Abstract:Abstract Abstract 5093 Objectives: To retrospectively analyze the significance of different clinical factors for predicting the prognosis of mature T-cell non-Hodgkin lymphoma (MTCL) patients. Methods: Two hundred and fifty-two MTCL patients admitted from 2005 to 2011 into the First Affiliated Hospital of the Zhejiang University were retrospectively reviewed. All diagnoses were confirmed by histopathological hematoxylin and eosin (HE) staining and prognostic values of β2-microglobulin (β2-MG) levels, lactate dehydrogenase (LDH) levels, B symptoms, Ann Arbor stages, international prognostic index (IPI), Eastern Cooperative Oncology Group (ECOG) performance status, bone marrow involvement (BMI) and extra nodal involvement (ENI) for the overall survival (OS) were evaluated. Additionally, we compared the OS of patients treated with CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisone) and patients treated with an alternative intensive chemotherapy. Results: At a median follow-up of 23 months, the OS rate was 23. 8%. Our results revealed that the presence of B symptoms (P <0. 001), ECOG score≥2 (P <0. 001), BMI (P <0. 001), elevated LDH levels (P <0. 001), elevated β2-MG levels (P<0. 001), Ann Arbor stages III/IV (P=0. 007) and IPI ≥3 (P=0. 001) were factors for a poor OS prognosis and intensive chemotherapy showed a better OS outcome than CHOP treatment. Conclusion: Elevated LDH and β2-MG levels, B symptoms, Ann Arbor stage III/IV, BMI, high IPI indexes and ECOG scores predict an unfavorable prognosis of MTCL patients. When compared with the classical CHOP regimen, the intensive chemotherapy treatment can improve the prognosis of patients with MTCL. Disclosures: No relevant conflicts of interest to declare.

Abstract:The prognostic value of absolute lymphocytic count (ALC) has been a recent matter of debate in the study of non-Hodgkin-lymphoma. To evaluate the prognostic value of ALC at diagnosis in patients with diffuse large B-cell lymphoma (DLBCL), we performed a meta-analysis of published studies that provided survival information with reference to ALC at diagnosis. Six studies covering a total of 1,206 subjects were included in this analysis. The summary hazard ratios of low ALC for overall survival were 2.72 (95% confidence interval (CI) 2.15–3.45, P < 0.001) in the entire population, 2.96 (95% CI 2.04–4.29, P < 0.001) in the population that received CHOP, and 2.78 (95% CI 1.87–4.13, P < 0.001) in the population that received R-CHOP. The corresponding ratios for progression-free survival were 2.79 (95% CI 1.90–4.11, P < 0.001) in the entire population, and 2.56 (95% CI 1.66–3.96, P < 0.001) in the population that received R-CHOP. In conclusion, our systematic analysis suggests that low ALC has an adverse effect on outcome in DLBCL. Although it should be borne in mind that this meta-analysis was mainly based on data abstracted from observational studies, these results may justify risk-adapted therapeutic strategies for DLBCL to account for ALC at diagnosis.

Abstract:Objective: We investigated the clinical significance and prognostic value of peripheral blood absolute lymphocyte count (ALC) in the diagnosis of diffuse large B-cell lymphoma. Methods:The medical records of 143 patients with diffuse large B-cell lymphoma were reviewed. These patients were diagnosed and underwent surgery in Tianjin Medical University Cancer Institute and Hospital, China, between February 2002 and April 2008. The correlation between different ALC levels and clinical features was statisti-cally analyzed. Results:Patients with low ALC (<1.0×109/L) at diagnosis showed B symptoms, an international prognostic index score of ≥2, advanced clinical stage (Ann Arbor III/IV), high lactate dehydrogenase level, and high β2-microglobulin level (P<0.05). Pa-tients with high ALC (≥1.0 × 109/L) responded well to treatment (P=0.003). In multivariate analysis, Ann Arbor stage and ALC level were independent prognostic factors of patients with diffuse large B-cell lymphoma. Patients with low ALC often exhibited advanced clinical stage (Ann ArborⅢ/Ⅳ). Conclusions:ALC level obtained at diagnosis is a novel, powerful prognostic factor of diffuse large B-cell lymphoma.

