A Clinical Trial of Maintenance Treatment for Lupus Nephritis with Leflunomide

Abstract:Objective:To compare the efficacy of prednisolone with Leflunomide (LEF) or Tripterygium wilfordii (TW) for the maintenance treatment in patients with lupus nephritis (LN). Methodology:One hundred and twenty two patients with LN(WHO class Ⅱ 24,class Ⅲ 9,class IV 65,class V 24),females 116,and males 6 cases with the average age of (29.6±8.3) years old,who received the complete remission state after induction treatments,were enrolled in this study. They were divided randomly into the LEF group (a dosage of 50 mg/d orally for 3 days then 20 mg/d,n=61) or the TW group (a dosage of 60 mg/d,n=61) according to the WHO renal histological classes. All patients were also taken predinisone (10 mg/d). No difference in sex,age,classes of lupus nephritis and induction therapy was found between the two groups. The patients followed for 24 months were collected for the analysis of clinical efficacy. The primary efficacy parameter was percentage of relapse,the secondary evaluation included immunology parameters (A-ds-DNA,C3,C4),urinary proteinuria and renal function. The adverse effects were also recorded in the all patients. Results:19 patients were withdraw from the study (14 in LEF group,5 in TW group),103 patients (47 in LEF group,56 in TW group) were followed for 24 months. 7 cases (14.9%) in LEF group (renal relapse in 4,extra-renal relapse in 3) and 9 patients (16.1%) in TW group (renal relapse in 6,extra-renal relapse in 3) relapsed after the maintenance treatment for (9.9±5.5) months and (12.4±4.3) months respectively. The percentage of cases with serum anti-ds-DNA autoantibody was significantly decreased in the LEF group at the end of the study (46.8% vs 23.4%,P0.05) while no significant decrement was found in the TW group (35.7% vs 26.8%,P0.05). No differences were found in proteinuria,serum albumin and complement level between the two groups. The rate of skin rashes (14.8% vs 0,P0.05) and alopecia (13.1% vs 0,P0.05) were much higher in LEF group than that in the TW group,while amenorrhea rate was much higher in the TW group than that in LEF group (58.1% vs 5.6%,P0.01). Other adverse effects in the LEF group and TW group included elevation of hepatic enzyme (6.6% and 6.6%),diarrhea (4.9% and 0),leukocyteopenia (4.9% and 1.6%),myalgia (3.3% and 0) and infection (3.3% and 3.3%). Conclusion:LEF and TW showed no differences in the relapse rate during the 24 months follow-up period. LEF could be used as a maintenance therapy for patients with lupus nephritis,but allergy reactions and other adverse effects should be monitored during the treatment.
