Effect of inhaled milrinone on ischemia-reperfusion injury in a rat model of lung transplantation

Abstract:Objective To investigate the effect and the related mechanism of inhaled milrinone on experimental ischemia-reperfusion injury(IRI) in rats with lung transplantation. Methods Thirty male Sprague-Dawley rats were randomly divided into sham operation group(group Ⅰ,n=10),lung transplantation group(group Ⅱ,n=10),and lung transplantation + milrinone group(group Ⅲ,n=10).In groupsⅡ and Ⅲ orthotopic left lung transplantation was performed,followed by 2-hour reperfusion.In group Ⅲ milrinone(0.4 mg/mL) was inhaled for 2 h during the reperfusion.Arterial blood gas analysis was done before ischemia and 2 h after reperfusion for the measurement of partial pressure of oxygen in the arterial blood(paO2)/fraction of inspired oxygen concentration(FiO2) and Qs/Qt.The ratio of wet to dry lung weight(W/D) and the concentrations of myeloperoxidase(MPO),malondialdehyde(MDA),inducible nitric oxide synthase(iNOS) and endothelial nitric oxide synthase(eNOS) were determined.And histopathological examination of left lung was carried out 2 h after reperfusion. Results The values of paO2/FiO2 and eNOS concentrations in groupsⅠand Ⅲ were significantly higher than those in group Ⅱ(all P<0.05).The W/D ratios,the values of Qs/Qt and the concentrations of MDA,MPO and iNOS in groupsⅠand Ⅲ were significantly lower than those in group Ⅱ(all P<0.05).Pathological examination displayed that,compared with group Ⅰ,there were obvious hyperemia and more inflammatory cells in the left lung in groups Ⅱ and Ⅲ,and that the inflammatory cells in the left lung in group Ⅲ were less than those in group Ⅱ. Conclusion Inhaled milrinone can reduce ischemia-reperfusion injury to the transplanted donor lungs,which may be associated with increased eNOS activity,decreased iNOS activity and infiltration of inflammatory cells,and the improvement of endothelial cell function.
