analysis(ICA) algorithm and real-time display of I/O waveform,etc.The clinical data and actual simulation data prove that this system can effectively extract fetal ECG from maternal ECG,and both the modules are able to satisfy the design requirements.The experiments demonstrate that the fetal ECG system introduced in this paper has high clinical value;further,it testifies the feasibility of ICA algorithms applied to fetal ECG extraction.In the end,it explains the advantages of this system over the relevant systems,and proposse updating direction and application prospects of the system.
FetalCare: A Home Telemonitoring System for Wearable Fetal-ECG and EHG Acquisition During Pregnancy

Abstract:A wearable and remote fetal monitoring system, FetalCare, is specifically developed for home use. It comprises a stretchable on-skin patch and a wearable pod to capture, process, and transmit fetal signals. Data received by a Bluetooth dongle is processed by edge computing to remove diverse noises and interferences and then displayed by a fetal monitoring platform integrated with cloud services. The highly integrated system can simultaneously measure up to 6-channel electrohysterography (EHG) and 3-channel fetal electrocardiogram (FECG) signals from the abdomen using just a 4-channel data acquisition chip. Electrically conductive composites (ECCs) and silicone implement the stretchable on-skin electrode. The compact wearable pod achieves low input noise of 0.191μVrms (250Hz bandwidth) while consuming 56mW and can operate continuously for up to 36 hours. We use edge computing to process fetal signals, which utilize the fast independent component analysis (FastICA) and Empirical Mode Decomposition (EMD) to obtain high-quality FECG and EHG signals. The fetal monitoring platform based on InfluxDB and Grafana supports real-time, remote monitoring with low latency and historical data retrieval. Finally, we used the Contraction Consistency Index (CCI) to assess the EHG and used metrics such as sensitivity (Se), positive predictive accuracy (PPV), accuracy (ACC), and their harmonic mean (F1) to assess the FECG. The CCI for EHG signals measured by the FetalCare system is 0.8, which implies a highly consistent result with the clinical CTG equipment. The average Se, PPV, ACC, and F1 scores for FQRS complexes extraction are 97.97%, 98.62%, 96.58%, and 98.25%, respectively.

Abstract:This article presents an innovative approach to analyzing and extracting electrocardiogram (ECG) signals from the abdomen and thorax of pregnant women, with the primary goal of isolating fetal ECG (fECG) and maternal ECG (mECG) signals. To resolve the difficulties related to the low amplitude of the fECG, various noise sources during signal acquisition, and the overlapping of R waves, we developed a new method for extracting ECG signals using blind source separation techniques. This method is based on independent component analysis algorithms to detect and accurately extract fECG and mECG signals from abdomen and thorax data. To validate our approach, we carried out experiments using a real and reliable database for the evaluation of fECG extraction algorithms. Moreover, to demonstrate real-time applicability, we implemented our method in an embedded card linked to electronic modules that measure blood oxygen saturation (SpO2) and body temperature, as well as the transmission of data to a web server. This enables us to present all information related to the fetus and its mother in a mobile application to assist doctors in diagnosing the fetus's condition. Our results demonstrate the effectiveness of our approach in isolating fECG and mECG signals under difficult conditions and also calculating different heart rates (fBPM and mBPM), which offers promising prospects for improving fetal monitoring and maternal healthcare during pregnancy.

Abstract:This paper reports the applications of a local non-linear projection technique to extract the fetal component in single-lead recordings from the abdomen of a pregnant woman. We propose the general planning, and then we determine and illustrate four important parameters in the algorithm. The results of experiments show that the method is effective for separating the fetal ECG from the maternal trace and from the other contaminations.

Abstract:Electronic fetal monitoring (EFM) is an effective approach to evaluate the status of fetal intrauterine growth and safety. In clinic, fetal monitoring parameters and curves are usually judged artificially, but there are serious problems of insufficient accuracy and uniformity. A PC-based and computer-aided analyzing system has been developed for fetal parameters, on the basis of scoring methods which are widely used in clinic. The functions of automatic analysis of characteristic parameters and automated fetal condition scoring are realized in this system. The management of fetus during pregnancy is available with the help of the scoring guidance. Verified by large clinical cases, there is good uniformity between the results processed by the system and obstetric experts.

