Abstract:Living arrangements in later life are dynamic, with changes associated with life events such as widowhood or moves into an institution. Previous research has found particular changes in living arrangements to be associated with an elevated risk of mortality. However, research in this area within the context of China is limited, despite China being home to the world's largest population of older people. This study investigates the impact of changes in living arrangements on older persons' survival using the Chinese Longitudinal Healthy Longevity Survey from 2002 to 2011. The original sample was 16,064 in 2002, and this study includes 6191 individuals who survived in 2005 and had complete information of track record in later waves. Changes in living arrangements are examined between 2002 and 2005. Cox-proportional hazards models are then used to investigate the association between the dynamics of living arrangements and respondents' survival status from 2005 to 2011. Results show that men and women who lived in an institution in both 2002 and 2005, or who moved into an institution from living with family faced a greater risk of dying compared to those continuing to live with family. By contrast, continuing to live with family or alone, or moving between living with family and living alone, were not associated with an increased mortality risk, although there were some differences by gender. The institutional care sector in China is still in its infancy, with provision based on ability to pay market fees rather than need associated with age-related function impairment. The findings show that living in, or moving into, an institution is associated with a high mortality risk therefore requires further investigation in the context of a rapidly changing Chinese society.

Abstract:This paper explores the changes in the prevalence of morbidity, morbidity-free life expectancy and its related factors in China from 2000 to 2010. The analysis uses health status data from three waves of the Sample Survey of the Aged Population in Urban/Rural China (SSAPUR). To explore the changes, three types of morbidity-free life expectancy (MFLE) are estimated using the Sullivan method. To identify the influencing factors of morbidity, we use regression models that consist of social and economic variables to identify these factors. Our results show that between 2000 and 2010, the prevalence of moderate morbidity based on activities of daily living (ADLs) declined across most age groups; however, the prevalence for instrumental activities of daily living (IADLs) and severe morbidity both increased, especially in the oldest old age group. Furthermore, the ratio of the MFLE to the total remaining life expectancy suggests the occurrence of a trend toward morbidity expansion. Our regression results suggest that age is the main factor in morbidity and impairment; however, education, health care, income and urbanisation play important roles in reducing the scores for IADLs and the prevalence of ADLs disability. Our findings imply that health care policy should assume a greater focus on healthy ageing, especially when people are expected to live longer. The findings also suggest that the government should prepare for the increasing demand for long-term care in the near future.

Abstract:This paper uses data from censuses and surveys to re-estimate mortality levels and trends in China from the 1960s to 2000. We use the General Growth Balance method to evaluate the completeness of death reporting above the youngest ages in three censuses of the People's Republic of China from 1982 to 2000, concluding that reporting quality is quite high, and revisit the completeness of death recording in the 1973-75 Cancer Epidemiology Survey. Estimates of child mortality from a variety of direct and indirect sources are reviewed, and best estimates arrived at. Our estimates show a spectacular improvement in life expectancy in China: from about 60 years in the period 1964-82 to nearly 70 years in the period 1990-2000, with a further improvement to over 71 years by 2000. We discuss why survival rates continue improving in China despite reduced government involvement in and increasing privatization of health services, with little insurance coverage.

Abstract:Population-wide increase in life expectancy is a source of aggregate longevity risk. Life insurance is a natural instrument to manage the risk. Previous studies used chronological age to examine the relationship between aging and life insurance purchase, which ignored the impact of subjective life expectancy-the real perception of remaining time. Therefore, this study aims to fill the lack in this area and to explore in depth the relationship between subjective life expectancy and purchasing life insurance among middle-aged and older adult at micro perspective. This paper utilizes data from the China Health and Retirement Longitudinal Study (CHARLS) over a period of 4 years to construct both Probit and Tobit models. The findings reveal that subjective life expectancy positively affects the likelihood of participation and the extent of life insurance among the middle-aged and older adult population in China. IV model estimation results show good robustness of the results. Meanwhile, there is also heterogeneity in the effect with respect to gender, hukou, education and wealth. The findings provide new perspective to explain the subjective motivation of purchasing life insurance in China.
Regional Differences in Life Expectancy in Mainland China

