Relationship between poor or nonvisualization of renal collecting system in IVU and clinicopathologic characteristics of TCC in the upper urinary tract

Abstract:Objective To investigate the positive rate of poor or nonvisualization(PNV) in intravenous urography in patients with transitional cell carcinoma of the upper urinary tract(UUT-TCC) and its correlations with pathological findings and clinical characteristics.MethodsBetween January 1990 and January 2005, a total of 172 consecutive patients with UUT-TCC underwent radical nephroureterectomy in our departments were analyzed retrospectively.Statistical analysis concerning the detection rate of UUT-TCC by preoperative urinary cytology, filling defect and PNV was done and the correlations of PNV with pathological findings and clinical characters were evaluated by logistic regression model.ResultsIn total, 144 cases were presented with gross hematuria(83.7%), 12 cases with microscopic hematuria(7.0%).Forty-six cases were detected by cytology(26.7%).Filling defect identified 36 positive cases of 172 patients(20.9%), PNV were present in 105 of 172 patients(61.0%).Both the detection rate by PNV of 61.0% versus 26.7% by cytology and PNV of 61.0% versus 20.9% by filling defect were significantly differ-ent(P =0.031, P =0.001 respectively).Univariate logistic regression analysis for PNV showed that tumor stage, grade and size were significant predictors(P =0.028;P =0.031;P =0.006, respectively).Tumor stage and size was identified as independent risk factors in the multivariate logistic regression model(P =0.042;P =0.014).ConclusionEx-cept for suspected urolithiasis, urinary infection, or congenital abnormalities et al, UUT-TCC should be strongly con-sidered if PNV in intravenous urography existed.It is supposed that PNV carries more risk of higher stage and larger tumor size in UTT-TCC.
