Surgical Patients at High Risk of Pressure Ulcer:Evidence-based Nursing Approach

Abstract:Objective To explore the nursing measures for preventing unavoidable pressure ulcer in high risk population. Methods A total of 100 patients with risk evaluation score of pressure ulcer less than 16, were randomly divided into an observation group and a control group equally. Patients in the observation group received evidence-based individualized nursing interventions, including using foam or air mattresses; turning over every 2 hours, from prone position to 30 degree left or right lateral position if patients’ condition permitted, and in prone position, raising patient’s head lower than 30 degree and putting a soft pillow under his/her heels; if the patient should keep prostration, upraised his or her hip every 2 hours by soft pillow from left or right to the lumbosacral portion; sanyrene was used on the surface of heels and lumbosacral portion every 2 hours except patients who suffered from skin ulcer; adjusting the diets, PN or EN by the level of serum albumin. While their counterparts in the control group were only given regular nursing interventions. Results The incidence rate of pressure ulcer was significantly lower in the observation group than that in the control group (P<0.05), and the occurring time was significantly delayed and the severity was milder (P<0.05). All pressure ulcers were healed. Conclusion The evidence-based individualized nursing interventions could lower the incidence rate of unavoidable pressure ulcer, reduce patients’ pain, and improve the basic nursing quality.
