Abstract:Objective To ivestigate the relationship between serum uric acid and lower extremity atherosclerosis in pa-tients with type 2 diabetes. Method 368 hospitalized type 2 diabetic patients were enrolled,body mass index( BMI) , waist-hip ratio(WHR),blood pressure,liver and renal funtion,fasting blood glucose,lipids,serum uric acid,HbA1c were measured or calculated. All the patients were examed with color doppler uhrasonography to find out if they had lower extremity atherosclerosis,and accordingly they were divided into two groups,NC group included 192 patients whose lower extremity artery intima-media thickness was less than 1. 0mm,while in AS group,176 patients,lower ex-tremity artery intima-media thickness was more than 1. 0mm or with plaque. The clinical and characteristic items were compared,and the relationship between serum uric acid and lower extremity atherosclerosis was statistical ana-lyzed. Result In all patients,the proportion of lower extremity atherosclerosis was 47. 82%,and hyperuricemia was 26. 63%. In AS group,the levels of BMI、age、WHR、duration of DM、HbA1c、LDL-C、uric acid、systolic blood pres-sure(SBP)、smoking were evidently higher than the NC group(P<0. 05). The level of serum uric acid in NC group was(323. 55±52. 75)μmol/L,the proportion of hyperuricemia was 21. 87%;while in AS group,level of serum uric acid was(361. 25±56. 90)μmol/L,the proportion of hyperuricemia was 32. 39%,both of them between the two groups were statistically different(P<0. 05). All the patients were divided into two groups based on whether the level of serum uric acid was high or nomal. The level of uric acid in normal group(270 patients)was(302. 49±46. 52)μmol/L,the proportion of lower extremity atherosclerosis was 44. 07%;the level of uric acid in hyperuricemia group was(396. 41±48. 61)μmol/L,the proportion of lower extremity atherosclerosis was 58. 17%,it was statistically dif-ferent between the two groups(P<0. 05). With lower extremity atherosclerosis as dependent variable,logistic regres-sion showed that age、duration of diabetes、WHR、HbA1c、uric acid、LDL-C、smoking were independent risk factors. Conclusion Hyperuricemia was an independent risk factor for lower extremity atherosclerosis in type 2 diabetic patients.