Abstract:China’s Central government is currently undertaking extensive pension reforms which requires accurate life expectancy forecasts at both national and regional levels. However, the lack of prior research in this area is an obstacle to policy formulation. The aim of this research is to investigate the trend of China’s regional differences in life expectancy. Three contributions of this thesis are as follows. First, we constructed 800 national and regional life tables (by province and for urban and rural populations) based on a unique hand collected mortality dataset. This extends investigations of life expectancy beyond just the national level. The analysis of the life tables reveals that significantly increasing life expectancy differences exist between urban and rural populations between 1990 and 2010. In addition, we find that across all regions the differences in life expectancy at birth and at age 65 were mainly driven by the differences in life expectancy of the rural population in each region. Second, estimations of future mortality improvements using constructed life tables and the extended Lee-Carted model which addresses the limited availability of mortality data shows a stable difference between the urban and rural population over next twenty-five years. Moreover, this thesis shows a narrowed regional difference for urban population and a widen regional difference for rural population in the future. Third, the growth rate of GDP, GDP per capita, income per capita and the change in the number of doctors per 10,000 people all show relatively strong correlations with the change of the index of kt. Moreover, the sensitivity analysis displays that life expectancy at birth in the least developed regions will catch up with the developing regions by 2040.

Abstract:Objective This paper studies the impact of the subjective life expectancy of the elderly on their commercial pension insurance participation at micro perspective, providing new evidence to explain the motivation of commercial pension insurance participation in China. Methods Using 4 waves of data from the China Health and Retirement Longitudinal Study (CHARLS) from 2011 to 2018, a multiple linear regression model is constructed to investigate the effect of subjective life expectancy on commercial pension insurance participation among the Chinese elderly, and IV model estimation shows that the results are robust. Meanwhile, the heterogeneity of the effect of elderly life expectancy on commercial pension insurance participation behavior among different characteristic groups is also studied. Results A one standard deviation improvement in our measure of subjective life expectancy predicts a 0.9% point higher participation rate in commercial pension insurance. We also find that there is significant heterogeneity in the effects of subjective life expectancy on the participation of elderly people in commercial pension insurance with respect to gender, education, hukou, and wealth. Conclusion This paper provides a new perspective to explain the factors influencing commercial pension insurance participation in China. We suggest that improving residents’ awareness of life expectancy is beneficial to their reasonable retirement planning, in the background of stepping into an aging society in China.

Abstract:This paper uses several sets of reliable life tables for different periods from 1930 to 2000 to examine the changes of male/female relative mortality risk in Chinese reproductive age groups and explore the role of Chinese reproductive health policies in these changes.The results show that there are two significant changes of male/female relative mortality risk in Chinese reproductive age groups in the twentieth century.The first occurred in the period of high mortality level for reproductive age groups.Before 1950s,females had a higher death risk than males.By 1950s,female′s risk of death transformed from higher than that for males to a lower level.The second took place after the 1970′s with low mortality level for reproductive age groups.From the 1970′s to the end of the century,the female′s risk of death reduced much more rapidly than that for male and sex differentials of mortality for reproductive age groups increased significantly with lapse of time.The two significant changes partly indicate the achievements made through maternal-child health care and family planning.

Abstract:Sullivan method is used to analysis Chinese elders' state-specific contribution to the change of unhealthy life expectancy based on 2005 population sample survey and 2010 population census.Results indicate :the unhealthy life expectancy of the elders of 60-97 is the unhealthy status but with ability to take basic daily activities(LUA),while the unhealthy life expectancy of the elders above 97 is their disability to do anything( LUUA),the elders' unhealthy life expectancy and its proportion in the life expectancy in 2010 has declined compared with 2005 which is in accordance with the pattern of morbidity compression,but the change of LUA and LUA are significantly different,the contribution rate of LUA to the compression of the unhealthy life expectancy is larger than that of LUUA.That is to say,not only the Chinese elders' total health status is improving,but the unhealthy state structure is improving,which means both the total demanding of the social care services and the types of services are to change accordingly.

Abstract:Already home to 23% of the global elderly population, China will experience further demographic change in the coming decades. To address the consequences of population ageing, the Chinese government is implementing major social insurance reforms and promotes the development of private insurance markets. We aim to inform these initiatives by developing a new method to project healthy life expectancy in different regions. Healthy life expectancy is an important population health measure which is increasingly used in the actuarial literature. Our new approach relies on publicly available data from the Global Burden of Disease Study for life expectancy and healthy life expectancy for 139 countries. We use the model to estimate healthy life expectancy at birth in 2015 for 31 province-level regions in China for both males and females. We discuss the implications of our results for planned increases in the retirement age in China and for long-term care insurance pricing.

