Abstract:Objective To analyze the risks of high alert drug concentrated potassium chloride injection in clinical application using failure mode and effect analysis (FMEA) method and make the relevant prevention measures.Methods A study team was established in the First Affiliated Hospital,Zhejiang University.The risk points in the application process of concentrated potassium chloride injection through literature search,questionnaires,and on-the-spot investigations were collected.Severity (S),frequency of occurrence(O),and likelihood of detection(D) of the risks related to these failure modes were scored and the risk priority numbers (RPN) were determined.The failure modes with RPN ＞ 10.0 or their severity up to 5.0 were screened out and the corresponding prevention strategies were formulated.Results Five risk points were collected through literature search;10 risk points were obtained through questionnaires,including 3 from physicians,3 from pharmacists,and 4 from nurses;and 5 risk points were obtained through on-the-spot investigations.Sixteen risk points with RPN ＞ 10.0 or severity up to 5.0 were further screened from the above links,including 3 from information system links,2 from drug storage links,4 from doctor prescription links,4 from pharmacist dispensing links,and 3 from nurse administration links.According to the selected risk points,22 prevention strategies were formulated.The prevention measures for the above 5 links were 4,4,6,5,and 3,respectively,including 11 mandatory strategies,9 recommended strategies,and 2 conditional strategies.Conclusion The risk points of concentrated potassium chloride injection application can be screened using FMEA method,which is helpful to formulate the preventive strategies for medication errors related to the drug.
Management Strategy Study on Lyophilizing Thrombin Powder Based on Failure Mode and Effect Analysis

Abstract:Objective To explore the application of failure mode and effect analysis (FMEA)in improving the management strategies of high alert medication (HAM),such as lyophilizing thrombin powder. Methods The study groups on medication risk and its prevention strategies of lyophilizing thrombin powder were set up in China-Japan Friendship Hospital and Xuanwu Hospital, Capital Medical University, respectively. The possible failure modes appearing in clinical application of lyophilizing thrombin powder were collected by questionnaire survey and on-the-spot investigation. The risk priority number (RPN)scores were calculated according to the score results of severity,frequency of occurrence,and likelihood of detection. Failure modes with higher RPN scores were screened out and the corresponding prevention strategies were formulated. The situations in related links before and after improvement of the strategies were analyzed comparatively. Results The results from questionnaire survey and on-the-spot investigation showed a total of 37 failure modes in the links of information system,physician prescription,drug dispensing,medical order execution by nurses,and medication error reporting. Ranking the RPN scores from high to low,15 major failure modes were screened out,including 4 modes in information system link (not setting the lyophilizing thrombin powder administration route as "oral",lack of HAM labels,lack of photo library of confusing drugs, and lack of real-time reporting program of medication errors),2 modes in physician prescription link (existence of confusing drugs and physician prescription errors),4 modes in drug dispensing link (lack of prescription/ medication orders auditing by pharmacist,absence of the pharmacist′ s double check in drug dispensing and distributing,lack of location mark of HAM and confusing drug for lyophilizing thrombin powder in the pharmacy,and no special storage area for lyophilizing thrombin powder in the pharmacy),4 modes in the link of medical order execution by nurses (negligence in check of the administration route of lyophilizing thrombin powder,existence of nurses in ward who were unfamiliar with the drug knowledge,no double check in drug distribution,and lack of location mark of HAM and confusing drugs in the ward),and 1 mode in medication error reporting link (no real-time information sharing system for medication errors among different wards). After drawing up the improvement measures of management,no medication errors occurred during the lyophilizing thrombin powder application in the 2 hospitals. Before and after the improvements,the percentages of physicians,pharmacists and nurses knowing that lyophilizing thrombin powder package should be with a special mark were 33. 3％ (12/ 36)and 83. 3％ (30/ 36),and the percentages of those knowing the right route to report medication errors were 77. 8％ (28/ 36)and 100％ (36/ 36),respectively,with statistically significant differences (P <0. 05 for both). Conclusions FMEA method can be used to screen the failure modes during the lyophilizing thrombin powder application effectively. The improvement of management measures for lyophilizing thrombin powder application has a positive effect on the improvement of medication safety.