Abstract:Objective To explore the prognostic value of peripheral blood absolute lymphocyte count (ALC) for patients with peripheral T-cell lymphoma,not otherwise specified (PTCL-NOS).Methods The clinical data of 69 patients with PTCL-NOS treated in Tianjin Union Medical Center from January 2008 to January 2016 were analyzed retrospectively.The relationship between different levels of ALC and clinical characteristics,therapeutic efficacy and prognosis was analyzed.Results Among 69 patients,23 cases (33.3％) had low ALC (＜1.0×109/L),and 46 cases (66.7％) had high ALC (≥ 1.0×109/L).Compared with the high ALC group,the low ALC group showed the higher International Prognostic Index (IPI) and PTCL-NOS Prognostic Index (PIT) scores,advanced clinical stage and higher lactate dehydrogenase level (all P ＜ 0.05).The total efficacy rate in the low ALC group was lower than that in the high ALC group [56.5％ (13/23) vs.67.4％ (31/46)],but the difference was not statistically significant (x2 =0.784,P =0.376).The 3-year survival rate in the low ALC group was significantly lower than that in the high ALC group,and the difference was statistically significant (40.5％ vs.68.6％,x2 =7.846,P =0.010).Univariate analysis showed that the US Eastern Cooperative Oncology Group performance status score ≥ 2,IPI score ≥2,Ann Arbor stage Ⅲ-Ⅳ and ALC＜1.0×109/L were the poor prognostic factors (all P ＜ 0.05),while Cox multivariate analysis showed that the Ann Arbor stage Ⅲ-Ⅳ and ALC＜1.0×109/L were the independent risk factors for prognosis of patients with PTCL-NOS (P =0.008,P =0.029).Conclusion The decrease of peripheral blood ALC in patients with PTCL-NOS at the initial diagnosis suggests a poor prognosis,and ALC can be used as a new indicator for prognosis evaluation of PTCL-NOS patients.

Abstract:Objective:To investigate the clinical significance and prognostic value of peripheral blood absolute lymphocyte count(ALC) for patients with extranodal diffuse large B cell lymphoma at admission. Method:Clinical features,immuno-pathological type,genetic markers and therapeutic effect of 59 patients with pathologically confirmed extranodal diffuse large B cell lymphoma were retrospectively reviewed.The relationship between different ALC levels and the clinical features,immuno-pathological type,genetic markers and therapeutic effect of extranodal diffuse large B cell lymphoma patients was statistically analyzed. Result:In this study,the average age of all 59 patients with extranodal diffuse large B cell lymphoma was 61.8 years.Taking ALC=1.0×109/L as cut-off points,18 patients with lymphopenia(ALC<1.0×109/L) at diagnosis were prone to manifeste with later clinical stage(P<0.05) and non-GCB subtypes(P<0.05),achieving a lower rate of overall reaction rate(ORR)(P<0.05) comparing with 41 cases without lymphopenia(ALC>1.0×109/L).No relationships were found between ALC level and age,gender,ECOG performance score,LDH level,IPI score,P53 gene deletion,overexpression of Bcl-6 gene or rearrangement of c-myc gene. Conclusion:Our data suggest that ALC at diagnosis is a novel,quite simple predictor of prognosis for patients with extranodal diffuse large B cell lymphoma.
Lymphopenia During Routine Follow-Up May Predict Relapse in Patients with Extranodal NK/T Cell Lymphoma

Abstract:Recently, absolute lymphocyte count (ALC) at diagnosis, as a surrogate marker of host immunity, has been reported to be a prognostic factor for clinical outcomes in extranodal NK/T cell lymphoma (ENKTL). In this retrospective study, we set out to investigate whether ALC at the time of confirmed relapse or at last follow-up is a marker for relapse after chemoradiotherapy in 84 patients with stage I/II ENKTL. Receiver operating characteristics (ROC) curve and area under the curve (AUC) analysis showed that ALC at follow-up was a significant marker for relapse (AUC = 0.883, P < 0.001). Using 1.215 × 10 9 /L as the optimal cutoff value of ALC, 44 patients (52.4 %) were in lower ALC group and 40 patients (47.6 %) were in higher ALC group. The sensitivity and specificity for ALC at the time of confirmed relapse or at last follow-up was 94.1 and 76.0 %, respectively. The relative risk of relapse with an ALC < 1.215 × 10 9 /L was 14.5. The positive predictive value with an ALC < 1.215 × 10 9 /L was 72.7 %, and the negative predictive value with an ALC ≥ 1.215 × 10 9 /L was 95.0 %. The 4-year cumulative incidence rate for an ALC < 1.215 × 10 9 /L was 73.2 % compared with 3.2 % for an ALC ≥ 1.215 × 10 9 /L ( P < 0.001). In a multivariate regression analysis, ALC at the time of confirmed relapse or last follow-up remained to be a significant factor for relapse ( P < 0.001). In conclusion, lymphopenia observed during routine follow-up can predict relapse in patients with ENKTL, which needs further validation in prospective trials.
Expression of CD14 + HLA-DRlow/- Monocytes in Peripheral Blood of Patients with Non-Hodgkin Lymphoma and Its Significance