Abstract:Between 1950 and 1980, China experienced the most rapid sustained increase in life expectancy of any population in documented global history. We know of no study that has quantitatively assessed the relative importance of the various explanations proposed for this gain in survival. We have created and analysed a new, province-level panel data set spanning the decades between 1950 and 1980 by combining historical information from China's public health archives, official provincial yearbooks, and infant and child mortality records contained in the 1988 National Survey of Fertility and Contraception. Although exploratory, our results suggest that gains in school enrolment and public health campaigns together are associated with 55-70 per cent of China's dramatic reductions in infant and under-5 mortality during our study period. These results underscore the importance of non-medical determinants of population health, and suggest that, in some circumstances, general education of the population may amplify the effectiveness of public health interventions.

Abstract:OBJECTIVE: To achieve the goal of "healthy China 2030", reasonable health policies must be developed based on the changes of death spectrum. We aim to investigate the temporal patterns of life expectancy (LE) and age/cause-specific contributions from 1990 to 2016.METHODS: Joinpoint regression model was used with Arriaga's decomposition method.RESULTS: LE in China has reached to 76.3 years in 2016 with an increase of 9.44 years from 1990. From 1990 to 2002, a remarkable reduction in infant mortality accounted for an increase of 1.27 years (35.39%) to LE which mainly resulted from diarrhea, lower respiratory, and other common infectious diseases (1.00 years, 27.79%). After 2002, those aged 65+ years contributed most to increased LE and the most prominent causes included cardiovascular diseases (0.67 years, 23.36%), chronic respiratory diseases (0.54 years, 18.76%) and neoplasms (0.39 years, 13.44%). Moreover, the effects of transport injuries changed from negative to positive. After 2007, contributions of transport and unintentional injuries increased especially for males. And for females contributions of cardiovascular diseases sharply increased LE by 1.17 years (32.26%).CONCLUSION: More attention should be paid to cardiovascular diseases, chronic respiratory diseases and neoplasms which were mainly attributed to the increase of LE, especially for males and elderly population.

Abstract:Abstract Determinants of mortality may differ depending on context. This study uses a nationally representative sample of persons aged 60 and over in China to determine if socioeconomic factors, early life conditions, community characteristics, biological and physical functioning, and disease burden predict four-year all-cause mortality. We employed a series of Weibull hazard models based on exact survival time to predict mortality. We find that current education and place of residence, physical functioning, uncontrolled hypertension, diabetes, cancer, a high level of systemic inflammation, and poor kidney functioning are strong predictors of mortality among older Chinese. We did not find linkages between early-life experiences or community infrastructure to mortality at older ages. Results from this study highlight the value of incorporating biological and performance measurements and the importance of social and historical context in studying old-age mortality.

Abstract:Abstract Background Improvements of population health in China have been unevenly distributed among different sexes and regions. Mortality Registration System provides an opportunity for timely assessments of mortality trend and inequalities. Methods Causes of death were reclassified following the method of Global Burden of Disease Study (GBD). Age-standardized mortality rate (ASMR) and ring-map of the rate by town were used to describe inequalities in changing mortality. Life expectancy (LE) and cause-deleted LE were calculated on the basis of life table technique. Results The burden of death from 2002 to 2018 was dominated by cardiovascular diseases (CVD), neoplasms, chronic respiratory diseases and injuries in Jiading district, accounting for almost 80% of total deaths. The overall ASMR dropped from 407.6/100000 to 227.1/100000, and LE increased from 77.86 years to 82.31 years. Women lived about 3.0–3.5 years longer than men. Besides, a cluster of lower LE was found for CVD in the southeast corner and one cluster for neoplasms in the southern corner of the district. The largest individual contributor to increment in LE was neoplasms, ranged from 2.41 to 3.63 years for males, and from 1.60 to 2.36 years for females. Conclusions Improvement in health was mainly attributed to the decline of deaths caused by CVD and neoplasms, but was distributed with sex and town. This study served as a reflection of health inequality, is conducive to formulate localized health policies and measures.

