Abstract:Drug administration is referred to the monitoring and management in the chain of RD, manufacturing, distribution and utilization of drugs as well as pricing. It promotes the safety and efficacy of drug utilization so as to maintain the health benefits for people. This paper mainly reviewed the development history of drug administration since 1978, when China’ s reform and opening up policy was first initiated, and summarized the impacts and lessons from the reform.

Abstract:Rationale: To ensure that quality medicines are available to the populace. Aim and objective: To provide a new perspective for drug sampling and testing using a risk-based approach globally, and provide a reference for improving the efficiency of the drug sampling and testing programme in China. Method: This study used a combination of theoretical analysis and problem analysis. First, we analysed the high-risk drugs that should be given attention to theoretically in the drug sampling testing programme based on the healthcare reform in China. Second, this study leveraged on data on the sampling and testing of drugs extracted from self-constructed data by the China Food and Drug Administration (CFDA). The integrated and structured data sets were used to conduct an innovative analysis that identifies the actual content of the drug sampling and testing programme and contrasts them with the theoretical content to understand the problems. This study used quantitative and qualitative approaches for data analysis. Results: Following analysis, we discovered that there is still a big gap between the reality of drug sampling and testing in China and the ideal state of the objective requirements of healthcare reforms. The major sources of risk are the retail pharmacies or hospital pharmacies in the distribution chain of drugs; however, the CFDA sampled mostly drugs from drug manufacturers. Additionally, the CFDA did not sample adequate essential medicines, bid-winning drugs (especially those with large price drops in centralized procurement), biological products and other high-risk drug varieties. The high-risk drugs produced by companies or enterprises with a history of noncompliance, especially bid-winning enterprises in centralized procurement, were not sampled. Conclusion: We suggest that the major content of drug sampling and testing in China should focus on the link of drug utilization, varieties of drugs with large price drops in centralized procurement and bid-winning enterprises with a history of noncompliance.

Abstract:OBJECTIVE:To provide policy recommendations for optimizing medicines distribution system reform in China.METHODS:Literature review and field investigation were used to mapping the medicines distribution system. Problems and influ-ential factors were analyzed and policy recommendations for optimizing the medicines distribution system reform were put forward. RESULTS & CONCLUSIONS:Medicines distribution system has been undergoing a rapid development and transformation under the overall health system reform. Most medicines distributors are still with small scale,low concentration,high cost,low profitabil-ity,lack of core competence, development of new form facing challenges. A large number of drug distributors still follow the tradi-tional business model,know little about modern drug supply chain management,and not understand their appropriate roles under the new social and economic development. Medical E-commerce face both opportunities and challenges in the process of develop-ing. Drug distribution industry needs vertical and horizontal integration, speeds up the socialization of the third party logistics and the healthy development of the medical electricity supplier, improves and perfects the drug distribution supervision system which is suitable for the healthy development of new form.

Abstract:The paper has summarized the achievements,issues and policy recommendations for implementing essential medicines system in China since 2009.A series of national policies have been established which include essential medicine list,production,quality supervision,pricing,tendering and procurement,rational use of essential medicines,monitoring and evaluation.About 98.8% public primary health institutions and 41.5% village health posts have been conducted the essential medicines system and zero mark-up on sale.However,the 307 essential medicines could not meet the need of medical service in the grass-root health facilities,it will be impact on the volume and quality of health care services,local government was required to give financial subsidy for the loss caused by zero mark-up.The coverage of essential medicine system needs to be expanded into secondary and tertiary hospitals as well as private health sector.The essential medicines list needs to be dynamically adjusted.The balance between the price cutting and quality assurance of essential medicines should be paid more attention.The scientific method of pricing of essential medicines and designated manufacturer to produce some essential medicines needs to be exploited.

Abstract:: Although the current drug service system can meet the fundamental healthcare needs of China, outstanding issues exist in all service sectors, including research and development, manufacturing, marketing, and clinical use. Through a literature search, data queries, specialist consultation, and a panel discussion, this study investigated the current status of the drug service system and identified the main problems. We propose an idea for constructing a new national drug service system to provide drugs to the Chinese people that are of high quality, have a reliable therapeutic effect, come at a reasonable price, and exhibit good medical compliance, thus ensuring the efficacy, safety, availability, and reasonability of drugs for clinical use, as well as promoting a structural transformation of the pharmaceutical industry that will enable China to finally transition from a big country into a great power in the global pharmaceutical industry.
Health System Reform in Rural China: Voices of Healthworkers and Service-Users.