Abstract:Objective To assess the immune function of newly diagnosed patients with non-Hodgkin lymphoma (NHL), and to compare the results with clinical data and the treatment effect to seek new immune targets related to prognosis. Methods The peripheral blood samples from 37 cases of new untreated NHL patients and 28 cases of healthy controls were collected.The absolute counts and relative proportion of different immune cells were detected by flow cytometry. The clinical data of the patients and controls were collected and software was used to do statistical analysis. Results The absolute number of monocyte was higher in NHL patients compared with healthy controls (536.20 ×109/μl vs. 374.90 ×109/μl, P<0.01). The classical CD14++CD16-monocyte counts were higher in NHL patients compared with healthy controls (502.30×109/μl vs. 325.10×109/μl, P<0.01). The ratio of absolute lymphocyte count to absolute monocyte count (ALC/AMC) was decreased in NHL patients compared with healthy controls (1.62 vs. 2.45, P<0.01). HLA-DR expression was decreased on each subtype of monocyte of NHL patients, among which the difference of intermediate monocyte (CD14++CD16+) (25.43 MFI vs. 33.52 MFI, P<0.01) and non-classical monocyte (CD14+CD16++) (11.40 MFI vs. 15.70 MFI, P<0.01) were of statistical significance. The ratio of CD14+HLA-DRlow/- monocyte was not associated with the patient's gender, pathological type, remission or relapse. For NHL patients, the ratio of CD14+HLA-DRlow/- monocyte in age below 57.8 years was higher than that of patients with age above 57.8 years (43.83 % vs. 23.85 %, P<0.01). The ratio of CD14+HLA-DRlow/-monocyte in patients with Ann Arbor stageⅢ-Ⅳwas higher than that of patients with Ann Arbor stage Ⅰ-Ⅱ(41.51%vs. 18.40%, P<0.01);The ratio of CD14+HLA-DRlow/-monocyte in patients with IPI score 3-4 points was higher than that of patients with IPI score 1-2 points (52.5%vs. 18.47%, P<0.01). Conclusions Peripheral CD14+HLA-DRlow/-monocyte is associated with poor prognosis in NHL patients. It may be considered as an important index to anticipate patients with disease progression and prognosis.
The Ratio of Absolute Lymphocyte Count at Interim of Therapy to Absolute Lymphocyte Count at Diagnosis Predicts Survival in Childhood B-lineage Acute Lymphoblastic Leukemia.

Abstract:Absolute lymphocyte count (ALC) after therapy has been reported to be an independent prognostic factor for clinical outcome in leukemia. This study mainly analyzed ALC at interim of therapy on day 22 (ALC-22) and the ratio of ALC-22 to ALC at diagnosis (ALC-0) on the impact of survival and the relation of ALC to lymphocyte subsets in 119 pediatric B-lineage acute lymphoblastic leukemia (B-ALL) patients. Univariate analysis revealed that ALC-22/ALC-0 ratio <10% was significantly associated with inferior overall survival (OS) (hazard ratio (HR) = 12.24, P=0.0014) and event-free survival (EFS) (HR = 3.3, P= 0.0046). In multivariate analysis, ALC-22/ALC-0 ratio remained an independent prognostic factor for OS (HR=6.92, P=0.0181) and EFS (HR=2.78, P=0.0329) after adjusting for age, white blood cell (WBC) count and minimal residual disease (MRD) status. A Spearman correlation test showed that CD3+ T cells had a negative correlation with ALC-0 (r=0.7204, P<0.0001) and a positive correlation with ALC-22 (r=0.5061, P= 0.0071). These data suggest that ALC-22/ALC-0 ratio may serve as a more effective biomarker to predict survival in pediatric B-ALL and ALC is mainly associated with CD3+ T cells. (C) 2014 Elsevier Ltd. Predictive Value of Serological Factors, Maximal Standardized Uptake Value and Ratio of Ki67 in Patients Diagnosed with Non-Hodgkin's Lymphoma