Abstract:Purpose To conduct a Health Care Failure Mode and Effects Analysis (HFMEA) of the chemotherapy preparation process to identify the steps with the potential to cause errors, and to develop further strategies to improve the process and thus minimize the risk of errors. Methods An HFMEA was conducted to identify and reduce preparation errors during the chemotherapy preparation process. A multidisciplinary team mapped the preparation process, formally identified all the steps, and then conducted a brainstorming session to determine potential failure modes and their potential effects. A severity and probability score for each failure mode, a hazard score (HS) and a total HS were calculated. A hazard analysis was conducted for each HS equal to or more than 8. Finally, an action plan was identified for each failure mode. After the action plan was implemented, failure modes were revaluated and a new HS score was calculated as well as the percentage decrease in risk. Results The team identified five main steps in the chemotherapy preparation process and nine potential failure modes. After implementing the control measures, all the HSs decreased. The total HS associated with the chemotherapy preparation process decreased from 54 to 26 (-52%). This reduction in the total HS was mainly achieved by updating the Standard Operating Procedures (SOPs) and implementing bar code and gravimetric control system. Conclusion The application of HFMEA to the chemotherapy preparation process in centralized chemotherapy units can be very useful in identifying actions aimed at reducing errors in the healthcare setting.

Abstract:Medication incident reporting systems play a vital role in improving medication safety through the identification of medication incidents/near misses and learning from them. FMEA is an excellent tool to review a system as it systematically identifies potential problems in a system and makes them transparent on a priority basis before they occur. To use failure modes and effects analysis (FMEA) to improve the medication incident reporting system in a hospital. The prospective risk analysis tool, FMEA, was used by a multidisciplinary team to identify key failures in the original medication incident reporting system. The likelihood of the failure occurring, the severity and detectability of the failure if it occurred were agreed by the team and used to calculate the risk priority number (RPN) of each failure mechanism. Potential failures with an RPN of greater than 45 were targeted in an FMEA action plan which was produced and implemented with the aim of reducing these priority risks. This action plan included the introduction of a new medication incident report form, a new database to record and analyse incidents for trends, a new medication safety newsletter for staff and the production of a key performance indicator to feedback information to management. By implementing the key measures identified in the FMEA process, an overall reduction of 75% was achieved in total risk priority number of the medication incident reporting system. Prospective risk analysis provides a great opportunity for system improvement. The inclusion of a multidisciplinary team at all stages in this analysis ensures a balanced approach to reviewing a system and identifying potential risks. Implementation of the FMEA action plan has lead to a significant risk reduction in the process and resulted in a robust medication incident reporting system in the hospital.

Abstract:It is crucial to investigate the risk factors inherent in the medication process for cancer patients since improper antineoplastic drug use frequently has serious consequences. As a result, the Severity, Occurrence, and Detection rate of each potential failure mode in the drug administration process for patients with lung cancer were scored using the Failure Mode and Effect Analysis (FMEA) model in this study. Then, the risk level of each failure mode and the direction of improvement were investigated using the Slacks-based measure data envelopment analysis (SBM-DEA) model. According to the findings, the medicine administration process for lung cancer patients could be classified into five links, with a total of 60 failure modes. The risk of failure modes for patient medication and post-medication monitoring ranked highly, with unauthorized use of traditional Chinese medicine and folk prescription and unauthorized drug addition (incorrect self-medication) ranking first (1/60); doctor prescription was also prone to errors. The study advises actively looking at ways to decrease the occurrence and difficulty of failure mode detection to continually enhance patient safety when using medications.

