Abstract:OBJECTIVE:To study the limb salvage methods and treatment outcomes in malignant periacetabular bone tumors.METHODS:The data of 31 patients of periacetabular malignant tumors who had limb salvage surgery between January 1999 and December 2006 was retrospectively reviewed. There were 14 females and 17 males with a mean age of 53 years (range, 42-75 years). Twenty-four patients had chondrosarcomas, 4 patients had Ewing sarcomas, 1 patient had osteosarcoma, and the remaining 2 patients had metastatic disease. Sixteen patients had Types II pelvic resections, 5 patients had Types I and II pelvic resections, another 5 had Type I and III pelvic resections, and Type I , II and III pelvic resections in the remaining patients. Seventeen patients had reconstructions after tumor resection.RESULTS:The mean follow-up time for all patients was 52 months (range, 12-84 months). Fourteen patients were alive with no evidence of disease, 4 patients were alive with disease at the most recent follow-up, and 13 patients died of disease. The local recurrence rate and mortality rate in 24 patients with chondrosarcomas was 20.8% and 33.3% respectively. Two patients with metastatic disease died at 11 and 34 months postoperatively. One patient with osteosarcoma and 2 patients with Ewing sarcoma died of lung metastases. Enneking scoring system was used to evaluate the functional outcome in 18 alive patients. In 13 patients who had reconstructions, 6 were in excellent, 6 were in good, and 1 was in poor. While in 6 patients who had no reconstructions, 3 were in excellent, 2 were in good, and 1 was in poor. Minor complications occurred in 6 patients.CONCLUSIONS:Clear margin tumor resection with decreased local recurrence rate is critical for limb salvage surgery in periacetabular sarcomas. The ranges of tumor invasion and resection, the principle of individual treatment should be considered in functional reconstruction.
Analysis of the efficacy and prognosis of limb-salvage surgery for osteosarcoma around the knee

Abstract:Aim: Limb-salvage surgery has become the standard of care for extremity osteosarcoma. In this study, we investigated the survival and functional outcomes of patients with osteosarcoma around the knee who were treated with limb-salvage surgery. Methods: We retrospectively reviewed the clinical data for 120 patients with osteosarcoma around the knee who were treated with limb-salvage surgery between 1998 and 2008. The sample included 75 males and 45 females. The mean age of the patients was 18.9 years. Osteosarcoma was diagnosed in the distal femur in 78 patients and in the proximal tibia in 42 patients. Statistical analyses were conducted to process and record the patient data and analyse the surgery's efficacy, prognosis and survival rates. Results: All patients were followed for 6-144 months (mean of 56.8 months). The overall 5-year survival rate was 61.8%. Lung metastasis developed in 31 patients. Local recurrence developed in 9 patients. The average Musculoskeletal Tumor Society Score (MSTS) was 25.5 points on a 30-point scale. Sixteen patients underwent prosthesis revision and twelve patients underwent amputation. The overall survivorship of the prosthesis based on Kaplan-Meier estimates was 77% at five years and 71% at ten years. There was a higher incidence of extensor lag for the patients with osteosarcoma in the proximal tibia than for those with osteosarcoma in the distal femur (P < 0.01). Conclusions: Treating osteosarcoma around the knee with limb-salvage surgery can preserve most of the knee's functionality. Attention must be paid to prevent the relatively high incidence of postoperative complications.
Treatment of Extremity Osteosarcoma with Neoadjuvant Chemotherapy and Limb Salvage