Abstract:The present study aimed to determine the prognostic value of serological factors, positron emission tomography/computed tomography maximal standardized uptake value (SUVmax) and the immunohistochemical index ratio of Ki67 (Ki67%) for patients diagnosed with non-Hodgkin's lymphoma (NHL). A total of 120 patients with NHL who received regular chemotherapy and underwent serological, radiological and pathological examinations at Shanghai Tongji Hospital between July 2015 and March 2019 were retrospectively analyzed. Spearman's correlation analysis was preformed to describe the associations between different categories of indicators. Kaplan-Meier analysis and log-rank test were used to compare the survival of different subgroups. Receiver operating characteristic curves were generated to assess the predictive value of prominent indicators derived from Cox regression analysis. The results indicated that inflammatory cytokines were strongly associated with tumor burden indicators. The correlation between SUVmax and Ki67% was significant, and SUVmax of the biopsy site exhibited a stronger association with Ki67% (Ρ=0.529, P<0.001) compared with SUVmax of the whole body (Ρ=0.395, P=0.017). C-reactive protein (CRP), lactate dehydrogenase (LDH) and interleukin-6 could differentiate the survival status of patients with NHL, whereas no statistical significance in the estimation of overall survival (OS) was obtained for SUVmax and Ki67%. SUVmax of the biopsy site had only a limited value in the estimation of progression-free survival (PFS), whereas LDH, β2-microglobulin (β2-mg) and CRP were independent predictors of both OS and PFS with high sensitivity and specificity. Among all indicators, CRP and β2-mg could predict both survival status and complete remission of patients with NHL, whereas the prognostic value of SUVmax and Ki67% requires further study and discussion.

Abstract:OBJECTIVE:To analyze the clinical characteristics and prognostic factors of patients with non-Hodgkin's lymphoma (NHL) in single center of the Northwest area in China for 10 years, so as to provide the evidences for early diagnosis, stratified treatmetn, evaluation of therapeutic efficacy and prognosis, as well as early prevation and so on.METHODS:The clinical data of 254 patients with NHL were analyzed retrospectively, the clinical characteristics were evaluated by unvariate analysis; then the single factors affecting prognosis were enrolled in multivariate analysis and the independent prognostic factors affecting the survival of patients were summarized.RESULTS:A total of 182 cases achieved CR（71.6%）, PR 30 cases（11.8%）, SD 22 cases（8.7%）, PD 20 cases（7.9%）, and RR 212 cases（83.5%). The statistically significant unfavorable prognostic factors for NHL revealed by univariate analysis included age, invasive, Ann Arbor stage, relapse, and total course of chemotherapy. Cox regression model analysis showed that the Ann Arbor stage, IPI, ECOG, B symptoms, peripheral blood cell levels, short-term efficacy, course to achieve CR, and total course of chemotherapy all were the independent prognostic factors.CONCLUSION:The incidence characteristics of NHL in this center displayed mainly middle and high-risk B cell type with attacks at young age, aggression and in lymph nodes. For aggressive lymphoma, the single and multiple prognostic factors may provide the significant guides for the treatment, individualized plan and evaluation of prognosis. The course number of chemotherapy is one of the important factors for survival and prognosis, possessed clinical significance, and worth further clinical research for aggressive lymphoma.

Abstract:The prognostic value of absolute lymphocyte count (ALC) has been a recent matter of debate in childhood acute lymphoblastic leukemia (ALL). In the current study, ALCs at the time of diagnosis (ALC-0), after 7 days of initial therapy (ALC-8) and at interim of the induction therapy (ALC-22) were examined in Chinese children with B-cell precursor (BCP) ALL and correlated with the level of minimal residual disease (MRD) at day 22 of induction therapy. Medical and laboratory records of 140 patients diagnosed with childhood BCP ALL were retrieved and analyzed. ALC-22 is significantly correlated with MRD level at day 22 of therapy and can be a good prognostic factor for childhood BCP-ALL. Furthermore, lymphocyte count at initial diagnosis is correlated with MRD level at day 22 in childhood BCP-ALL with the immnunophenotype of CD19(pos)/CD10(pos)/CD34(pos)/CD45(neg) and role as a new prognostic factor was determined.