Abstract:RATIONALE, AIMS, AND OBJECTIVES:Failure mode and effects analysis (FMEA) is a valuable reliability management tool that can preemptively identify the potential failures of a system and assess their causes and effects, thereby preventing them from occurring. The use of FMEA in the healthcare setting has become increasingly popular over the last decade, being applied to a multitude of different areas. The objective of this study is to review comprehensively the literature regarding the application of FMEA for healthcare risk analysis.METHODS:An extensive search was carried out in the scholarly databases of Scopus and PubMed, and we only chose the academic articles which used the FMEA technique to solve healthcare risk analysis problems. Furthermore, a bibliometric analysis was performed based on the number of citations, publication year, appeared journals, authors, and country of origin.RESULTS:A total of 158 journal papers published over the period of 1998 to 2018 were extracted and reviewed. These publications were classified into four categories (ie, healthcare process, hospital management, hospital informatization, and medical equipment and production) according to the healthcare issues to be solved, and analyzed regarding the application fields and the utilized FMEA methods.CONCLUSION:FMEA has high practicality for healthcare quality improvement and error reduction and has been prevalently employed to improve healthcare processes in hospitals. This research supports academics and practitioners in effectively adopting the FMEA tool to proactively reduce healthcare risks and increase patient safety, and provides an insight into its state-of-the-art.

Abstract:BACKGROUND:Medical risks are al unsafe events or that damage to the patient during medical services. Present medical risk management is mainly qualitative experience, and lacks of regular failure analysis and risk assessment for established medical treatment. <br> OBJECTIVE:To construct models of medical process failure analysis and risk assessment, find possible risks during medical treatment, and propose effective measures to eliminate or decrease above risks. <br> METHODS:Failure mode and effects analysis during reliability engineering were used in production. That was, risk assessment was conducted in the possible technical failure modes, causes and al impacts on the product during each process. Improvement measures were made for weak link during the process. The risk could reach an acceptable level. In accordance with failure mode and effects analysis during production, the procedure of medical process failure analysis and risk assessment could be made to analyze the potential failures during medical treatment. Moreover, the improvement measures were proposed for weak link with high risks so as to prevent the occurrence of risk of significant adverse effects on patients. <br> RESULTS AND CONCLUSION:The methods and basic procedures of medical process failure analysis and risk assessment were established by using the experience of failure mode and effects analysis. Taking the rescue process of myocardial infarction in the emergency of a hospital as an example, the analysis of failures, reasons and impacts was performed taking“chewing 300 mg aspirin”in the rescue steps as a key. The improvement measures and suggestions were proposed for unacceptable failures and reasons. Seen from the analysis results, proposed improvement measures and suggestions can obviously decrease the risks of failures caused by this step to patients. Therefore, the application of failure mode and effects analysis in medical treatment has a strong practical value.

Abstract:Abstract Purpose The purpose of this study was to identify potential failure points in a new chemotherapy preparation technology and to implement changes that prevent or minimize the consequences of those failures before they occur using the failure modes and effects analysis (FMEA) approach. Methods An FMEA was conducted by a team of medication safety pharmacists, oncology pharmacists and technicians, leadership from informatics, investigational drug, and medication safety services, and representatives from the technology vendor. Failure modes were scored using both Risk Priority Number (RPN) and Risk Hazard Index (RHI) scores. Results The chemotherapy preparation workflow was defined in a 41-step process with 16 failure modes. The RPN and RHI scores were identical for each failure mode because all failure modes were considered detectable. Five failure modes, all attributable to user error, were deemed to pose the highest risk. Mitigation strategies and system changes were identified for 2 failure modes, with subsequent system modifications resulting in reduced risk. Conclusion The FMEA was a useful tool for risk mitigation and workflow optimization prior to implementation of an intravenous compounding technology. The process of conducting this study served as a collaborative and proactive approach to reducing the potential for medication errors upon adoption of new technology into the chemotherapy preparation process.