Abstract:Objective To analyze the outcomes of treatment of extremity osteosarcoma after neoadjuvant chemotherapy and limb salvage operation.Methods On the basis of Enneking grading,preoperative chemotherapy with methotrexate, cisplatin, doxorubicin, and ifosfamide was given in 121 patients of extremity osteosareoma, some of whom with liB tumors received extra interventional embolism followed by postoperative chemotherapy according to the tumor necrosis rate.Most of the patients underwent limb salvage operation(99/121), and the Musculoskeletal Tumor Society(MSTS) score was used to evaluate the recovery of the limb function.Results The follow up lasted for 16-101 months with a survival rate of 62.8%(76/121).In 47 cases underwent osteoarticular allografts, the mean MSTS score was 22.60±4.13,and the overall excellent and good function outcome was obtained in 76.3% of the patients.Fifty patients underwent custom-made or modu[ar tumor endoprosthesis replacememt bad MSTS of 24.32 ± 3.85 with an overall excellent rate of 84.6%.Conclusion Neoadjuvant chemotherapy and limb salvage surgery are effective in treating osteosarcoma.
Limb Salvage in Treatment for 47 Cases with Long Bone Tumors

Abstract:[Purpose] To investigate the methods and clinical efficacy of limb salvage in treatment for long bone tumors. [Methods] Forty-seven cases with long bone tumor stage Ⅲ underwent with bone tumor resection and massive bone allograft reconstruction. All the 47 cases were followed-up. [Results] According to the Enneking system, the good-excellent rate of function outcome was 73%. [Conclusion] Limb salvage treatment is becoming the major means for treating long bone tumors with certain curative effect.
Pathologic Fracture Does Not Influence Local Recurrence and Survival in High‐grade Extremity Osteosarcoma with Adequate Surgical Margins

Abstract:Background and Objectives: The purpose of this study was to estimate the risk factors having relationships with pathologic fracture of osteosarcoma of extremities and to evaluate the role of limb salvage surgery in this group of patients.Methods: We retrospectively analyzed 28 consecutive cases of pathologic fracture of primary high-grade localized osteosarcoma of extremities between June 1, 2001 and June 30, 2009. All patients underwent limb salvage surgery and neo-adjuvant chemotherapy. They had a median age of 14 years (range, 630 years). The average follow-up time was 40.7 months (range, 9108 months). Clinicopathological factors were analyzed in relation to pathologic fracture. Their recurrence and survival rates were compared to those in cohort of 171 osteosarcoma patients without pathologic fracture who underwent limb salvage surgery during the same period at the same institution.Results: Less than 15 years, telangiectatic histological subtype, tumor located at the proximal humerus, and radiographical manifested as osteolytic features were risk factors in relation to pathologic fracture. The overall 3- and 5-year survival rates were 50.5% and 45.4%, respectively, in the fracture group, and were not significantly different from those in the control group (71.0% and 61.9%, respectively). Of all 28 fracture patients, 4 experienced local recurrences (14.2%) and 14 developed distant metastasis (50%), which were not significantly different from the rates in the control group (8.8% and 37.4%, respectively).Conclusion: Limb salvage surgery with adequate margins combined with neo-adjuvant chemotherapy for pathologic fracture of osteosarcoma did not seem to significantly increase the risk of local recurrence or distal metastasis. Impact of Limb Salvage on Prognosis of Patients Diagnosed with Extremity Bone and Soft Tissue Sarcomas

Abstract:Background Although clinicians and patients with extremity bone and soft tissue (EBST) are increasingly interested in limb salvage surgery (LSS), because of the minimal damage to physical appearance and function, however, there is still a lack of large-scale population studies on whether LSS improves the prognosis of patients. Purpose The aim of this study was to compare the survival of patients with EBST sarcomas after receiving LSS and amputation. Methods To conduct the population-based study, we identified 6,717 patients with a histologically diagnosed bone sarcoma and 24,378 patients with a histologically diagnosed soft tissue sarcoma from the Surveillance, Epidemiology, and End Results database. We analyzed overall survival (OS), cancer-specific survival (CSS), and non-sarcoma survival (NSS) using the Kaplan–Meier method, log-rank test or Gray test, Cox regression model, propensity score-matched analysis, and landmark analysis. Results LSS could improve the prognosis in patients with most EBST subtypes, except for Ewing sarcomas and MPNST. However, in the subgroup without distant metastases, limb salvage increased CSS only for patients with osteosarcoma, Ewing sarcoma, and leiomyosarcoma, as well as NSS for patients with chondrosarcoma and synovial sarcoma. Landmark analysis further demonstrated that sarcoma survivors surviving <10 years could benefit from LSS but not for long-term survivors ≥10 years. Moreover, for patients with distant metastases, LSS could improve survival of osteosarcoma patients but worsen CSS among patients with MPNST. Landmark analysis further demonstrated that LSS improved survival among osteosarcomas patients with distant metastases only within 1 year after surgery. Moreover, patients receiving LSS and those receiving amputation had a high risk of dying from different non-sarcoma diseases during the postoperative follow-up. Conclusions The impact of limb salvage on the prognosis of patients depends on the pathological subtype and stage of EBST sarcomas.
The assessment of the effectiveness of the adjuvant chemotherapy regimen for the treatment of osteosarcoma

