Poster 056: Surgical Management of the Asymmetric Patients with Unilateral Temporomandibular Joint Ankylosis

Abstract:The correction of facial asymmetry after unilateral temporomandibular joint ankylosis presents a challenging problem for reconstructive surgeons. Various techniques have been defined for the treatment of the condition. We present our experience with a minimum of 1-year follow-up over a 3-year period with 12 patients who underwent bony reconstruction for facial contour. From July of 2004 to June of 2007, 12 patients with unilateral temporomandibular joint ankylosis were used in this investigation. Orthognathic surgeries such as maxillary Le Fort I osteotomy, bilateral sagittal split ramus osteotomies (SSRO), mandibuloplasty, mandibular osteodistraction, or genioplasty, were performed for those patients, which depended on their facial characteristics and malformation of jaws. Simultaneous arthroplasty was performed in 6 patients. TMJ functions, facial profile, and occlusion were evaluated after surgical treatment. No complications were observed in any of the patients. Good results were obtained during a follow-up period ranging from 1 to 3 years, regarding the restoration of joint function, improvement in esthetic appearance and relief of respiratory obstruction. Application of orthognathic surgery provides the surgeon with the ability to correct facial asymmetry in patients with facial deformity after TMJ ankylosis. Orthodontic treatment before surgery is necessary to produce a symmetrical face with good occlusion.
