Abstract:Dear Editor, It was proposed by Haider and Ashraf1 that cytokine and growth factor mediated mobilization of endogenous mesenchymal stem cells from their bone marrow niches for homing into the infarcted myocardium could be a better therapeutic strategy than the explantation and subsequent injection of these cells (either autologous or from a donated allogenic source) directly into the damaged heart.2 Besides being less invasive to the patient, it was argued that this approach would obviate the need for ex vivo processing of the putative stem cells, which could in turn reduce the risk of nonspecific differentiation into undesired lineages at the transplantation site.3–5 Nevertheless, such an approach could be severely hampered by the age related decline in the regenerative capacity of the putative mesenchymal stem cell population within the bone marrow, as has been demonstrated by a number of studies.6,7 It must be remembered that the overwhelming majority of myocardial infarct cases occur in the older segment of the population.8 Quarto and colleagues6 demonstrated in a rat model that there was an age associated decline in the ability of marrow stromal cells to repair bone, which could be the underlying cause of age related osteodegenerative diseases. Subsequently, D’Ippolito and colleagues7 reported a similar finding with the human model. This would be expected because, with increasing age, the putative mesenchymal stem cell population within the bone marrow would have gone through more rounds of cell division, in addition to having been exposed to free radicals and metabolic toxins over a longer lifespan. Hence it is no surprise that there is a gradual decline in their regenerative capacity with increasing age. Although there have been no studies to date that have conclusively demonstrated an age associated decline in the capacity of bone marrow cells to repair the infarcted heart, we could make such a conjecture based upon extrapolation from the bone repair model.6,7 The obvious solution to overcome this age related decline in regenerative capacity would be to look at donated allogenic sources of stem cells for myocardial repair. In this regard, human embryonic stem (hES) cells or their differentiated progenies would hold much promise because these potentially represent an abundant and readily available source of transplantable cells. Recently, Zandstra and colleagues9 successfully developed a scalable, robust, and reproducible process for obtaining purified cardiomyocytes from hES cells. Nevertheless, the immunologic barrier10 probably represents the most formidable challenge to the allogenic transplantation of hES cells or their differentiated progenies into the infarcted myocardium. It is hoped that this could somehow be overcome in the near future. Abstract:Introduction: Ischemic heart diseases, especially the myocardial infarction, is a major hazard problem to human health. Despite substantial advances in control of risk factors and therapies with drugs and interventions including bypass surgery and stent placement, the ischemic heart diseases usually result in heart failure (HF), which could aggravate social burden and increase themortality rate. The current therapeutic methods to treat HF stay at delaying the disease progression without repair and regeneration of the damagedmyocardium. While heart transplantation is the only effective therapy for endstage patients, limited supply of donor heart makes it impossible to meet the substantial demand from patients with HF. Stem cell-based transplantation is one of the most promising treatment for the damaged myocardial tissue.Sources of data: Key recent published literatures and ClinicalTrials. gov.Areas of agreement: Stem cell-based therapy is a promising strategy for the damaged myocardial tissue. Abstract:Ischemic heart disease, which is one of the top killers worldwide, encompasses a series of heart problems stemming from a compromised coronary blood supply to the myocardium. The severity of the disease ranges from an unstable manifestation of ischemic symptoms, such as unstable angina, to myocardial death, that is, the immediate life-threatening condition of myocardial infarction. Even though patients may survive myocardial infarction, the resulting ischemia-reperfusion injury triggers a cascade of inflammatory reactions and oxidative stress that poses a significant threat to myocardial function following successful revascularization. Moreover, despite evidence suggesting the presence of cardiac stem cells, the fact that cardiomyocytes are terminally differentiated and cannot significantly regenerate after injury accounts for the subsequent progression to ischemic cardiomyopathy and ischemic heart failure, despite the current advancements in cardiac medicine. In the last two decades, researchers have realized the possibility of utilizing stem cell plasticity for therapeutic purposes. Indeed, stem cells of different origin, such as bone-marrow- and adipose-derived mesenchymal stem cells, circulation-derived progenitor cells, and induced pluripotent stem cells, have all been shown to play therapeutic roles in ischemic heart disease. In addition, the discovery of stem-cell-associated paracrine effects has triggered intense investigations into the actions of exosomes. Notwithstanding the seemingly promising outcomes from both experimental and clinical studies regarding the therapeutic use of stem cells against ischemic heart disease, positive results from fraud or false data interpretation need to be taken into consideration. The current review is aimed at overviewing the therapeutic application of stem cells in different categories of ischemic heart disease, including relevant experimental and clinical outcomes, as well as the proposed mechanisms underpinning such observations.