Abstract:Using data from Population Censuses, 1% National Population Sample Surveys of China and the Human Mortality Database, this article adopts robust percentile-based methods to analyze the changing trend of life expectancy of the Chinese elderly especially the young–old and rural–urban disparity from 1989 to 2015, and attempt to explain the disadvantage of old-age mortality improvement in China compared to developed countries. We find that life expectancy at age 65 in China has increased continuously in recent decades, but at a lower speed than in developed countries, leading to a widening gap between China and developed countries, and the increase in e65 has not shown a clear catch-up trend that has been observed in the life expectancy at birth. Similar patterns are found when we explore the rural–urban disparity of China. Based on the age-at-death distribution, we find that the old-age deaths in rural areas are more concentrated at relatively younger ages compared to urban areas due to the higher death risks and slower improvement in mortality of the young–old in rural China. Our findings describe the age-patterns underlying the rural–urban disparity in life expectancy of the elderly within China, and also the main reason for the slower improvement of life expectancy among the Chinese elderly compared with those in developed countries. Survival improvement of the young–old and equalization of available health services are key to reducing the rural–urban bias and achieving accelerated increase in life expectancy among the elderly in China.

Abstract:BackgroundIn the 12th Five-Year Plan, the Chinese government set the goal of increasing life expectancy by one year. The purpose of this study is to examine the impact of major causes of death on the life expectancy of the Chinese people between 1950 and 2010 and predict changing trends to identify major issues requiring future attention.MethodsA continuous database organised by population and death data on diseases by age group between 1950 and 2010 were created from A Province in Eastern China. The diseases were classified into four categories by the International Classification of Diseases-10 (ICD-10): infectious and parasitic diseases, chronic diseases, accidental injuries, and maternal diseases. Potential gains in life expectancy (PGLEs) were applied to reflect the impact on life expectancy caused by deaths from various diseases, by using the cause-eliminated life table.ResultsThe PGLEs of infectious and parasitic diseases decreased from 15.59 years in 1950, to 0.07 year in 2010, and have remained low since 2000. However, the PGLEs of chronic diseases increased from 8.70 years in 1950, to 13.36 years in 2010, and indicated an increasing future trend. The two opposite trends exhibited a ‘scissors-like difference’. The proportion of accidental injuries and maternal diseases in the death spectrum was low. The PGLEs of accidental injuries decreased from 2.95 years in 1950, to 0.86 year in 2010, maintaining a low level, while the PGLEs of maternal diseases dropped from 0.56 to 0.002 year during the same period, approaching zero.ConclusionsThe findings of this study provide useful information, which could contribute to a more effective allocation of public health programmes. In recent years, chronic diseases and accidental injuries have emerged as major factors influencing life expectancy. Primary and secondary prevention actions, such as public education, modification of behaviours, and introduction of safety measures should be emphasised in efforts to promote life expectancy. The morbidity and mortality rates of infectious, parasitic, and maternal diseases should be maintained at low levels.

Abstract:This article investigates determinants of China’s mortality levels and trends since 1981. Econometric results show that both economic development and social policies matter to survival outcomes, but in different magnitudes for various mortality measures. Economic growth plays a key role in reducing mortality in childhood and especially adulthood, and raises life expectancy. Declining adult illiteracy is a critical factor in reducing child mortality and increasing life expectancy. Stock of health practitioners, share of government expenditure for health and education, and policy biases favoring cities and coastal areas selectively affect survival. Authors propose policies for further reducing mortality in China today.

Abstract:The provincial mortality data of China's 2010 population census sustains the study of regional mortality disparities, but the data suffer from the disadvantage of underestimation. Due to the deficiency of relevant information, little research on the quality of province-specific mortality data has been published with respect to the 2010 census. We estimated provincial underestimation of infant and old-age mortality, analysed regional disparities using the Developing Countries Mortality Database model life table and re-calculated sex-specific life expectancy at birth and at age 60 for the 2010 census. Results show that in most provinces the infant mortality rate is underestimated by at least 70% and that the related data for the eastern and southwestern provinces show better quality than that of other areas. Old-age mortality underestimate rates vary among provinces: the eastern, south-central and northern provinces present less severe underestimations than the southwest, northwest and northeast provinces. After adjustments, the province gap in e0 ranged from 0 to 14.8 years for boys and 0 to 15.9 years for girls, that in e60 ranged from 0 to 8.8 years for men and 0 to 9.9 years for women. Sex differences in life expectancy increased compared to the reported values of the 2010 census.