Abstract:Abstract Abstract 4993 Objective: To investigate prognostic significance of cytogenetics abnormalities, staging, patient factors, serum p53 protein concentration with Non–Hodgkin lymphoma (NHL). Methods: 43 patients with NHL were identified form August 2003 to December 2010. The clinical characteristics of these cases were performed by morphological, immunohistochemical, cytogenetic analyses and quantitative detection of human serum p53 protein were performed. Results: After conventional chemotherapy, a higher proportion of complete/ partial remission (CR/PR) rates and a prolonged overall survival (OS) in patients with early-stage(I–II) lymphoma, normal karyotype and lower level of Serum p53 protein compared with patients with advanced-stage (III–IV) lymphoma, cytogenetic abnormalities and higher p53 protein level (≥0.35U/ml) in serum, respectively. Bone marrow infiltration is also a predictor of poor response and OS. No statistical difference was noted with respect to the rate of disease remission between male versus female(<70 years versus ≥70 years), B-cell lymphoma versus T-cell lymphoma subgroups. Conclusions: Staging is an effective way of assessing disease extent; Cytogenetic examination provided useful information on diagnosis, staging and prognostic estimation; Serum p53 protein level may be a potential prognostic marker in patients with NHL. Abstract:Background: Accumulating evidence suggested that tumor microenvironment and host immune system played important roles in determining the clinical course and outcome of human malignancies. The derived neutrophil to lymphocyte ratio (dNLR) and absolute lymphocyte count (ALC) were demonstrated to act as a prognostic factor in several malignancies. Nevertheless, the prognostic significance of them in extranodal natural killer/T-cell lymphoma (ENKTL) patients has never been explored. Patients and methods: A total of 33 newly diagnosed patients with ENKTL were included in this study. Clinicopathological characteristics were collected and prognostic significance of dNLR and ALC were evaluated. Results: Elevated dNLR and low ALC were both associated with poor survival rates. Patients with dNLR >= 3.6 revealed significantly shorter overall survival (OS) (P=0.001) and progression-free survival (PFS) (P=0.008) than those with dNLR <3.6. Patients with ALC <0.8x10(9)/L had worse OS (P=0.008) and PFS (P<0.001) than those with ALC >= 0.8x10(9)/L. An independent significant association between low ALC and poor clinical outcome in multivariate analysis for OS (HR, 36.023; 95% CI, 2.438-532.243; P=0.009) as well as PFS (HR, 7.698; 95% CI, 1.573-37.679; P=0.012) was identified. Conclusion: In this study, we validated for the first time the prognostic value of dNLR and ALC in ENKTL patients. Elevated dNLR and low ALC were both associated with aggressive tumor process and poor survival. ALC value at diagnosis represented an independent favorable prognostic factor for the clinical outcome of ENKTL patients.
The Prognostic Significance of Peripheral Blood ALC/AMC in Follicular Lymphoma Patients Treated with R-CHOP-like Chemotherapy

Abstract:Background and purpose:The prognostic capability of traditional prognostic index like follicular lymphoma international prognostic index (FLIPI) is limited in the rituximab era. This study was to investigate the prognostic significance of peripheral blood absolute lymphocyte count/absolute monocyte count (ALC/AMC) in Chinese patients with follicular lymphoma (FL).Methods:This study retrospectively analyzed 136 newly diagnosed FL patients who received rituximab plus cyclophosphamide, doxorubicin, vincristine and prednisone (R-CHOP)-like chemotherapy in Department of Hematology, Ruijin Hospital from Jan. 2003 to Dec. 2013, and further classified these patients according to FLIPI scoring system.Results:According to FLIPI, 61 patients (44.9%) were stratified into the low-risk (0-1 points) group, 42 cases (30.9%) into the intermediate-risk (2 points) group, and 33 cases (24.2%) into the high-risk (3-5 points) group. The overall response rate and 2-year progression-free survival (PFS) of the 3 risk groups were 88.5%, 95.2%, and 78.8% (P=0.090), and 91.4%, 74.6%, and 47.8% (log-rank=23.3,P<0.001), respectively. The overall response rate and 2-year PFS for patients with ALC/AMC≥4.7 and <4.7 were 91.9%, 68.6% (P=0.005) and 96.0%, 69.7% (log-rank=13.0,P<0.001), respectively. In the multivariate study, ALC/AMC≥4.7 was independent of FLIPI and was able to distinguish the FLIPI low-risk and intermediate-risk patients (log-rank=7.535,P=0.006).Conclusion:For FL patients treated with R-CHOP-like regimens, ALC/AMC is a simple and effective biomarker reflecting tumor microenvironment and human immunity, and could be considered for prognosis evaluation.