Abstract:Ischemic heart disease (IHD) is the leading cause of mortality worldwide. Stem cell transplantation has become a promising approach for the treatment of IHD in recent decades. It is generally recognized that preclinical cell-based therapy is effective and have yielded encouraging results, which involves preventing or reducing myocardial cell death, inhibiting scar formation, promoting angiogenesis, and improving cardiac function. However, clinical studies have not yet achieved a desired outcome, even multiple clinical studies showing paradoxical results. Besides, many fundamental puzzles remain to be resolved, for example, what is the optimal delivery timing and approach? Additionally, limited cell engraftment and survival, challenging cell fate monitoring, and not fully understood functional mechanisms are defined hurdles to clinical translation. Here we review some of the current dilemmas in stem cell-based therapy for IHD, along with our efforts and opinions on these key issues.

Abstract:Ischemic heart diseases (IHDs) remain major public health problems with high rates of morbidity and mortality worldwide. Despite significant advances, current therapeutic approaches are unable to rescue the extensive and irreversible loss of cardiomyocytes caused by severe ischemia. Over the past 16 years, stem cell-based therapy has been recognized as an innovative strategy for cardiac repair/regeneration and functional recovery after IHDs. Although substantial preclinical animal studies using a variety of stem/progenitor cells have shown promising results, there is a tremendous degree of skepticism in the clinical community as many stem cell trials do not confer any beneficial effects. How to accelerate stem cell-based therapy toward successful clinical application attracts considerate attention. However, many important issues need to be fully addressed. In this Review, we have described and compared the effects of different types of stem cells with their dose, delivery routes, and timing that have been routinely tested in recent preclinical and clinical findings. We have also discussed the potential mechanisms of action of stem cells, and explored the role and underlying regulatory components of stem cell-derived secretomes/exosomes in myocardial repair. Furthermore, we have critically reviewed the different strategies for optimizing both donor stem cells and the target cardiac microenvironments to enhance the engraftment and efficacy of stem cells, highlighting their clinical translatability and potential limitation.

Abstract:Stem cell-based therapy is emerging as a promising strategy to treat end-stage heart failure, a leading cause of morbidity and mortality. Stem cells can be isolated from a variety of sources and exhibit unique characteristics that impact their potential therapeutic utility. The adult heart contains small populations of committed, multipotent cardiac stem cells (CSC), which are adapted to the cardiac microenvironment and participate in postnatal physiological and pathological cardiac renewal or repair. These cells can be isolated, expanded in culture, and administered therapeutically to improve cardiac function in the setting of heart failure. CSC can be differentiated into three distinct cardiovascular lineages and exhibit enhanced paracrine factor production and engraftment as compared with other types of mesenchymal stem cells, which in turn may translate into improved therapeutic efficacy. The cell surface marker expression and phenotype of these CSC, however, depends on the method of isolation, selection and propagation, which likely explains the variable experimental results obtained to date. Moreover, invasive procedures are required to obtain CSC from humans. Early trials using autologous CSC in patients with ischemic cardiomyopathy have demonstrated feasibility and safety, along with variable degrees of therapeutic efficacy in terms of enhancing myocardial viability and cardiac function. Further studies are needed to optimize methods of CSC isolation, manipulation and delivery. If fully realized, the potential of CSC therapy could fundamentally change the approach to the treatment of end-stage heart failure.
Angiotensin Receptor Type 2 Stimulation Increases the Migration of Bone-Marrow Mononuclear Cells into the Infarcted Myocardium and Improves Heart Function