Abstract:Objective To evaluate the clinical results of treatment for osteosarcoma of the extremities with adjuvant chemotherapy regimen Methods Sixty seven patients with ⅡB extremity osteosarcoma were treated with the combination of adjuvant chemotherapy regimen and definitive surgery Forty seven of them were male and twenty four were female The mean age of the patients at the time of diagnosis was 20 years (ranged 10～51 years) Sixty four lesions involved the lower extremity, with 45 involving the distal femur, 14 the upper tibia, 3 the upper fibula and 2 the proximal femur; Three involved the upper end of the humerus The study of subtype of osteosarcoma revealed that 51 were osteoblastic, 6 chondroblastic, 5 fibroblastic and 5 the other subtypes The patients were divided into three groups according to the adjuvant chemotherapy regimen given, the first group consisted of 23 cases, who were treated with the combination of the presurgical cisplatin hyperthermic isolated limb perfusion (CDP-HILP) and postoperative chemotherapy; the second group of 20 cases, who were treated with the combination of the presurgical CBP-HILP and postoperative chemotherapy; and the other 24 cases were treated with postoperative chemotherapy alone Twenty nine of the 67 patients treated surgically had limb salvage procedures; 35 had amputations and 3 had Tikhoff-Limbergs surgery Multiple histologic sections were procured and examined from each of the surgical specimens in the patients who were treated with preoperative HILP to determine the effect of chemotherapy All patients were followed from 3 to 9 years, and the data were statistically analeyed using SPSS 7 0 Results Among 67 cases, 41 (61 19%) had relapse free survival(RFS) in 2 years after surgery, 37 (55 22%) had RFS in 3 years after surgery, and RFS probability at 5 years after surgery was 49 56% Log-rank analysis showed no significant difference in RFS probability among the three groups(P=0 517 1), but there was a significant difference in RFS probability between patients who had limb salvage procedure and those had amputation(P=0 009 7) Age, sex, site and subtype were not significant prognostic variables in this group of patients Correction analysis revealed that the RFS probability were not related with the rate of the tumor necrosis (P=0 058) Conclusion The data revealed that 1) The chemotherapy regimen mainly including platinic drugs was effective treatment for osteosarcoma ,and was more simple and less expensive than any previous reported chemotherapy for steosarcoma, 2) CDP of chemotherapy regimen for osteosarcoma, which had more nephritic and audio toxicity could be replaced by CBP , And 3)Whether the preoperative chemotherapy is suitable for the patients in whom amputation is inevitable (based on age ,size, and involvement of local structure) remains unclear