Abstract:Background Extranodal natural killer/T-cell lymphoma (ENKL) has heterogeneous clinical manifestations and prognosis. This study aims to evaluate the prognostic impact of absolute monocyte count (AMC) in ENKL, and provide some immunologically relevant information for better risk stratification in patients with ENKL. Methods Retrospective data from 163 patients newly diagnosed with ENKL were analyzed. The absolute monocyte count (AMC) at diagnosis was analyzed as continuous and dichotomized variables. Independent prognostic factors of survival were determined by Cox regression analysis. Results The AMC at diagnosis were related to overall survival (OS) and progression-free survival (PFS) in patients with ENKL. Multivariate analysis identified AMC as independent prognostic factors of survival, independent of International Prognostic Index (IPI) and Korean prognostic index (KPI). The prognostic index incorporating AMC and absolute lymphocyte count (ALC), another surrogate factor of immune status, could be used to stratify all 163 patients with ENKL into different prognostic groups. For patients who received chemotherapy followed by radiotherapy (102 cases), the three AMC/ALC index categories identified patients with significantly different survivals. When superimposed on IPI or KPI categories, the AMC/ALC index was better able to identify high-risk patients in the low-risk IPI or KPI category. Conclusion The baseline peripheral monocyte count is shown to be an effective prognostic indicator of survival in ENKL patients. The prognostic index related to tumor microenvironment might be helpful to identify high-risk patients with ENKL.

Abstract:Objective:To investigate the value of peripheral blood absolute lymphocyte count (ALC),absolute monocyte count (AMC) and ALC/AMC ratio (LMR) in predicting the prognosis of patients with newly diagnosed multiple myeloma (MM).Methods:Retrospective analysis was performed in 190 patients with newly diagnosed MM from Tianjin Medical University Cancer Institute and Hospital between January 2005 and December 201 5.The relationships of peripheral blood ALC,AMC and LMR with peripheral blood hemoglobin (Hb),β2-microglobulin (β2-MG) and lactate dehydrogenase (LDH) and the proportion of bone marrow plasma cells were analyzed.The cutoff values of ALC,AMC and LMR were determined by the receiver operating characteristic (ROC) curve in patients with newly diagnosed MM.Survival analysis was performed using Kaplan-Meier method and the log-rank test for univariate analysis of prognosis,and a COX proportional hazard model was used for multivariate analysis of prognosis.Results:The cutoff values of ALC,AMC and LMR determined by ROC curve were 1.24×109/L,0.60×109/L and 3.90,respectively.According to these cutoff values,the patients were divided into high value groups and low value groups.The results of multivariate analysis showed that ALC ＜ 1.24×109/L [hazard ratio (HR):0.544,95％ confidence interval (CI):0.301-0.984);P =0.044],LMR≤3.90(HR:2.284,95％ CI:1.018-5.124;P =0.045) and LDH ＞ 247 U/L (HR:1.972,95％ CI:1.087-3.576;P =0.025) were independent poor prognostic factors in untreated MM patients.According to the number of poor prognostic factors (each poor prognostic factor was scored as one),the patients were divided into score 0,1-2 and 3 groups,the overall survival and progression-free survival of the three groups were significantly different (all P ＜ 0.05).Conclusion:Lower ALC and LMR values may indicate poor prognosis.ALC＜1.24×109/L and LMC≤3.90 maybe the independent poor prognostic factors in patients with newly diagnosed MM.
Prognostic Factors of Chinese Patients with T/NK-cell Lymphoma: a Single Institution Study of 170 Patients

