Difference and relationship between arterial and end-tidal carbon dioxide pressures during gynecologic laparoscopic surgery

Abstract:【Objective】 To assess the difference and relationship between arterial and end-tidal carbon dioxide pressures during gynecologic laparoscopic surgery.【Methods】 24 ASA gradeⅠ~Ⅱ patients undergoing gynecologic laparoscopic surgery without cerebral or cardiovascular system diseases were selected.Anesthesia were induced and maintained with fentanyl,vecuronium and propofol or isoflurane.Ventilation parameters(tidal volume=body weight 7 mL,respiratory rate 14 times/min) were constant before and during pneumoperitoneum(PP).The PaCO2 and PETCO2 were measured before PP,10min,20min and 60min after PP respectively.【Results】 Compared with before PP,PaCO2 and PETCO2 increased significantly after PP(P <0.01).The Pa-ETCO2 increased significantly at 10min and 60min after PP(P <0.05 and P <0.01).The correlation between PaCO2 and PETCO2 was not well at 20 min after PP.【Conclusions】 Because of increased PaCO2 and PETCO2,extra Ventilation were needed during PP.PETCO2 is an imperfect index of PaCO2 under these circumstances.