Abstract:OBJECTIVE:To analyze the oncological and functional outcome of limb salvage in this location.METHODS:From November 2003 to January 2010, 20 patients with primary malignant bone tumors of the distal lower extremity were treated. There were 15 male and 5 female, the mean age was 20 years. Among 14 patients with malignant distal tibial sarcoma, 11 patients had ankle arthrodesis reconstructed by using allograft (7 patients) or autografts (4 patients), the other 3 patients underwent below-knee amputation. Three patients with osteosarcoma of distal fibula reconstructed with ipsilateral fibular head, 3 patients with calcaneus osteosarcoma underwent total calcanectomy and reconstructed with fibular segment and iliac crest. The mean follow-up was 36.4 months.RESULTS:Five of 7 (71.4%) patients with allograft reconstruction were associated with delayed wound healing, however, only 1 of 10 patients with autograft had this problem (P = 0.036). One local recurrence was observed, 2 osteosarcoma patients died of disseminated disease. The estimated 2-year and 5-year overall survival for the patients with malignant tibial sarcomas were 92.9% and 79.6% respectively, for the patients with distal tibial high-grade osteosarcoma were 87.5% and 70.0% respectively. The mean functional MSTS score was 82% for limb salvage patients.CONCLUSIONS:The survival of patients with primary malignant bone tumor of distal lower extremity seems to be better than that of other sites. Limb salvage can provide satisfactory local control and functional results.

Abstract:Osteosarcoma is the most common primary malignant bone tumor, occurring mainly in children and adolescents, and the limbs are the main affected sites. At present, limb‐salvage treatment is considered as an effective basic standard treatment for osteosarcoma of the limb. China has a vast territory, but the development of technology is not balanced,which requires sufficient theoretical coverage, strong technical guidance and the application of limb‐salvage treatment guidelines to the treatment of osteosarcoma. Therefore, to standardize and promote the development of limb‐salvage surgery technology and improve the success rate of limb‐salvage treatment, this guide systematically introduces limb‐salvage techniques for the treatment of patients with limb osteosarcoma through definition of limb‐salvage treatment, surgical methods, efficacy evaluation, postoperative treatment and prevention of complications, rehabilitation guidance, and follow‐up advice.
[Limb Salvage Surgery for Osteosarcoma Around the Knee in Children and Adolescent Patients].

Abstract:OBJECTIVETo discuss the reconstruction methods and treatment effects of osteosarcoma around the knee in skeletal immature children and adolescent patients.METHODSBetween October 1996 and December 2005, 89 children and adolescent patients with osteosarcoma around the knee received limb salvage surgery. The average age was 13 years old. Tumor involved distal femur in 52 cases, proximal tibia in 36 cases, and both parts in one patient. Different limb salvage methods, including tumor recycle bone reimplantation (n = 20), partial endoprosthesis (n = 19) and total knee megaprosthesis replacement (n = 50) were chosen according to both the age of the patient and the skeletal. Expandable and special designed pediatric prosthesis some times might also be considered.RESULTSAfter average follow up of 42 months, 4 cases of local recurrence and 11 of metastasis occurred in tumor recycle bone group; 1 case of local recurrence and 4 of metastasis in partial endoprosthesis group; another 4 cases of local recurrence and 9 patients died of disease in total knee megaprosthesis group. The five-year survival rate was 47.1%, 71.7% and 68.2% in 3 groups respectively. There were 3 deep infections in all the cases, 2 subluxations and 1 amputation after operation for limb ischemia in partial endoprosthesis group. The average functional score of MSTS 93 was 21.43 point, 23.40 point and 25.32 point; and the average leg length discrepancy was 5.48, 4.50 and 3.12 cm in 3 groups respectively.CONCLUSIONSPartial-joint prosthesis or other methods which can avoid injury of the opposite epiphysis should be used to decrease the possibility of limb length discrepancy and ease the following revised arthroplasty. For patient with almost mature skeletal, total knee megaprosthesis may be a proper choice for better function result.
Clinical Results of Limb Salvage Operation with Major Vascular Reconstruction for Sarcomas of Extremities