Abstract:Like many other countries China is undergoing major health system reforms, with the aim of providing universal health coverage, and addressing problems of low efficiency and inequity. The first phase of the reforms has focused on strengthening primary care and improving health insurance coverage and benefits. The aim of the study was to explore the impacts of these reforms on healthworkers and service-users at township level, which has been the major target of the first phase of the reforms. From January to March 2013 we interviewed eight health officials, 80 township healthworkers and 80 service-users in eight counties in Zhejiang and Yunnan provinces, representing rich and poor provinces respectively. Thematic analysis identified key themes around the impacts of the health reforms. We found that some elements of the reforms may actually be undermining primary care. While the new health insurance system was popular among service-users, it was criticised for contributing to fast-growing medical costs, and for an imbalance of benefits between outpatient and inpatient services. Salary reform has guaranteed healthworkers' income, but greatly reduced their incentives. The essential drug list removed perverse incentives to overprescribe, but led to falls in income for healthworkers, and loss of autonomy for doctors. Serious problems with drug procurement also emerged. The unintended consequences have included a brain drain of experienced healthworkers from township hospitals, and patients have flowed to county hospitals at greater cost. In conclusion, in the short term resources must be found to ensure rural healthworkers feel appropriately remunerated and have more clinical autonomy, measures for containment of the medical costs must be taken, and drug procurement must show increased transparency and accountability. More importantly the study shows that all countries undergoing health reforms should elicit the views of stakeholders, including service-users, to avoid and address unintended consequences.

Abstract:Solving the present issue of the hardship and expensive to seek medical care is related to the access, equity and cost of health service. But it is not the final outcomes of health reform and development. It reflects the cause of institutional issue in China. The paper puts forward a blueprint of China's health reform and development. It contains one goal: universal coverage of basic health service, strengthening three systems: basic medical insurance system, basic healthcare system and basic essential drug system as well as six strategic measurements. In the future 15 years, China will under the ideology of people-oriented and scientific development develop a harmonious society and socialist "Xiao Kang" society. Universal coverage of basic health service is the vision of Chinese population. To strengthening the responsibility of government and promoting the equity of health service, the author suggests making the universal primary education and universal coverage of basic health service as two basic social policies. However, the goal of universal coverage of basic health service is a long-term process, it requires mobilizing community involvement and coping with the social and economic development.

Abstract:According to the overall requirements of Healthy China 2020 Strategic Planning,this study comprehensively analyzes the demands and challenges for the domestic and abroad development of clinical medicine.It recommends a strategic design o the clinical medicine research,including three major solutions as such the construction of science and technology based support system,the key research areas,and the science and technology action plan.The purpose of this study is to realize the goal of Healthy China 2020 Strategic Planning in providing the basic medical services and healthcare for everyone and to improve the overall health quality of all citizens.

Abstract:ObjectiveTo review drug policies, health technology assessment (HTA), and HTA’s use in drug policies in China, to further improve the quality and efficiency of drugs.MethodsThis study draws on multiple methods. A systematic review of the literature, review of Chinese government documents and statistical handbooks, and authors’ experiences in drug policies and HTA in China were combined to achieve the objective.ResultsOf 571 studies identified in the initial search, 14 eligible articles (6 English, 8 Chinese) were finally included. On the Web site of the National Health and Family Planning Commission, the National Development and Reform Commission, and the Ministry of Human Resources and Social Security, we found that HTA or pharmacoeconomics evaluation is mentioned in recent years and its frequency has been increasing; however, there was not one hit about HTA or PE on the Web site of China Food and Drug Administration.ConclusionsThe decision makers have realized the importance and value of HTA and have tried to integrate HTA into drug policies and regulations. However, the application of HTA findings to drug policymaking is not yet widespread and there are a number of challenges in using HTA in China. Therefore, it is necessary to establish a national HTA commission and develop pharmacoeconomics guidelines to support the use of HTA in decision making. Moreover, the most important steps are to encourage technology innovation, groom more HTA experts, and build reliable databases in China.

