Clinical and Radiological Manifestations of Pulmonary Involvement in Systemic Sclerosis

Abstract:Objective To summarize the clinical and radiological characteristics of the pulmonary involvement of the systemic sclerosis.Methods The clinical data of 43 patients with systemic sclerosis in our hospital from 2009 to 2011 were retrospectively collected and analyzed.Chest high resolution computerized tomography(HRCT) and cardiac ultrasound were performed to evaluate the pulmonary involvement and pulmonary artery involvement in different subtypes.Results The clinical manifestations included:Respiratory symptoms were seen in 19 cases(44.2%),in which exertional dyspnea(19/19) and cough(10/19) were the most common symptoms;Acroscleriasis was seen in 43 cases(100.0%),while sclerosis in trunk was seen in 21 cases(48.8%);Raynaud phenomenon was seen in 32 cases(74.4%);Only 1 case(2.3%) presented as CREST syndrome.Chest HRCT verified interstitial changes of the lungs in 30 cases(69.8%),of which 40.0%(12/30) had no obvious respiratory symptoms.Ten cases(23.3%) of pulmonary hypertension were detected by cardiac ultrasound.The prevalence of Raynaud phenomenon in diffusive and limited sclerosis(61.9%(13/21) vs.86.4%(19/22))and the prevalence of pulmonary hypertension in diffusive and limited sclerosis(23.8%(5/21)vs 40.9%(9/22)) showed no significant difference(P0.05).The interstitial lung disease was more common in the subtype of diffusive systemic sclerosis than that of the limited systemic sclerosis(85.7%(18/21) vs.54.5%(12/22),P0.05).Patients with Raynaud phenomenon had higher prevalence rate of pulmonary hypertension than those without Raynaud phenomenon(31.2% vs.0,P0.05).Conclusion Diffusive systemic sclerosis has common pulmonary involvement with insidious and non-specific symptoms.Chest HRCT is more sensitive than the clinical manifestations in assessing the pulmonary involvement of the systemic sclerosis,especially at the early stages.