Abstract:Objective To determine the feasibility of limb salvage with major vascular reconstruction for sarcomas of extremities, focusing on the early complications, oncological and functional outcomes.Methods Between August 2004 and June 2009, 13 patients (mean age 38 years, range 14-63 years) underwent wide resection of upper and lower extremity sarcomas to include the involved arterial and venous segments. 4 patients had bone sarcomas and 9 soft tissue sarcomas, 6 patients only had underwent artery replacement and the other 7 had underwent both artery and vein. 8 resected vessels were reconstructed with vascular graft and 5 with ipsilateral or contralateral saphenous vein. Results No patient suffered from reperfusion injury and infection. The patient with osteosarcoma in proximal humerus had suffered acute arterial graft thrombosis at first day postoperatively; however, the limb was salvaged after successful thrombectomy. 5 of 8 patients with synthetic graft developed minor delayed wound healing and hematoma, but the other 5 patients with autologous saphenous vein replacement did not. The mean follow-up was 19.4 months (7-45months). One patient died of pulmonary metastasis; the patient with osteosarcoma in proximal humerus had developed local recurrence, but refused amputation and was alive with metastasis. No arterial occlusion was observed at final follow-up or at the time of death, but the patency of artery and vein was 100% and 28.6%respectively. One patient who has pulmonary metastases before operation died of metastases 7.5 months after operation. Four patients developed pulmonary metastases, therefore, Kaplan-Meier survival analysis showed that 2-year overall and metastasis-free survival rate was 90.9% and 63.6% respectively. Functional status were judged as good or excellent (mean MSTS score 72%) in 12 of 13 patients. Conclusion The study indicate that malignant involvement of major vessels is not a contraindication for limb-salvage. Vascular reconstruction is a feasible option in limb salvage surgery. Wide resection with vascular reconstruction provide acceptable oncological and functional outcome of limb salvage.
Global Research Trends in Limb Salvage Surgery for Osteosarcoma: Findings from a Bibliometric and Visualized Analysis over 15 Years

Abstract:Background: Limb salvage surgery in osteosarcoma requires a multidisciplinary team of experts, due to which research interest has remained limited globally. This article analyzes research trends over 15 years from 2007 to 2022. Materials and methods: Publications on limb salvage surgery in osteosarcoma were retrieved using the Web of Science. Bibliometric analysis of the publication metadata was done using R software. VOS viewer software was used to analyze the bibliographic coupling, co-citation, co-authorship, and co-occurrence to report the current trends in global research on limb salvage surgery in osteosarcoma. Results: A total of 693 articles were retrieved. On applying the inclusion and exclusion criteria, a publication metadata of 276 articles was analyzed using the methodology mentioned. Annual scientific production on the subject has shown a steady rising trend globally. China has the highest number of publications on the topic; however, the USA has the highest citations globally. The Journal "Clinical Orthopedics and Related Research" remains the pioneer in the topic with the highest number of publications and H index among all journals. Most of the research interest is generated in the developed countries of the USA, Europe, and China. Keyword analysis suggested 4 clusters of surgical reconstruction, Survival, Chemotherapy, and general management related. Newer keywords such as biological reconstructions, allograft, metastases, cell, and chemotherapy suggest future research topics in the field. Conclusion: Research interest in limb salvage surgery in osteosarcoma continues to grow with the introduction of concepts such as biological reconstructions and allografts. However, for more inclusive research on the topic, research interest must also be encouraged in underdeveloped and developing countries.
Clinical Factors Affecting Prognosis of Limb Osteosarcoma in China: a Multicenter Retrospective Analysis.