Abstract:Objectives Stem cell therapy is a potential option for treating myocardial infarction. However, the effect is not as well as we expected. As a matter of fact, cell therapy is faced a lot of challenges, such as poor survival of the transplanted cells in the infracted region, low number of cells migrated into myocardial lesions, and so on. So the aim of this study is to explore a new strategy that angiotensin receptor type 2 stimulation can improve the engraftment of bone marrow mononuclear cells in the infracted myocardium and recover heart function. Methods Mononuclear cells (MNCs), isolated from SD rat bone marrow, were treated with several reagents which can change the activity of AT2R of MNCs. The experiment group designation for in vitro study were: MNCs (control), MNCs+AngII, MNCs+CGP42112A, MNCs+AngII+ARB, MNCs+AngII+PD123319 and MNCs+CGP+ARB. The ability of migration and cardioprotection were examined by tran-swell and co-culturing with neonatal cardiomyocytes. The experiment group designation for in vivo study were: DMEM, MNCs, MNCs+CGP42112A and MNCs+AngII+ARB. Cells from different groups were intramyocardially injected into the peri-infarct region. Inflammatory cytokine expression, death of cardiomyocyte, anti-apoptotic protein expression were assessed 3 days after transplantation. Meanwhile we transplant above-mentioned female rats'cells into male rats via tail vein, and evaluate the number of engrafted cells 1 day after transplantation by real-time PCR. Infarct size, angiogenesis and heart function were measured 1 month after transplantation. Results Bone marrow mononuclear cells, when their AT2R were stimulated, can prevent neonatal cardiomyocytes from apoptosis in vitro. Also more cells migrated into low chamber in same situation. Transplantation of AT2R-activated bone marrow cells intramyocardially after myocardial infarction resulted in decrease in inflammatory cytokines (IL-1, IL-6, MCP-1) expression, death rate of cardiomyocytes in peri-infarcted region, as well as increase in anti-apoptotic protein expression (Bcl2). Also more bone marrow mononuclear cells were found in myocardial lesions 1 d after transplantation via tail vein. No matter which transplanted routes we choose, transplantation of AT2R-activated bone marrow cells can improve heart function significantly. Conclusions Angiotensin receptor type 2 stimulation can increase the migration of bone-marrow mononuclear cells into the infracted myocardium and prevent cardiomyocytes from apoptosis, thus activate AT2R of bone marrow stem cells maybe a novel option for enhancing benefits of stem cell therapy.

Abstract:BACKGROUND: As a group of primary cells, stem cells have the potentials of self-renewal and multi-directional differentiation, and can replace the damaged cardiomyocytes to construct new vessels, and enable the asthenic heart to obtain the blood pumping function again. This study summarized the progress of stem cell transplantation in treating cardiovascular diseasesDATA SOURCES: A computer-based online search of Pubmed database was undertaken to identify English articles published from January 2000 to December 2005, the keywords were "stem cell transplantation, cardiovascular diseases".STUDY SELECTION: The articles were primarily checked, and the references of each literature were looked up. Inclusive criteria: articles correlated with the classification and the transplanted pathways of stem cells, the present status,problems and prospect of its basic and clinical application in treating cardiovascular diseases. Exclusive criteria: repetitive study or Meta analysis.DATA EXTRACTION: Totally 50 relative literatures were collected, and 31 of them accorded with the inclusive criteria,and the other 19 were excluded due to the old or repetitive contents. Of the 31 involved literatures, 19 were about the classification of stem cells used in cardiovascular system, 1 about the transplanted pathways of stem cells, 8 about the application of stem cells in treating cardiovascular diseases, and 3 about the existing problems and prospect.DATA SYNTHESIS: With the continuous improvement of living level and aging, cardiovascular diseases have become the main causes for death and disability in modern society. Stem cells have the potentials of multi-directional differentiation, and have become the main seed cells for cell replacement therapy, and provided important investigative and applicating field for the treatment of cardiovascular diseases. At present, there are many stem cells to be used in cardiovascular system, including embryonic stem cells, umbilical cord blood stem cells, peripheral blood stem cells, bone marrow stem cells, skeletal muscle satellite cells, myocardial stem cells, adipose tissue-derived stem cells, etc. The common pathways for stem cell transplantation are direct injection into myocardium, intracoronary artery injection and intravenous injection. Stem cell transplantation can treat cardiovascular diseases by promoting angiogenesis and myocardial regeneration, decreasing myocardial apoptosis and reducing myocardial collagen, then ameliorate the cardiac function, and breakthrough and progress have been made in both animal models and clinical studies. However, problems sill exist to be solved, and there is agreat distance to the routine clinical application.CONCLUSION: Stem cells are the important fields of the basic research and clinical application for preventing and treating cardiovascular diseases, thus it will provide powerful theoretical and practical supports for the more effective prevention and treatment of cardiovascular diseases to solving the key points and difficulties in cardiovascular diseases.