Abstract:Failure mode and effect analysis (FMEA) is a team-based technique for prospectively identifying and prioritizing failure modes of products, processes, and services. Given its simplicity and visibility, FMEA has been widely used in different industries for quality and reliability planning. However, various shortcomings are inherent to the traditional FMEA method, particularly in assessing failure modes, weighting risk factors, and ranking failure modes, which greatly reduce the accuracy of FMEA. Additionally, the classical FMEA focuses on the risk analysis problems in which a small number of experts participate. Nowadays, with the increasing complexity of products and processes, an FMEA may require the participation of larger number of experts from distributed departments or organizations. Therefore, in this article, we present a novel risk priority approach using cluster analysis and prospect theory for FMEA when involving a large group of experts. Furthermore, an entropy-based method is proposed to derive the weights of risk factors objectively by utilizing the risk-evaluation information. Finally, we take an empirical healthcare risk analysis case to illustrate the proposed large group FMEA (LGFMEA) approach, and conduct a comparative study to evaluate its validity and practicability.

Abstract:Failure mode and effects analysis (FMEA) is a widely applied risk assessment method in engineering and management fields. In a typical FMEA, the ranking orders of failure modes are determined by risk priority number (RPN) which can be determined by multiplication of the scores of risk factors. However, the typical RPN has been criticized for several limitations when employed in practical situations. In this paper, a novel FMEA method using extended matter-element model and analytic hierarchy process (AHP) is developed to overcome the limitations of the typical RPN. The scores of failure modes with respect to risk factors are obtained according to the typical RPN method, and the weights among risk factors are derived from AHP. The extended matter-element model integrating with AHP is developed to calculate the closeness coefficients of failure modes, according to which the ranking orders of failure modes are determined. An illustrative example, which implements FMEA to evaluate an ocean fishing vessel, is presented to demonstrate the application and effectiveness of the proposed method. Finally, a sensitivity analysis is implemented to explore the influences of the weights of risk factors and a comparison analysis is carried out to show the advantages of the proposed method.

Abstract:Failure mode and effects analysis (FMEA) is a systematic bottom-up structured method used originally in aerospace systems then electronic systems to implement failure analysis, criticality analysis, and failure clearance. With its effective application, FMEA has been extended rapidly into mechatronic systems and others to improve system reliability. However, there exist different compositions, loading forms, operation modes, and failure mechanisms for diverse kinds of systems, so the formulaic extension of FMEA usually is not suitable for the actual situation of followers. In this paper, we propose a function-motion-action (FMA) hierarchy structure especially suitable for mechatronic systems, which focuses on describing and analyzing failure behaviors such as identifying failure modes, analyzing failure causes, and mapping failure effects at each system structure layer without vague and overlapping interpretations. Three risk factors namely occurrence, detection or non-detection, and severity are both defined for each of failure behaviors at each system structure layer, in which the occurrence describes all failure behaviors and the severity is reliant on the condition of the detection and non-detection about all failure behaviors. A primary risk calculation model using quantitative risk data is established to garner the risk level of each failure, based on which multiple and specific risk levels of each failure are explained when considering different levels of failure effects and different combinations of control methods.

Abstract:Medical device systems have become increasingly complex, interconnected, and interoperating. A major challenge is how to ensure and improve the safety, security, and reliability of medical devices. An efficient human reliability analysis and assessment for medical devices is essential for improving the quality of medical treatment and preventing an iatric accident. This paper explores qualitative and quantitative methods to analyze human reliability for medical devices. First, the SHELL (named after the initial letters of its components' names, Software, Hardware, Environment, Live-ware and Central Live-ware) model is developed to make a qualitative analysis for human reliability of medical devices. The SHELL model is to consider human as an integrated and inseparable component of the productive system. After that, failure modes and effects analysis (FMEA) is proposed to evaluate the potential failures in human reliability of medical devices. Failure mode and effects analysis (FMEA) is a method to assess a system, design, process or service for possible ways, in which failures (problems, errors, risks and concerns) can occur. The most important issue of FMEA is the determination of risk factors like the occurrence, severity, and detection using the opinions of different experts. This paper applies fuzzy linguistic theory to convert the subjective cognition of experts into an information entity to obtain the numerical values of risk factors. Abstract:With the rapid development of radiotherapy technology, traditional quality management will shift from a device-centric strategy to a process involving multiple people. In recent years,failure mode and effect analysis(FMEA)based on predictive risk assessment has gradually been applied in the quality management of radiotherapy process. The FMEA approach mainly includes four steps: process tree management,FMEA, error tree analysis, and the design of scheme for process quality management. The specific methods and results are different due to the specificity of radiotherapy process and the subjectivity of participants. Its reliability and feasibility have also been considered and verified. This article reviews the application of FMEA approach in some radiotherapy centers.

