Clinicopathological Factors Associated with Recurrence in Malignant Ovarian Germ Cell Tumor: a Report of 145 Cases

Abstract:Background and purpose:Recurrence rate of malignant ovarian germ cell tumor (MOGCT) has been greatly reduced since the wide use of effective multiple drug combined chemotherapy. However, approximately 10%~40% of the patients will suffer recurrence after failure of primary therapy, and their prognoses are rather dismal. This study was undertaken to investigate the clinicopathological factors associated with recurrence in MOGCT. Methods:A total of 145 patients with MOGCT treated at our institution from April 1990 to December 2004 were retrospectively identified and analyzed. Recurrent rates were analyzed by the Kaplan-Meier, and the statistical significance of the difference among various factors associated with recurrence was tested by both the log-rank and the Cox proportional hazards model.Results:Of the study patients,43 were diagnosed as dysgerminoma (DSG), 47 immature teratoma (IMT),37 endodermal sinus tumor(EST),5 embryonal carcinoma and 13 mixed form. The 1-,3-,5-year recurrent rates for the whole group were 21.07%、36.32% and 42.95%, respectively. There were statistically significant differences in terms of histology(DSG versus non-DSG)(P=0.0117)、FIGO stage(Ⅰ versus Ⅱ+Ⅲ +Ⅳ)(P=0.0000)、tumor size(≤15cm versus 15cm)(P=0.0420)、residual tumor after surgery(≤1cm versus 1cm)(P=0.0000)and the type of adjuvant chemotherapy implemented (non-standard chemotherapy versus standard chemotherapy)( P=0.0028). Among them tumor size was exclusively an independent factor.Conclusions:Histology、FIGO stage、residual tumor after surgery and the type of adjuvant chemotherapy were independent factors associated with recurrence in MOGCT .For those patients with high risks of recurrence, full dose and cycles of standard BEP/PVB regimen should be used.