Abstract:Fetal heart sound is an important part of fetal monitoring and has attracted extensive research and attention from scholars at home and abroad in recent years. The fetal heart rate, extracted from the fetal heart sound signal, is one of the important indicators that reflect the health of the fetus in the womb. In this study, a maternal-fetal Holter monitor based on f ECG technology was used to collect maternal heart rate, fetal heart rate, and uterine contractions signals, isolate the fetal heart rate, and design an algorithm to extract the fetal heart rate baseline, acceleration, variation, wake-up cycle, and nonlinear parameters. Using statistical methods to analyze the average value and range of various characteristic parameters of fetal heart rate under continuous long-term monitoring, the results show that the baseline has a downward trend from 10 o'clock in the night to 4 o'clock in the morning and is the lowest around 2 o'clock in the morning. The area and acceleration time were significantly higher than those in the suspicious group. However, there was no significant difference in the number of acceleration values between the two groups; the proportion of small mutations in the normal group was lower than that of the suspicious group and the proportion of medium mutations was higher than that of the suspicious group. There is no statistically significant difference in maternal age, gestational age at childbirth, pregnancy comorbidities, and complication rates in the five-level interpretation system of ACOG (2009), RCOG (2007), SOGC (2007), and the United States (2007). The difference of pregnancy and parity in various images was statistically significant, P < 0.05. The second type of fetal heart rate monitoring images appeared in the highest among the diagnostic standards, and the difference in the second type of fetal heart rate monitoring images between the various diagnostic standards was statistically significant, P <= 0.001.

Abstract:Fetal heart rate monitoring during pregnancy can help to diagnose distress and morbidity for a fetus. Noninvasive fetal electrocardiography (NI-FECG) is a promising technology that provides critical information about a fetus's health status. Our study applies the template subtraction (TS) algorithm for fetal ECG signal extraction from abdominal ECG (AECG) recordings. The combination of discrete wavelet transform (DWT) and adaptive double threshold (ADT) algorithm is utilized to detect QRS waves for fetal ECG signals. To verify the algorithm's effectiveness, the Cardiology Challenge 2013 database (PCDB) and abdominal and direct fetal ECG database (ADFECGDB) are employed. The proposed method detects 8999 fetal QRS complexes, wrongly locates 707 fetal QRS complexes, and miss detects 583 fetal QRS complexes for the PCDB. The diagnostic Se, PPV, and F1 scores are 93.92%, 92.72%, and 93.31% of the PCDB. In addition, our work detects 3191 fetal QRS complexes, wrongly locates 62 fetal QRS complexes, and miss detects 38 fetal QRS complexes for AECG recordings of the ADFECGDB. Diagnostic Se, PPV, and F1 scores are 98.81%, 98.07%, and 98.44% of the ADFECGDB. The results show that our proposed method can improve the accuracy of fetal QRS complex detection to a certain extent.

Abstract:BACKGROUND: In the fetal electrocardiogram (ECG) acquisition process, it is interfered by maternal ECG and other noise. It becomes important that how to extract rapidly and effectively fetal ECG. OBJECTIVE: Using independent component analysis and wavelet analysis to isolate maternal detection signals to obtain effective fetal ECG. METHODS: Fetal ECG was extracted using the method combining independent component analysis and wavelet analysis. First, noised signals underwent wavelet transform to eliminate strange signals and non-stable random signals, followed by rapid independent component analysis. RESULTS AND CONCLUSION: Fetal ECG collection was interfered by maternal ECG and other noise, but these signals are random, not related, and can be considered independent of each other. Each independent component could be extracted effectively by using the method combining independent component analysis and wavelet analysis. Experiments show that the method is effective.

Abstract:Despite wearable electronic devices having enormous potential applications, the recording quality has not sufficiently met the requirements for fetal disease detection. In this article, we aim to propose a novel framework for fetal electrocardiography (ECG) extraction, where the hybrid approach mainly considers the combination of a novel adaptive maternal QRS removal (AQRSR) algorithm and a tensor-based joint blind source separation (JBSS) approach. To adapt to the dynamic change of the ECG signal and suppress the powerful maternal heartbeat, AQRSR is explored with an adaptive template, which is generated by a model consisting of a generator, discriminator, and transformer. By replacing the oldest segmentation with the newest cycle, the approach is capable of quickly adapting to the estimated output to match the new input. In addition, due to the significant crossover between the target signal and the noise, even if the maternal ECG removal processing is implemented, the residual signal inevitably contains fetal ECG, part of maternal ECG, and noise. As a consequence of this, a novel JBSS that incorporates tensor decomposition is formulated to separate fetal heartbeat from long-term recordings contaminated with maternal movements noise and heartbeat interference. By dividing the collected data into multiple segments, this method can extract fetal ECG signals by exploiting the signals that are not only statistically independent within each segment but can be dependent on different segments. Experimental results show that even if the recorded signals are contaminated by the noise, the proposed framework has comparable better performance on fetal ECG extraction.