Abstract:The sound implementation of a legal system depends on scientific ideas and an efficient supervision system. Presently, the drug supervision system is a period of transition, with the prospects of establishing a modern supervision system that incorporates the ideals of risk management. To this end, the responsibilities of the drug super-vision agency shall be adjusted horizontally and vertically so as to enhance the efficiency of supervision and to avoid an overlap of functions or supervision vacancy. We should enhance the capacity for governance and cultivate the self-discipline of the industry and associations. The implementation of administrative accountability is necessary to ensure that institutional responsibilities are fulfilled and thereby realize public health.

Abstract:China's healthcare system has made great achievements in the management of medical services and public health challenges for the Chinese people. However, the issue of an aging population in Chinese society is becoming more and more salient. The gap between demand and supply of healthcare is expanding. China's healthcare system is facing unprecedented challenges. These problems include an insufficient medical insurance fund, nonuniform insurance reimbursement policies, a poor integrity system, and a lack of supervision in the management of the medical insurance fund. To address these challenges, some practical solutions are worth considering. A national medical insurance supervision platform should be strengthened. Besides, blacklists for illegal medical institutions and individuals engaged in malicious medical disturbances should be created. The country should also introduce policies to narrow the differences in regional medical insurance policies and balance the reimbursement levels of residents in different regions. Big data and artificial intelligence can be used to monitor the entire process of medical insurance fund utilization. The government needs to establish relevant laws and regulations to optimize the medical insurance system and ensure the safe and effective operation of the medical insurance fund.

Abstract:Background China’s pharmaceutical market is growing at a healthy pace. This is due to potential rising per capita income, an increasing elderly population, urbanisation demographics and more access to health care. Methodology The methodology used is a comparative study on the basis of original empirical research. More specifically, the part of examination facts and regulation has been written by conducting empirical research on current international and national resource concerning the subject from books, various guidelines, rules and regulation, articles, published reports and internet. Results Generally the regulatory environment in China has been considered as an very challenging one. From regulatory perspective, CFDA regulations have not been stringent enough nor has it been robust enough to create a quality supply of medications, with (1) concerns in the subject matter, quality comparison between international regulatory standard and usually accepted local products and manufactures set standards; (2) a timeline for regulatory assessment and authorization of new drugs which is much longer in comparison to most countries; (3) less number of regulatory reviewer which has led to a backlog of earlier application. The journey to reshape China from ‘made-in-China’ to ‘discovered-in-China’ is highly challenging. (4) Complications with local protection, corruption and a lack of transparency. The directives from Government has recognized the need for significant system-level transformations for streamlining the pharmaceutical regulatory environment. Conclusion Continuing on the route of reform is promising that all Chinese citizens is eventually having access to safe, effective and high-quality medications in affordable price. These reformations are encouraging and promising not only for China’s health care system, but also for global Pharmaceutical industry and that’s the reason, now many big Pharma companies are considering China as a prime focus. The companies need to make use of the new opportunities, accessible to experience the benefit of faster time-to-market, and cause rewarding growth prospect for both domestic/ multinational companies.
A Review of Promoting Access to Medicines in China - Problems and Recommendations.