Abstract:Objective This study was performed to explore the relationship between various clinical factors and the prognosis of limb osteosarcoma. Methods We retrospectively analyzed the clinical data of 336 patients with limb osteosarcoma treated from June 2000 to August 2016 at 7 Chinese cancer centers. Data on the patients’ clinical condition, treatment method, complications, recurrences, metastasis, and prognosis were collected and analyzed. Kaplan–Meier analysis and Cox regression models were used to analyze the data. Results The patients comprised 204 males and 132 females ranging in age from 6 to 74 years (average, 21.1 years). The overall 3- and 5-year survival rates were 65.0% and 55.0%, respectively. The 5-year overall survival rate was 64.0% with standard chemotherapy and 45.6% with non-standard chemotherapy. Cox regression analysis demonstrated that standard chemotherapy, surgery, recurrence, and metastasis were independent factors associated with the prognosis of limb osteosarcoma. Conclusion The survival of patients with limb osteosarcoma can be significantly improved by combining standard chemotherapy and surgery. The overall survival rate can also be improved by adding methotrexate to doxorubicin–cisplatin–ifosfamide triple chemotherapy.
What Are the Results of Limb Salvage Surgery for Primary Malignant Bone Tumor in the Forearm?

Abstract:Background and Objectives: After diagnosing a primary bone tumor involving the forearm, various excision strategies and reconstruction methods must be considered. This study explored the oncological and functional outcomes of limb salvage surgery for primary malignant bone tumors in the forearm. Methods: Patients with primary forearm bone tumors (n = 369) were retrospectively analyzed between 2000 and 2017. There were 266 patients with radial tumors, and 46 (17.3%) were malignant, whereas 103 patients had ulnar lesions and 22 (21.4%) were malignant tumors. The oncological results, prognostic factors, and functional results after limb salvage surgery of forearm malignancies were analyzed. Results: The follow-up averaged 72.1 (7–192, median 62.5) months. Fifty-six patients who received limb salvage surgery were included in the final evaluation. Radius resection was performed in 38 patients, and distal radius (25 patients) was most frequent. Ulnar resection was performed in 18 patients, and the proximal ulna (13 patients) was most frequent. The surgical margins obtained were intralesional in 3 patients, marginal in 8 patients and wide in 45 patients. Local recurrence occurred in 11 patients (19.6%), and distant metastasis occurred in 14 patients (25%). The 5-year recurrence-free survival rate was 79.8%. Unplanned excision, ulnar involvement, proximal forearm location and inadequate surgical margins were associated with recurrence. The overall 5-year and 10-year survival rates were 83.5 and 71.7%, respectively. Distant metastasis was a poor prognostic factor for the survival rate. Forty-two patients were evaluated by MSTS score with an average of 27.9 ± 1.5. Conclusions: The incidence of radial malignant tumors is higher than that of ulnar lesions. The distal radius and the proximal ulna are the most frequently involved sites. Unplanned excisions, ulnar tumors, proximal forearm tumors, and inadequate surgical margin are the risk factors for local recurrence. Distant metastasis is an independent poor prognostic factor of death. The oncology control and functional results of limb salvage surgery were satisfactory.

Abstract:Objective To evaluate the results of intra-arterial chemotherapy and wide resection in patients with extremity soft tissue sarcoma.Methods From 1997 to 2003,21 patients with extremity soft tissue sarcoma were treated with intra-arterial chemotherapy preoperatively and limb-salvage two weeks later.Of them,13 cases (62%) were confirmed to be adherent to the major vessels and bone by ultrasonic and CT scan. After chemotherapy,the limb-salvage was undergone,resecting tumor tissues and meanwhile,preserving the main function of the extremities.Results The size of tumor decreased gradually with necrosis of the cells and the less invasive margin around tumor.Also,mass was detached easily from adjacent tissue.All cases were followed up for a period of 6-62 months,the functions of extremities of these patients remain satisfactory.Conclusions The use of intra-arterial chemotherapy and limb-salvage surgery can increase the possibility of resection and decrease the local recurrence in soft tissue sarcoma and remain the main function of affected extremity.
A multi-center retrospective study on the clinical prognosis of limb osteosarcoma