Abstract:Objective To standardize the procedure and ensure the safety of oral administration,and to achieve the "three-accurate",that is right medicine,right time,and right patients． Methods The concept of "Health Care Failure Mode and Effect analysis" was used to analyze the procedure of in-patients' oral administration,and adjust the system according to insecurity factors to improve the hospital oral administration system constantly． Results The systematic reconstruction of the oral administration improved nurses' and patients' satisfaction,reduced the error rate of in-patients' oral administration and improved the safety of oral administration． Conclusion The application of"Health Care Failure Mode and Effect analysis" to forward-looking analysis in-patient oral administration system,combined with the actual situation of our hospital to develop and implement plans are the guarantees of oral administration security.

Abstract:Medical devices play a critical role in care and treatment. The human-related failures can significantly affect the safety of patients in clinical use of medical devices. This study develops a comprehensive risk assessment model for identification and evaluation of failures which may occur in the clinical use of medical devices. First, the “Swiss cheese” model and SHEL model (the acronym of software, hardware, environment, and liveware) are integrated to comprehensively identify the potential human errors. Then, a new failure mode and effects analysis (FMEA) approach improved by rough set theory and grey relational analysis is developed to assess the risk of the identified failures. The proposed method integrates the strengths of the “Swiss cheese” and SHEL model in identifying human failures from both the vertical and horizontal perspectives of the system, and the advantages of the improved FMEA approach in flexibly manipulating vague information in risk evaluation without much priori information. Finally, the proposed method is applied in clinical use of respirator to verify its efficiency and effectiveness.

Abstract:The aim of this study was to evaluate the use of Healthcare Failure Mode and Effect Analysis (HFMEA) in Dutch health care by means of user feedback. Thirteen HFMEA analyses of various health care processes were successfully concluded and on average took 69 person-hours (excluding reporting). These results show that HFMEA can successfully be applied in Dutch health care. However, the user feedback also uncovered several perceived drawbacks, such as the fact that HFMEA is very time-consuming and that, particularly, the risk assessment part of HFMEA is difficult to carry out. Moreover, a lack of guidance with regard to the identification of failure mode causes and effective actions might influence the quality of the outcomes of an HFMEA analysis. Several suggestions are put forward to improve the perceived utility and acceptance of HFMEA. Nevertheless, future research is necessary to evaluate the actual effects of these recommendations. Error modelling and risk analysis, and their contribution to explaining human performance in socio-technical systems, traditionally belong to the field of ergonomics. The user feedback on HFMEA and the suggestions that are put forward may also be useful for (H)FMEA and hazard analysis and critical control point applications in sectors other than health care.

Abstract:Failure mode, effects analysis (FMEA) is widely recognized and applied in different fields, such as aviation, aerospace, automotive industries. And later FMEA has been extended to other industries, such as electronics, medical and so on. For key products, FMEA is an indispensable part in reliability analysis. However, many products are newly researched and developed, with high integrated level and complicated failure mode, reasons and effects. Historical data and similar products data are also scarce, which constrain the effectiveness of hardware FMEA. Besides, technicians are relatively young and inexperienced with incomplete understanding of failure and effects, which will result in incomplete analysis results. These problems affect implementation effect of hardware FMEA and will affect the subsequent reliability, maintainability, testability and supportability analysis work. Aiming at the above problems, this paper proposes an integrated method for hardware FMEA of new electronic products which integrates present advanced failure mode acquisition, failure mechanism analysis and failure effect analysis together. This method can effectively solve the above problems, meanwhile make supplement for the problem not taking redundant design into consideration in traditional FMEA method. It has been proved that this integrated method achieved good effect on a type of new research aviation electronic products.