Abstract:BACKGROUND:Despite recent reforms, distorting funding mechanisms and over-prescribing still maintain severe financial barriers to medicines access in China. Complicated and interrelated problems in the pharmaceutical sector require a common framework to be resolved as fragmented solutions do not work. We present a preliminary assessment of the impact of the national healthcare reforms on access to medicines, and propose policy recommendations for promoting universal access to medicines in China.METHODS:Drawing on multiple sources of information, including a review of published literatures and official national data, field investigations in six provinces and interviews with key opinion leaders, this paper presents a preliminary assessment of the impact of the national healthcare reforms on access to medicines, and proposes policy recommendations for promoting universal access to medicines in China.RESULTS:Public expenditure on medicines has been strictly controlled since the national healthcare reforms of 2009. Yet total pharmaceutical expenditure (TPE) and total health expenditure growth rates continuously outpaced the growth of gross domestic product (GDP). With 2.4% of GDP, TPE now exceeds that of most high income countries. The distorted provider and consumer incentives in the Chinese health system have not fundamentally changed. Price-setting and reimbursement mechanisms do not promote cost-effective use of medicines. Inappropriate price controls and perverse financial incentives are the un-resolved root causes of preference of originator brands for some major diseases and shortages of low-cost and low-consumption medicines. In addition, access to expensive life-saving medicines is yet systematically addressed.CONCLUSIONS:The complicated and interdependent problems interact in a way that leads to significant system problems in China, which create dual challenges that both the developing country and the developed countries are facing. To further promote access to medicines, China should speed up the re-assessment of the quality and efficacy of domestically produced generic medicines; coordinate various reforms of price determination, insurance payments, and procurement policies; address medicine shortages through comprehensive policies and legislation; establish specific mechanisms to achieve sustainable equitable access to expensive essential medicines with health technology assessment as a tool to ensure that policy and priority setting are created in a coherent and evidence-based way.

Abstract:The aim of the national essential medicines system is to ensure the people's accessibility to quality drugs.It is the key to the success of drug policy reform that the responses of the relevant stake-holders are consistent with the designed objectives of the reform measures.The zero mark-up drug policy for primary health facilities brings many problems:it is not consistent with the situation that there exists great imbalance in economic development and financing capacity between areas in China,there is no economic stimulus for primary health facilities to achieve the policy objectives,and it hurts both the accessibility of the people to non-essential drugs and the objective of "strengthening the primary health facilities".To overcome these problems,the following alternative measures are recommended:price mark-up policy should be retained for essential drugs,and non-essential drugs should be permitted to use in primary health facilities without price mark-up.

Abstract:Chinese health system remains the crucial one for understanding the wider healthcare landscape across the Global South and in particular the leading Emerging Markets. Purpose of our observation was to understand the inner dynamics of mainland Chinese health reforms adopting a lengthy time horizon. We have analysed the public reports and seminal evidence on Chinese of multiple waves of national health reforms taking place since 1980s in terms of medical care and pharmaceuticals provision and financing. Chinese international trade with ASEAN nations and wider South-East Asia is accelerating its growth after the recovery of trade routes. In terms of health sector this means that global demand and supply of medical goods, services and pharmaceuticals remains largely driven by Chinese domestic developments. Furthermore, Chinese domestic manufacturing and sales of decent quality medical devices and services have grown exponentially. Some temporary pitfalls and increasing in rural-urban inequalities in equity of access and affordability of medical care and pharmaceuticals did take place. Despite these difficulties to generate a balanced development strategy for the largest global market, this is a clear path upwards. Further upcoming improvements expanding health insurance coverage are in strong demand for certain layers of the society. Domestic bottleneck weaknesses yet remain manufacturing, import and market penetration of cutting-edge pharmaceuticals such as monoclonal antibodies and targeted oncology agents. Yet some of these obstacles are likely to be overcome in foreseeable future with the adoption of responsible strategies by governmental agencies in health care arena.

Abstract:SummaryThis paper discusses the progress and prospects of China's complex health care reform beginning in 2009. The Chinese government's undertaking of systemic reform has achieved laudable achievements, including the expansion of social health insurance, the reform of public hospitals, and the strengthening of primary care. An innovative policy tool in China, policy experimentation under hierarchy, played an important role in facilitating these achievements. However, China still faces gaps and challenges in creating a single payer system, restructuring the public hospitals, and establishing an integrated delivery system. Recently, China issued the 13th 5‐year plan for medical reform, setting forth the goals, policy priorities, and strategies for health reform in the following 5 years. Moreover, the Chinese government announced the “Healthy China 2030” blueprint in October 2016, which has the goals of providing universal health security for all citizens by 2030. By examining these policy priorities against the existing gaps and challenges, we conclude that China's health care reform is heading in the right direction. To effectively implement these policies, we recommend that China should take advantage of policy experimentation to mobilize bottom‐up initiatives and encourage innovations.