Abstract:Objective To explore effects of clinical factors on the prognosis of limb osteosarcoma. Methods Clinical data of 479 patients with limb osteosarcoma, registered from June 2000 to March 2016 in seven cancer centers in China, were retrospectively collected. Clinical conditions, treatment method, complication, recurrence and metastasis as well as prognosis were recorded and analyzed. Kaplan-Meier survival curve and Cox regression model were used to analyze the correlation between the survival rate and variables factors. Results There were 292 males and 187 females among 479 patients, aged from 3 to 74 years ( average 21.4 years ). Tumors located around the knee joint in 396 patients ( 253 cases of the distal femur, 125 cases of the proximal tibia and 18 cases of the proximal fibula ), while beyond the knee in 83 patients ( 24 cases of the proximal femur, 17 cases of the distal tibia, 34 cases of the proximal humerus, 5 cases of the distal humerus and 3 cases of the radius ). 3 and 5-year overall survival were 71.0％, 60.0％, respectively. 3 and 5-year disease-free survival were 54.0％, 46.0％, respectively. 5-year overall survival were 73.0％ standard chemotherapy and 55.9％ non-standard chemotherapy; 48.7％ three medicine combination ( DDP +IFO + ADM ) and 66.0％ four medicine combination ( DDP + IFO + ADM + MTX ) preoperatively; 51.5％ three medicine combination ( DDP + IFO + ADM ) and 66.2％ four medicine combination ( DDP + IFO + ADM + MTX ) postoperatively; 93.3％ tumor necrosis rate ≥90.0％ and 40.3％ tumor necrosis rate < 90.0％; 61.9％ limb-salvage surgery and 43.3％ amputation; 61.9％ Enneking stage II and 25.1％ Enneking stage III; 93.5％ no recurrence, 46.3％local recurrence, 20.7％ lung metastasis and 10.1％ recurrence and metastasis postoperatively. Single factor analysis showed that standard chemotherapy, chemotherapy regimens, tumor necrosis rate, surgery, the Enneking stage and recurrence and metastasis were important to the prognosis of limb osteosarcoma. Cox regression analysis showed that standard chemotherapy, tumor necrosis rate, surgery approach and recurrence and metastasis were independent factors to the prognosis of limb osteosarcoma. Conclusions Survival rate of patients with limb osteosarcoma can be significantly improved by combining the standard chemotherapy and surgery. Recurrence and metastasis, non-standard chemotherapy severely affect the prognosis of limb osteosarcoma.

Abstract:Objective To investigate the outcome and affecting factors of patients with osteosarcoma after recurrence.Methods Retrospective analysis was used to analyze the achieved data and prognosis-related factors of 72 patients with extremity osteosarcoma after recurrence from June 2000 to September 2012 through multicenter joint treatment.37 males and 35 females were included.The overall age was 7 to 61 years old (average age22.2 years old).Classification based on the tumor recurrent parts:42 cases (58.3％) on femur,18 cases (25％) on tibia,7 cases (9.7％) on humerus and 5 cases (6.9％) on fibula.Classification based on metastases:14 cases of simple recurrence (19.4％),58 cases (80.6％) of recurrence with metastasis:29 cases (40.3％) of recurrence occurred first,9 cases (12.5％) of metastasis occurred firstly,20 cases (27.8％) of simultaneous recurrence with metastasis;12/14 patients (16.7％) of simple recurrence had long-term survival,but 2 died (2.8％).Six of 58 patients (8.3％) of recurrence with metastasis had long-term survival but 52 died (72.2％).Results The overall 10-year postrelapse survival rate of the 72 patients was 25％.Kaplan-Meier univariate analysis revealed that postrelapse survival was significantly influenced by the continued treatment after recurrence and the number of postoperative chemotherapy,but it was not significantlyinfluenced by age,gender and the number of preoperative chemotherapy.Multivariate analysis revealed that the continued treatment after recurrence and the number of postoperative chemotherapy were independent prognostic factors (P=0.002,P=0.007).At the same time,according to the survival curve grouped by different indicators,the number of chemotherapy after recurrence has no significant effect on TFS.However,the continued treatment after recurrence has obvious statistical significance on improving TFS and OS of patients (P=0.026,P=0.002).Conclusion Resection and standard postoperative chemotherapy had significant effects on the postrelapse survival in patients with osteosarcoma.There are good prospects for further multimodal therapy and multicenter cooperation for osteosarcoma after recurrence.