Abstract:<p>Cardiovascular disease (CVD) is one of the leading causes of death worldwide. Currently, many methods have been proposed by researchers for the prevention and treatment of CVD; among them, stem cell-based therapies are the most promising. As the cells of origin for various mature cells, stem cells have the ability to self-renew and differentiate. Stem cells have a powerful ability to regenerate biologically, self-repair, and enhance damaged functional tissues or organs. Allogeneic stem cells and somatic stem cells are two types of cells that can be used for cardiac repair. Theoretically, dilated cardiomyopathy and acute myocardial infarction can be treated with such cells. In addition, stem cell transplantation procedures, including intravenous, epicardial, cardiac, and endocardial injections, have been reported to provide significant benefits in clinical practice; however, there are still a number of issues that need further study and consideration, such as the form and quantity of transplanted cells and post-transplantation health. The goal of this analysis was to summarize the recent advances in stem cell-based therapies and their efficacy in cardiovascular regenerative medicine.</p>

Abstract:Though the conventional treatments of ischemic heart disease can improve coronary artery blood supply and save the ischemic myocardium,it is impossible to enable regeneration of myocardial necrosis.Therefore,using stem cells in the replacement of the injured and damaged myocardial cells has gradually become the research focus of improving cardiac function.Bone marrow mesenchymal stem cells could be easily obtained,amplified and cultured in vitro,and could transferred efficiently,they can be collected autologous and avoid ethical dispute and immunological rejection,and could become the ideal source of cells in the treatment of ischemic heart disease.

Abstract:Stem cell therapy has become a promising method for many diseases, including ischemic heart disease and heart failure. Several kinds of stem cells have been studied for heart diseases. Of them, bone marrow stem cells (BMSCs), which have been used in many clinical trials, are the most understood one. But the effect of BMSCs is mediated by paracrine factors instead of direct turning into cardiomyocytes. On the other hand, a lot of evidences have shown that resident cardiac stem cells could turn into cardiomyocytes directly in vivo. Currently, seven kinds of resident cardiac stem cells have been discovered. However, their mechanisms, development origins, and relationships have yet to be fully understood. Moreover, two Phase I clinical trials have been performed recently. They show promising results. In this review, we will summarize the current research on these cardiac stem cells and the methods to enhance their effects in clinical applications.

