Abstract:Aim: To observe the activity of serum neutrophil elastase and α 1-antitrypsin, level of α 1-anti-trypsin and changes of inflammatory factors in patients with acute lung injury (ALI), and compare with the healthy people. Meanwhile, to analyze the correlation of activity of elastase with injured degree and prognosis of ALI. Methods: (1) Thirty-five patients with ALI, who were treated at the Mature Intensive Care Unit and Surgical Care Unit, Xinhua Hospital from October 2003 to March 2004, were selected. Twenty-two patients with ALI were included, including 13 males and 9 females aged from 25 to 75, averagely (45±10) years old, and they were all agreed to take part in the experiment. (2) The patients were assigned into 2 groups according to prognosis: Death group (n=10) and survival group (n=12). Fifteen healthy volunteers, who had health examination at the out-patient clinic at the same period, were selected as control group. They were all agreed to take part in the experiment. (3) The venous blood was extracted from the included people. The activity of elastase in serum was detected with chromatometry; The level of α 1-anti-trypsin was detected with ratenephelometry; The activity of α 1-anti-trypsin was detected with Dietz method; The levels of interleukin 6 and 8 were detected with double-sandwich ABC-enzyme-linked immuno-sorbent assay (ELISA). (4) The measurement data of multiple group were compared with single factor analysis of variance, and the comparison between two groups was performed with LSD t-test. The correlation between two variances was analyzed with Spearman rank correlation analysis. Results: Twenty-two patients with ALI and 15 healthy people were all involved in the result analysis. (1) The activity of elastase in serum, levels of interleukin 6 and 8 in patients in the death group and survival group were all higher significantly than those in the control group (P < 0.05), and the activity of α 1-anti-trypsin was significantly lower than that in the control group (P < 0.01,0.05 ). The activity of elastase in serum, levels of interleukin 6 and 8 in patients in the death group were higher significantly than those in the survival group (P < 0.05-0.01), and the activity of α 1-anti-trypsin was significantly lower than that in the survival group (P < 0.01). (2) The activity of serum elastase in ALI patients had significant negative correlation with arterial PaO 2/FiO 2 ratio (r =-0.874, P < 0.01). Conclusion: (1) In ALI patients, the activity of elastase elevates significantly, and the activity of α 1-anti-trypsin reduces, appearing disequilibrium of protease-antiproteinases with the increase of inflammatory medium, interleukin 6 and 8, which is the main reason for causing pulmonary tissue damage. (2) The activity of serum elastase has relation with severity and prognosis of pulmonary damage of patients.