Abstract:T/natural killer-cell lymphoma (T/NKCL) is a heterogeneous group of lymphoma and has a higher incidence in Asia than in Western countries. T/NKCL is presented with various clinicopathologic features, and in general, associated with a poor clinical outcome. This study aims to analyze the clinical prognostic factors in patients with T/NKCL. From January 1999 to December 2009, a total of 170 patients with T/NKCL, except mycosis fungoides, were included in this retrospective study. The diagnosis was established according to World Health Organization classification. The clinical characteristics and prognostic factors were evaluated. Of the 170 patients, mainly peripheral T-cell lymphoma-unspecified (65 cases), precursor T-lymphoblastic lymphoma/leukemia (31 cases) and nasal NK/T-cell lymphoma (NKTCL, 19 cases), advanced disease (Ann Arbor stages III-IV) was presented in 68.8% and extranodal involvement was in 71.2% of the patients. According to the international prognostic index (IPI), 77 cases were categorized as high/intermediate or high-risk group. Using the prognostic index for peripheral T-cell lymphoma-unspecified (PIT), 87 cases were classified as group 3 or 4. Most of the initial regimens were CHOP (cyclophosphamide, doxorubicin, vincristine and prednisone)-based chemotherapy (87.6%). Cumulative probability of overall survival at 5 years was 43%, and the median survival time was 44.5 months. Univariate analysis revealed that factors associated with a poor outcome were poor performance status (ECOG > 1) (P = 0.001), advanced disease (P = 0.009), the presence of B symptom (P = 0.001), multiple extranodal involvement (P = 0.005), bone marrow involvement (P = 0.003), elevated lactic dehydrogenase level (P = 0.019), IPI (P < 0.001), PIT (P < 0.001), abnormal white blood cell count (P = 0.016), decreased platelet count (P = 0.005) and serum Epstein-Barr virus (EBV) IgA positivity (P = 0.016). In the multivariate analysis, PIT (P < 0.001; relative risk, 3.221; 95% CI = 2.115-4.907) and EBV serum IgA (P = 0.049; relative risk, 1.901; 95% CI = 1.002-3.606) remained independent factors predictive for overall survival. The PIT may therefore be a useful index for risk stratification in patients with T/NKCL. The serum EBV antibody test could be a simple and quick marker to predict the outcome of the patients.

Abstract:OBJECTIVETo analyse the influence of factors on the prognoses of non-Hodgkin's lymphoma (NHL) and the clinical usage of IgH and T cell receptor(TCR) gene rearrangement for NHL typing.METHODSImmunological phenotyping was carried out by SABC, and IgH(FR2A, FR3A) and TCR(beta, gamma) detection by PCR.RESULTSThe age curve of NHL was increased parallel with the patient's age increasing, hazard ratio was increased 1.039n annually. The incidence of B cell NHL(B-NHL) was 66.7%, T cell NHL(T-NHL) was 31.1%, low grade NHL was 56%, middle and high grade NHL were 44%. The positivity of IgH and TCR gene rearrangement in NHL patients were 75% by PCR detection, T and B classification was same as phenotyping. The 3 and 5 years survival ratio: HD were 83.3% and 62.5%, stage I-II of NHL were 88.9% and 66.7%, stage III-IV of NHL were 39.9% and 33.3%, Low grade NHL were 65.1% and 48.8%, middle and high grade NHL were 47.6% and 39.6%. The survival time of APBSCT group was longer than that of the conventional therapy group.CONCLUSIONAge, T,B classification, grading and staging are the important factors which affect on NHL prognoses. APBCST can improve NHL prognoses, especially for those of stage III-IV patients. Molecular biological methods can help T/B classification when it couldn't be confirmed by phenotyping.
The Prognostic Significance of the Serum P53 Protein Concentration in Chinese Patients with Non-Hodgkin Lymphoma.

Abstract:OBJECTIVE:To investigate the prognostic significance of cytogenetic abnormalities, staging, patient factors, and the serum p53 protein concentration in Chinese non-Hodgkin lymphoma (NHL) patients.MATERIAL AND METHODS:The study included 43 patients with NHL that were identified between August 2003 and December 2008. Patient clinical characteristics patients were determined based on morphological, immunohistochemical, and cytogenetic analysis, and the serum p53 protein concentration was measured quantitatively.RESULTS:Following conventional chemotherapy, the complete/partial remission (CR/PR) rate was significantly higher and overall survival (OS) was significantly longer in the patients with early-stage (stage I-II) lymphoma, normal karyotype, and a low serum p53 protein concentration than in those with advanced-stage (stage III-IV) lymphoma, cytogenetic abnormalities, and a high serum p53 protein concentration (≥0.35 U/mL). Bone marrow infiltration was also a predictor of poor response and OS. There weren't any significant differences in disease remission between the male and female patients, older and younger patients (aged <70 years vs. ≥70 years), or B-cell lymphoma and T-cell lymphoma patients.CONCLUSION:Staging is an effective means of assessing the severity of NHL. Cytogenetic examination can provide useful information for diagnosis, staging, and prognostication. The serum p53 protein level may be a potential prognostic marker in patients with NHL.CONFLICT OF INTEREST:None declared.