Abstract:OBJECTIVE:Some studies have indicated that stem cells transplantation therapy can replace necrotic myocardial cells,increase the number of functional myocardial cells,and build new blood vessels to improve blood supply and heart function.In this paper,we summarized the domestic development and achievement in this field in recent years.DATA SOURCES:Computer-based online search of Medline,CBM and CNKI databases was undertaken to identify related clinical(regardless of age and sex of subjects) and basic researches either in vivo or vitro dated from January 2001 to November 2006 with the Keywords of "stem cell,bone marrow,mesenchymal,transplantation" in English and "stem cell,transplantation,clinical research,myocardial infarction,heart failure" in Chinese.STUDY SELECTION:820 literatures were collected.All titles and most abstracts of articles were read through.Only articles ①related to stem cells transplantation therapy,②related to treatment of angiocardiopathy,and ③related to clinical treatment were selected.Review,repeated study and Meta analytical papers were excluded.DATA EXTRACTION:Totally 225 articles matched the inclusive criteria,and 595 were excluded.Of 225 articles,30 were analyzed involved 13 in English,and 19 in Chinese.One Chinese article was searched manually from supplement.DATA SYNTHESIS:①Since 2002,domestic researchers began to explore how to use stem cells transplantation to treat angiocardiopathy.②Nowadays,the majority of stem cells used in clinical practice are bone marrow mononuclear cells.Other kinds of stem cells including bone marrow mesenchymal stem cells,skeletal muscle stem cells,and peripheral blood stem cells are also applied in clinic.③Intracoronary injection is the main way in stem cell injection.It is mainly used to treat some clinical diseases such as acute and olden myocardial infarction and chronic cardiac insufficiency.CONCLUSION:Stem cells transplantation can replace necrotic myocardial cells,increase the number of functional myocardial cells,and build new blood vessels to improve the blood supply and heart function.Clinical researches have showed that it is one of the methods to deal with angiocardiopathy due to its safety and efficacy.

Abstract:Heart failure (HF) after myocardial infarction (MI) represents a worldwide public health issue with gradually increasing incidence. HF may eventually affect all organs and tissues and is associated with significant morbidity and mortality. Current therapeutic methods to treat HF stay at only delaying the disease progression without repair and regeneration. Recently, cell transplantation has gradually become the most promising treatment for the damaged myocardial tissue. Meanwhile, the bone marrow-derived mesenchymal stem cells (MSCs) are the most used cells for the treatment of HF. In the present review, we summarize the current literature on MSCs and their potential use to treat HF after MI. So far, MSCs have been demonstrated beneficial competence in animal studies and clinical trials, which may open novel avenues for further clinical studies.

Abstract:Ischemic heart diseases remain the leading cause of death globally, and stem cell-based therapy has been investigated as a potential approach for cardiac repair. Due to poor survival and engraftment in the cardiac ischemic milieu post transplantation, the predominant therapeutic effects of stem cells act via paracrine actions, by secreting extracellular vesicles (EVs) and/or other factors. Exosomes are nano-sized EVs of endosomal origin, and now viewed as a major contributor in facilitating myocardial repair and regeneration. However, EV/exosome therapy has major obstacles before entering clinical settings, such as limited production yield, unstable biological activity, poor homing efficiency, and low tissue retention. This review aims to provide an overview of the biogenesis and mechanisms of stem cell-derived EV/exosomes in the process of cardiac repair and discuss the current advancements in different optimized strategies to produce high-yield EV/exosomes with higher bioactivity, or engineer them with improved homing efficiency and therapeutic potency. In particular, we outline recent findings toward preclinical and clinical translation of EV/exosome therapy in ischemic heart diseases, and discuss the potential barriers in regard to clinical translation of EV/exosome therapy.

Abstract:The current pharmacotherapy is inadequate in preventing the progression of ventricular remodeling and congestive heart failure. Cellular cardiomyoplasty, through cell transplantation or regeneration of cardiomyocytes is a potential therapeutic approach to prevent left ventricular remodeling and the development of congestive heart failure after myocardial infarction. Recent research has focused on stem cells, which are undifferentiated pluripotent cells and can differentiate into a wide variety of cells including cardiac myocytes and endothelial cells. The majority data on stem cell transplantation were preclinical animal studies, all showing that stem cell therapy may replace lost heart muscle (myogenesis) and enhance cardiovascular revascularization (angiogenesis and vasculogenesis). These findings have been rapidly applied to human trials, but there were many questions remain to be solved. Although the results are interesting and safe, early phase Ⅰ clinical studies are small in scale and very preliminary. Data from large, randomized controlled trials are needed to confirm the short- and long-term effects of cellular cardiomyoplasty.