Abstract:The incidence of malignant bone tumors has remained relatively stable over the past two decades between 8% and 9% per 1,000,000 in North America. Multidisciplinary treatment is paramount for optimal care combining surgical resection, chemotherapy, and rehabilitation. Surgical treatment aims for a negative margin resection of the sarcoma with a personalized reconstruction plan. Limb salvage surgery (LSS) is possible in the majority of cases; however, amputation (including rotationplasty) may be required or preferred. Reconstruction can be achieved utilizing endoprostheses, allograft, autograft, or a combination of these techniques. Emerging technologies such as 3D printing of implants and cutting guides, and intraoperative navigation have helped to improve options for LSS.

Abstract:Purpose: Limb salvage surgery is a surgical procedure for tumour resection in bone and soft-tissue cancers. Guidelines aim to preserve as much function and tissue of the limb as possible. Surgical outcome data is routinely available as part of surgical reporting processes. What is less known are important non-oncological outcomes throughout recovery from both clinical and patient perspectives. The objective of this review was to explore non-oncological outcomes in patients diagnosed with sarcoma around the knee following limb salvage surgery. Materials and Methods: A scoping review methodology was used, and results analysed using CASP checklists. Results: Thirteen studies were included and following appraisal and synthesis, three themes emerged as providing important measures intrinsic to successful patient recovery: (1) physical function, (2) quality of life and, (3) gait and knee goniometry. Specifically, patients develop range of motion complications that alter gait patterns and patients often limit their post-operative participation in sport and leisure activities. Conclusions: This study has shown the importance of exploring confounding factors, adopting a holistic view of patient recovery beyond surgical outcomes, proposing evidence-based guidance to support and inform healthcare providers with clinical decision-making. This review highlights the paucity and lack of quality of research available, emphasising how under-represented this population is in the research literature.Patients having undergone LSS often have limited participation in sport and leisure activities.Patients can develop range of motion complications, such as flexion contracture or extension lag, which may affect the pattern of gait.Clinical consideration should be given to walking ability and gait patterns during the rehabilitation phase to prevent poor functional outcomes during recovery.Variation of treatment protocols, outcome measurement and rehabilitative care has been identified as important in predicting the outcomes in recovery from LSS procedures.

Retrospective study on the secondary amputation after the limb salvage surgery for bone tumor

Abstract:Background:Endoprosthetic replacement is a main technique for reconstructing the bone defects after the bone tumor resection.So far,secondary amputations happened after the limb salvage surgery are rarely reported.Objective:To analyze the risk factors of amputation after the patients with bone tumor receive the limb salvage surgery.Methods:From October 1995 to October 2011,1459 patients with bone tumor received the limb salvage surgery and were followed up.The clinical data of medical history,diagnostic and treatment process,postoperative complications,and prognosis were collected and analyzed statistically.Results:70 out of 1459 patients failed the limb salvage surgery and received the secondary amputation surgery,the rate of the secondary amputation was 4.80%.Reasons for the amputation included 54 cases of local recurrence,14 cases of deep infection around the artificial joints,and 2 cases of acute ischemia and necrosis at the distal limb after the limb salvage surgery.The amputation surgery was performed after 6 days to 10 years after the limb salvage surgery,with an average of 18.1 months.Conclusions:The limb salvage surgery can effectively reconstruct the bone defects of patients with bone tumors.There are few long-term complications of it,however,there still exists the risk of secondary amputation following the failure of that surgery.Local recurrence and deep infections are the main causes of the secondary amputation.Pathologic fracture is an important risk factor of secondary amputation to the patients with limb tumors receiving limb salvage surgeries.An improved method of preventing the local recurrence and deep infection will effectively reduce the risk of secondary amputation.The risk of secondary amputation decreases with time.