Abstract:Failure mode and effect analysis (FMEA) is a powerful risk discerning technique for identifying, evaluating, and reducing possible failures of products or processes. However, the classical FMEA has been criticized for inherent limitations, such as equal weights of risk elements and lack of capability in handling inaccurate information. Although fuzzy-based modified FMEA methods are frequently utilized to handle vagueness of experts' judgments, they still have some drawbacks, for example, requiring extra assumptions, neglecting experts' bounded rationality and psychological effects, lacking consideration of randomness, and only considering three classical risk elements among most of them. Therefore, this study develops an extended risk assessment method to enhance the performance of FMEA, which integrates the superiority of rough number theory in handling subjective and inaccurate information and the advantage of cloud model theory in reflecting the randomness of qualitative evaluations. Moreover, two synthetic weighting methods are developed to determine the weights of risk elements and handle the experts' individual effects, respectively, which consider both subjective and objective aspects. In addition, maintenance is added into the classical risk elements, and then a hierarchical structure containing four risk dimensions is built to evaluate failures' risk levels comprehensively. Finally, an application case to demonstrate the effectiveness of the developed FMEA model is presented.

Abstract:Objective To analyze the risks of hypertonic glucose injection (20％) (hypertonic glucose) in clinical application using failure mode and effect analysis (FMEA) method and improve the relevant management measures.Methods A study group on risk prevention strategies of hypertonic glucose was established in the First People's Hospital of Hefei.Domestic and foreign literature on medication error cases,preventive measures or management strategies,and data from questionnaires and on-the-spot investigations were collected.The failure modes in the process of hypertonic glucose application were analyzed and assessed.Severity (S),frequency of occurrence (O),and likelihood of detection (D) of the risks related to these failure modes were scored and the risk priority numbers (RPN) were determined.The failure modes with higher RPN were screened out and the corresponding prevention strategies were formulated.The incidence of medication errors and potential hazards,cognition rate of knowledge on drugs,qualification rate of drug storage,and satisfaction rate for management measures before and after the improvement were analyzed comparatively.Results According to the results from literature search,questionnaires,and on-the-spot investigations,a total of 24 failure modes were found in the 6 links relevant with hypertonic glucose application in clinical,which included links of prescription writing and delivery,pharmacist dispensing,nurse dispensing,post-administration monitoring,drug management,and information technology.Ranking the RPN from high to low,7 major failure modes with higher RPN were selected from the 24 failure modes,including the existence of drugs with similar name and drugs with similar package,wrong drug dispensing,absence of post-administration monitoring,lack of marked labels for drugs,garbles caused by lack of medication knowledge,and untimely record and report of medication errors.The incidences of medication errors and potential safety hazards before and after management improvement were 3.2％ (7/216) and 0.4％ (1/234),respectively and the difference had no statistical significance (P =0.058).The average cognitive rates of physicians,nurses and pharmacists before and after the improvement were 65.2％ (15/23),54.2％ (13/24),67.9％ (19/28) and 95.8％ (23/24),91.7％ (22/24),95.5 ％ (21/22),respectively and the differences were statistically significant (P 50.05 for all).The qualification rates of drug storage before and after management improvement were 70.2％ (151/215) and 98.7％ (230/ 233),respectively and the difference was statistically significant (P 5 0.05).The average satisfaction degrees of medical staff with hyperosmotic glucose management were 81.3％ (61/75) and 95.7％ (65/70),respectively before and after the management improvement and the difference was statistically significant (P 5 0.05).Conclusion Failure modes in the process of hypertonic glucose use can be effectively identified using the FMEA method,and the risk events can be effectively reduced after the corresponding improvement measures.