Abstract:Cardiovascular disease remains the single highest global cause of death and a significant financial burden on the healthcare system. Despite the advances in medical treatments, the prevalence and mortality for heart failure remain unacceptably high. New approaches are urgently needed to reduce this burden and improve patient outcomes and quality of life. One such promising approach is stem cell therapy, including embryonic stem cells, bone marrow derived stem cells, induced pluripotent stem cells and mesenchymal stem cells. However, the cardiac microenvironment following myocardial infarction poses huge challenges with inflammation, adequate retention, engraftment and functional incorporation all crucial concerns. The lack of cardiac regeneration, cell viability and functional improvement has hindered the success of stem cell therapy in clinical settings. The use of biomaterial scaffolds in conjunction with stem cells has recently been shown to enhance the outcome of stem cell therapy for heart failure and myocardial infarction. This review outlines some of the current challenges in the treatment of heart failure and acute myocardial infarction through improving stem cell therapeutic strategies, as well as the prospect of suitable biomaterial scaffolds to enhance their efficacy and improve patient clinical outcomes.

Abstract:The selective transplantation of autologous bone marrow cells (chronic infarction 10 (9) million cells) as well as the intracoronary approach, represents a novel and effective therapeutic procedure. The improvement of autologous stern cell therapy is achieved in addition to the catheter interventional procedures and is a procedure for regeneration of destroyed heart muscle in the early phase after myocardial infarction. In patients with chronic coronary artery disease (mean 108 months after myocardial infarction) intracoronary stern cell therapy leads to significant increase of left ventricular pump function and contractility, reduction of infarct size, increase of myocardial glucose storage and an increase of physical ability (functional capacity) and feeling of well-being. Autologous stern cell therapy in patients with dilated cardiomyopathe seems to be a new option for myocardial restitution. A significant improvement of the subjective aas well as the objective functional capacity was documented. Also a significant reduction of ventricular arrhythmias was revealed in patients with chronic coronary artery disease and non-ischemic cardiomyopathy. Stern cells have the important properties of self-regeneration and organ plasticity. Therefore they are ideal candidates for regeneration of myocardial tissue. The regenerative potential of bone-marrow-derived stern cells may be explained by four mechanisms: 1) direct cell differentiation from monoclear cells to cardiac myocytes, 2) cytokine-induced growing and increase of residual viable myocytes, especially within the border zone of the infracted area, 3) stimulation of resident cardiac stern cells (endogenous stern cells), and 4) induction of cell fusion between transplanted bone marrow cells and resident myocytes. For this method of therapy, no ethical problems exist, and no side effects were observed. The therapeutic benefit for the patient's heart seems to prevail. Peripheral arterial occlusion disease The combined intraarterial and intramuscular transplantation of autologous, mononuclear bone marrow stern cells is a clinical feasible and safe therapeutical option for patients with severe chronic limb ischemia. It leads to a significant increase of the perfusion indices and of the quality of life. Further studies are required to prove the benefit of these new therapeutic approach.
MiRNAs:A New Approach for Treatment of Ischemic Heart Disease by Stem Cell Transplantation

Abstract:MicroRNAs are an abundant family of endogenous non-protein-coding small RNAs, which negatively regulate gene expression at the posttranscriptional level in many developmental and metabolic processes. It has been demonstrated that some miRNAs are especially expressed in some stem cells and play an important role in regulating their functions such as migration, survival and differentiation. Moreover, stem cells are used to treat ischemic heart disease and, to some extent, improve the functions of ischemic myocardium. However, some problems, for example, how to further augment the implanted cells survival and other functions to overcome the limitations for their clinical applications, are still to be explored. In this present review, the role of miRNAs and their regulatory mechanisms in stem cells are discussed systemically. Finally, it is reasonable to speculate that miRNAs may be a novel but realistic regulator of stem cell implantation for ischemic heart disease, and bring a promising strategy for improving myocardial infarction.
